The crown-root ratio (C/R) theory of natural teeth has been widely recognized in the field of stomatology,and has important clinical significance in predicting and assessing the prognosis of natural teeth as well as for abutment selection during denture restoration. In the past few decades, scholars have advocated for the implantation of implants as long in length as possible to improve the success rate according to the theory of crown-root ratio of natural teeth. However, with the application of short implants, our philosophy of implantation has changed, and the relationship between the crown-implant (C/I) ratio and complications has become one of the current research hotspots. In this paper, the concept of the crown-implant ratio, the research progress of the C/I ratio, the implant survival rate and clinical complications of implant restoration were reviewed and summarized, and the following suggestions were put forward: although most studies have shown no significant correlation between the C/I ratio and implant survival or marginal bone loss, this relationship may increase the risk of mechanical complications. A C/I < 3 and a crown length < 15 mm are recommended in implant restoration; when ultra-short implants are applied, the implant system can increase the bone-to-implant contact area, and splint prostheses such as crown or bridge are recommended.

Objective To evaluate the clinical effects of full-arch cement-retained implant-supported combined crowns and screw-retained implant-supported bridge dentures in complete or half edentulous patients. Methods A total of 25 patients with complete or partial edentulous dentures followed up for 1, 3, and 5 years in our hospital from June 2013 to June 2018 and were treated with Straumann bone horizontal implantation, cobalt-chromium stenting and cobalt-chromium porcelain restoration with cement-retained and screw-retained implant-supported fixed dental prostheses to evaluate the accumulative implant survival rate, accumulative prosthesis survival rate, mechanical complications, and biological complications in both groups. Results There were 25 complete or half edentulous patients who received 165 Straumann implants and 28 implant-supported fixed dental prostheses in this study. There were 11 cases with 69 implants in the cement group and 17 cases with 96 implants in the screw group. The accumulative implant survival rate was 100% in the cement group and 96.9% in the screw group. The accumulative prosthesis survival rate was 100% in both groups. The cumulative peri-implant mucositis rate was 23.2% in the cement group and 29.2% in the screw group, and the peri-implantitis rate was 6.8% in the cement group and 7.3% in the screw group. There was 1 case of porcelain collapse (n=1/11) and no screw of abutment loosening in the cement group and 4 cases of porcelain collapse (n=4/17) and 1 case of screw loosening in the screw group. No fracture of abutment was observed in either group. There was no difference in bone loss between the two groups in the first year (P > 0.05), and a higher rate of bone loss was found in the screw group in the third and fifth years (P < 0.05). There was no difference in the sulcus bleeding index(mSBI) between the two groups in the first year and the third year (P > 0.05) and a higher modified mSBI value in the cement group in the fifth year (P < 0.05). Conclusion The survival rates of the implant and prosthesis for cement-retained or screw-retained implant-supported fixed dental prostheses were both high, but there were more mechanical and biological complications in the traditional cobalt-chromium alloy screw-retainer group. The removal of residual adhesives must be reasonably considered when choosing the cement retention method.

Objective To investigate the effect of immediate and delayed implant placement on dimensional changes in hard and soft tissues as well as esthetic outcomes. Methods A total of 40 maxillary single anterior teeth with a dehiscence defect on the labial bone (≤4 mm) were categorized into two groups according to the timing of implant placement: immediate implant placement (n = 20) or delayed implant placement (n = 20). Guided bone regeneration (GBR) was conducted at the sites using a flap approach, and the implants were given immediate provisionalization. Implant survival rates, dimensional changes in hard and soft tissues during the six- and twelve-month follow-ups, and pink esthetic scores (PESs) were measured. Results The implant survival rates in both groups were 100%, and no complications occurred during the follow-up time. There was no significant difference between the two groups at the measurement sites in the dimensional change of hard and soft tissues during the six- and twelve-month follow-ups. The largest resorption was observed at the implant neck, with a loss of (1.29 ± 0.71) mm in the immediate implant placement group and (1.43 ± 0.19) mm in the delayed implant placement group. The mean PES scores were (10.95 ± 1.51) for the immediate implant placement group and (11.05 ± 1.23) for the delayed implant placement group. Conclusion Immediate implantation or delayed implantation combined with GBR and immediate provisionalization might both be a prospective treatment strategy for a maxillary single anterior tooth with a dehiscence defect on the labial bone.

Objective This study aims to evaluate the short-term clinical outcomes and patient satisfaction of anterior and pterygoid implants in the rehabilitation of edentulous maxilla with posterior atrophy. Methods Given a minimum follow-up of 1 year, 25 patients with fixed maxillary rehabilitation over anterior and pterygoid implants were enrolled in this retrospective study. The implant survival rates, peri-implant soft tissue status (including probing depth, modified sulcus bleeding index, and plaque index), marginal bone loss, and patient satisfaction were measured. Results The survival rates for anterior and pterygoid implants at 1-year follow-up were 96.5% and 97.8%, respectively (P>0.05). No statistically significant difference in probing depth, modified sulcus bleeding index, and plaque index was observed between the two types of implants (P>0.05). The marginal bone losses of anterior implants were 0.62 mm±0.44 mm (mesial) and 0.61 mm±0.40 mm (distal), and those of pterygoid implants were 0.64 mm±0.46 mm (mesial) and 0.68 mm±0.41 mm (distal) mm. These results showed no statistical difference in mesial and distal sites (P>0.05). Patients indicated a high degree of satisfaction with the full-arch prostheses supported by anterior and pterygoid implants. Conclusion For the edentulous maxilla with posterior atrophy, full-arch fixed prostheses supported by anterior and pterygoid implants has an acceptable short-term clinical outcome and excellent patient satisfaction. PURPOSE: To evaluate the surgical outcome of tilted implants as an alternative to sinus grafting and the prosthodontic outcome of the bridges on tilted implants in severely resorbed posterior maxilla. METHODS:A total of twenty--one patients were rehabilitated with 27 fixed prostheses supported by 78 Brånemark and ITI System implants between January 2005 and December 2007. Forty-four implants were placed in an axial and 34 in a tilted direction. In these patients, the maxillary bone volumes were insufficient for conventional placement of implants to support a fixed prosthesis. After an average healing period of 3 months, all implants were functionally loaded with cemented bridges or screw retained bridges. Each patient was recalled at 12th, 24th and 36th month for clinical and radiographic examinations. SPSS11.0 software package was used for statistical analysis. RESULTS: One of the 44 axial implants was lost 2 months postoperatively. After 3 years, the implant survival rate was 100% for the tilted implants and 97.72% for the axial implants, and the prosthesis survival rate was 100%. The peri--implant soft tissues were stable over time; the mean probing depths and mean attachment levels did not change during the follow -up period. CONCLUSIONS: This study demonstrates that patients with a severely resorbed maxilla can be treated successfully with tilted implant treatment; the use of tilted implants is an effective and safe alternative to maxillary sinus floor augmentation procedure. PURPOSE: The aim of this study was to evaluate the osteotome sinus floor elevation procedure with Straumann implants without bone grafting through observing the implant survival rate and the change of bone around the implant, and explored new bone formation and stability when the implants protruding in the sinus less than 3mm. METHODS: 23 patients with single upper maxillary molar lost were included in this study. The mean residual bone height (RBH) under the maxillary sinus was 5-8 mm and the distance of the implants protruding into the sinus was less than 3mm without bone grafting. A healing period of 3-4 months was allowed before abutment tightening at 35 N.cm. All the patients were followed up for 2 years to evaluate the implant survival rate. X-ray measurements (immediately after implant placement, after 3,6,12,24 months postoperatively) were performed to calculate the length of the implant protruding into the sinus and if the new bone formed near it. RESULTS: At the end of 2-year, the implant survival rate was 100% and the implants resisted the applied 35 N·cm torque and supported the definitive prosthesis well. The mesial and the distal sides of the implants protruding into the sinus had new bone formation. No hard tissue could be seen around the apical implant 3 months after inserting and the boundary between the new bone and the original sinus floor could not be seen 2 years after surgery. Objective To observe the changes of the knee joint line position after total knee arthroplasty (TKA), and to investigate the relationship with the clinical features. Methods Eighty-nine patients who received TKA were selected randomly. Posterior stabilized prosthesis (GENESISⅡ) was used in all patients. According to the method recommended by Hofmann,the distance between adductor tubercle of femur and distal line of femoral condylar which represented the joint line position was recorded.Pre-and post-operative knee joint line was measured, and the changes of pre-and post-operative knee joint line were calculated.The KSS and Feller scores were used to assess pre-and post-operative knee and knee extension apparatus function.The correlations between the changes of joint line and Feller scores were analyzed.Results The implant survival rate was 100%(89/89) during the postoperative follow-up. The anteroposterior and lateral X-ray of knee did not show radiographic loosening, and the knee KSS, function, mobility and Feller patellar scores increased as compared with that preoperative (all P<0.01).The knee KSS, function, mobility and Feller patellar scores were higher in the patients whose JL level changes ≤4 mm than those whose JL>4 mm (P<0.05 or P<0.01). The Feller patellar scores in patients whose JL changes ≤4 mm was not related with the altitude of changed JL (r=-0.162, P=0.205); while the Feller patellar scores in patients whose JL changes >4 mm was negatively correlated with the altitude of changed JL ( r=-0.724, P=0.000).Conclusions The joint line changed within 4 mm in patients who underwent TKA for the first time by using posterior stabilized prosthesis may result in a better knee joint function after TKA. PURPOSE: To evaluate the clinical outcomes as well as related factors of dental implants replaced in previously failed sites. METHODS: A total of 12 patients (15 implants) who received replacement in previously failed sites during January 2005 to December 2011 were included. Outcomes of redo-implants were collected and clinical data was recorded and analyzed regarding risk factors related to redo-implants' survival using SPSSI3.0 software package. RESULTS: The survival rate of redo-implants was 100% [(33.5±15.4) months]. The mean redo-implant diameter (4.5±0.6) mm was significantly larger than the previous one (4.1±0.7) mm (P<0.05). The mean survival time of implants placed for the first time was (12.9±15.9) months. Implant replacement occurred (6.8±4.4) months after original implant removal. No significant influence was observed on patient and implant-related factors as well as surgery and prosthesis-dependent factors. CONCLUSIONS: Reimplantation in previously failed site is considered as one of feasible prosthesis plans, and surgeons ought to get enough confidence to achieve satisfactory implant survival rates. PURPOSE: To study the clinical effect of osteotome sinus floor elevation (OSFE) combined with simultaneous implant placement in the treatment of edentulous posterior maxilla subject to insufficient bone height in the periodontally compromised patients. METHODS: Forty-seven Straumanns implants were placed in the posterior maxilla in 35 patients with the procedure of OSFE. The final prostheses were restored after 3 to 6 months. The follow-up period was 6 to 30 months. Radiographs were taken and PD, PLI, BOP were measured and analyzed. RESULTS: The overall survival rate was 95.74% during the study period. Forty-five out of the 47 implants were clinically stable and loaded without pain or any subjective sensation. The perforation ratio of the membrane was 4.26%. The average of PD around the implants was (3.22±1.07) mm. The average of the marginal bone loss was (1.38±0.59) mm. CONCLUSIONS: OSFE without bone graft proves to be an effective and predictable treatment for atrophic edentulous posterior maxillary region in patients with periodontitis, but the long-term effect needs further observation. PURPOSE: To evaluate the influence of smoking on survival rate of endosseous implant. METHODS: The literatures published before the year 2012 involving influence of smoking on survival rate of endosseous implant were searched in PubMed,Embase and Cochrane databases. Data from the literatures conforming to the inclusion criteria and exclusive criteria were selected and a meta-analysis was carried out. RESULTS: Sixteen literatures conforming to the inclusion criteria and exclusive criteria were screened out, which were prospective or retrospective cohort studies. Meta analysis using random effects model showed smoking patients had higher risk to lose endosseous implants than non -smoking patients(synthesized OR 2.24,95%CI 1.66-3.01,P<0.01), which implied the survival rate of endosseous implant in smoking patients was lower than in non-smoking patients. Heterogeneity source was not figured out by meta regression and subgroup analysis. When sensitivity analysis was done by using fixed and random effects model alternatively or excluding articles in which the distribution of implant surface characteristics were uneven between smoking and non-smoking group, result was not changed obviously. CONCLUSIONS: The existing limited evidences show that smoking is harmful to the survival of endosseous implants. Short implants can be used as an alternative in cases of insufficient vertical bone volume after dentition defect or absence of dentition to simplify or avoid bone augmentation procedures. Short implants (≤ 6 mm) are reported to have a mean survival rate of 96% after a period of 1-5 years in function and have been widely used in cases of dentition defects or absence of dentition. Compared with conventional implants (≥ 10 mm) combined with bone augmentation procedures, short implants have fewer surgery-related complications, less marginal bone loss, shortened treatment times and reduced costs, and are preferred by patients. Due to a lack of evidence, a high crown-implant ratio should not be an obstacle for the use of short implants. In addition, most of the current literature has not enough follow-up time, the long-term implant survival data of short implants remain unclear. To improve the clinical outcomes of short implants, attention should be paid to the implant site, bone quality, and occlusal force as well as to the presence of oral health maintenance, periodontal diseases and habits through a careful intraoral and radiographic examination. The choices of wider implant use and splint restoration are recommended, occlusal force should be paid attention during implant maintenance. Inappropriate stress on restorations should be avoided. Future studies should be focused on the long-term clinical outcomes of short implants.

PURPOSE: To study the clinical results of XIVE implants placed immediately after sinus floor elevation using osteotomes without bone grafting. METHODS: Totally 14 XIVE implants were placed in 8 patients immediately after sinus floor elevation using osteotomes without bone grafting. The survival rates of the implants during the prosthodontic process and six months after the crowns fabricated were recorded and analyzed. RESULTS: One implant was loosen and extracted during the prosthodontic process and no implant failed six months after the crowns were placed. The survival rate of the implants in this study was 92.9%. CONCLUSIONS: Implants placed immediately after sinus floor elevation using osteotomes without bone grafting could achieve higher survival rates, which is similar to the reported survival rates in dental implants with bone grafting. Objective The aim of the present study was to evaluate the clinical outcomes of implant⁃supported pros⁃ theses for oral function rehabilitation in patients with ectodermal dysplasia. Methods Thirteen patients were included in the present study. After bone augmentation, zygomatic implants (ZIs) or regular implants (RIs) were placed, fabrica⁃ tion of dental prostheses were applied, and psychological and oral education was carried out. Implant survival rates, pa⁃ tient satisfaction and other related evaluation indicators were assessed. Results The ilium was chosen for autogenic bone grafts in two patients. The fibula was used in two other patients and the mandibular ramus in one other patient. One patient was treated through alveolar distraction osteogenesis of the mandible. Guided bone regeneration was applied in seven other patients. Bone graft resorption in the maxilla was observed in one patient; bone augmentation of the man⁃ dible was successful in all patients, and no obvious bone resorption was observed. One hundred and eighteen implants were placed, among which 22 were ZIs, and 96 were RIs. Five RIs failed and were removed. The survival rate for ZIs was 100%, and the survival rate for RIs was 94.79%, in a follow up after 3 years. All patients were satisfied with the res⁃ toration of their oral function. More than 50% of the patients exhibited self⁃confidence. Conclusion Oral function can be restored in edentulous ectodermal dysplasia patients using bone augmentation and implant⁃supported prostheses, and patient self ⁃confidence can be enhanced. However, the resorption of grafted bone in the anterior region of the maxilla cannot be ignored.

Objective To investigate the short ⁃term efficacy of implant in the treatment of patients with dentition defect caused by chronic periodontitis. Methods 53 patients with mild to moderate chronic periodontitis treated in our hospital from June 2014 to May 2015 were selected as observation group, and 53 patients without periodontal diseases in the same period were selected as control group. The plaque index (PLI), sulcus bleeding index (SBI), periodontal probing depth (PD) and gingival papilla index (PIS) were measured at six months and one year after operation, respectively. Interleukin⁃6 (IL⁃6), interleukin⁃8 (IL⁃8), high sensitive C⁃reactive protein (hs⁃CRP) and tumor necrosis factor⁃α (TNF⁃ α) were detected at six months and one year after operation, respectively. At the last follow⁃up, the survival rate of two groups were calculated. Results One year after operation, 5 of the 64 implants in the observation group failed compared with 2 of the 71 implants in the control group. There was no statistical difference in the implant survival rate between the two groups (92.19% vs 97.18%, P＝0.191). There was no significant difference in SBI (t＝0.936, P＝0.351)、PLI (t＝0.457, P＝0.649)、PIS (t＝1.132, P＝0.217)、PD (t＝0.957, P＝0.341) between the two groups at six months after operation. One year after operation, the periodontal index of the observation group was higher than that of the control group SBI (t＝5.297, P＝0.000)、PLI (t＝2.341, P＝0.021)、PIS (t＝8.218, P＝0.000) and PD (t＝6.492, P＝0.000). The levels of IL ⁃ 6 (t＝6.463, P＝0.000)、IL ⁃8 (t＝7.202, P＝0.000)、hs ⁃CRP (t＝4.237, P＝0.000) and TNF ⁃α (t＝ 6.194, P＝0.000) in the observation group were higher than those in the control group at six months after operation. One year after operation, the level of inflammatory factors in the observation group was higher than that in the control group IL⁃6 (t＝12.835, P＝0.000)、IL ⁃8 (t＝13.207, P＝0.000)、hs ⁃CRP (t＝11.319, P＝0.000) and TNF ⁃α (t＝8.117, P＝ 0.000). Conclusion Implant as the treatment of patients with dentition defect caused by chronic periodontitis obtained satisfied short⁃term effect, but its long⁃term efficacy remains to be further verified.

Objective To analyze implant survival rate and incidence rate of complications after All⁃on⁃4 implanta⁃ tion, and to discuss the technical points and consideration of All⁃on⁃4. Methods According to the criteria of inclusion and exclusion, 23 cases treated with All ⁃ on ⁃ 4 immediate restoration were referred, from January 2007 to December 2015, including 5 cases of edentulous maxillary, 14 cases of edentulous mandibular and 4 cases of both maxillary and mandibular were edentulous. All the patients received immediate function with provisional prostheses at the surgery day. Definitive prostheses were delivered to patients after 3⁃6 months post operation and follow⁃up visits were performed up to 9 years (average 3 years) after placement of definitive prostheses. Results In this study, the rate of implant re⁃ tention of 108 implants and 27 final restorations was 100% until the last review. Among the 23 patients, 3 patients had complications and the incidence of complications was 13.0%. Conclusion All⁃on⁃4 is an effective and reliable meth⁃ od in the treatment of dentures denture. The incidence of complications is low. All⁃on⁃4 is a practical method.
