Objective To investigate the preoperative hope level of elderly patients with femoral neck fracture and analyze its influencing factors, so as to provide a practical basis for improving the preoperative hope level of elderly patients with femoral neck fracture and promoting postoperative rehabilitation. Methods A total of 206 cases of elderly patients with femoral neck fractures who were eligible for the surgical treatment were included as the research objects. The general information questionnaire, Herth Hope Index (HHI)Scale, General Self-Efficacy Scale(GSES) were analyzed. Results The total HHI score of 206 elderly patients with femoral neck fractures was (31.73+-9.68) points, and scores of each dimension were: positive attitude towards reality and the future (9.14+-1.10), taking positive actions (11.69+-5.36), maintaining close relationships with others (10.89+-3.22); Among them, 63 cases (30. 6%) were high hope levels, 131 cases (65. 6%) were medium hope levels, and 12 cases (3. 8%) were low hope levels. Background Pneumocystis jirovecii (PJ) can colonize the lungs, and cause pulmonary infections. It is essential to distinguish between PJ colonization and infection in the lung to avoid clinical overtreatment and delayed treatment. Many studies have shown that both the mNGS technique and serum G test can be used to aid the diagnosis of PJ pneumonia, but it is still unclear whether they have the appropriate cut-off value to distinguish PJ colonization from PJ infection. Objective To investigate the values of mNGS of alveolar lavage fluid and serum G test in distinguishing between infection and colonization of PJ and to assess the correlation between them. Methods A retrospective study design was used. Forty inpatients with pulmonary infections were recruited from Hubei No.3 People's Hospital of Jianghan University from September 2018 to May 2022, and divided into PJ infection group (n=21) and colonization group (n=19), according to the prevalence of anti-PJ treatment based on mNGS results of alveolar lavage fluid. Objective To explore the effect of massage based on midnight-noon ebb-flow theory on milk secretion primiparas. Methods A total of 70 primiparas were selected and divided into the control group and the study group, with 35 cases in each. Both groups were given breastfeeding for the first time within 2 hours, the control group was given regular massage, and the study group was given massage based on midnight-noon ebb-flow theory. The milk production, prolactin level, breast tenderness rating and breastfeeding rate were compared between the two groups. Taking the water soluble phenolic-formadehyde risin as the basis, impregnating high silica-glass-fiber-cloth, high silica-glass-fiber-cloth/phenolic resin composites are obtained by high temperature formed after the high temperature curing. The microstructure of composites was characterized by FT-IR and SEM.and curing formation,the thermal stability and mechanical properties were tested by TG and universal testing machine. The influence of the molar ratio of formaldehydes to phenol on the thermal properties of the composites was discussed. The tensile properties and hot metal filtration experiments were carried out for composites with the best thermal properties. The results show that the thermal residual rate of the composites at 900°C increased firstly and then decreased with the increase of formaldehyde/phenol molar ratio. When the molar ratio of formaldehyde to phenol was 1. 25, the thermal residual rate of composites reached the maximum, 77. 38% and its fracture strength was 89. 6 MPa. Objective To investigate the effect of CTGF on the proliferation of cultured RAW264. 7 cells and the differentiation of mature multinuclear osteoclasts induced by RANKL. Methods RAW264. 7 cells were cultured with the intervention of 200ng/ ml CTGF. The cell proliferation rate was detected using (³H)-thymidine (³H-TdR) incorporation method. After the intervention with or without RANKL, TRAP staining was performed to observe TRAP-positive multinuclear cells (MNCs). The expression of carbonic anhydrase II was detected using Western blotting. Results CTGF could significantly promote the proliferation of RAW264. 7 cells. The intervention of 200ng/ml CTGF combined with RANKL could promote RAW264. 7 cell differentiation into mature multinuclear osteoclasts. It also could promote the expression of carbonic anhydrase II in RAW264. 7 cells. Objectives: To observe the therapeutic efficacy of acupuncture and its effects on abdominal circumference, separation distance of rectus abdominis and quality of life in patients with postpartum diastasis recti abdominis on the basis of diastasis recti abdominis exercise., Methods: A total of 87 postpartum women with diastasis recti abdominis were randomly divided into an observation group (44 cases) and a control group (43 cases) . The control group was treated with conventional diastasis recti abdominis rehabilitation exercise, including abdominal breathing training and supine leg lifting training, 3 times a day for 2 weeks. On the basis of the treatment in the control group, the observation group was treated with acupuncture at Zhongwan(CV 12), Qihai(CV 6)and bilateral Shenshu(BL 23), Daimai(GB 26), Daheng(SP 15), Zusanli (ST 36), etc., 30 min each time, once a day for 2 weeks. Before and after treatment, the separation distance of rectus abdominis, low back pain visual analogue scale (VAS) score, abdominal circumference and 36-item short form health survey questionnaire (SF-36) score in the two groups were compared, and the clinical effect was evaluated., Results: After treatment, the separation distance of rectus abdominis, low back pain VAS scores, abdominal circumference of the two groups were lower than those before treatment( P <0.05), and the physiological function, physiological role, pain, mental health, emotional role, social function, energy, general health scores and total scores of SF-36 were higher than those before treatment( P <0.05); the separation distance of rectus abdominis, low back pain VAS score, abdominal circumference of the observation group were lower than those in the control group( P <0.05), the sub-item scores and total score of SF-36 of the observation group were higher than those in the control group( P <0.05).The effective rate of the observation group was 95.5% (42/44), which was higher than 79.1% (34/43) in the control group( P <0.05)., Conclusions: Acupuncture combined with diastasis recti abdominis exercise can effectively relieve the low back pain of postpartum diastasis recti abdominis patients, promote the recovery of recti abdominis function, and improve the quality of life. The clinical effect is superior to diastasis recti abdominis exercise alone.

Objective To explore the effects of microRNA-155-5p (miR-155-5p) on lipopolysaccharide (LPS)-induced neuroinflammatory damage of human SH-SY5Y neuroblastoma cells. Methods SH-SY5Y cells line was overexpressed miR-155-5p or transfected with negative control (miR-155-5p mimic group, mimic-NC group), down-expressed miR-155-5p or transfected with its negative control (miR-155-5p inhibitor group, inhibitor-NC group). The cells with successful transfection in the above groups were treated with LPS for 24 hours. The cells without SH-SY5Y cells transfection and those with LPS treatment were included into control group and LPS group, respectively. The activity of SH-SY5Y cells was detected by MTT assay. The apoptosis rate was detected by flow cytometry. The mRNA levels of tumor necrosis factor α (TNF-α), interleukin 1β (IL-1β), interleukin 6 (IL-6) and interleukin 10 (IL-10), and expression of miR-155-5p were detected by reverse transcription PCR. The levels of cleaved caspase-3 (c-caspase-3), B-cell lymphoma/leukemia-2 (Bcl2), Bcl2-associated X protein (BAX), phosphorylated nuclear factor κB p65/nuclear factor κB p65 (p-NF-κB p65/NF-κB p65) and phosphorylated p38 mitogen-activated protein kinase/p38 mitogen-activated protein kinase (p-p38 MAPK/p38 MAPK) were detected by Western blot analysis. The expression of miR-155-5p in SH-SY5Y cells was regulated by miR-155-5p mimic and miR-155-5p inhibitor. The target relationship between miR-155-5p and suppressor of cytokine signaling 1(SOCS1) was predicted by bioinformatics, which was verified by luciferase assay. SH-SY5Y cells with down-regulation of both miR-155-5p and SOCS1 were constructed (miR-155-5p inhibitor/si-SOCS1 group). The cells activity, apoptosis, mRNA expressions of inflammatory cytokines, expression of SOCS1 protein, ratios of p-NF-κB p65/NF-κB p65 and p-p38 MAPK/p38 MAPK were detected by the above methods. Results Compared with control group, activity of SH-SY5Y cells, expression of Bcl2 protein, level of IL-10 mRNA and expression of SOCS1 protein decreased in LPS group while apoptosis rate, expressions of c-caspase-3 and BAX proteins, and levels of TNF-α, IL-1β and IL-6 mRNA were increased, along with the increased miR-155-5p level and ratios of p-NF-κB p65/NF-κB p65 and p-p38 MAPK/p38 MAPK. The activity of SH-SY5Y cells, expression of Bcl2 protein, level of IL-10 mRNA and expression of SOCS1 protein increased in miR-155-5p inhibitor group, compared with LPS group, whereas decreased miR-155-5p level, apoptosis rate, expressions of c-caspase-3 and BAX proteins, levels of TNF-α, IL-1β and IL-6 mRNA, and expressions of p-NF-κB p65/NF-κB p65 and p-p38 MAPK/p38 MAPK proteins were observed. Compared with LPS group, activity of SH-SY5Y cells, expression of Bcl2 protein, level of IL-10 mRNA and expression of SOCS1 protein were decreased in miR-155-5p mimic group, while miR-155-5p level, apoptosis rate, expressions of c-caspase-3 and BAX proteins, levels of TNF-α, IL-1β and IL-6 mRNA, and ratios of p-NF-κB p65/NF-κB p65 and p-p38 MAPK/p38 MAPK proteins were increased. Targeted relationship was identified between miR-155-5p and SOCS1. Compared with miR-155-5p inhibitor group, cells activity and level of IL-10 mRNA decreased in miR-155-5p inhibitor/si-SOCS1 group, while apoptosis rate, ratios of p-NF-κB p65/NF-κB p65 and p-p38 MAPK/p38 MAPK, and levels of TNF-α, IL-1β and IL-6 mRNA increased. Conclusion Inhibiting miR-155-5p can alleviate neuroinflammatory damage induced by LPS, which may be related to down-regulating SOCS1 level.

The quality control problems of 49 institutional level medical device clinical trials in hospital from 2016 to 2021 were summarized, and the causes of the problems were analyzed from the perspective of all parties involved in the clinical trial. The improvement measures were discussed, which including improving the management system, strengthening the training of researchers and optimizing the selection method, strengthening the responsibility and regulation consciousness of applicants, and formulating the selection basis of applicants and so on. All the above aims to ensure the quality of clinical trials and provide reference for clinical trial managers and implementers.

A thermal desorption electrospray ionization-triple quadruple mass spectrometry (TD-ESI-MS/MS) method was developed for the rapid screening of poppy husk in hot pot sauce and soup. The solid surface or liquid sample was directly touched by a simple metal probe. The analytes collected on the probe were desorbed and ionized using a TD-ESI source, after which the analyte ions were detected by MS/MS in multiple reaction monitoring (MRM) mode. The results were compared with those of colloidal gold card rapid detection and verified by high performance liquid chromatography coupled with triple quadrupole mass spectrometry (HPLC-MS/MS). The instrument gave the best response with the highest sensitivity under the following conditions: thermal desorption temperature, 260 ℃; injection solvent, 0.1% formic acid aqueous solution containing 10 mmol/L formic ammonium-acetonitrile (1:1, v/v); flow rate, 200 μL/h. The limits of detection (LODs) for papaverine, noscapine, and thebaine in five alkaloids were 2 μg/kg, while those for codeine and morphine were 10 μg/kg in hot pot sauce and 5 μg/kg in soup. The sensitivity of this method was significantly superior to that of the colloidal gold card rapid detection. The method was applied to 50 batches of hot pot sauce and soup. Noscapine, papaverine, thebaine, and morphine were detected in a positive sample of chicken soup, which was consistent with the result of the HPLC-MS/MS method. This method without sample preparation and chromatographic separation is fast, green, and environmentally benign, thus being suitable for the rapid qualitative analysis of poppy husk in food.

Several new varients related to the case-control designs have been developed in the recent decades, and this article briefly summarized four new designs: two-stage design, case-specular study, exposure-crossover study and case-case-time-control study. This paper involved principles of study design, requisites for application, advantages and disadvantages on all the studies.

A 61 year-old male patient, plasma fibrinogen level was 2.98 g/L, endoscopic sinus surgery was performed under general anesthesia for polypoid of uncinate process with mycotic maxillary sinusitis. Hemocoagulase were given in pre- and post-operative for treatment. The patient was found postoperative drain blood continuously since 3 days after surgery, when the dose of hemocoagulase reach 26 KU, and fibrinogen determined in Plasma was 0.48 g/L. Coagulation returned to normal and nasal bleeding stopped after discontinuation of the hemocoagulase and supplement with fibrinogen.

Objective: This study investigated the role and mechanism of calcineurin (CaN)-nuclear factor of activated T cells (NFAT) pathway in the myoblast apoptosis induced by cyclic tensile strain., Methods: Myoblasts were cultured using an in vitro-mechanical stimulation model and imposed with tension for different hours with a multi-channel cell stress loading system. Cyclosporine (CsA) was used as CaN inhibitor to clarify the role of CaN in the apoptosis induced by cyclic stress. Hochest 33258 staining and flow cytometry detection were performed to detect the apoptotic cells. Real-time polymerase chain reaction was conducted to detect the mRNA expression of CaN and NFAT. Protein levels of NFAT3 were evaluated by Western blot., Results: The apoptosis rate increased with the extension of loading time. The mRNA expression of the CaN subunits, CnA and CnB, and the protein levels of NFAT3 also increased. When the myoblasts were incubated with CsA, the apoptosis rate decreased, the mRNA expression of CnA and NFAT3 significantly decreased, and the NFAT3 protein expression levels became significantly lower than those of the groups without CsA., Conclusion: Continuous cyclic tensile stress can induce myoblast apoptosis. The CaN-NFAT signaling pathway may be involved in the cyclic stretch-induced apoptosis of myoblasts.

Objective: To investigate the mechanisms of Porphyromonas gingivalis (Pg) infection-mediated enhancement of adhesion between monocytes THP-1 and human umbilical vein endothelial cells (HUVEC) by detecting the effect of erythropoietin producing hepatomocellular receptor interacting protein B2 (Ephrin B2) and its receptors on the adhesion., Methods: PgATCC33277 was cultured in an anaerobic jar, and THP-1 cells were infected with various concentrations of Pg at multiplicity of infection (MOI) of 1:100 for 8 and 24 h, respectively. The expression of Ephrin B2 receptor of THP-1 cells was detected. After removal of the free Pg, THP-1 cells were cocultured with HUVEC (overexpress of EphrinB2 or not) for 24 h to detect the expression of Ephrin B2 of HUVEC cells after additional cultivation for 23 h., Results: The adhesion of THP-1 cells post infection by Pg to HUVEC was enhanced. The mRNA levels of Ephrin B2 receptors, including EphB3 (5.169±0.152, P = 0.005), EphB4 (11.040±1.195, P = 0.001), and EphA4 (4.976± 0.122, P = 0.001) expressed by THP-1, and Ephrin B2 (8.938±0.962, P = 0.008) expressed by HUVEC were significantly elevated 24 h post infection of Pg. Over expression of Ephrin B2 in HUVEC promoted the adhesion of THP-1 to HUVEC., Conclusions: Ephrin B2 and its receptors are involved in Pg infection mediated enhancement of the adhesion of THP-1 to HUVEC cells, suggesting that Ephrin B2 participates in the development of atherosclerosis.

Objective: To explore the best time of intratympanic dexamethasone injection to treat sudden sensorineural hearing loss (SSNHL) as salvage therapy so that to improve the curative efficacy on sudden deafness at the utmost., Method: A total of 192 patients with SSNHL were included in this study, among whom 63 cases received the systemic steroid therapy throughout the study, while the other ones were treated with systemic steroid as initial treatment and were given intratympanic steroid administration as salvage treatment starting at different time point. The salvage treatment started on the 3rd day after the beginning of the initial treatment for 29 cases, on the 7th day for 38 cases, on the 14th day for 43 cases, and 1 month later for 19 cases. All the patients were followed up for 2 months., Result: The recovery rates and total effective rates showed no statistically significant difference between the patients received only systemic steroid therapy and the ones received intratympanic steroid administration on the 3rd, 7th day and 1 month later after the initial treatment. The recovery rate and total effective rate exhibited statistically significant difference between the patients received intratympanic steroid administration since the 14th day after the initial treatment and the ones received only systemic steroid therapy, with the numerical value of P 0. 037 and 0. 034, respectively., Conclusion: (1) As an initial management plan, the curative effects. between the intratympanic steroid administration and the systemic steroid therapy were not significantly different. (2) As a salvage treatment, intratympanic steroid was a better choice for patients who have not completely recover from ISSNHL after failure of initial management with systemic steroid only. (3) The best time point of salvage treatment with intratympanic steroid was about 2 weeks after initial management with systemic steroid.

Objective: To evaluate the effect of long-term fertilization on soil microbial community and soil chemical and physical properties., Methods: Using a high-throughput pyrosequencing technique, we studied microbial community in the 0-300 cm soil samples covering a 20-year field-experiment with different fertilization applications including inorganic fertilizer alone (N 300 kg/hm2, P2O5 150 kg/hm2 and K2O 60 kg/hm2) and inorganic fertilizer combined with straw (same application rate of N and P fertilizer combined with 5.4 t straw)., Results: Actinobacteria and alpha-proteobacteria were the predominant groups in the topsoil (0-20 cm). As the soil depth increased, the relative abundance of actinobacteria decreased whereas that of proteobacteria, especially gamma-proteobacteria and beta-proteobacteria increased and gradually became the dominant groups in the subsoil (20-300 cm). Long-term fertilizing applications significantly affected soil microbial communities throughout the soil profile, and increased the relative abundance of ammonia-oxidizing archaea at 0-40 cm depth. In addition, agriculture management, e. g. irrigation may be an important driving factor for the distribution of ammonia-oxidizing bacteria in soil profile. Total nitrogen and organic carbon contents were the most influential factors on microbial community in the topsoil and in the subsoil, respectively., Conclusion: Long-term fertilizer applications altered soil nutrient availability within the soil profile, which was likely to result in the different microbial community structure between the fertilizer treatments, especially for the subsoil.

Objective: To observe if Porphyromonas gingivalis (Pg) infection could enhance the adhesion of human monocytic cell line (THP-1) to human umbilical vein endothelial cells (HUVEC)., Methods: PgATCC33277 was cultured in anaerobic jar, and THP-1 was infected with various concentrations of PgATCC33277 at multiplicity of infection (MOI) of 1: 100 for 8 and 24 hours, respectively. After removal of the free Pg, THP-1 cells were cocultured with HUVEC for 1 hour to observe the adhesion of THP-1 to HUVEC.HUVEC with adhesive THP-1 cells were co-cultured for additional 23 hours. The medium and cells were separately collected. The expression of related chemotactic cytokine[monocyte chemotactic protein 1(MCP-1) and interleukin 8(IL-8)] and intercellular adhesion molecule-1(ICAM-1) were detected with enzyme-linked immunosorbent assay., Results: The adhesion of THP-1 to HUVEC was enhanced (13.8%-35.2%, P = 0.006) and the expression of ICAM-1 of HUVEC was increased from (132.5 ± 7.7) to (164.9 ± 9.1) ng/L (P = 0.005) after infection for 24 hours by Pg. Both of the secreted MCP-1 and IL-8 elevated after infection of Pg for 24 hours from (183.2 ± 3.1) to (221.0 ± 4.9) ng/L (P = 0.012) and from (587.2 ± 5.1) to (787.2 ± 10.3) ng/L (P = 0.002), respectively., Conclusions: Pg could enhance the adhesion of monocytes to endothelial cells and stimulate the inflammation, suggesting that Pg infection may be one of the risk factors in promoting the development of atherosclerosis.

Objective: To express and purify the recombinant major outer membrane protein (MOMP) of Legionella pneumophila (Lp) as diagnostic antigen, and explore its practical value in the serological diagnosis of Lp infection., Methods: The recombinant plasmid pET-momp was transformed into the E.coli BL21 competent cells. The recombinant MOMP was induced to express, and then analyzed by SDS-PAGE electrophoresis, purified by affinity chromatography. We screened and obtained 58 positive blood serum and 32 negative blood serum using the DRG (Germany, IgG/IgM/IgA) Lp kit. The blood serum samples were detected for IgG, IgM, IgA antibody levels by indirect ELISA that we had established with the purified MOMP as the coating antigen, as well as by R&D (USA, IgG/IgM/IgA) Lp kit. Then using the receiver operating characteristic (ROC) curve, we compared these two methods in the sensitivity, specificity and consistency of the test results, for evaluating the application value of the indirect ELISA of recombinant MOMP., Results: The approximately 45 000 recombinant MOMP was successfully expressed and purified. Compared with the indirect ELISA we established with the R&D Lp kit for detecting Lp antibody IgG, IgM and IgA in blood serum, the sensitivity of the indirect ELISA of recombinant MOMP to IgG was 90.9%, the specificity was 91.7%, the Kappa value was 0.784 (P < 0.05), and the area under the ROC curve was 0.913; the sensitivity to IgM was 91.4% and the specificity was 90.6%, the Kappa value was 0.809 (P < 0.05), and the area under the ROC curve was 0.910; the sensitivity to IgA was 92.1% and the specificity was 88.9%, the Kappa value was 0.793(P < 0.05), and the area under the ROC curve was 0.905., Conclusion: The recombinant MOMP was successfully induced to express and purified. The indirect ELISA we established with the recombinant MOMP protein as a diagnostic antigen showed good specificity, sensitivity and consistency, which laid a foundation for the development of serological diagnosis kit of Legionnaires' disease.

Objective: To explore the association of soy food intake with breast cancer risk., Methods: The epidemiological studies on soy food intake and breast cancer risk published from 1991 to April 2011 were searched from MEDLINE, Pubmed, EMBASE, Sci and CNKI databases. Meta-analysis was used to estimate the combined OR and 95% CI., Results: Twenty eight studies were selected for the meta-analysis, including 23 case-control studies and 5 cohort studies. The overall analysis showed that soy food intake was inversely associated with the risk of breast cancer. The pooled OR was 0.86 (95% CI 0.78 - 0.94) for the combination of both study designs. However, the protective effect was only observed in case-control studies (the pooled OR = 0.86, 95% CI 0.77 - 0.95) but not in cohort studies. The protective effect of soy food intake on breast cancer risk was observed among studies conducted in Asian countries (OR = 0.76, 95% CI 0.67- 0.87), whereas no association was found in studies from Western countries (OR = 1.01, 95% CI 0.93-1.09). The further stratified analysis showed that the protective effect was only found in studies conducted in Asian countries (pooled OR = 0.76, 95% CI 0.65 - 0.89 for 14 case-control studies and OR = 0.81,95% CI 0.70 - 0.92 for 3 cohort studies, respectively). Additionally, the protective effect was only found among postmenopausal women but not among premenopausal women., Conclusion: According to the meta-analysis of studies conducted in Asian countries, soy food intake is inversely associated with the risk of breast cancer among Asian women, especially based on the results of meta-analysis of case-control studies.

Purpose: To analyze the relationship of the pathological and clinical features of patients with ocular adnexal mucosa-associated lymphoid tissue (MALT) type lymphoma, and to provide a suggestion of preoperative diagnosis and treatment of MALT lymphoma., Methods: To analyze the clinical, imaging and histopathologic features of 20 cases (25 eyes) of MALT lymphoma retrospectively in the department of ophthalmology., Results: Among the 20 cases (25 eyes), 18 cases (21 eyes)were located in the orbit, and 2 cases (4 eyes) were at conjunctiva. MALT lymphoma had typical characters in CT, MRI and B scan imaging, Histopathological examination showed that all of the 20 cases (25 eyes) were B cell lymphoma. Monoclonal immunophenotype of 21 eyes were positive, 15 eyes kappa positive, 6 eyes lambda positive. 15 eyes CD45RA positive, 18 CD45RO low positive, 23 eyes CD20 positive and 24 eyes CD3 negative., Conclusion: Patients with primary ocular adnexal MALT lymphoma had a similar pathological morphology with other locus. Through imaging we could get a preoperative diagnosis.
