Objective To investigate the impact of combining triamcinolone acetonide injections with negative pressure suction therapy on plaque size, pain, and outcomes in patients with penile induration (PD). Methods A retrospective analysis was conducted on the clinical data of 60 PD patients admitted to Suixi County Hospital from January 2018 to January 2023. They were divided into the control group (27 cases) and the study group (33 cases) by different treatment methods. The control group was treated with verapamil injection, and the study group was treated with triamcinolone acetonide injection combined with negative pressure suction therapy. The clinical efficacy, penile curvature, length, volume, plaque area, pain level, erectile function, adverse reactions, and recurrence rate of two groups of patients were compared. Results In comparision with the control group, the total effective rate of treatment in the study group was higher, with a statistically significant difference (P＜0.05). After treatment, the penile curvature in both groups was smaller than before treatment, and the length and volume were greater, in comparision with the control group, the penile curvature of the study group was smaller, and the length and volume were greater, with a statistically significant difference (P＜0.05). After treatment, the plaque area in both groups were smaller than before treatment, and the visual analogue scale (VAS) score was lower, in comparision with the control group, the plaque area of the study group was smaller, and the VAS score was lower, with a statistically significant difference (P＜0.05). The International Erectile Function Index Score-5 (IIEF-5) scores in both groups after treatment were higher than those before treatment, in comparision with the control group, the IIEF-5 scores of the study group after treatment were higher, with a statistically significant difference (P＜0.05). Conclusion The utilization of triamcinolone acetonide injection therapy in conjunction with negative pressure suction in the treatment of PD has been shown to enhance clinical effectiveness, reduce penile curvature, increase length and volume, decrease plaque area, alleviate pain, improve erectile function, and prevent adverse reactions or disease recurrence. Objective L-carnitine plays a crucial role in sperm maturation and energy metabolism, exerting multiple functions such as improving sperm quality, maintaining sperm DNA integrity, and facilitating sperm-egg binding. Guidelines and consensus, including the Expert Consensus on the Clinical Application of L-carnitine in Male Infertility (2014 Edition) and the Guidelines for the Diagnosis and Treatment of Male Infertility (2022 Edition), have clarified the clinical value of L-carnitine in the treatment of male infertility and provided recommendations on its standardized application. With further study on the physiological effects of L-carnitine, it has been found that L-carnitine exerts multifaceted effects such as improving mitochondrial function and energy metabolism, possessing the ability of anti-oxidation and anti-apoptosis, promoting nerve regeneration and nutrition and neurotransmitter regulation, as well as facilitating gene expression regulation. Meanwhile, several clinical studies have demonstrated that in addition to male infertility, L-carnitine also plays a role in the treatment of many other andrological diseases, including Peyronie's disease, late-onset hypogonadism, and erectile dysfunction. This consensus is hereby developed to further clarify the clinical application of L-carnitine in various andrological diseases, giving detailed information on the distribution of L-carnitine in the human body, its basic physiological effects as well as its pharmacological effects, clinical research progress, and standardized medication in various andrological diseases to ultimately provide guidance and reference for clinical medication. Objective Explore differentially expressed genes of Peyronie’s disease（PD） and their related signal pathways and biological processes through bioinformatics analysis. Methods Based on the GEO（Gene Expression Omnibus）, the microarray dataset GSE146500 was downloaded for screening DEGs. DEGs were visualized by R language tool（version 4.1.0）. The clusterProfiler package was used to perform gene function annotation（GO）, Kyoto Encyclopedia of Genes and Genomes（KEGG） pathway enrichment analysis, gene set enrichment analysis（GSEA）. STRING database and Cytoscape software were applied to construct protein-protein interaction（PPI） Network and obtain hub genes. Results A total of 1326 differential genes were identified, of which there were 513 up-regulated genes and 813 down-regulated genes in the PD group. GO enrichment analysis showed that DEGs mainly participated in the process of extracellular matrix organization, extracellular structure organization, and collagen content. The enrichment analysis of KEGG signaling pathway mainly included extracellular matrix receptor interaction, cell adhesion molecules, focal adhesion, TGF-β signaling pathway, PI3K/AKT signaling pathway, MAPK signaling pathway, receptor interaction between cytokines, etc. GSEA analysis revealed that the biological pathways mainly included TGF-β signaling, epithelial mesenchymal transition, TNF-α signaling via NF-κB, etc. PPI analysis and screening obtained 10 hub genes, including ASPM, CDK1, BUB1, BUB1B, KIF11, CCNB2, CDC20, TOP2A, DLGAP5, CCNB1. Conclusion DEGs, signaling pathways, and hub genes were identified to be associated with the pathogenesis of PD via bioinformatics analysis, which laid a theoretical foundation for further studies. Objective : To analyze the clinical effect of pelvic floor muscle function training combined with penile clamp in the treatment of urinary incontinence after prostatectomy. Method : From April 2017 to April 2019, 72 patients with urinary incontinence after prostatectomy in our hospital were randomly divided into control group and observation group with 36 cases in each group. Both groups were given pelvic floor muscle function training, and the observation group was added with penile clamp to control urination. The improvement of urinary incontinence, the improvement of micturition, the change of ICI-Q-SF score, the grading evaluation of stress urinary incontinence and the economic cost were compared between the two groups. Result : The incidence of urinary incontinence in the observation group was lower than that in the control group (P<0.05). 90 d after intervention, the total daily urine volume in the two groups was higher than that before intervention, and the total times of urinations per day and total times of leakage of urine per day were lower than that before intervention; the total daily urine volume in the observation group was higher than that in the control group, and the the total times of urinations per day and total times of leakage of urine per day were lower than that in the control group 90d after intervention, with statistical significance (P<0.05). The ICI-Q-SF score 90 d after intervention in the two groups was lower than that before intervention, and the ICI-Q-SF score 90d after intervention in the observation group was lower than that in the control group, the difference was statistically significant (P<0.05). The clinical cure rate of stress urinary incontinence in the observation group was higher than that in the control group (P<0.05), and there was no significant difference in the stress urinary incontinence grade between the two groups (P>0.05). The cost of penile clamp in the observation group was (70.26依8.51) yuan, lower than that (388.71依26.44)yuan in the control group, the difference was statistically significant (P<0.05). Conclusion : On the basis of pelvic floor muscle function training, combined with penile clamp can effectively improve the symptoms of urinary incontinence and quality of life of patients after prostatectomy, and help to reduce the economic burden of patients, which is worthy of clinical application.
