Objective By population survey, to explore the epidemiological characteristics of gastric precancerous lesions in Lishui District of Nanjing and provide objective basis for the prevention and treatment of early gastric cancer. Methods From July 2021 to December 2022, 21 977 patients who received endoscopy and/or 13C-UBT in Lishui District People's Hospital and 6 medical community units in Nanjing City were retrospectively analyzed for demography characteristics, detection rate of gastric precancerous lesions, and H. Pylori infection rate. Results （1） 590 cases of gastric precancerous lesions were detected （detection rate 2.68%）; （2） The total detection rate of precancerous lesions and three pathological types in males were all higher than those in females （all P < 0.001）; （3） The minimum age for the total detection rate of precancerous lesions in males and the minimum age for each pathological type were lower than in females （P < 0.001, 0.009, 0.005, 0.002）; （4） The population total H. pylori infection rate was 23.10%, the H. pylori infection rate in patients with precancerous lesions was higher than that in non-precancerous lesions （P < 0.001）, both H. pylori infection rate of male and female in precancerous lesions were all higher than those of non-precancerous lesions of the same sex （all P < 0.001）, in addition, the H. pylori infection rate of male whether in precancerous or non-precancerous lesions was higher than that of female （all P < 0.001）; （5） The precancerous lesions detection rate in male, female, and the overall age range of 20 ~ 29 to 70 ~ 79 years is positively correlated with age growth （P < 0.001）, and rapidly decreases after the age of 79, the of H. pylori infection rate was also positively correlated with age growth （P < 0.001）, and the trend of age change （P < 0.001） was parallel to the precancerous lesions detection rate. Conclusions The detection rate of gastric precancerous lesions in this region is above the average level in China;the total H. pylori infection rate is at a relatively low level in China; the H. pylori infection rate is parallel to the age trend of the detection rate of gastric precancerous lesions, and increases with age. Objective To explore the predictive value of carbohydrate antigen 125 (CA125) combined with platelet-to-lymphocyte ratio (PLR) in the diagnosis of early gastric cancer. Methods A total of 150 suspected gastric cancer patients admitted to this hospital from January 2020 to January 2023 were selected as the study subjects. Peripheral venous blood samples were collected to detect the levels of CA125, PLR, and their combined levels. Among them, 87 cases were confirmed as benign lesions by gastroscopy or surgical pathological biopsy (54 cases of chronic non-atrophic gastritis and 33 cases of chronic atrophic gastritis）, and 63 cases were diagnosed as early gastric cancer (including severe dysplasia). The levels of CA125 and PLR were compared between the two groups, and the influencing factors of early gastric cancer were analyzed. Receiver operating characteristic (ROC) curves were plotted to analyze the value of the above indicators alone and in combination in the diagnosis of early gastric cancer. Results Significant differences were observed in CA125, PLR, tumor location, smoking status, histological type, and depth of invasion between the two groups (P＜0. 05). Multivariate logistic regression analysis revealed that tumor location, smoking status, CA125, and PLR were independent influencing factors for early gastric cancer (P＜0. 05). The ROC curves showed that CA125 and PLR had similar diagnostic efficacy for early gastric cancer, but the combination of the two could effectively improve the diagnostic performance of each individual marker. The area under the curve (AUC) of the combined predictor for early gastric cancer was 0.856, with a 95% confidence interval of 0.837-0. 951, a sensitivity of 81. 60%, and a specificity of 88. 70%. Conclusion Tumor location, smoking status, CA125, and PLR are independent risk factors for early gastric cancer. CA125 and PLR may be helpful in identifying patients with early gastric cancer, and they are more easily accessible and cost-effective, making them more acceptable to patients. Objective To analyze the influencing factors of delayed postoperative bleeding (DPPB) after endoscopic submucosal dissection (ESD) for early gastric cancer and construct a Nomogram model. Methods The clinical information of 234 patients who underwent ESD treatment at our hospital and pathologically diagnosed with early gastric cancer from April 2021 to April 2023 were collected. The patients were grouped based on the presence or absence of DPPB. Logistic regression analysis was applied to screen for risk factors affecting DPPB after ESD in early gastric cancer patients. R software was applied to construct a Nomogram model for predicting the risk of DPPB in early gastric cancer patients after ESD. Receiver operator characteristic (ROC) curve, calibration curve, and Hosmer-Lemeshow goodness of fit test were plotted to evaluate the Nomogram model. Results A total of 23 (9.8%) of 234 early gastric cancer patients who underwent ESD treatment developed DPPB. Multivariate Logistic regression analysis showed that the number of biopsies ≥ 3 (95%CI: 2.482-20.424, P=0.000), the maximum diameter of lesions ≥ 3 cm (95%CI: 1.620-13.244, P=0.004), the depth of invasion to submucosa (95%CI: 1.421-10.536, P=0.008), and intraoperative bleeding (95%CI: 1.160-11.300, P=0.027) were independent risk factors for DPPB after ESD in early gastric cancer patients. The area under ROC curve (AUC) was 0.838 (95%CI: 0.743-0.932). The slope of the calibration curve was close to 1. Hosmer-Lemeshow goodness of fit test showed χ2=3.328, P=0.505. Conclusions The number of biopsies ≥ 3, the maximum diameter of lesion ≥ 3 cm, the depth of invasion to submucosa, and intraoperative bleeding are independent risk factors for DPPB after ESD in early gastric cancer patients. The Nomogram model based on these four independent risk factors can effectively predict the risk of DPPB after ESD in early gastric cancer patients.

Objective To explore the related factors of lymph node metastasis in early gastric cancer and to analyze the clinical efficacy between laparoscopic D2 radical resection and endoscopic submucosal dissection (ESD). Methods A retrospective cohort study was conducted on 404 patients with early gastric cancer undergoing surgical treatment in our hospital from June 2018 to June 2022 were recruited in this study. According to their surgical treatment, they were divided into laparoscopy group (n=271) and ESD group (n=133). The lymph node metastases were compared in the patients with different clinical features, and the factors affecting lymph node metastasis were analyzed with multivariate logistic regression analysis. The lymph node metastasis rate of patients with absolute and expanded indications of ESD was calculated. Kaplan-Meier survival analysis was used to compare the long-term efficacy of ESD and laparoscopic D2 radical resection in these patients. Stratified regression analysis was applied to explore the relationship between surgical methods and long-term outcome in them. Results In the 404 patients with early gastric cancer, the rate of lymph node metastasis was 12.38% (50/404). Univariate analysis showed that 7 clinical features, such as ulcer, tumor pathological features and pathological differentiation, were related to lymph node metastasis. Multivariate logistic regression analysis revealed that low differentiation and tumor infiltration depth of submucous membrane (SM) were related factors affecting lymph node metastasis in early gastric cancer. The overall survival rate was 96.29% (389/404) in the 404 patients, 96.31% in the laparoscopic group and 96.24% in the ESD group, and no statistical difference was seen between the 2 groups (P>0.05). There was no obvious difference in 5-year survival rate between the patients who met the absolute indications for ESD and underwent laparoscopic D2 radical surgery and those receiving ESD treatment (96.00% vs 96.55%), and between those who were in accordance with the expanded indications for ESD and underwent laparoscopic D2 radical surgery and those receiving ESD treatment (94.74% vs 91.67%). After adjusting the clinical features such as lesion size, pathological differentiation and depth of tumor invasion by stratified regression analysis, the mode of operation for early gastric cancer was still correlated with the long-term efficacy (β=1.173, P=0.003). Conclusion The depth of tumor infiltration and differentiation are closely related to lymph node metastasis in early gastric cancer patients. The efficacy of laparoscopic D2 radical surgery on the patients with absolute and expanded indications for ESD is comparable to that of ESD treatment.

Objective To explore the risk miRNAs during the development of gastric cancer, providing the evidence for early gastric cancer identification in upper gastrointestinal opportunistic screening. Methods The patients who underwent opportunistic screening population for upper gastrointestinal cancer in the Guangxi Medical University Cancer Hospital, the Affiliated Hospital of Youjiang Medical University for Nationalities, and Guilin People's Hospital from June 2021 to August 2023 were enrolled. A total of 107 healthy individuals, 71 patients with early gastric cancer, and 97 patients with advanced gastric cancer were selected. The differential expression of miRNAs was screened by using transcriptome sequencing, and the differential expression of miRNAs was validated by RT-qPCR in prospective blood samples of the 3 prospective groups. The diagnostic efficacy of miRNAs was evaluated by using receiver operating characteristic (ROC) curves. Results The differential gene analysis by transcriptome sequencing screened out 6 differentially expressed miRNAs in the development of gastric cancer development, including miR-3176, miR-885-5p, miR-203a-3p, miR-452-5p, miR-223-3p, and miR-219a-2-3p. RT-qPCR results showed that miRNA-452-5p was up-regulated in both the early gastric cancer and the advanced gastric cancer (all P<0.001). The ROC curves indicated that the area under the curve (AUC) of miRNA-452-5p for diagnosing early gastric cancer and advanced gastric cancer were 0.900 and 0.975, respectively. The AUC for distinguishing early gastric cancer and advanced gastric cancer was 0.843. Conclusions miRNA-452-5p demonstrates a good diagnostic efficacy in early gastric cancer, and may serve as a potential biomarker for the diagnosis of early gastric cancer by liquid biopsy. Endoscopic resection (ER) and laparoscopic surgery are the main methods for treatment of early gastric cancer (EGC). Radical gastrectomy(RG) and lymph node dissection are required for the patients who don’t meet the indications of ER, but it may cause postoperative stomach dysfunction and reduce quality of life (QOL). Pylorus-preserving gastrectomy (PPG) is one of the function-preserving gastrectomy for EGC. The short-term complications of PPG are equal to that of RG and it is also beneficial to long-term nutritional status and QOL. The 5-year survival rate of PPG is similar to that of distal subtotal gastrectomy (DSG) according to a retrospective study of the matched cohort. But delayed gastric emptying (DGE) is a common problem in the short and long term after PPG. The limited lymph node dissection of PPG makes its application not widely promoted. Sentinel node navigation surgery (SNNS) may provide more personalized and minimally invasive surgery, in order to preserve the gastric function to the maximum extent.

Objective To establish a prediction model for non-curative resection in patients with early gastric cancer (EGC) who underwent endoscopic submucosal dissection (ESD), and to evaluate its predictive value. Methods Clinical data of EGC patients in the Second Hospital & Clinical Medical School, Lanzhou University from January 2014 to July 2023 were retrospectively collected. According to the postoperative pathological results of ESD, the patients were divided into curative resection group and non-curative resection group. Multifactorial Logistic regression analysis was used to screen the risk factors for non-curative resection after ESD surgery and establish a prediction model, and the model was evaluated using receiver operating characteristic(ROC) curves, calibration curves and clinical decision curve analysis. Results A total of 479 EGC patients who underwent ESD were included, with 60 cases in the non-curative resection group and 419 cases in the curative resection group. The results of multifactorial Logistic regression analysis showed that the lesion diameter > 2 cm (OR=3.017, 95% CI: 1.483-6.136, P=0.002), flat lesion morphology (OR=2.712, 95% CI: 0.774-9.497, P=0.043), undifferentiated/mixed histologic type (OR= 4.199, 95% CI: 1.621-10.872, P=0.003), and submucosal infiltration (OR=30.329, 95% CI: 13.059-70.436, P < 0.001) were independent risk factors for non-curative resection after ESD in EGC patients. The area under the curve of ROC validated within the column-line graph prediction model constructed accordingly was 0.867 (95% CI: 0.811-0.923), the calibration curve showed that the model had good calibration, and decision curve analysis showed the model had a good clinical usefulness. Conclusions The prediction model constructed based on lesion diameter, lesion morphology, histologic type, and depth of mucosal infiltration has good differentiation, calibration, and clinical utility. This model is expected to assist in the early clinical screening of the population at high risk for noncurative resection after ESD in patients with EGC, and to provide a basis for the development of optimal clinical decisions.

Objective To evaluate the effect of modified nursing shift schedule based on SBAR communication model in perioperative management of patients with early gastric cancer. Methods Totally 100 patients who received endoscopic submucosal dissection (ESD) for early gastric cancer between January and May 2021 were included as the control group, another 100 patients who received ESD for early gastric cancer between August and December 2021 were included as the observation group. The routine nursing shift management was adopted in the control group, and the modified nursing shift schedule based on SBAR communication model was carried out in the observation group. The quality of nursing shift, patient’s satisfaction and nurse’s critical thinking ability were measured and compared between two groups. Results After implementation of modified nursing shift schedule, the quality of nursing shift was improved, and nurses achieved a higher score of The California Critical Thinking Disposition Inventory (CCTDI) compared with that before implementation of modified nursing shift schedule (259. 95±38. 60 vs. 228. 68±36. 44, P＜0. 05). The score of satisfaction assessment in the observation group was higher than that in the control group (21. 47±2. 96 vs. 7. 39±2. 90, P＜0. 05). Conclusion The modified nursing shift schedule based on SBAR communication model is an effective tool of improve quality of nursing shift, patient’s satisfaction and nurse’s critical thinking ability. (目的 探讨基于SBAR模式改良护理交班表在早期胃癌患者围手术期管理中的应用效果。方法 选取2021年1月—5月医院消化内科收治的100例行内镜黏膜下剥离术(ESD)治疗的早期胃癌患者作为对照组, 2021年8月—12月收治的100例行ESD治疗的早期胃癌患者作为观察组。对照组使用传统交班模式, 观察组采用基于SBAR模式改良的护理交班表完成护理交班管理。评估两组的交班质量、患者对护士的满意度及护士的评判性思维能力。结果 实施改良护理交班表后, 护士交班质量全面性明显提高, 护士的评判性思维得分(259. 95±38. 60)分也高于实施前的(228. 68±36. 44)分, 差异有统计学意义(P＜0. 05)。观察组患者对护士满意度评分(21. 47±2. 96)分, 高于对照组的(17. 39±2. 90)分, 差异有统计学意义(P＜0. 05)。结论 采用基于SBAR模式改良的护理交班表可以提高交班质量以及患者对护士的满意度, 还能提高护士的评判性思维能力。)

To analyze the efficacy of endoscopic submucosal dissection (ESD) in the treatment of early gastric cancer, and to provide reference for the treatment of such patients. 60 patients with early gastric cancer were selected as the main body. The patients were randomly divided into routine group (30 cases) and experimental group (30 cases). The patients were treated with surgical resection in the routine group and gastrointestinal ESD in the experimental group. After treatment, the clinical efficacy, surgical indicators, serum tumor markers, PG levels (PG Ⅰ, PG Ⅱ) and follow-up were compared in each group. The case selection time was from November 2019 to November 2021. There was no difference between the total effective rate of patients in the experimental group and the routine group (P>0.05); There was no difference in the levels of CEA, CA199 and CA125 between the two groups before treatment (P>0.05). After treatment, the levels of CEA, CA199 and CA125 in the two groups decreased, and the decrease in the experimental group was greater than that in the conventional group (P<0.05); The operation time, blood loss, hospitalization time and blood transfusion rate of the experimental group were less than or less than those of the conventional group (P<0.05); Before treatment, there was no difference in the levels of PG Ⅰ, PG Ⅱ and PG Ⅰ /pg Ⅱ between the two groups (P>0.05). After treatment, the levels of PG Ⅰ, PG Ⅰ /pg Ⅱ in the two groups increased, and the level of PG Ⅱ decreased. The range of change in the experimental group was greater than that in the conventional group(P<0.05); The recurrence rate of the experimental group was similar with the conventional group (P>0.05). the effect of gastrointestinal ESD in the treatment of patients with early gastric cancer is ideal, which can effectively improve the level of serum proteasome and tumor markers. It has the advantages of less bleeding and short operation time. At the same time, it can also reduce the risk of complications, which can be applied and promoted in the treatment of such diseases. The detection rate of gastric submucosal tumor (SMT) and early gastric cancer has been increasing year by year. The type of treatment should be selected to keep gastric function as more as possible because of both SMT and early gastric cancer with good prognosis. Endoscopy and the combination of laparoscopic and endoscopic cooperative surgery can be the choice. Laparoscopic intragastric surgery (LIGS) contains the advantages of both laparoscopy and endoscopy and can be performed by the surgeon only. LIGS has been applied to gastric submucosal tumors and early gastric cancer based on the continuous improvement of the procedure and instruments of LIGS. The application including the advantage and disadvantage of LIGS were reviewed in this paper.

Objective：This study aims to investigate the relationship between D⁃dimer in different periods and postoperative deep venous thrombosis（DVT）in patients with early gastric cancer and premalignant lesions underwent endoscopic submucosal dissection （ESD）. Methods：A retrospective cohort study was carried out，176 patients with early gastric cancer were collected，including 117 men with an average age of（62.9±9.7）years. According to the results of vascular ultrasound examination of the lower extremities at 3 days after surgery，the patients were divided into two groups：venous thrombosis group（12 people）and non ⁃venous thrombosis group （164 people）. General clinical data of patients were collected. Postoperative DVT risk score was calculated by Padua scale. The blood D ⁃dimer level of all patients were measured 1 day before operation，immediately after operation and 1 day after operation. The ROC curve and area under curve（AUC）of multiple variables were analyzed to predict DVT after ESD. The relationship between D ⁃dimer level and DVT after ESD was analyzed by multiple regression model. Results：There were 176 patients underwent vascular ultrasound examination of lower extremities 3 days after surgery，and 12 patients（6.82%）had lower extremity DVT. There was statistical significance in D⁃dimer levels between the two groups before ESD and 1 day after ESD（all P < 0.05）. The sensitivity，specificity and AUC of D ⁃ dimer 1 day after ESD to predicted DVT were 91.7%，67.1%，0.841（95% CI：0.778~0.892）. Multivariate regression analysis showed that the level of D⁃dimer 1 day after ESD was significantly correlated with the increased risk of DVT after ESD（OR：82.756，95% CI：6.000~1 141.349，P < 0.001）. Conclusion：The increased level of D ⁃ dimer 1 day after ESD is associated with an increased risk of DVT in patients with early gastric cancer and premalignant lesions underwent ESD. Objective To investigate the related factors and risk model prediction of pneumonia in patients with early gastric cancer after ESD. Methods We enrolled 171 patients with early gastric cancer who underwent endoscopic submucosal dissection (ESD) from May 2016 to February 2019. Logistic regression analysis was used to screen independent risk factor of pneumonia in patients with early gastric cancer after ESD. Then, based on the selected independent risk factors, we established a nomogram prediction model and verified the accuracy and predictability of the model. Results Hyperlipidemia, antibacterial usage, ICU treatment or not, swallowing disorder, infiltration depth, oral care and indwelling gastric tube were independent risk factors for pneumonia after ESD. Based on the selected seven independent risk factors, we established a nomogram model to predict the risk of pneumonia after ESD. The validation of the model showed that the predicted values and the observed values were basically consistent, indicating that the nomogram prediction model had a good predictive ability. We used the Bootstrap internal verification method to validate the model. The C-index index was 0.937 (95%CI: 0.929-0.945), indicating that the model had good precision and discrimination. Conclusion For patients with early gastric cancer, timely consideration of hyperlipidemia, antibiotic usage, ICU treatment or not, dysphagia, depth of infiltration, oral care and indwelling gastric tube could improve the diagnostic efficacy for the incidence of pneumonia after ESD.

Sentinel node (SN) detection and biopsy are important for timely diagnosis and treatment of cancer. Endoscopic submucosal dissection technique is clinically used to treat early gastric cancer, but there is not accurate intuitive judgment of lymph metastasis so as to increase the risk of missing the timing of treatment. Malignant gastric tumor contains lymphatic basin with the sentinel lymph node. Sentinel lymph node can be obtained to guide the surgery by lymphatic basin dissection. So, laparoscopic local gastrectomy combined with lymphatic basin dissection is a recommendable operation, and it can not only achieve the therapeutic effect of endoscopic submucosal dissection but also avoid the risk of lymph node micrometastasis. In this paper, the related difficulties and problems of this technology are summarized for clinical reference.

Objective: To analyze the long-term Therapeutic effect of endoscopic submucosal dissection (ESD) on the treatment of early gastric cancer (EGC). Methods: We retrospectively reviewed EGC patients who underwent ESD at Cancer Hospital, Chinese Academy of Medical Sciences (CAMS), from January 2009 December 2016. The incidence rates of local recurrence, synchronous cancer and heterogeneous cancer were analyzed. Kaplan-Meier method was used to analyzed 5-Years recurrence free survival (RFS) and 5-years disease special survival (DSS) of all patients. Results: A total of 255 EGC patients were enrolled in this study, included 175 differentiated early gastric cancer (D-EGC) patients and 80 undifferentiated early gastric cancer (UD-EGC) patients. Among them, 171 patients within the extended indication of ESD while 84 patients beyond the extended of ESD. Among the 255 Patients, the incidence rates of local recurrence, synchronous cancer and heterogeneous cancer were 2.0%,2.0% and 2.4% respectively. The local recurrence rates of D-EGC group and UD-EGC group was 1.7% and 2.5%, respectively, without significant difference (X² =0.176, P=0.675). The incidence rates of synchronous and heterogenous cancer in the D=EGC group were 2.3% and 3.4%, higher than 1.2% and 0 of UD-EGC group, although there was no significant difference (x² =0.306, p=0.580 vs 2.809, p=0.094). There were no significant difference in 5-years RFS (91.3%vs 95.9%, P=0.236) and 5-years DSS (100% vs 98%, P=0.156) between D-EGC group. Conclusion: The long-term outcome of ESD in the treatment of EGS is good. More attention should be paid to the occurrence of local recurrence and heterogenous cancer in EGC cancer in EGC patients undergoing ESD for more than once. [ABSTRACT FROM AUTHOR]

Objective: To explore the diagnostic value of mi R-17-92 cluster in the peripheral blood of early gastric cancer and provide references for the early diagnosis and treatment of gastric cancer. Methods: A total of 125 cases with gastric cancer(stage I, 35 cases,stage II, 28 cases, stage III, 39 cases, stage IV, 23 cases) and 24 cases with precancerous lesion(including intestinal metaplasia and intraepithelial neoplasia)were collected in this experiment. Meanwhile, 65 cases with chronic gastritis was selected as the control group.Real-time quantitative PCR(RT-qPCR) was used to detect the expression of mi R-17-92 cluster in the serum. The receiver operating curve(ROC) and area under the curve(AUC) were used to assess the sensitivity and specificity of the expression of mi R-17-92 cluster in the diagnosis of early gastric cancer. Results:(1)The expression of mi R-17-92 cluster was not significantly different between chronic gastritis and precancerous lesion(P>0.05);(2)The expression of mi R-17-5 p in early gastric cancer and advanced gastric cancer was higher than that of chronic gastritis(P<0.05). The level of mi R-19 a-3 p, mi R-19 b-3 p, mi R-20 a-5 p and mi R-92 a-3 p were significantly lower than those of chronic gastritis and advanced gastric cancer(P<0.05);(3)The AUC of mi R-17-5 p is higher than mi R-19 a-3 p, mi R-19 b-3 p,mi R-20 a-5 p and mi R-92 a-3 p in early gastric cancer;(4)High and low expression of mi R-19 a-3 p, mi R-19 b-3 p, mi R-20 a-5 p, mi R-92 a-3 p is associated with invasive depth of gastric cancer(P<0.05) and low expression of mi R-19 b-3 p was related to clinical stage of gastric cancer(P<0.05);(5)The positive rate of mi R-17-5 p, mi R-19 a-3 p, mi R-19 b-3 p, mi R-20 a-5 p, mi R-92 a-3 p are higher than CEA and CA199 in the diagnosis of early gastric cancer. Conclusion: The diagnostic value of mi R-17-92 cluster in the peripheral blood is better than CEA and CA199 for the early gastric cancer, which may provide a new strategy for the early diagnosis and treatment of gastric cancer. Objective: To establish the specific serum polypeptide spectra and explore specific and sensitive serum markers for diagnosis of early gastric cancer. Methods: Serum samples of 10 early gastric cancer cases and 10 normal cases were enrolled, using protein precipitation to remove out high abundance ratios serum protein. Mass spectrometry detection for peptides repeated three times by liquid chromatography tandem-mass spectrometry technology(LC-MS/MS), and applied Max Quant algorithm of Label free method to quantitate peptides relatively, and analysed the differential polypeptides and the matching protein between the two groups. Results: Three repeated mass spectrograms in EGC group and N group are consistent generally, and the detected polypeptide repeated rates are 87.54%and 85.67% respectively. There were 65 unique serum peptides that were detected in EGC group, matching with 31 proteins, and there were 22 differential unique serum peptides between the EGC group and N group, matching 11 proteins. Biological analysis of the serum different polypeptides matched proteins showed most of these proteins were found in extracelluar domain, and the functional categories mainly involved sequence variant, signal peptide, glycosylation site: N-linked, etc. The signal pathways were mainly enriched in the coagualtion cascade pathway and the complement cascade pathway. Conclusion: Establishing the early gastric cancer serum specificity polypeptide spectra is expected to become serological markers in diagnosis of early gastric cancer. The following experiments will further analyse these differential peptides and screen early gastric cancer signal peptides, after verificating with large sample, then screen out specific and effective serum markers for early gastric cancer. Objective: To evaluate the diagnostic performance of narrow-band imaging combined with magnifying endoscopy (NBI-ME) for diagnosis of early gastric cancer. Methods: Patients (115 cases) with early gastric lesions were enrolled in the study and examined by NBI-ME. Biopsy specimen was taken from suspicious lesions for pathological examination. All patients received endoscopic resection. The diagnostic ability of different methods will be compared. Results: A total of 115 patients in the study, 65 men and 50 women. The final pathology showed 16 cases of low grade intraepithelial neoplasia, high grade intraepithelial neoplasia in 30 cases, 59 cases of differentiated gastric cancer, and 10 cases of undifferentiated gastric cancer. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of NBI-ME for diagnosing early gastric cancer were 98.0 %, 81.3%, 97.0%, 86.7%, 95.7%, respectively. The counterparts of endoscopic biopsy for diagnosing early gastric cancer were 82.8 %, 87.5 %, 97.6 %, 45.2 %, 83.5 %, respectively. The sensitivity and accuracy of NBI-ME were significantly better than that of endoscopic biopsy (P<0.05). Conclusion: NBI-ME has value in diagnosis for early gastric cancer. [ABSTRACT FROM AUTHOR]

Objective: To evaluate the pathological characteristics of endoscopic submucosal dissection (ESD) specimens for early gastric cancer and precancerous lesions, accumulating experience for clinical management and pathological analysis., Methods: A total of 411 cases of early gastric cancer or precancerous lesions underwent ESD. According to the Japanese guidelines for ESD treatment of early gastric cancer and classification of gastric carcinoma, the clinicopathological data, pathologic evaluation, concordance rate of pathological diagnosis between preoperative endoscopic forceps biopsies and their ESD specimens (in 400 cases), as well as the risk factors of non-curative resection of early gastric cancer, were analyzed retrospectively., Results: 23.4% (96/411) of the 411 cases were adenoma/low-grade dysplasia and 76.6% (315/411) were early gastric cancer. The latter included 28.0% (115/411) non-invasive carcinoma/high-grade dysplasia and 48.7% (200/411) invasive carcinoma. The concordance rate of pathological diagnosis between endoscopic forceps biopsies and ESD specimens was 66.0% (264/400), correlating with pathological diagnosis and lesion location ( P < 0.01). The rate of upgraded diagnosis and downgraded diagnosis after ESD was 29.8% (119/400) and 4.2% (17/400), respectively. Among the 315 cases of early gastric cancer, there were 277 cases (87.9%) of differentiated type and 38 cases (12.1%) of undifferentiated type. In the study, 262 cases (83.2%) met with absolute indication, while 53 cases (16.8%) met relative indication. En bloc and curative resection rates were 98.1% and 82.9%, respectively. Risk factors for non-curative resection included a long diameter >20 mm ( OR =3.631, 95% CI : 1.170-11.270, P =0.026), tumor infiltration into submucosa ( OR =69.761, 95% CI : 21.033-231.376, P < 0.001)and undifferentiated tumor histology ( OR =16.950, 95% CI : 4.585-62.664, P < 0.001)., Conclusion: Several subjective and objective factors, such as the limitations of biopsy samples, the characteristics and distribution of the lesions, different pathological understanding, and the endoscopic sampling and observation, can lead to the differences between the preoperative and postoperative pathological diagnosis of ESD. In particular, the pathological upgrade of postoperative diagnosis was more significant and should receive more attention by endoscopists and pathologists. The curative resection rate of early gastric cancer in ESD was high. Non-curative resection was related to the long diameter, the depth of tumor invasion and histological classification. ESD can also be performed in undifferentiated early gastric cancer if meeting the indication criteria. The comprehensive and standardized pathological analysis of ESD specimens is clinically important to evaluate the curative effect of ESD operation and patient outcomes.

Objective: To explore the differential expression of low abundance proteins in peripheral blood of patients with early gastric cancer and to screen tumor markers for the diagnosis of early gastric cancer. Methods: Three patients suffered from early gastric cancer were collected from January 2013 to June 2013, and another three healthy people were collected from healthy examination. They were devided into gastric cancer group and normal control group. The serum low abundance proteins were tested with biotin label-based antibody chips, which could detect 1000 kinds of proteins levels in one-time detection both in patients with early gastric cancer(case group) and in normal control persons(control group). The differential expression profile of low abundance proteins were analyzed by bioinformatics software. Results: Compared with that in the control group, the expressions of 10 kinds of proteins were significantly elevated(P<0.05) and 52 kinds of proteins were obviously lowered in the early gastric cancer group(P<0.05). A series of bioinformatics analysis was carried out on the differential proteins, which were mainly associated with the blood vessel morphogenesis, enzyme linked receptor protein signaling, immune process, positive regulation of signal transduction and apoptosis. Among these factors, VEGI, CD40 L,SMAD7, PLUNC, NTN, LTβR and HEVM could become the serum tumor markers of early gastric cancer. Gastric cancer is one of the common gastrointestinal cancers, and is one of the highest mortality cancers in China.Compared with in developed countries such as Japan or South Korea, most patients with gastric cancer in China have an advanced diagnostic during treatment. The diagnosis of early gastric cancer is less than 10%. Traditional open surgery of gastric cancer is still the primary means of treatment of early gastric cancer. Laparoscopic surgery has an obvious advantage in early gastric cancer treatment.Patients with early gastric cancer have faster recovery and better quality of life after laparoscopic surgery. Endoscopic submucosal dissection appears as a new means of treatment of early gastric cancer in the recent years. In this paper, the traditional open surger,laparoscopic surgery and ESD applied in the treatment of early gastic cancer were reviewed. Objective: To explore the application value of white light image (WLI), endoscopic ultrasonography (EUS) and magnifying endoscopy with narrow band imaging (ME-NBI) in the endoscopic treatment of early gastric cancer (EGC), and to provide basis for decision-making in clinical diagnosis and treatment., Methods: The clinicopathological data of EGC patients who underwent endoscopic submucosal dissection (ESD) at West China Hospital, Sichuan University between December 2013 and October 2020 were included. The accuracy, sensitivity, specificity, positive predictive value and negative predictive value of EGC invasive depth were compared between WLI and EUS. The role of ME-NBI in predicting the differentiation types of EGC was analyzed., Results: A total of 280 patients (291 lesions) were enrolled in the study. Among them, 199 patients (207 lesions) received EUS and 160 patients (168 lesions) received ME-NBI. The overall accuracy of WLI in diagnosing the invasive depth of EGC was 87.0%, significantly higher than that of EUS (46.4%, P <0.001). When WLI was combined with EUS, the diagnostic accuracy (87.4%) was not significantly improved. The overall accuracy of determining the differentiation degree of EGC with ME-NBI was 92.3% (155/168), and the accuracy of determining undifferentiated EGC with ME-NBI was significantly lower than that of differentiated EGC (41.2% vs. 98.0%, P <0.001)., Conclusion: In the evaluation of indications for endoscopic treatment of EGC, WLI showed better performance in predicting the invasive depth of EGC, while EUS demonstrated limited value. Objective: To analyze the long-term therapeutic effect of endoscopic submucosal dissection (ESD) on the treatment of early gastric cancer (EGC). Methods: We retrospectively reviewed EGC patients who underwent ESD at Cancer Hospital, Chinese Academy of Medical Sciences (CAMS), from January 2009 to December 2016. The incidence rates of local recurrence, synchronous cancer and heterogeneous cancer were analyzed. Kaplan-Meier method was used to analyze 5-years recurrence free survival (RFS) and 5-years disease special survival (DSS) of all patients. Results: A total of 255 EGC patients were enrolled in this study, included 175 differentiated early gastric cancer (D-EGC) patients and 80 undifferentiated early gastric cancer (UD-EGC) patients. Among them, 171 patients within the extended indication of ESD while 84 patients beyond the extended indication of ESD. Among the 225 patients, the incidence rates of local recurrence, synchronous cancer and heterogeneous cancer were 2.0%, 2.0% and 2.4%, respectively. The local recurrence rates of D-EGC group and UD-EGC group was 1.7% and 2.5%, respectively, without significant difference ( χ (2)=0.176, P =0.675). The incidence rates of synchronous and heterogenous cancer in the D-EGC group were 2.3% and 3.4%, higher than 1.2% and 0 of UD-EGC group, although there was no significant difference ( χ (2)=0.306, P =0.580 vs χ (2)=2.809, P =0.094). There were no significant differences in 5-years RFS (91.3% vs 95.9%, P =0.236) and 5-years DSS (100% vs 98.6%, P =0.156) between D-EGC group and UD-EGC group. Conclusions: The long-term outcome of ESD in the treatment of EGS is good. More attention should be paid to the occurrence of local recurrence and heterogeneous cancer in EGC patients undergo ESD. These patients still have a good long-tern outcome even undergoing ESD for more than once.

Gastric cancer is the fourth most common cancer in the world and the second leading cause of death in cancer worldwide. In order to reduce the mortality rate of gastric cancer, the problem that needs to be solved at present is to find new specific markers of early gastric cancer and increase the detection rate of early gastric cancer, thus fundamentally solving the problem of high mortality of gastric cancer. Previous studies in our laboratory found that Peroxiredoxin 4 (PRDX4) has the potential for early gastric cancer markers. In this study, the role of PRDX4 in transformed cells was studied by establishing a malignant transformation model and transforming cell overexpression, the experiment proves that PRDX4 is responsible for growth and proliferation by reducing the content of reactive oxygen species (ROS) in transformed cells. In the microenvironment, it promotes the malignant transformation of cells, that is, PRDX4 promotes the development of gastric cancer via eliminating ROS.
