2. An Ethical Reflection on the Museum Narrative in News from Nowhere.

Museum was a public space to display order, cultivate citizens, and construct cultural identity, which exerted a major influence on modern life, and then became an object of literary writing. William Morris affirmed the museum's education role, and viewed museums as a field to realize his socialist ideals. However, under the capitalist system, museums are subject to political power and follow the logic of capital. The direction the museums taking is contrary to Morris's political ideal. In News from Nowhere, Morris attacks the manipulative purpose of museum visits, and attempts to construct a museum as an equal, interactive and sharing space to highlight art and social ethics. From the perspective of Ethical Literary Criticism, the political and capital intervention of collection and exhibition serves as the ethical knot of the museum narrative, while the visitors' identity anxiety as its ethical line, and they interweave with each other to constitute the museums' existential crisis. 3. Analysis on the application of vascular closure system in portal vein reconstruction of liver transplantation: report of 137 cases

Objective To analyze the application effect of non-penetrating vascular closure system in portal vein reconstruction of allogenic liver transplantation in adults. Methods Clinical data of 222 patients undergoing allogeneic liver transplantation were retrospectively analyzed. According to whether vascular closure system was used in portal vein reconstruction during operation, all patients were divided into vascular clip group (n=137) and traditional suture group (n=85). Perioperative conditions, clinical prognosis and complications were statistically compared between two groups. Results All patients successfully completed the surgery. The success rate of one-time portal vein anastomosis was 93.4% (128/137) in the vascular clip group. A total of 14 patients died during perioperative period in this study with a mortality rate of 6.3% (14/222). No portal vein-related complications occurred during perioperative period in both groups. The time of portal vein anastomosis in the vascular clip group was (5.6±1.7) min, which was significantly shorter than (10.7±3.6) min in the traditional suture group (P < 0.05). The incidence and grade of perioperative complications did not significantly differ between two groups (all P > 0.05). Conclusions It is safe and feasible to utilize vascular closure system to reconstruct the portal vein during liver transplantation. Compared with traditional suture, it can effectively shorten the time of portal vein anastomosis.

Objective: To explore the molecular reasons of weak expression of B antigen on the red cell., Methods: Serological test for blood group was carried out, including red cell and plasma grouping, and anti-A1 and anti-H testing, and confirming weak A or B antigens by adsorption and elution. Exons 1-7 were sequenced directly, and one of them was cloned and sequenced., Results: All of the 23 samples showed the weak B antigen by serological method. The alleles of the subgroups were identified by DNA sequencing, including 2 Bel subgroup, 4 B3 subgroup, 14 Bw subgroup, 2 CisAB subgroup and a novel allele. The novel allele showed a nucleotide substitution 662G>A in the exon 7, and the sequence was submitted to Blood Group Antigen Gene Mutation Database, and the novel allele was named Bel10., Conclusion: Nucleotide substitution in exon results in blood subgroup, which showed that the antigens were weakened, and Bw phenotype was the most frequently subgroup.

- Abstract
Objective: To explore the roles of bone metabolism-related molecules in bone metabolic disturbance and disease progression of patients with rheumatoid arthritis (RA)., Methods: A total of 66 RA patients and 20 healthy controls were included, and their relevant clinical information was gathered. Electrochemiluminescence immunoassay (ECLIA) was used to detect the levels of osteocalcin N-terminal middle (OC-N-MID) and C-terminal cross-linked telopeptides of type 1 collagen (CTX) in serum. The serum levels of Wnt inhibitory factor Dickkopf1 (DKK1) and receptor activator of nuclear factor-κB ligand (RANKL) were determined by magnetic luminex assays. Flow cytometry was used to detect the RANKL level on peripheral blood T cells., Results: Compared with healthy controls, the levels of OC-N-MID and CTX in the sera of the RA patients showed no significant difference. Level of RANKL in the sera of the RA patients was higher than that in the controls, while level of DKK1 was lower. The level of RANKL on peripheral blood T cells increased in the RA patients, especially on CD3(+) T cells., Conclusion: In RA patients, level of RANKL on T cells is elevated, and the serum level of RANKL increases, but DKK1 decreases.

Objective: To compare the effectiveness of external fixation and volar locking compression plate in the treatment of distal radius fractures of type C., Methods: Between March 2012 and March 2013, 122 patients with distal radius fractures of type C were enrolled in the prospective randomized study. Fractures were treated by external fixation in 61 patients (external fixation group) and by open reduction and internal fixation using a volar locking compression plate in 61 patients (plate group). There was no significant difference in age, gender, fracture side, weight, height, body mass index, fracture type, and interval of injury and operation between 2 groups (P > 0.05). The blood loss, operation time, hospitalization days, fracture union time, wrist function, and complications were compared between 2 groups. And the quality of reduction was observed, including volar tilting angle, ulnar deviation, radial height, and articular reduction., Results: The blood loss, operation time, and hospitalization days in plate group were significantly higher than those in external fixation group (P < 0.05). All of the patients in both groups were followed up 12-28 months. Postoperative complications occurred in 4 patients (6.6%) of external fixation group (pin tract infection in 2 cases and radial nerve neuritis in 2 cases) and in 5 patients (8.2%) of plate group (wound infection in 1 case, carpal tunnel syndrome in 2 cases, and tendon rupture in 2 cases), showing no significant difference between 2 groups (P = 0.500). The X-ray films showed fracture healing in all patients of 2 groups; the union time of plate group was significantly longer than that of external fixation group (P < 0.05). At last follow-up, there was no significant difference in ulnar deviation and radial height between 2 groups (P > 0.05), but the volar tilting angle of plate group was significantly larger than that of external fixation group (P < 0.05). There was no significant difference in wrist function and articular reduction between 2 groups (P > 0.05)., Conclusion: For distal radius fractures of type C, the use of external fixation or volar locking compression plate can obtain satisfactory clinical outcomes, but the external fixation has the advantages of less invasion, shorter hospitalization days, minor complications, and faster fracture union.

- Abstract
Objective: To assess the clinical value of serum matrix metalloproteinase-3 (MMP-3) in evaluating joint destruction and therapeutic effect in rheumatoid arthritis (RA) patients., Methods: The study included 109 RA patients and 23 healthy volunteers. Serum MMP3, C-reactive protein (CRP), rheumatoid factor (RF) and anti-cyclic citrullinated peptide (CCP) antibody levels were detected. All relevant clinical information of RA patients were recorded., Results: Serum RF, anti-CCP antibodies and MMP3 concentrations in RA patients were markedly higher than those in healthy controls, and serum anti-CCP antibodies and MMP3 levels significantly increased in active RA as compared with those in stable RA (P<0.05). Serum CRP, anti-CCP antibodies and MMP3 levels gradually rose with the progress of joint destruction, but only the increase of serum MMP3 was significant statistically (P<0.05). Serum MMP3 level was found positively correlated with serum CRP or RF levels or joint injury. After the combination treatment of TNF antagonist and methotrexate, DAS28 score, serum CRP, anti-CCP antibodies and MMP3 levels were markedly reduced (P<0.05), and the decrease of MMP3 level was the most obvious., Conclusion: Serum MMP3 was a good laboratory index to evaluate the joint injury status and therapeutic effect, which is superior to other traditional and routine laboratory indexes.

- Abstract
Objective: To introduce the approach and technique of endoscopic transseptal transsphenoidal pituitary adenoma microsurgery., Method: Endoscopic transseptal transsphenoidal pituitary adenoma microsurgery were performed in 23 patients., Result: Tumors were total resected in all of the patients. The mean volume of bleeding was 50 ml and there were not any severe complications., Conclusion: Compared with transsphenoidal pituitary adenoma microsurgery and endoscopic transsphenoidal pituitary adenoma surgery, endoscopic transseptal transsphenoidal pituitary adenoma microsurgery could combine the benefits of the two approaches. The approach has the benefit of microtrauma, convenience and could resect the tumor thoroughly. We could do best in treating suitable patients with good endoscopic technique.

- Abstract
Objective: For establishment and application of Xa Factor Inhibitory Assay to detect plasma heparin concentrations during and after cardiopulmonary bypass (CPB) and to evaluate the treatment with low-dose protamine reversal of heparin., Methods: Venous blood samples were drawn from 25 patients with rheumatic valvular disease undergoing valvular replacement at different points (before CPB, after 5 min of heparinization, at termination of CPB, after 10 min, 1 hr and 5 hr of protamine reversal of heparin), and the plasma heparin concentrations were detected., Results: The mean of plasma heparin concentration was the highest (13.16 U/ml) after 5 min of heparinization. At termination of CPB, it decreased (11.06 U/ml) and was significantly different from the concentrations at other points (P < 0.05). After 10 min of low-dose protamine reversal of heparin, the heparin concentration (0.45 U/ml) decreased to the same as that before CPB and continued to be stable at 1 hr and 5 hr; there was no significant difference in heparin concentration between these 4 points (P > 0.05). The highest batch CV in standard curves was 11.65%; the lowest was 0.37%., Conclusion: Factor Xa Inhibitory Assay is of good accuracy and repetition. The results from its clinical application demonstrated that the plasma heparin concentration of the patient at termination of CPB should be used for reference in determining the dose level of protamine for that every patient subjected to CPB.
