Background The "Fourteenth Five-Year Plan for Healthy Aging" proposes to promote the realization of healthy aging, which not only requires the elderly to be healthy, but also pays attention to the security. With the deepening of the aging society, there is a significant gap in the level of participation of the current pension insurance system for urban and rural residents in various provinces. Under this background, it is of great significance to study the endowment insurance system. Objective To describ the spatial distribution of the basic old-age insurance participation level of urban and rural residents in 31 provinces in China from 2013 to 2020, to observe the spatial correlation and difference of the participation level among provinces, to analyze the relevant influencing factors, and to provide suggestions for the government and medical and health administrative departments to formulate the old-age insurance policy. Methods The data for this study is sourced from the China Statistical Yearbook from 2014 to 2021. The level of participation in urban and rural residential insurance is calculated based on the ratio of the number of urban and rural residents participating in basic pension insurance by region in the public management, social security, and social organization modules to the end of year population in the population clump. The various indicators of influencing factors are all from the 2014-2021 China Statistical Yearbook. Using ArcGIS 10.8 statistical analysis software, the basic pension insurance participation levels of urban and rural residents in 31 provinces were matched with provincial administrative divisions in China. Descriptive research and analysis were conducted on the differences in pension insurance participation levels among 31 provinces. Using methods such as global spatial autocorrelation and Getis Ord Gi * local hotspots, the spatial correlation and hot and cold spots of participation levels were studied, Study the relevant influencing factors by combining geographic detectors. Results Overall, from 2013 to 2020, the number and level of urban and rural resident insurance participants in China had been continuously increasing year by year; from a vertical structure perspective, there were significant differences in the level of insurance participation among provinces; from a horizontal perspective, from 2013 to 2020, the growth rate of most provinces was positive. The participation level of basic pension insurance for urban and rural residents in 31 provinces of China from 2013 to 2020 had spatial correlation. The central provinces were hot areas, while Shanghai is a cold area. The influencing factors on the participation rate were economic development factors, social development factors, policy factors, and cultural and educational factors in descending order. Conclusion The overall level of participation in basic pension insurance for residents has been increasing year by year, but there are differences in the level of participation and growth rate among provinces. The level of economic development is an important factor affecting the level of participation in basic pension insurance for residents.

Objective This study aimed to explore the mediating effect of digital health literacy on the aging expectations and healthy aging of rural empty nest elderly. Methods Using a multi-stage sampling method, a questionnaire survey was conducted on 457 rural empty nest elderly individuals in Chengde, Hebei Province, with the use of general information questionnaires, digital health literacy assessment scales, healthy aging scales, and aging expectation scales. Mediation effect testing procedures and Bootstrap procedures were applied to analyze and validate the mediating effect. Results The scores for healthy aging, aging expectations, and digital health literacy among rural empty nest elderly in Chengde were (96.77±11.90), (34.98±8.96), and (34»90±17.18), respectively. Correlation analysis showed that digital health literacy, aging expectations, and healthy aging were positively correlated with each other among rural empty nest elderly in Chengde (P<0.001). The results of the mediation effect test indicated that digital health literacy partially mediated the relationship between aging expectations and healthy aging among rural empty nest elderly, with a mediation effect ratio of 14.53%. Conclusion The healthy aging of rural empty nest elderly in Chengde was at a moderate level. With the rapid growth of the aging population in China, the social problems caused by aging population continue to intensify. As an emerging industry, elderly care has emerged and received high attention from all sectors of society. The development of the in elderly care service cannot be separated from the cultivation of talents. This article aims to analyze the current situation of elderly care education in China and learn from the experience of developed countries, draw essence from it, put forward scientific and specific suggestions for the cultivation of elderly nursing professional and technical talents, strengthen the cultivation of elderly nursing talents and team building, and improve the training mode of professional talents to meet the needs of the elderly nursing industry in China, promote the health and longevity of the elderly population in China . With the aggravation of population aging, the health of the elderly has become an important issue in the aging society. The Mediterranean diet (MD) contributes to healthy aging by changing dietary patterns to delay and prevent disease onset. This paper summarizes the concepts related to the MD pattern and its relationship with cardiovascular diseases, cognitive dysfunction, depression, chronic inflammation and other brain and heart comorbidity in the elderly, so as to provide thinking and reference for the optimization and upgrading of the elderly, and contribute to the development of healthy China.

The rapid increase of older population in China has brought about a huge demand for health services, which is the basis for achieving healthy aging. Improving the health service system and its services efficiency has been an inevitable choice for actively response to the aging population. However, few studies have been able to reveal the complex mechanism of improving the service efficiency of healthy aging, and propose specific ways to effectively promote the service efficiency of healthy aging. This paper constructs a comprehensive framework for the efficiency-driven paths of healthy aging service based on the theory of competitive advantage. Using national macro statistical data and micro survey data, we explore the complex causal mechanisms that improve healthy aging service efficiency through a combination of necessary condition analysis (NCA) and qualitative comparative analysis (QCA). Findings show that the improvement of service efficiency for healthy aging is a result of multiple factors working together. Economic development and financial capacity are necessary conditions for improving the efficiency of healthy aging services, which have broad value for improving the efficiency of healthy aging services. Economic development and financial capacity are bottlenecks at 70% and 80%level of the efficiency of healthy aging services, respectively. Additionally, there are four equivalent paths that have achieved the improvement of service efficiency of healthy aging system, including comprehensive development, diversified joint force driven, factor-driven in government-led logic, and dual wheel is driven by government and market. This paper contributes to the existing literatures on the following three aspects. First, studying on the service efficiency of healthy aging system has to some extent broaden findings on health service quality from a new perspective. Second, based on the theory of competitive advantage, we construct a comprehensive framework for the efficiency-driven path of health aging services, which provides new ideas and a reference for subsequent research on healthy aging services. Last, by using the methods NCA and QCA, this study explores multiple conditional configurations for achieving high efficiency in healthy aging services and recognizes specific paths to improve the efficiency of healthy aging services. This research reveals the necessary conditions and feasible paths for improving the efficiency of healthy aging services, which helps China actively respond to the growing demand for healthy aging services in the context of deepening population aging. It also provides decision-making reference and path inspiration for government departments to optimize the healthy aging service system and improve the efficiency of healthy aging services. Background There are few epidemiological analyses on mental illness deaths among Chinese residents. As an economically developed region, Shanghai has more complete health information system, which can provide data support for deeper analysis of smaller categorical disease characteristics and causes of death among residents, and analysis of the circumstances and trends of deaths related to mental disorders among women in Shanghai will help to understand the local burden of mental disorders in women. Objective To analyze the death status and trend of mental disorders among female residents in Pudong New Area of Shanghai from 2005 to 2021. Methods Cause of death monitoring data of registered residents of the whole population in Pudong New Area from 2005 to 2021 were collated, and deaths of local female residents with mental disorders were analyzed by using indicators such as crude mortality rate, standardized mortality rate, years of life loss and rate of life loss, and annual percentage change (APC) and average annual percentage change (AAPC) were calculated by using Joinpont regression model for trend analysis. Results From 2005 to 2021, there were 1 165 female deaths from mental disorders in Pudong New Area of Shanghai, accounting for 0.32% of the total deaths in the same period. The crude mortality rate was 4.82/100 000, and the standardized mortality rate was 1.85/100 000. The crude mortality rate of female mental disorders in Pudong New Area of Shanghai showed a downward trend from 2005 to 2012 (APC=-13.05%, P=0.002), and an upward trend from 2012 to 2021 (APC=7.66%, P=0.012). Coronavirus disease 2019 (COVID-19) is the most widely spread epidemic in the past century, which poses a major threat to the safety and health of the whole population, especially to those of the elderly. China carried forward the traditional virtue of respecting and caring for the elderly in special times, adhered to unified leadership, joint prevention and control, society-wide prevention and control, and precise measure implementation for the elderly in the face of challenges such as poor physical and mental health, weak protection ability, and the "digital divide". During the period of COVID-19 prevention and control, China did its utmost to ensure life support and health service for old people. In this article, we summarized successful experience of COVID-19 epidemic prevention and control among the elderly in China and proposed some suggestions on improving health service system for elderly and constructing an elderly friendly society during the period of normalized COVID-19 epidemic prevention and control.

Establishing and improving the hospice care system is an integrant part of healthy ageing. Currently, hospice care has been a part of the national healthcare services, and models of provision of hospice care using multi-agent approaches have been formed in China. Furthermore, the inpatient-outpatient-home-based hospice care model has begun to take shape. And some regions have also explored the guidance center-demonstration base-professional institutions-based system for promoting hospice care. However, many problems have been revealed during the development of the hospice care system, such as low coverage of hospice care, uneven distribution of hospice care resources, imperfect mechanisms for eligibility approval, performance assessment and ineligibility exit targeting hospice care provision institutions, and the absence of an integrated hospice care model. In view of this, we put forward the following recommendations: legally defining hospice care belonging to essential healthcare services in essence, improving the inpatient-outpatient-home-based hospice care model, building an integrated hospice care system focusing on home-based hospice care in communities, and establishing a national center-regional center-professional institutions-based hospice care promotion system.

Community home-based hospice care service plays an important role in hospice care service. Focusing on community home-based hospice care is the future direction of hospice care service development. As the early countries to develop community home-based hospice care delivery, the United Kingdom and the United States have accumulated rich experience in patient admission standard, service team and content, and funding guarantee of community home-based hospice care delivery, a relatively sound community home-based hospice care delivery system has been established in these two countries. China can appropriately learn from the development experience of community home-based hospice care in the United Kingdom and the United States, in order to formulate the admission standards of community home-based hospice care, strengthen the construction of multidisciplinary hospice care teams, incorporate the non-drug therapy into the scope of community home-based hospice care, implement the payment method of per-diem, so that the patients can die peacefully and dignifiedly in a familiar environment with their hospice rights protected.

In the context of high⁃quality social development，population aging is becoming more and more severe，and residents’active health awareness is constantly improving. Various countries have proposed different forms of health management for the growing demand of chronic disease services for the elderly population. However，China’s management of chronic diseases among the elderly is still in the preliminary exploration stage with insufficient construction experience. Based on this，this paper used foreign practical experience as a reference to summarize and analyze the current situation of domestic chronic disease management service under the background of aging from various aspects. This paper found that the current domestic health management of the elderly is mainly based on community work， with medical care， coordination of traditional Chinese and western medicine and smart senior care as the primary service content. There are still deficiencies in supporting policies，community health service capacity and construction, and active health awareness of the population. It is still necessary to focus on medical insurance and medical policies，health talent construction，community performance assessment mechanism， information platform construction, health education and other aspects to improve the health service system and quality of health management. Objective: To explore a definition of healthy longevity in the Chinese population based on the Delphi method. Methods: Through a comprehensive literature review and expert consultation, the dimensions in the definition of healthy longevity were identified, and a preliminary list of questions was created. Experts in clinical medicine, public health, basic research, and the elderly care service industry, who had been working in the field of geriatric health for at least 5 years, were invited to participate in the Delphi survey from August to December 2022. The survey questionnaires were administered via email in two rounds, and experts were asked to select the optimal options from the provided questions. The active coefficients were expressed by the response rate, and a consensus was reached when the largest number of experts agreed for single-choice questions and more than 70% agreed for multiple-choice questions. Results: In the two rounds, the active coefficients were 96.00% (24/25) and 79.17% (19/24), respectively, and a consensus was finally reached on nine items, including age, physical health, common metabolic indicators, mental health, cognitive function, functional ability, social activity, self-rated health, and subjective well-being. Following discussions among the research team and experts, a final definition of healthy longevity was determined. Healthy longevity could refer to a state of good physical, psychological, cognitive function and social adaptation, as well as subjective well-being, in individuals aged 90 and above. Specifically, individuals with healthy longevity should be free from diseases associated with high disability rates and mortality, such as stroke, cancer, and Parkinson's disease. They should also maintain reasonable levels of common non-communicable disease indicators, such as blood pressure and blood glucose, and exhibit favorable mental health and cognitive function using validated measurement tools. In addition, individuals with healthy longevity should engage in social interactions with friends and relatives, care for family members, and go out to do things. Meanwhile, with the ability to complete the visual and hearing functions of daily life and communication, and the ability to complete basic activities such as walking, eating, bathing, toileting, dressing, continence of urination, and bowel movement independently, they could rate themselves to be in good health and experience a relatively high level of life satisfaction. Conclusion: A definition of healthy longevity in the Chinese population is established through the two-round Delphi consultation.

Population ageing has become a major social issue of concern worldwide in recent years, with significant implications for national economic and social development. Globally, Singapore is one of the first countries to address ageing as a population issue and has implemented relatively well-developed initiatives to promote healthy ageing. Similar to China, Singapore has a sharp decline in the total fertility rate, resulting in changes in the population structure. This paper briefly introduces Singapore's healthy ageing measures, summarizes Singapore's practical measures and coping concepts in scientific research on ageing, healthcare programs for the elderly, elderly -friendly environment construction, artificial intelligence big data application, and puts forward that China should pay attention to the effectiveness of population growth incentive measures, pay attention to the scientific and technological response, increase the development and application of artificial intelligence, improve primary health care and long-term health care, and update scientific concepts.

Objective: To construct an epigenetic clock model for assessing and calibrating human biological age. Methods: Convenience sampling was used to select 186 subjects from the longevity cohort of Guangxi Zhuang Antonornous Region from July 1 to November 30, 2019, and 124 subjects from the physical examination population of the Seventh Medical Center of the PLA General Hospital from October 1 to December 31, 2020. Self-designed questionnaire was applied to collect demographic characteristics and family history of disease. Physical examination was applied to determine heart rate and blood pressure. Fasting peripheral venous blood was drawn for determination of fasting plasma glucose, plasma total cholesterol, triglyceride, plasma high-density lipoprotein cholesterol, plasma low-density lipoprotein cholesterol and telomere length. Methylation levels of EDARADD cg09809672, IPO8 cg19722847, NHLRC1 cg22736354, P2RX6 cg05442902 and SCGN cg06493994 were detected by targeted methylation site sequencing. A total of 54 subjects with unqualified quality control of DNA methylation and telomere length were excluded, and 256 subjects' data were finally analyzed. Trend test was used for the change of methylation level among different ages groups, multiple linear regression method was used to build prediction models of biological age. Kendal rank correlation analysis was used to evaluate the correlation of age gap (Gregorian calendar age minus biological age) with telomere length. Independent sample t -test was used to compare the health-related indicators between subjects with different age gap within different age groups. Results: The M ( Q 1 , Q 3 )of age of subjects were 67 (51, 91) years old, including 166 females (64.84%). With increase of age, the methylation levels of gene loci were decreased (EDARADD cg09809672, IPO8 cg19722847 and P2RX6 cg05442902) and increased (NHLRC1 cg22736354 and SCGN cg06493994) (all P values<0.05). The established biological age prediction model was as follows: Y =-53.121×EDARADD cg09809672-137.564×IPO8 cg19722847+141.040×NHLRC1 cg22736354-67.893×P2RX6 cg05442902+149.547×SCGNcg06493994+4.592×sex+64.185 ( R 2 =0.86, P <0.001), where Y was the biological age, and the items in the equation were methylation level, sex (male =1, female =2) and intercept in sequence. The Kendall rank correlation coefficient between age gap and telomere length was 0.731 ( P <0.001). Compared with the subjects whose age gaP<0, the subjects with age gaP≥0 had higher systolic blood pressure in adolescence [(88.50±8.89) and (109.83±9.48) mmHg, respectively, 1 mmHg=0.133 kPa]; lower TC [(5.48±0.23) and (3.98±0.54) mmol/L, respectively, ] and TG [(3.51±0.32) and (3.41±0.20) mmol/L] in young adults; lower fasting blood glucose in middle age [(6.17±0.67) and (5.37±0.79) mmol/L, respectively, ] and higher diastolic blood pressure in nonagenarian age [(76.99±6.78) and (83.97±9.36) mmHg, respectively, ] (all P values<0.05). Conclusion: The constructed epigenetic clock model can be used to evaluate and calibrate human biological age.

Objective: To establish an index that can reflect the level of healthy aging promotion in a region. Methods: Establish an indicators system using expert consultation and then determine the weight for each indicator using the analytic hierarchy process. Finally, we can get the regional healthy aging promotion index. Results: Regional healthy aging promotion indicator system was established, including five first-level indicators (residence environment, medical service, public health, nurse and care, and supporting system) and 21 second-level indicators. The weight of every level-one indicator ranges from 0.073 to 0.346. Two indicators with the highest weight are residence environment and public health (0.346 and 0.325, respectively), while the indicator with the lowest weight is nurse and care (0.073). The importance of every level-two indicator ranges from 0.011 to 0.162. The consistency ratio of the regional healthy aging promotion index is 0.021, and the consistency test is qualified. Conclusion: Regional healthy aging promotion index established in this study is very scientific, reasonable, and applicable. It can be used to evaluate the region's situation or level of healthy aging promotion.

Geriatric epidemiology deals with the application of epidemiological techniques to study the distribution and prevention of health status, diseases, and risk factors among the elderly, to develop intervention measures for promoting healthy ageing. The ageing of the population is a gradually developed phenomenon that is accelerating in China, geriatric epidemiology is one of the important branches of epidemiology and the major pillars of geriatric preventive medicine that play an important role in disease prevention and health promotion. In this article, we reviewed the recent Chinese studies in geriatric epidemiology, and synthesized the distributions and determinants of health-related status in Chinese elderly, and finally highlighted the future trends and expectations of the geriatric epidemiological research.

With the worsening of population ageing in China, the life quality of the elderly is deeply affected by comorbidity, disability, dementia and psychological problems. In China, great achievements have been made in the research of healthy ageing, but due to limitations in research hypothesis, sample size, study design and quality of follow-up, the research findings are insufficient to provide high-quality evidence chain or can't be adopt in health guidelines or standards for the elderly in China, especially the oldest-old. In this special issue, the team of Healthy Aging and Biomarkers Cohort Study reported a series of new findings on health pattern of the elderly, influencing factors and biomarkers in healthy ageing research to provide more comprehensive scientific evidences for health service and to support scientific implementation of health promotion program for the elderly and promote healthy ageing.

China has entered an aging society since 1999. By the end of 2017, the number of persons aged ≥60 years had increased to 240.90 million. At present, the average life expectancy of China's population is 76.4 years, and the healthy life expectancy is around 68.7 years, which means that the elderly will be accompanied by disease for about 8 years. The main health problems of the elderly in China include increased prevalence of chronic diseases, increased prevalence of disability, insufficient attention to older persons' mental health and area specific differences in the health level of the elderly. The disease status of the elderly cannot be ignored. Strengthening the medical services in western and rural areas, emphasizing the psychological and mental concern for the elderly, formulating unified health evaluation criteria, promoting a new model of medical and nursing care combination, establishing an elderly security system with equal responsibilities and rights, jointly managing under the cooperation of whole society and multi-dimensions to protect the health of the elderly are essential to the construction of an active ageing society in China and the realization of the strategic goal of healthy China.

In recent years, great effort has been made in the promotion of healthy aging in China. The core indicator of a country's healthy aging is the level of body health, mental health, social adaptation and quality of life (QOL) of the elderly. This paper summarizes the concept and the progress in the research of healthy ageing and the leading role of geriatric epidemiology. Articles on column "Pay attention to function and health of elderly" analyze the basic conditions affecting the QOL of the elderly, such as disability, comorbidity and Alzheimer disease prevalence, in the elderly in China and related risk factors to describe the severity of different health problems affecting the QOL of the elderly in different areas of China and provide essential data for developing effective health approaches and conducting targeted intervention foe healthy aging.

Objective: To explore the relationship between successful aging (SA) index and the survival status among elderly from longevity areas of China. Methods: A total of 2 296 old people aged ≥65 years old from 8 longevity areas were enrolled in our baseline survey in 2012. The information of demographics characteristic, life style, self-assessed health and mood or emotional state, cognitive function, abilities of daily living and physical activity were collected by questionnaire. There were 891 SA people in total, whose SA score was 4-5. 891 SA and 1 396 non-SA participated in the follow-up study in 2014. Finally, 860 SA and non-SA people were matched by orientation analysis. A Cox proportional hazards model was used to explore the relationship between successful aging index and survival status. Results: After 2 years of follow-up, a total of 1 442 elderly survived, with an average baseline age at (83.69±10.81) years old and 278 elderly people died, with an average baseline age at (93.41±9.05) years old. The mortality rate was 16.16% (278/1 720) in total, and it was 13.14% (113/860) in SA group, which was lower than it in non-SA group (19.19%, 165/860), and the difference was statistically significant ( P =0.001). The analysis of the multivariate Cox regression showed that SA was the protective factor of the survival of the elderly after age and sex adjusted. Compared to the non-SA group, the risk of mortality rate in SA group decreased by 38%, the HR (95% CI ) was 0.62 (0.49-0.79). Conclusion: The mortality rate in the SA group was low in the longevity areas in China; compared with non-SA group, the death rate in SA group reduced by 38%.
