Objective: To explore the difference of peripheral blood mononuclear cells (PBMC) transcripts between atopic dermatitis (AD) and healthy controls, and to screen and preliminarily validate potential biomarkers of AD. Methods: From January 2021 to May 2022, blood samples from 9 AD patients and 10 healthy controls were collected from the Dermatology and Cosmetic Center of the Third Affiliated Hospital of Chongqing Medical University, ribonucleic acid-sequencing (RNA-seq) was used to determine the transcriptome and relative expression of PBMC, the differentially expressed genes (DEGs) were analyzed by gene ontology (GO), Kyoto Encyclopedia of Genes and Genomes (KEGG) and protein-protein interaction networks (PPI) analysis, and the potential biomarkers were identified by quantitative real-time PCR (qRT-PCR). Results: The age of patients in the AD group [ M ( Q 1 , Q 3 )] was 26.50 (22.75, 30.50) years old, and the course of disease [ M ( Q 1 , Q 3 )] was 15 (10, 20) years,and the age of the healthy control group [ M ( Q 1 , Q 3 )] was 37.00 (27.75, 40.25) years old. Compared with healthy controls, 1 044 DEGs were detected in PBMC samples in AD group, including 668 up-regulated genes and 376 down-regulated genes. Differential variable splicing (AS) showed that mutually exclusive exons (46.74%) and skipped exon (31.01%) accounted for a large proportion. GO and KEGG enrichment analysis revealed that AD is closely linked to DEGs implicated in the inflammatory response and cytokine interaction and signal pathway. Comprehensive enrichment analysis and PPI analysis selected the expression of 8 candidate genes (CCL4, CCR3, CXCR5, NFKBIA, CXCL1, IL-1B, CCL20, LY96), which was confirmed by qRT-PCR and were consistent with that of RNA-seq. Conclusions: CCL4, CCR3, CXCR5, NFKBIA, CXCL1, IL-1B, CCL20 and LY96 might be potential biomarkers of AD, participating in the occurrence and development of AD.

Objectives: To explore the effects of somatostatin on the levels of gastrointestinal hormones and clinical outcomes in critically ill infants after gastrointestinal surgery., Methods: Using a random number table method, critically ill infants after gastrointestinal surgery who were admitted to the Intensive Care Unit of Xuzhou Children's Hospital from June 2019 to June 2021 were randomly divided into an observation group (29 cases) and a control group (30 cases). The control group received routine treatment such as anti-infection and hemostasis after surgery, while the observation group received somatostatin in addition to the routine treatment [3.5 μg/(kg·h) infusion for 7 days]. The levels of serum gastrin (GAS), motilin (MTL), insulin, and glucagon-like peptide-1 (GLP-1) before surgery, on the 3rd day after surgery, and on the 7th day after surgery were compared between the two groups. The recovery progress and incidence of complications after surgery were also compared between the two groups., Results: There was no significant difference in the levels of serum GAS, MTL, insulin, and GLP-1 between the two groups before surgery ( P >0.05). On the 3rd and 7th day after surgery, the levels of serum GAS, MTL, insulin, and GLP-1 in the observation group were higher than those in the control group ( P <0.05). In the observation group, the levels of GAS, MTL, insulin, and GLP-1 on the 7th day after surgery were higher than those before surgery and on the 3rd day after surgery ( P <0.05), and the levels on the 3rd day after surgery were higher than those before surgery ( P <0.05). There was no significant difference in the levels of serum GAS, MTL, and insulin before surgery, on the 3rd day after surgery, and on the 7th day after surgery in the control group ( P >0.05). The level of GLP-1 on the 7th day after surgery was higher than that before surgery and on the 3rd day after surgery ( P <0.05), and the level on the 3rd day after surgery was higher than that before surgery ( P <0.05) in the control group. The observation group had shorter first time of anal exhaust, recovery time of bowel sounds, and first time of defecation after surgery compared to the control group ( P <0.05). The incidence of complications after surgery in the observation group was lower than that in the control group (10% vs 33%, P <0.05)., Conclusions: Somatostatin can increase the levels of serum GAS, MTL, insulin, and GLP-1 in critically ill infants after gastrointestinal surgery, promote the recovery of gastrointestinal function, and reduce the incidence of postoperative complications.

Objectives: To study the effect of early use of sodium valproate on neuroinflammation after traumatic brain injury (TBI)., Methods: A total of 45 children who visited in Xuzhou Children's Hospital Affiliated to Xuzhou Medical University from August 2021 to August 2022 were enrolled in this prospective study, among whom 15 healthy children served as the healthy control group, and 30 children with TBI were divided into a sodium valproate treatment group and a conventional treatment group using a random number table ( n =15 each). The children in the sodium valproate treatment group were given sodium valproate in addition to conventional treatment, and those in the conventional group were given an equal volume of 5% glucose solution in addition to conventional treatment. The serum concentrations of nucleotide-binding oligomerization domain-like receptor protein 3(NLRP3), high-mobility group box 1 (HMGB1), tumor necrosis factor-α (TNF-α), and interleukin-1β (IL-1β) were measured in the healthy control group on the day of physical examination and in the children with TBI on days 1, 3, and 5 after admission. Glasgow Outcome Scale-Extended (GOS-E) score was evaluated for the children with TBI 2 months after discharge., Results: Compared with the healthy control group, the children with TBI had significantly higher serum concentrations of NLRP3, HMGB1, TNF-α, and IL-1β on day 1 after admission ( P <0.017). The concentration of NLRP3 on day 5 after admission was significantly higher than that on days 1 and 3 after admission in the children with TBI ( P <0.017). On days 3 and 5 after admission, the sodium valproate treatment group had a significantly lower concentration of NLRP3 than the conventional treatment group ( P <0.05). For the conventional treatment group, there was no significant difference in the concentration of HMGB1 on days 1, 3, and 5 after admission ( P >0.017), while for the sodium valproate treatment group, the concentration of HMGB1 on day 5 after admission was significantly lower than that on days 1 and 3 after admission ( P <0.017). On day 5 after admission, the sodium valproate treatment group had a significantly lower concentration of HMGB1 than the conventional treatment group ( P <0.05). For the children with TBI, the concentration of TNF-α on day 1 after admission was significantly lower than that on days 3 and 5 after admission ( P <0.017). On days 3 and 5 after admission, the sodium valproate treatment group had a significantly lower concentration of TNF-α than the conventional treatment group ( P <0.05). The concentration of IL-1β on day 3 after admission was significantly lower than that on days 1 and 5 after admission ( P <0.017) in the children with TBI. On days 3 and 5 after admission, the sodium valproate treatment group had a significantly lower concentration of IL-1β than the conventional treatment group ( P <0.05). The GOS-E score was significantly higher in the sodium valproate treatment group than that in the conventional treatment group 2 months after discharge ( P <0.05)., Conclusions: Early use of sodium valproate can reduce the release of neuroinflammatory factors and improve the prognosis of children with TBI.

Objective: To investigate the role and possible pathogenesis of high mobility group protein B1 (HMGB1) in lipopolysaccharide (LPS)-induced acute lung injury/acute respiratory distress syndrome (ALI/ARDS)., Methods: (1) In vivo, 24 SPFC57BL/6 male mice were randomly divided into normal control group, ALI/ARDS model group, ethyl pyruvate (EP) treatment group and EP control group, with 6 mice in each group. The ALI/ARDS model was established by intraperitoneal injection of 20 mg/kg LPS. Mice in normal control group and EP control group were intraperitoneally injected with the same amount of sterile normal saline. Then, mice in the EP treatment group and EP control group were intraperitoneally injected with 40 mg/kg HMGB1 inhibitor EP. After 6 hours, the mice were sacrificed and lung tissues were collected. The expressions of heparan sulfate (HS), syndecans-1 (SDC-1), heparanase (HPA) and matrix metalloproteinases-9 (MMP-9) in lung tissues were detected by immunofluorescence technique. Orbital blood of mice was collected and serum was extracted to detect the content of HMGB1 by enzyme linked immunosorbent assay (ELISA). (2) In vitro, human umbilical vein endothelial cells (HUVECs) were randomly divided into 6 groups: normal control group, HUVECs damage group (treated with 1 mg/L LPS for 6 hours), HMGB1 group (treated with 1 μmol/L recombinant HMGB1 for 6 hours), HMGB1+EP group (treated with recombinant HMGB1 for 1 hour and then added 1 μmol/L EP for 6 hours), LPS+EP group (treated with LPS for 1 hour and then added 1 μmol/L EP for 6 hours), EP group (treated with 1 μmol/L EP for 6 hours). The expressions of HS, SDC-1, HPA and MMP-9 in endothelial cells were detected by immunofluorescence technique., Results: (1) In vivo, light microscopy showed that the alveolar space was thickened after LPS stimulation, and there were a large number of inflammatory cells infiltrating in the alveolar space. Compared with ALI/ARDS model group, the expressions of HS and SDC-1 in lung tissue of EP treatment group were significantly increased [HS (fluorescence intensity): 0.80±0.20 vs. 0.53±0.02, SDC-1 (fluorescence intensity): 0.72±0.02 vs. 0.51±0.01, both P < 0.05], and the expressions of HPA and MMP-9 were significantly decreased [HPA (fluorescence intensity): 2.36±0.05 vs. 3.00±0.04, MMP-9 (fluorescence intensity): 2.55±0.13 vs. 3.26±0.05, both P < 0.05]; there were no significant changes of the above indexes in EP control group. Compared with ALI/ARDS model group, the content of serum HMGB1 in EP treatment group decreased significantly (μg/L: 131.88±16.67 vs. 341.13±22.47, P < 0.05); there was no significant change in the EP control group. (2) In vitro, compared with HMGB1 group, the expressions of HS and SDC-1 in HMGB1+EP group were significantly higher [HS (fluorescence intensity): 0.83±0.07 vs. 0.56±0.03, SDC-1 (fluorescence intensity): 0.80±0.01 vs. 0.61±0.01, both P < 0.05], and the expressions of HPA and MMP-9 were significantly lower [HPA (fluorescence intensity): 1.30±0.02 vs. 2.29±0.05, MMP-9 (fluorescence intensity): 1.55±0.04 vs. 2.50±0.06, both P < 0.05]; the expression of HS, SDC-1, HPA and MMP-9 had no significant changes in EP group., Conclusions: HMGB1 participates in LPS-induced injury of endothelial cell glycocalyx, leading to increased lung permeability, and inhibition of HMGB1 can alleviate lung injury.

Objectives: To study the effect of somatostatin on postoperative gastrointestinal function and stress level in children with acute abdomen., Methods: A total of 102 children with acute abdomen who underwent surgery in Xuzhou Children's Hospital from August 2019 to June 2021 were enrolled as subjects and were randomly divided into an observation group and a control group, with 51 children in each group. The children in the control group were given conventional treatment such as hemostasis and anti-infective therapy after surgery, and those in the observation group were given somatostatin in addition to conventional treatment. Peripheral blood samples were collected from both groups before surgery and on days 1 and 5 after surgery. The two groups were compared in terms of the serum levels of endothelin-1 (ET-1), adrenocorticotropic hormone (ACTH), cortisol, gastrin, and motilin, postoperative recovery, and the incidence rate of complications., Results: There was no significant difference in the serum levels of ET-1, ACTH, cortisol, gastrin, and motilin between the two groups before surgery ( P >0.05). Compared with the control group, the observation group had significantly lower serum levels of ET-1, ACTH, and cortisol on days 1 and 5 after surgery ( P <0.05) and significantly higher levels of motilin and gastrin on day 5 after surgery ( P <0.05). Compared with the control group, the observation group had significantly shorter time to first passage of flatus, first bowel sounds, and first defecation after surgery, as well as a significantly shorter length of hospital stay ( P <0.05). The incidence rate of complications in the observation group was significantly lower than that in the control group (6% vs 24%, P <0.05)., Conclusions: In children with acute abdomen, somatostatin can significantly reduce postoperative stress response, improve gastrointestinal function, and reduce the incidence rate of complications, thereby helping to achieve a good prognosis.

8-hydroxy-2'-deoxyguanosine (8-OHdG) is a sensitive and stable biomarker for evaluating DNA oxidative damage. A rapid and sensitive colloidal gold immunochromatographic strip was developed for 8-OHdG detection by a competitive method. The sample pad (glass cellulose film), bonding pad (glass cellulose film), nitrocellulose film and absorbent pad were pasted on the polyvinyl chloride (PVC) base plate to construct the test strip. Colloidal gold (AuNPs) was prepared by the reduction of chloroauric acid with sodium citrate. 8-OHdG antibody (Ab) was coated on the outer layer of AuNPs to form Ab@AuNPs as a probe. Bovine serum albumin (BSA) and 8-OHdG were conjugated with carbodiimide hydrochloride to prepare an artificial antigen, which was used as the coating antigen of detection line. Goat anti mouse polyclonal antibody IgG was used as the coating antibody of control line. The experimental parameters were optimized including the type of nitrocellulose membrane, the formula of loading solution, and the spraying amount of gold labeled antibody. The results showed that the appropriate nitrocellulose membrane was CN 95. The optimal loading solution included BSA (1%), Tween-20 (3%), sucrose (3%) and NaCl (0.9%). The optimal spraying amount of gold labeled antibody was 4 μL. 8-OHdG can be detected by the strip under visible light, and the level of 8-OHdG in urine can be preliminarily determined by comparing the color intensity of T line and C line. The 8-OHdG concentration in urine was further calculated by the gray value of T line and the threshold of detection was 2.55 μg/L. This colloidal gold immunochromatographic strip is simple, rapid and specific for detecting 8-OHdG in human urine to preliminarily evaluate the human status.

Objectives: The integrated model of prenatal diagnosis and postnatal treatment for congenital heart disease (CHD) leads to an increasing number of operation in infants. This study aims to reveal the risk factors for postoperative early mortality and delayed recovery in infants less than 3 months old, who underwent surgical treatment for CHD in the Department of Cardiovascular Surgery, Second Xiangya Hospital, Central South University during the past 5 years., Methods: Clinical variables were collected via medical records. Delayed recovery was defined as the time of postoperative intubation, or cardiac intensive cure unit (CICU) stay, or hospital stay longer than its third quartile. Risk factors for early postoperative prognosis and the odds ratio (OR) were analyzed with logistic regression analysis., Results: A total of 511 infants underwent surgical treatment for CHD from January 2016 to June 2020 were retrospectively reviewed, including 217 (42.5%) infants with complex CHD. The median age was 60 days (3 hours-90 days); and median weight was 4.5 (1.7- 8.4 kg). There were 26 postoperative mortalities, making the incidence at 5.1%, including 5 (5/294, 0.7%) mortalities in patients with uncomplicated CHD, and 21 (9.6%) mortalities in patients with complex CHD. Based on multivariable analysis, risk factors for postoperative mortality were diagnosis of complex CHD (OR=5.53, P <0.001), weight under 4.0 kg (OR=9.86, P <0.001), preoperative symptoms (OR=3.17, P =0.012), and emergency operation (OR=11.66, P <0.001). The median time for postoperative intubation, CICU stay, and hospital stay were 21.0 (0.3-979.0) hours, 3.0 (0.5-91.0) days, and 11.5 (3.0-105.0) days, respectively. A total of 177 (34.6%) infants delayed recover, with risk factors including diagnosis of complex CHD (OR=3.41, P =0.001), weight under 4.0 kg (OR=4.55, P <0.001), and preoperative symptoms (OR=3.91, P <0.001)., Conclusions: Surgical treatment for infants (<3 months) with CHD is still a challenge, particularly for infants with complex CHD and weight under 4.0 kg. We can improve the prognosis of CHD treatment in infants by establishing the integrated model of prenatal diagnosis and postnatal treatment to choose the most suitable time window, avoid symptoms before surgery, and reduce emergency operation.

Objectives: Due to the lack of large-sized pulmonary valved conduit products in clinical practice, hand-sewn expanded polytetrafluoroethylene (ePTFE) valved conduit has been used for right ventricular outflow tract (RVOT) reconstruction in many heart centers around the world. This study aims to summarize the early results of the ePTFE valved conduit and the sewing technology of the conduit in combination with the latest progress, and to provide a reference for the application of ePTFE valved conduit., Methods: A total of 21 patients using ePTFE valved conduit for RVOT reconstruction in the Second Xiangya Hospital, Central South University from October 2018 to October 2020 were prospectively enrolled in this study. The age at the implantation of the conduit was 4.3 to 43.8 (median 15.1) years old, with weight of (38.9±4.1) kg. In this cohort, 14 patients underwent re-reconstruction of RVOT, including 12 patients with pulmonary regurgitation at 6.3 to 31.0 (median 13.8) years after tetralogy of Fallot (TOF) repair, and 2 patients with failed bovine jugular vein conduit (BJVC). Seven patients underwent Ross operations. Among them, 3 were for aortic valve stenosis, 2 were for aortic regurgitation, and 2 were for both stenosis and regurgitation. The ePTFE valved conduits were standard hand-sewn during the surgery. The 3 leaflets were equal in size with arc-shaped lower edge of the valve sinus. The free edge of the valve leaflets was straight with the length of about 1 mm longer than the diameter. The height of the valve sinus was 4/5 of the diameter. The junction of the valve leaflet was 3/4 of the height of the sinus. The designed leaflets were then continuous non-penetrating sutured into the inner surface of Gore-Tex vessel to make a valved conduit. Valved conduits with diameter of 18, 20, and 22 mm were used in 2, 9, and 10 cases, respectively. The surgical results, postoperative recovery time, and serious complications were summarized, and the changes of postoperative cardiac function status and hemodynamic status of the conduits were investigated., Results: During the implantation of ePTFE valved conduit for RVOT reconstruction, 2 patients underwent mechanical mitral valve replacement with Ross operation, 2 patients with pulmonary regurgitation with repaired TOF underwent left and right pulmonary artery angioplasty, and 1 patient with failed BJVC underwent tricuspid valvuloplasty. The cardiopulmonary bypassing time for patients underwent re-reconstruction of RVOT was (130.9±16.9) min, with aorta clamping for 1 patient to repair the residual defect of the ventricular septum. The cardiopulmonary bypassing and aorta clamping time for Ross operation were (242.7±20.6) min and (145.6±10.5) min, respectively. The duration of postoperative ventilator assistance, intensive care unit stay, and hospital stay were 3.5 h to 7.7 d (median 17.1 h),11.2 h to 29.5 d (median 1.9 d), and 6.0 to 56.0 (median 13.0) d, respectively. All patients survived after discharge from hospital. The follow-up rate after discharge was 100% with median time at 15.0 (13.0 to 39.0) months. No death happened during the follow-up. One patient underwent stent implantation due to right coronary stenosis 2 months after Ross operation. One patient underwent balloon dilation due to right pulmonary artery ostium stenosis 1 year after re-reconstruction of RVOT. The cardiac function of all patients recovered to NYHA class I 6 months after operation. The peak pressure gradient across the valve measured by transthoracic echocardiography before discharge was (9.4±2.6) mmHg (1 mmHg=0.133 kPa), and (18.3±6.1) mmHg at the last follow-up. There was no significant increase in the gradient during the follow-up ( P =0.134). No patient suffered from mild or more pulmonary regurgitation., Conclusions: Hand-sewn ePTFE valved conduit is feasible for RVOT reconstruction. It is a promising material for RVOT reconstruction which can effectively meet clinical need. In our experience, the ePTFE valved conduit is simple to manufacture with satisfactory early outcomes.In the application of ePTFE valved conduit, attention should be paid to implantation indications and postoperative anticoagulation management, especially to the preparation details of the valved conduit, to obtain better function and durability of the conduit after implantation.

Objective: To explore the prognosis effect of the expression of long-chain non-coding RNA (lncRNA) MBNL1-AS1 on acute myeloid leukemia (AML) patients. Methods: One hundred and twenty-five AML patients of the Cancer Genome Atlas (TCGA) from November 2001 to March 2010 were involved, including 70 patients who received chemotherapy only and other 55 patients treated with allogeneic hematopoietic stem cell transplantation (allo-HSCT) in addition to chemotherapy. According to the median expression of lncRNA MBNL1-AS1, patients of chemotherapy group were divided into high expression sub-group( n =35) and low expression sub-group ( n =35), and patients of allo-HSCT group were also divided into high expression sub-group ( n =28) and low expression sub-group ( n =27) for prognosis analysis. Clinical characteristics at diagnosis, including peripheral white blood cell counts (WBC), blast percentages in peripheral blood and bone marrow (BM), French-American-British (FAB) subtypes and the frequencies of common genetic mutations in AML were described. The event-free survival (EFS) rate and overall survival (OS) rate of patients in different groups were analyzed, and the influence of the clinical characteristics of patients on the prognosis of AML was analyzed by COX multivariate analysis. Results: In the chemotherapy group, patients with low lncRNA-MBNL1-AS1 expression had significantly lower EFS and OS (60.0%, 8.6%) than patients with high lncRNA-MBNL1-AS1 expression (68.6%, 34.3%) ( χ²= 7.817, 10.880, all P< 0.01). However, in the alloHSCT group, no significant differences were observed in EFS and OS of patients between high and low expression groups of lncRNA-MBNL1-AS1 (all P >0.05). COX multivariate analysis confirmed that age≥60 years old (EFS: HR (95% CI ): 6.934 (1.918-25.075), P =0.003;OS: HR (95% CI ): 4.119 (1.812-9.364), P =0.001), and low expression of lncRNA MBNL1-AS1 (EFS: HR (95% CI ): 0.354 (0.126-0.941), P =0.038; OS: HR (95% CI ): 0.424 (0.231-0.778), P =0.006)were independent risk factors for EFS and OS in the chemotherapy group. Conclusion: The long-chain non-coding RNA MBNL1-AS1 is related to the prognosis of AML, and its low expression is an independent poor prognostic factor in AML patients.

Based on the seasonal changes in the nitrogen and phosphorus concentrations in Hanfeng Lake from March 2017 to February 2018, the nutrient limitation status was evaluated by the stoichiometric molar ratio of nitrogen and phosphorus. The results showed that the average concentrations of TN, DN, and NO 3 - -N were 1.60, 1.25, and 0.91 mg·L -1 in Hanfeng Lake, respectively. The seasonal changes of those indicators were similar, showing the highest concentration in winter and lowest in summer. NO 3 - -N accounted for TN significantly in the water body, and the concentrations of NH 4 + -N and NO 2 - -N remained at low levels and changed steadily. The average concentrations of TP, DP, and PO 4 3- -P were 0.13, 0.09, and 0.06 mg·L -1 , respectively. The changes in the concentrations of TP and DP were similar, showing a trend of increasing in spring and summer, and then decreasing in autumn and winter, while the PO 4 3- -P concentration showed the trend of fluctuated decrease. TN/TP varied from 11.07 to 56.02, with an average value of 29.23. TN/TP changed seasonally, with the highest value occurring in winter and the lowest value in summer. The conditions of the water body were conducive to growth and reproduction of algae for most of the time during sampling months. The water body was occasionally nitrogen limited and rarely phosphorus limited. The seasonal variation in TN/TP ratio was affected by several factors such as rainfall runoff, fertilizer use, sewage discharge, and aquatic biological activities. Further, protection strategies were proposed for the improvement of the water body in terms of present water quality characteristics in Hanfeng Lake.

The hydrograph of Hanfeng Lake, which is the largest pre-dam of theThree Gorges Reservoir, varied between the fluvial stage and lake stage after the lake was officially operated, resulting in large shifts in the aquatic biogeochemical processes. To explore the spatial-temporal distribution of nitrogen and phosphorus concentrations and identify their influencing factors in Hanfeng Lake, seven sampling sites were set up to monitor the changes of nutrients and other water indicators at different water depths monthly from January to December 2018. The results showed that completely vertical mixing across water profiles was observed. The nutrient concentrations were not significantly different between the top, middle, and bottom water depths ( P >0.05). Total nitrogen concentration decreased from January to September but gradually increased from October to December, with a monthly average concentration of 1.52 mg·L -1 . NO 2 - -N concentration decreased in the first four months, increased sharply from May to June, and decreased from July to December with a monthly average concentration of 0.05 mg·L -1 . NO 3 - -N concentration gradually decreased from January to June, and gradually increased from July to December. NH 4 + -N concentration was the highest in July, with a concentration of 0.44 mg·L -1 , and the change in other months was not notable, with a monthly average concentration of 0.09 mg·L -1 . The concentrations of total phosphorus (TP), dissolved phosphorus, and soluble reactive phosphorus showed insignificant changes in trends throughout the year, with monthly average concentrations of 0.17 mg·L -1 , 0.11 mg·L -1 , and 0.05 mg·L -1 , respectively. The phosphate concentration was mainly sourced from the upstream Nan River and Taoxi River, and gradually decreased from upper Zhendong to the downstream regulating dam. Of these nutrients, TP was the key factor in the growth of algae in Hanfeng Lake.

Objective: To investigate the causes of misdiagnosis of suprasellar arachnoid cysts, analyze its characteristics and put forward the diagnostic basis and differential points. Methods: The clinical data fo 97 cases of suprasellar arachnoid cysts diagnosed and treated in the neurosurgery department of Beijing Tiantan Hospital and Hebei General Hospital from March 2015 to March 2019 were analyzed retrospectively. All patients underwent CT and MRI scans with obstructive hydrocephalus. 13 cases were misdiagnosed, including 7 males and 6 females. First visit age 1-31 years old, with an average age of 6.3 years. There were 10 patients younger than 6 years old. The remaining 15-year-old patients, 31-year-old patients and 26-year-old patients each have one case. 11 cases were misdiagnosed as obstructive hydrocephalus, 2 cases as cystic craniopharyngioma. Results: 13 cases were misdiagnosed and mistreated, 11 cases were treated with intraventricular and abdominal shunt, 9 cases were treated with neuroendoscopy and recovered well. One cases of intracranial hematomas underwent craniotomy again, the hematomas were removed again and the bone slise were decompressed. One case had fissured stable after shunt. There were no operative deaths and no complications in this group. After endoscopic reoperation, CT and/or MRI scans showed that the ventricle narrowed in varying degrees, some of them returned to normal size and the flow of cerebrospinal fluid (cerebrospinal fluid) was unobstruct at the end of magnetic resonance cerebrospinal fluid angiography (MRI) fistula after endoscopic reoperation. Conclusions: The incidence of suprasellar arachnoid cysts is low, it is rare in clinic and it is easy to misdiagnose and mistreate. At present, it is recognized that the best treatment methods are partial resection of endoscope cyst wall, cyst ventricle fistula and third ventricle floor fisthla.

Objective: To assess the clinical significance of transient elastography (Fibroscan) in detection of clonorchiasis, so as to provide new insights into the assessment of therapeutic efficacy of deworming., Methods: The liver stiffness measurement (LSM) values were measured in parasitologically diagnosed clonorchiasis patients using FibroScan before and after deworming, and the patients'age, gender, body mass index (BMI), duration of raw fish consumption and total amount of raw fish consumption were collected for correlation analyses., Results: The clonorchiasis patients'age, gender, BMI, duration of raw fish consumption and total amount of raw fish consumption had no associations with pre-treatment LSM values ( r / r s = 0.189, 0.073, 0.180; 0.071, -0.098, 0.033; 0.166, 0.309, 0.172; 0.235, 0.247, 0.209; 0.164, 0.277, 0.088; all P values > 0.05). There was a significant difference in the LSM values from the seventh, eighth and ninth intercostal space prior to deworming ( F = 3.259, P < 0.05), and no significant difference was detected after deworming ( F = 0.851, P > 0.05). The LSM values from the seventh, eighth and ninth intercostal space were significantly lower pre-deworming than post-deworming ( t = 6.724, 5.603, 2.884; all P values < 0.05)., Conclusions: FibroScan is feasible to assess the therapuetic efficacy of deworming in patients with clonorchiasis; however, measurement at various sites affects the LSM value.

Retinitis pigmentosa (RP) is a genetically heterogeneous group of hereditary retinal disorders characterized by photoreceptor cell death, associated with night blindness, vision loss, progressive peripheral visual field loss and abnormalities in the electroretinogram. A number of gene defects have so far been associated with RP, which cause a progressive loss of rod photoreceptor function, followed by cone photoreceptor dysfunction and eventually complete blindness. The rate of blindness related to RP is high. At present there is no effective therapeutic strategy for RP. In recent years, with the progress of molecular biology technique, many new therapeutic approaches have become promising. This article summarizes the pathogenesis of RP and gives a brief overview of related research progress of RP therapeutic strategies. (Chin J Ophthalmol, 2017, 53: 148-153) .

Objective: To analyze the differences of coagulation parameters between patients with advanced schistosomiasis cirrhosis and hepatitis B cirrhosis under different degrees of liver function, so as to provide the evidence for further guidance of judging clinical condition and prognosis., Methods: Sixty-three patients with advanced schistosomiasis cirrhosis and eighty patients with hepatitis B induced cirrhosis hospitalized in Jingzhou Central Hospital from January 2014 to June 2016 were served as an advanced schistosomiasis cirrhosis group and a hepatitis B cirrhosis group, respectively, and ninety-six gastropathy patients excluded from other diseases that might affect coagulation in the same period were served as a control group. The levels of PT, INR, Fib, TT, APTT, and PLT of the patients among the above 3 groups and the cirrhosis patients with different Child-Pugh classes were detected and compared., Results: The differences of the levels of PT, INR, Fib, TT, APTT, and PLT among the 3 groups were statistically significant ( F = 84.512, 81.672, 37.612, 104.475, 52.497, 102.233; all P ＜0.05). The further analysis showed that PT, INR, TT, and APTT of both the advanced schistosomiasis cirrhosis and hepatitis B induced cirrhosis groups were longer than those of the control group, and the PLT levels of both the former 2 groups were lower than that of the control group (all P < 0.05). Compared with the advanced schistosomiasis cirrhosis group, PT, INR, TT, and APTT of the hepatitis B induced cirrhosis group were longer, and the levels of Fib and PLT were lower (all P < 0.05). Among those with the liver function of Grade A, the reduced degree of PLT in the patients with hepatitis B induced cirrhosis was more serious, while for those with the liver function of Grade B and C, the difference between the patients with advanced schistosomiasis cirrhosis and hepatitis B induced cirrhosis was not statistically significant (both P > 0.05). Under all the grades of liver function, TT and APTT of the hepatitis B induced cirrhosis group was longer than those of the advanced schistosomiasis cirrhosis group, and the Fib level of the former was lower than that of the latter. Among those with the liver function of Grade A and B, PT and INR of the hepatitis B induced cirrhosis group were longer than those of the advanced schistosomiasis cirrhosis group; while for those with the liver function of Grade C, the difference between the two groups was not statistically significant (all P > 0.05)., Conclusions: There exist differences of the damage degrees of coagulation function between the patients with advanced schistosomiasis cirrhosis and hepatitis B cirrhosis. When the damage degree of the liver function is lighter, the coagulation function in the patients with hepatitis B cirrhosis decreases more significantly; when the damage degree becomes more severe, APPT in the patients with hepatitis B cirrhosis extends more significantly, while the differences of PLT and PT between them are little.

Objective: To investigate the characteristics and outcomes of necrotizing enterocolitis (NEC) in Chinese term infants population., Methods: A national neonatal necrotizing enterocolitis network was established. Neonates as having necrotizing enterocolitis with gestation age ≥37 weeks were identified if they met the accepted diagnostic criterion during the study period from Jan 1(st) 2011 to Dec 31(st) 2011. The data of maternal and neonates' characteristics, the comorbidities, the clinical interventions prior to NEC, the clinical courses and radiology results, the medical and surgical treatment and the outcomes were collected. SPSS 19.0 software was used to do statistic analysis. Logistic-regression models were used to analyze the risk factors for death in infants with NEC, odds ratio (OR) and 95% confidence interval (CI) were calculated., Results: There were 231 067 newborn infants, 164 307 of them were term infants, admitted to 95 hospitals in main land China. There were 718 term infants were diagnosed as necrotizing enterocolitis with the incidence of 0.44%. There were 294 term infants cases identified as ≥stage 2 necrotizing enterocolitis for the analysis, including 193 cases of stage 2 and 101 cases of stage 3.The mean gestation age was (39.0±1.3) weeks, and the mean birth weight was(3 087.4±548.3)g. The percentage of small for gestation age was 20.4%. The onset age of NEC was 5 (2-11)d. The percentage of cases received breast milk feeding was 23.7%. The most common comorbidities were sepsis (9.5%, 28/294), asphyxia (9.5%, 28/294), pneumonia (7.8%, 23/294) and congenital megacolon (7.5%, 22/294). The bowel perforation rate was 13.9%. The rate of cases who received surgical treatment was 25.2%(76.6% small intestinal necrosis and 65.8% small intestinal perforation). The mortality rate was 28.9%(the mortality rate were 20.7% and 44.6% in stage 2 and stage 3 NEC, respectively). Noninvasive continuous positive airway pressure treatment for NEC (OR=5.278, 95% CI: 2.058-13.533, P<0.01) and NEC staging 3 (OR=3.156, 95% CI: 1.766-5.642, P<0.01) were statistically significantly associated with mortality of NEC., Conclusions: The term infants with necrotizing enterocolitis usually have the underlying comorbidities. The breastmilk feeding rate is low. Necrotizing enterocolitis remains high mortality in term neonates in Chinese neonatal units. Noninvasive continuous positive airway pressure treatment for NEC is statistically significantly associated with mortality of NEC.

Objective: To understand the infection status and gene types of Cryptosporidium among HIV/AIDS patients in Guangxi Zhuang Autonomous Region., Methods: The fecal samples were collected from 285 HIV/AIDS cases in Nanning, Guilin, Qinzhou, Baise, Hechi cities of Guangxi and 150 HIV negative persons in Nanning City. The modified acid-fast staining and nested-PCR based on 18S rRNA were employed to detect the infection status of Cryptosporidium . The nested PCR products were sequenced, and the homology searches and identification for the gene types of Cryptosporidium were done by DNAStar software., Results: The infection rate of Cryptosporidium in HIV/AIDS patients was 0.70% (2/285), and the rate of those with chronic diarrhea was 6.67% (2/30), the latter was significantly higher than that of the HIV negative persons (0, 0/150) ( P = 0.002). Both the two HIV/AIDS patients infected with Cryptosporidium were from Guilin City. By molecular identification, the Cryptosporidium strains which the above 2 patients were infected with were Cryptosporidium andersoni and Cryptosporidium hominis respectively., Conclusions: Cryptosporidium co-infection can be found in HIV/AIDS patients in Guangxi. The genotypes of the infection strains include Cryptosporidium andersoni and Cryptosporidium hominis.

Objective: To investigate the incidence and influencing factors of term small for gestational age infants (SGA ) in China., Methods: This study is a multicenter study. Data were collected from Chinese Neonatal Network with term infants, born from May 1st 2010 to February 28th 2014, from 83 nationwide hospitals., Results: A total of 170 565 neonates were recorded from 83 nationwide hospitals; 144 659 cases were term newborn, and 3 543 neonates were found to be term SGA. The incidence of term SGA was 2.45%. In term SGA, the girl's incidence (2.88%, 1 985/69 003) was higher than boy (2.05%, 1 531/74 707), P<0.001. Furthermore, the incidence of in vitro fertilization (IVF) and artificial fertilization of term SGA was 9.12% (66/724), which was higher than the natural conception (2.42%, 3 465/143 283), P<0.001. The incidence of term SGA in multiple births (30.37%, 741/2 440) was higher than that of single birth (1.97%, 2 787/141 366), P<0.001. The incidence of SGA with maternal age <18 years old was 4.86% (27/556) and was 3.20% (307/9 592) with maternal age > 35 years old, meanwhile both were higher than that with maternal age as 18-35 years old (2.39%, 3 197/133 662). The incidence of SGA with maternal occupation as farmers was 4.67% (854/18 284), which was significantly higher than other professions. The incidence of SGA with maternal education as illiterate and elementary education was 8.65% (187/2 162), 5.17% (397/7 678), respectively, both were significantly higher than those with mother graduated from university (1.72%, 853/49 692). Moreover, the geographical distribution of the main residence of the term SGA mothers showed Northeast Area (1.54%, 30/1 946)and East Area (2.21%, 1 267/57 334) had lower incidence of term SGA than those of Central South Area (2.71%, 493/18 223) and Southwest Area (3.67%, 876/23 852)., Conclusions: The incidence of term SGA was 2.45% in this study, which was influenced significantly by gender, IVF, multiple births, maternal factors (age, maternal occupation, level of education) and geographical distribution. This multicenter study with large data, to a certain extent, reflected status of the incidence and influencing factors of term SGA in China, which will help prevent and treat term SGA.

Purpose: Sprague-Dawley rat models of tooth movement were established to investigate the expression of transient receptor potential vanilloid 1 (TRPV1) and calcitonin gene related peptide (CGRP) in rat trigeminal ganglion during orthodontic tooth movement, and to explore the roles of TRPV1 and CGRP in orthodontic pain., Methods: Sixty-six Sprague-Dawley rats were randomly divided into control group (n=6), sham operation group (n=6), and experimental group (n=54). Tooth movement models were established, orthodontic force (50 g) was applied on the maxillary first molar in the rats of experimental group, and then the trigeminal ganglia were collected at 4, 8 h, 1 d (3 subgroups were set up according to the force: 1 d-30 g, 1 d-50 g, 1 d-80 g), 3, 5, 7, 14 d after tooth movement. The changes of TRPV1 and CGRP expression were detected by immunofluorescence staining. SPSS16.0 software package was used for statistical analysis., Results: According to immunofluorescence staining, the TRPV1-IR and CGRP-IR neurons were mostly small to medium sized. The percentages of TRPV1-IR and CGRP-IR neurons in trigeminal ganglion increased after applying force, and reached the peak at 1-3 d and then fell to the initial level gradually. In addition, the application of greater force during experimental tooth movement induced higher percentages of TRPV1-IR and CGRP-IR neurons in trigeminal ganglion., Conclusions: Experimental tooth movement leads to the regular changes of TRPV1-IR and CGRP-IR neurons in trigeminal ganglion, indicating that TRPV1 and CGRP may play important roles in orthodontic pain.

Objective: To investigate the prognostic value of the size of primary tumor (T staging) and international prognostic index (IPI) for patients with non-Hodgkin's lymphoma (NHL) of the tonsil, and to recommend the treatment strategy for early stage patients., Methods: 306 patients with untreated NHL of the tonsil were reviewed. According to Ann Arbor staging classification, 35 patients had stage I, 178 stage II, 49 stage III and 44 stage IV disease. According to 1997' AJCC staging system, 29 patients had T1, 142 T2, 117 T3 and 18 T4 disease. Twelve stage I patients were given radiotherapy alone and 23 stage II patients were given combined modality therapy (CMT). For patients with stage II lesion, 57 were given radiotherapy alone, 2 chemotherapy alone and 119 CMT. Chemotherapy was the main treatment in patients with stage III or IV lesions., Results: The 5-year cancer specific survival (CSS) was 74% for patients with T(1), 59% for T(2), 56% for T(3) and 26% for T(4), respectively (P = 0.000). The 5-year CSS was 70% for patients with 0 risk factor, 49% for 1 risk factor, 25% for 2 or 3 risk factor, respectively (P = 0.000). CMT significantly improved disease free survival (DFS) from 46% (radiotherapy alone) to 60% (CMT) for stage II patients (P = 0.046). Multivariate analysis showed that performance status, Ann Arbor staging, T staging, B symptom, and IPI were independent prognostic factors., Conclusion: The T staging of the primary tumor and IPI are the important prognostic factors of patients with NHL of the tonsil. Combined modality therapy significantly improves the disease free survival of stage II patients.

Objective: To evaluate the results of radiotherapy for early stage anal cancer., Methods: During 1960 through 1993, 27 patients with stage T1-2N0 anal cancer were treated by radiotherapy alone. Seven cases received pelvic irradiation plus local field boost, 20 cases received local field irradiation alone. Kaplan-Meier method was used in survival analysis., Results: The overall 5-year survival rate in this series of patients was 79.1%. Eighteen of the 27 cases had their anal functions preserved. Local recurrence occurred in 7 cases and regional lymph node metastasis in one case. Five of them were salvaged, two by surgery and three by irradiation., Conclusion: Radiotherapy alone for early anal cancer is effective. Anal function is preserved in about two-thirds of the patients so treated. Local treatment failure can be salvaged by surgery or radiotherapy.

Objective: To analyse the prognosis of patients with inguinal cryptorchic seminoma., Methods: From 1985 through 1991, 43 patients with inguinal seminoma of undescended testis were treated and the therapeutic effect retrospectively reviewed. There were 33 patients in stage I, 8 in stage II, 1 in stage III, and 1 in stage IV. All patients had inguinal orchiectomy. Patients in stage I and II were primarily treated with radiotherapy, whereas one patient in stage III and another in stage IV were treated with radiotherapy or chemotherapy., Results: The 5- and 10-year overall survival was 93.0% and 90.3%, respectively. The corresponding disease-free survival was 88.1% and 85.3%, respectively. The overall 5- and 10-year survival by stage was 100% and 100% for stage I, 75.0% and 60.0% for stage II, respectively. Two patients relapsed and were successfully treated with radiotherapy., Conclusion: Prognosis for inguinal cryptorchic seminoma is as good as that of scrotal testicular seminoma. Postorchiectomy radiotherapy encompassing para-aortic and ipsilateral iliac nodes is effective for stage I and early stage II inguinal crytorchic seminoma. The inguinal nodes should be included in the treatment ports only when they are involved or if there is obvious involvement of surrounding tissues by the primary tumor.

Sixty-five patients with stage II testicular seminoma treated from 1960 through 1990 were reviewed. According to Royal Marsden Hospital staging, there were 11 stage IIA, 13 stage IIB, and 41 stage IIC. Thirty Patients were treated with combination of radiotherapy and chemotherapy, 11 chemotherapy alone, 24 rediotherapy alone. The overall 5-year survival rate was 75.6%, with 100% for stage IIA, 92.3% for stage IIB and 62.9% for stage IIC. The differences were statistically significant. In stage IIC patients, there was no significant difference between those tumors > 10cm and < 10cm in diameter. Of the 41 IIC cases, 4 were treated with chemotherapy, 15 were treated with whole abdominal rediation, 22 with paraaortic and ipsilateral pelvic radiation. The 5-year survival rate was 91.7% and 51.2%, respectively. These differences was statistically significant. We recommand that stage IIC disease should be treated primarily with whole abdominal radiation. Regardless of the stage of the disease, prophylactic mediastinal irradiation did not influence the survival nor the relapse rate and may therefore be omitted.

Brainstem auditory evoked potentials (BAEP) were measured in 20 chronic renal failure patients and 8 maintenance hemodialysis patients. The BAEP in chronic renal failure patients was showed that wave I, III, V peak latencies (PL) and I-III, I-V interpeak latencies (IPL) were markedly increased. The significant reductions, although not to normal, of wave III, IV PL and I-III, I-V IPL were recorded in postdialysis (average 4.2 months) compared with that in predialysis. A correlation analysis revealed no consistent relationships between plasma proteins, hemoglobin, urea, creatinine serum electrolytes and BAEP PL or IPL. The authors realize that BAEP could be used as a subjective index in deciding the inner ear and nervous system damage in chronic renal failure, and it also provided an electrophysiologic evidence for the relationship of Kidney and ear in traditional Chinese medicine.

Sixty cases of nephrotic type of primary glomerulonephritis (PGN), who were mostly insensitive to the corticosteroids, received killed BCG inoculation as a main measure of combined therapy. After this treatment the complete and approximate remissions accounted for 80% in all. The urinary protein, serum albumin, BUN and SCr all had very significant improvement. As to those with similar pathologic and clinical findings, the therapeutic effects were much better in the BCG group than in the control. 13/15 cases followed up over 1-2 years had complete remission at the latest follow up and 5/6 followed below 1 year did so. The longer the courses of BCG, the lower the recurrence rates. The therapeutic effects of retreatment to those recurrent cases were much more rapid than those of initial treatment. The phagocytic function of the monocyte in another 24 PGN cases was studied. This function was found to be significantly depressed and could be normalized through BCG inoculation.

From Mar. 1958 through 1985, 50 male patients with breast cancer were treated in our hospital. The ratio of male and female with primary breast cancer was 1:90 during the same period. All were proven by pathology except one case. Forty seven have been followed for more than 5 years. There were 12 stage I lesions, 19 stage II and 16 stage III. Radical mastectomy was performed in 22 patients, total mastectomy in 11 and lobectomy in 12, supplemented by radiotherapy, chemotherapy, hormonal therapy or their combination. Two were not indicated for surgery and were only given a combination combination therapy without operation. The irradiation after radical mastectomy was to deliver to the regional lymph nodes and for the rest, a dose of 5,000-6,000 cGy/5-6 weeks should be added to the chest wall. The chemotherapy, including Thio-TEPA, 5-fluorouracil, cyclophosphamide and methotrexate etc, in the form of single or multi-drug for 1-3 courses, was given. The overall 5 and 10 year survival rates were 51% and 22.5%. The 5 year survival rates of stage I, II and III patients were 83%, 53% and 25%. The 5 year survivals of radical mastectomy, total mastectomy and lobectomy combined with radiotherapy, chemotherapy and hormonal therapy were 59%, 27% and 67%. In those who failed, 50% developed local recurrence or regional metastasis or both and 60% had extensive dissemination. The results indicate that the combination therapy comprising radical mastectomy and lobectomy has a good prognosis. The authors believe that the male breast cancer is more hormone-dependent than the female breast cancer, orchiectomy plays an important role in the treatment of advanced lesions.
