ObjectiveTo explore the causes of related medical damage risks and preventive measures by analyzing the identification results of medical damage in 20 urological death cases.MethodsA retrospective analysis was conducted on 20 death cases of medical damage identification involving urology diagnosis and treatment accepted by the Judicial Appraisal Center, School of Forensic Medicine, Guizhou Medical University, and Academy of Forensic Science from 2010 to 2023.ResultsAmong the 20 cases, the male-to-female ratio was 1.5∶1. The age of the identified persons ranged from 37 to 84 years old, with an average age of 59.2 years. There were 21 medical institutions involved, with 81.0% being tertiary medical institutions. The statistical results of the causes of death showed that 60.0% deaths were due to infectious shock or hemorrhagic shock.ConclusionThe causative potency analysis of medical damage in urological death cases is closely related to the patients’ diseases, the death consequences of medical damage, and medical errors. Analyzing characteristics of typical cases can improve the comprehensive analysis ability of forensic examiners in similar cases, help them understand the rationality and compliance of medical behaviors in different situations, in order to make more accurate and comprehensive judgments.

Objective Virtual marshalling technology requires high precision and accuracy of train positioning, and traditional train positioning technologies are unable to meet the safety requirements of the virtual marshalling SIL4. Therefore, it is necessary to further study the positioning algorithm, estimate more accurate positioning error envelope, and shorten the train chasing distance in virtual marshalling to improve the marshalling safety. Method Traditional train positioning technologies divide the positioning process into train speed measuring, distance measuring and initial positioning. Based on the error factors existing in the three parts of the positioning process, a new calculation method of train positioning error envelope is proposed. The upper and lower boundary envelope for the three parts of the positioning process are obtained by mathematical derivation and calculation of each type of the error respectively. Result & Conclusion The proposed method fully considers the error factors in train speed measuring, distance measuring and initial positioning, making the upper and lower boundary envelope of positioning errors more accurate and reducing the conservation in traditional positioning error calculations. Meanwhile, the proposed method can reach the safety level of SIL4, meet the safety requirements of virtual marshalling control system in train close chasing scenario, and further shorten the train chasing distance, improving transportation efficiency.

Objective·To investigate the effect of hypoxia inducible factor-1α (HIF-1α) inhibitor YC-1 on the progression of diabetic nephropathy (DN) in mice and the potential mechanism.Methods·Ten-week-old male db/db mice (DN model) and their nondiabetic wild-type (WT) littermates were divided into 4 groups (n=6) according to whether treated with YC-1 or not: WT group, WT+YC-1 group, DB group, and DB+YC-1 group. The treatment groups were intraperitoneally injected with YC-1 (20 mg·kg-1) once a day, while the non-treatment groups received the same volumes of DMSO injection. After a total of 8 weeks of intervention, blood glucose, body weight, and kidney weight of all mice were measured. Serum, urine and kidney tissue samples were harvested. Serum creatinine, urinary albumin-to-creatinine ratio (UACR), and urine neutropil gelatinase-associated lipocalin (NGAL) levels were detected. The kidneys were stained with ‎hemat‎oxyli‎n-eosin (H-E) and periodic acid-Schiff (PAS) to observe the pathological changes. Masson staining was used to detect fibrosis, collagen-Ⅰ was detected by immunohistochemistry, and α-smooth muscle actin (α-SMA) was detected by Western blotting. The expression of HIF-1α was detected by both Western blotting and immunohistochemistry. TUNEL staining and Western blotting for apoptosis-related proteins were used to observe the cell apoptosis level. Superoxide dismutase (SOD) activity and malondialdehyde (MDA) level were detected by the kits. Endoplasmic reticulum stress (ERS) markers, including immunoglobulin heavy chain binding protein (BiP, also known as GRP78), phospho-protein kinase R-like endoplasmic reticulum kinase (p-PERK), total PERK, phospho-eukaryotic initiation factor 2α (p-eIF2α), total eIF2α, activating transcription factor 4 (ATF4), and C/EBP homologous protein (CHOP), were determined by Western blotting.Results·Compared with the WT group, the DB group showed significant rise of blood glucose, loss of renal function, severe kidney histopathology injuries and kidney fibrosis, increase of renal HIF-1α expression, and aggravated oxidative stress and ERS. Whilst there were no significant changes in blood glucose, YC-1 treatment notably reduced kidney weight/body weight ratio, serum creatinine, UACR, and urine NGAL levels in db/db mice. YC-1 treatment ameliorated kidney histopathology injuries and kidney fibrosis, and decreased the expressions of collagen-Ⅰ and α-SMA. YC-1 treatment also reduced the number of TUNEL positive cells, the expression of HIF-1α and pro-apoptotic proteins including BAX and cleaved caspase-3, and MDA level in the kidneys of db/db mice, while promoting anti-apoptotic protein BCL-2 expression and SOD activity. The expressions of ERS markers GRP78, p-PERK, p-eIF2α, ATF4, and CHOP were likewise significantly decreased in DB+YC-1 group.Conclusion·HIF-1α inhibitor YC-1 inhibits oxidative stress and abnormal activation of ERS, improving cell apoptosis and fibrosis in the kidneys of DN mice, which would attenuate the aggravation of pathological damage and loss of kidney function.

Objective To investigate the effect of evidence-based nursing combined with individualized health education intervention on knowledge about deep vein thrombosis (DVT), self-management ability and discharge readiness of patients with gynecological surgery. Methods A total of 160 patients who underwent gynecological surgery in the hospital from January 2021 to December 2021 were selected as the research subjects. According to different health education methods, they were divided into control group and observation group, 80 cases in each group. The control group received routine health education, and the observation group received evidence-based nursing combined with personalized health education. During the epidemic of COVID-19, the routine clinical treatment for gynecological cancer patients has been disturbed due to the redistribution of medical resource. Due to the systemic immunosuppression caused by the malignancy and anticancer treatments, gynecological cancer patients are more susceptible to COVID-19. With the improvement of the epidemic, the treatment needs of gynecological cancer patients are extremely strong. During this special period, it should carefully identify fever and respiratory symptoms of gynecological cancer patients receiving chemotherapy, immunotherapy and operations. Therefore, it is quite necessary to carry out comprehensive clinical management. We introduce a clinical management of gynecological cancer patients in three aspects of outpatient, in-hospital and out-of-hospital management during this period, in order to maximize the treatment of tumors and effectively prevent COVID-19.

In order to establish a rapid and effective method for the quantitative determination of microcystins (MC-LR, MC-YR and MC-RR) in bivalves such as mussels, by taking the mussels as the research object and optimizing the factors such as the extracting solvent, the concentration of extracting solvent, and dilution ratio, etc., a method for the determination of three microcystins (MC-LR, MC-YR and MC-RR) in mussels was established by using the direct dilution combined with ultra-high performance liquid chromatography-tandem mass spectrometry. The results showed that : the determination method of three toxins in mussels was as follows : after being extracted with 90% methanol aqueous solution, the extract fluid was diluted with 0.5% acetic acid methanol solution, and the diluent was directly determined by ultra-high performance liquid chromatography-tandem mass spectrometry. The limit of detection (LOD) was 5 µg ⋅ kg-1 and the limit of quantification (LOQ) was 15 µg ⋅ kg-1. The mixed standard substance of 15, 30, and 150 µg ⋅ kg-1 microcystins was added to mussels for the spiked recovery test, and the recovery rates of three microcystins (MC-LR, MC-RR and MC-YR) were 89.4%-91.8%, 77.1%-106.3%, and 75.8%-102.3%, respectively. The relative standard deviations were 3.5%-13.5%, 5.6%-9.2%, and 4.5%-8.1% (n= 6), respectively. The determination method was simple and fast, with low cost and strong operability, which could meet the technical requirements of the current batch quantitative detection of three microcystins in mussels. The ' Policies and Actions to Address Climate Change' part of China' s Fourth National Assessment Report on Climate Change comprehensively summarizes the policies and actions implemented to tackle climate change during the middle and late stages of the 12th Five-Year Plan and the 13th Five-Year Plan, and makes an in-depth and systematic assessment of the effects of the policies. This paper mainly introduces the overall framework and main features of this part of the report, interprets its main conclusions, and proposes prospects for future policies. Since the middle and late stages of the 12th Five-Year Plan, China has adopted a series of policies and actions to tackle climate change, continued to promote the establishment and improvement of climate policies, and formed a policy system with distinctive Chinese characteristics involving the planning objectives, the administrative measures, the market-based policies, the pilot and demonstration actions, etc. During this period, China made great achievements in tackling climate change, fulfilled the 2020 carbon emission reduction target in advance, and reversed the rapid growth of carbon dioxide emissions. To achieve the goal of reaching peak carbon emissions by 2030 and achieving carbon neutrality by 2060, China needs to make climate change solutions a part of the overall goal and strategy for national modernization in the future, formulate and implement stronger climate policies, and put them into action. Glomerular diseases are a leading cause of chronic kidney disease and end-stage renal failure. Understanding the molecular mechanism underlying the initiation and progression of glomerular diseases is important to early identify risk factors for disease development and rescue renal function. The kidney is composed of complex and highly differentiated cells, which is a major challenge in the study of glomerular diseases. Bulk RNA sequencing cannot assess differential transcription or the interactions between cell types. Single-cell RNA sequencing (scRNA-seq) technology has developed rapidly in recent years, making it possible to study a wide range of cell types in kidney, urine and blood. The application of scRNA-seq to glomerular diseases helps to construct a comprehensive cell atlas, elucidate complex cellular and molecular mechanisms, develop efficient and accurate prognostic biomarkers and cell-specific therapies. ObjectiveTo observe the clinical effect of extracorporeal diaphragmatic pacing combined with four-point knee-ling position training on pulmonary function in patients with ischemic stroke.MethodsA total of 60 patients with ischemic stroke admitted to the Rehabilitation Department of the Fourth Affiliated Hospital of Soochow University from January 1, 2023 to June 30, 2023 were enrolled and randomly divided into control group and treatment group, with 30 cases in each group. Both groups received regular basic training. The control group was treated with extracorporeal diaphragmatic pacing, and the treatment group was treated with extracorporeal diaphragmatic pacing, while combined with four-point kneeling training. After 8 weeks of continuous treatment, the patients were evaluated for changes of clam endexpiratory diaphragmatic thickness (CEEDT), maximum end-inspiratory diaphragm thickness (MEIDT), quiet breathing diaphragm mobility (QBDM), deep breathing diaphragm mobility (DBDM), forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1), FEV1/FVC, peak expiratory flow (PEF), 6-minute walking distance (6MWD) and Borg score of patients were detected and evaluated in both groups.ResultsThere were no significant differences in the pre-treatment general data and baseline evaluation results at baseline, before treatment between the two groups (P>0.05). To solve the problem that the satin bowerbird optimizer (SBO) is prone to low accuracy and slow convergence, this paper proposes an improved satin bowerbird optimizer (ISBO). Firstly, the non-uniform mutation operator is introduced to dynamically adjust the search step size of each iteration bowerbird, so that the algorithm can quickly and efficiently find the global optimal value. Secondly, the mutually beneficial factor is used to introduce more combinatorial modes to the social part of the algorithm so as to no longer searches around the previous bowerbird, thus obtaining a better optimal solution. Finally, in order to better balance the local and global search ability of the algorithm, the inertia weight factor of cosine change is introduced to update the bowerbird position formula. Convergence rate analysis, Wilcoxon test and 8 benchmark functions are used to evaluate the efficiency of the improved satin bowerbird optimization algorithm. The results show that the improved algorithm has better global search capability and solution robustness, and the optimization precision and convergence speed are also better than the original algorithm. Objective To investigate the relationship of soluble CD146 (sCD146) with atherosclerosis and future cardiovascular events in patients with diabetic kidney disease (DKD). Methods A total of 105 DKD patients at chronic kidney disease(CKD) (68 male, 37 female) stage 1-3 were enrolled and another 94 type 2 diabetes mellitus patients without DKD (64 male, 30 female) entered the control group from January 2013 to December 2015 in our hospital. Plasma concentration of sCD146 was measured. Doppler ultrasounds of carotid and lower extremity artery were performed. All the DKD patients were retrospectively followed up (medium follow-up 28 months) .The differences of sCD146 between DKD patients and control group was analyzed and the relationship between sCD146 and proteinuria and renal function was evaluated in DKD patients. The association between sCD146 and atherosclerosis was explored in DKD patients. Kaplan-Meier method was used to evaluate the predictive value of sCD146 in future cardiovascular events. Results The level of sCD146 in diabetes mellitus patients was lower than that of DKD group [the level of sCD146 was (435± 150) and (602 ± 274) µg/L, respectively, t=-5.246, P<0.001]. Spearman analysis showed the positive association between sCD146 and serum creatinine (r=0.36, P<0.001), urinary albumino-to-creatinine ratio (r= 0.225, P=0.001) and its negative correlation with eGFR (r=-0.376, P<0.001). The elevation of sCD146 was further associated with the progression of CKD [the levels of sCD146 in patients of CKD stage 1, 2 and 3 were (472 ± 172), (590 ± 223), (685 ± 340) µg/L, respectively, F=164.203, P<0.001]. In DKD group, the upregulation of sCD146 was associated with both the increase of carotid intima-media thickness (r=0.577, P< 0.001) and femoral intima-media thickness (r=0.765, P<0.001). Patients with higher level of sCD146 tended to have more carotid plaques [OR(95%CI)=17.302(2.752-108.780), P=0.002], unstable carotid plaques [OR (95%CI)=6.404(1.036-39.608), P=0.046] and unstable plaques in lower extremity arteries [OR(95%CI)= 13.641(1.942- 95.825), P=0.009] after the adjustment of other cardiovascular risks. Patients with higher concentration of sCD146 tended to have poorer cardiovascular outcomes (Log rank χ²=9.366, P=0.049). Conclusions The measurement of plasma sCD146 is a noninvasive way which can sensitively reflect the progression of renal function and atherosclerosis, and it can also be a good marker to predict cardiovascular outcomes in DKD patients at CKD stage 1-3. Objective To investigate the clinical efficacy and complications of different treatments for progressive subdural effusion (SDE) after decompressive craniectomy for craniocerebral injury. Methods We retrospectively analyzed the clinical and follow-up data of 65 patients with progressive SDE after decompressive craniectomy for craniocerebral injury, who were admitted in our hospital between February, 2013 and March, 2019. Thirty-four of the patients (21 male and 13 female patients) received borehole drainage or local puncture drainage combined with compression dressing with elastic bandage and continuous drainage of the lumbar cistern (observation group), and 31 (17 male and 14 female patients) received borehole drainage or local puncture drainage with compression dressing with elastic bandage (control group). The occurrence time of SDE, retention time of head drainage tube, hospital stay after treatment, one-off cure rate and the incidence of complications were analyzed in the 2 groups, and the Glasgow Outcome Scale (GOS) scores of patients before and at 3 and 6 months after the treatment were compared. Results The retention time of head drainage tube, the time from local drainage to SDE disappearance and hospital stay after treatment were significantly shorter (P < 0.05), and the one-off cure rate (P < 0.05) was significantly higher in the observation group than in the control group. The overall incidence of complications was 23.53% in the observation group and 48.39% in the control group, showing a significant difference between the 2 groups (P < 0.05). There was no significant difference in GOS scores between the 2 groups before the treatments (P>0.05), but at 3 and 6 months after the treatments, GOS scores was significantly higher in the observation group than in the control group (P < 0.05). Conclusion For patients with progressive SDE after decompressive craniectomy for craniocerebral injury, combined treatment with borehole drainage or local puncture drainage, compression dressing with elastic bandage and continuous drainage of the lumbar cistern can improve the one-off cure rate of SDE, reduce the incidence of the overall complications, shorten the duration of hospital stays and improve the prognosis of the patients.

In liquid crystal display, the uniformity of spacers influences contrast degree and color tone change, et al. Thus, the test method of uniformity of spacers in LCD is researched in this paper. Firstly, the liquid crystal cells are made by using dry spray method and wet spray method, then the distribution character of spacers is studied by using the polarizing microscope. It is found that, in normal case, the least distance of spacers in LCD is close to 20 µm and the largest distance exceeds 100 µm. The cluster of spacers is the main defect which leads to badly distribute. Under laboratory conditions, the cluster of spacers is more serious by using dry spray method but this phenomenon doesn't exist basically by using wet spray method. Therefore, by using the way of polarization interference and through adjusting the relative angle of up and down polarized plates of the polarizing microscope, the spacers and distribution of spacers can be directly observed without destroying the LCD. Objective: To investigate the efficiency and safety of benazepril plus amlodipine in the clinical treatment of hypertension. Methods: 90 patients with hypertension were randomly divided into the control group 1 (n=30) which was treated by benazepril alone, the control group 2(n=30) which was treated by amlodipine alone as well as the observation group (n=30) which was treated by benazepril and amlodipine for 12 weaks. The systolic blood pressure, diastolic blood pressure, serum total cholesterol, triglyceride, and high density lipoprotein cholesterol were detected and compared before treatment and 12 weeks after treatment. Results: After treatment, the total effective rate of observation group was 93.3%, which was significantly higher than those the control group 1(63.3%) and the control group 2 (56.7%)(P<0.05). The degree of blood pressure reduction of observation group was more significant compared with the control group 1 and controlgroup 2 (P<0.05), while there was no significant difference between the control group 1 and control group 2(P>0.05). The improvement of TC, TG and HDL-C of observation group were obviously better than those of the control group 1 and control group 2 (P<0.05). The incidence rate of adverse reactions was 6.7% in the observation group, which was significantly lower than those of the control group 1(26.7%) and the control group 2(23.3%)(P<0.05). Conclusions: The efficacy and safety of benazepril combined with amlodipine were evidently better than benazepril or amlodipine alone in the treatment of hypertension, which was worthy of clinical popularization. [Objective] The study aimed to improve the agricultural development environment and define the regional agricultural investment direction, by which can promote the coordinated development of regional agriculture in Hainan Province. [Method] Using the date of 2011, the study focused on the agriculture development environment estimation of the 18 cities (counties) in Hainan Province based on their agricultural development conditions and features by using the Principal Component Analysis method. [ Result ] Due to the different topography, geomorphology and cultivated land area, the agriculture development environment score of cities (counties) in central Hainan Province is low and the agriculture is not suitable in these regions, while the cities ( counties ) in the coastal plain terrain have strong agricultural investment potential. There is regional imbalance in agricultural development of Hainan Province. Land and capital both play the key role in agriculture development of Hainan Province. But the factor of agriculture structure and technology could not promote the agriculture development well in most cities (counties). This paper describes a high precision non-contact physiological signal acquisition system using oversampling technology, which sacrifices sampling rate for achieving a higher accuracy. As we known, due to the low resolution of traditional ADC and the high cost of high-resolution ADC, oversampling technology is used to achieve a high precision data acquisition in this paper. This technology has been remerged to be studied due to its advantage in the acquisition and analysis of weak biomedical signals by using biomedical wireless sensor networks. In this paper, the traditional sampling and the oversampling technologies were compared by quantifying the parameters of signal-to-noise ratio and dynamic range through simulations and experimental measurements. Oversampling technology reduces the quantization noise about 40dB, in other word, the signal-to-noise increases 40dB, which has great improvement in the acquisition system for physiological signals. This paper studied the effects of different mulching modes on the soil moisture in a semi-arid rainfed area of Loess Plateau, Northwest China. Seven treatments were installed, i. e. , mulching plastic film in summer (T1 ), mulching plastic film in autumn (T2 ), mulching 5 cm long wheat straw in summer (T3), mulching whole wheat straw in summer (T4), mulching plastic film in summer plus wheat straw (T5), mulching used plastic film after harvest (T6), and un-mulching (CK). In T6, the soil moisture in different layers at different crop growth stages was all higher than that in CK. In the other five mulching treatments, the soil moisture in 0-90 cm layer before flowering stage was obviously higher, but that in 0-90 cm layer after flowering stage and in 90-200 cm layer during the whole growth season was lower than that of CK. The soil moisture in 0-200 cm layer in T6 during the whole growth period was significantly higher than that in CK, with a difference of 0. 9%, but the soil moisture in 0-200 cm layer in other mulching treatments was lower. As compared with plastic film mulching, straw mulching increased the soil moisture in 0-200 cm layer. The soil moisture under mulching with used plastic film after harvest was higher than that under mulching with new plastic film. As compared to CK, the grain yield of winter wheat with plastic film mulching was increased by 20. 3% -29. 0%, and that with straw mulching was increased by 5.0% -16. 7%. Field tests (inclusive of material property, static and dynamic structural properties) were conducted on a reinforced concrete arch bridge, which has been in service for more than 40 years; Based on the similarity theory of model design, a scaled model of one-span bridge was fabricated by organic-glasses, and the scale factor of geometry is 20. To keep the structural integrity, added mass were added on different locations, of the arch ribs to change the structural dynamic properties. Taking structural dynamic properties as damage fingerprint, dynamic tests were performed respectively. Dynamic responses (structural damping, frequency, mode shape, etc. ) of the damaged bridge models are achieved. It is shown that local variation of arch rib makes a great influence on the acceleration amplification along arch ribs, while it has a little effect on the structural natural frequency and damping ratio. To investigate the deteriorating process of reinforced concrete beams corroded by sulfate, the section analysis method and fractal theory were employed to analyze and calculate the ductility of the corroded beams respectively. According to the corrosion mechanism of the concrete and reinforcing steel in sulfate environment, the normal assumption of plane section behavior was proved to hold true. The available ductility of corroded doubly reinforced concrete beam sections with a range of reinforcement corrosion and concrete degradation was discussed. Based on the mechanical test of corroded beams, a simplified expression is put forward to predict the ductility factor for the beams corroded by sulfate. Based on an old reinforced concrete arch bridge, which has been in-service for about 40 years, a scaled one-span bridge model was fabricated by organic-glasses. Notches fabricated by saw cutting were introduced into the arch ribs to simulate the damage. Dynamic experiments were executed on the undamaged and damaged model. Dynamic responses (structural frequency, mode shape, dynamic strain) of different damaged models were achieved. Numerical simulation was performed by ANSYS software. Sensitivity analyses of the dynamic signature to the damage location were investigated. Relation between structural dynamic signature and the damage was discussed. It is shown that the lst-order horizontal frequency, vertical mode shape and the dynamic strain are sensitive to the damage on the arch rib, and the structural frequency is more sensitive to the damage on 1/8-1/4 of the arch rib span than the damage on other locations, and the structural mode shape is more sensitive to the mid-span damage. According to equivalent model of asymmetrical magnetic coupled resonant wireless power transmission system, a view was pointed out that magnetic coupled resonant wireless power transmission which could maintain long distance transmission at resonance frequency was a special case of magnetic coupled inductive wireless power transmission. Methods of restraining frequency splitting were given, namely changing offset angle and radial distance of axial direction of transmitting coil and receiving coil to reduce mutual inductance coefficient and improve peak voltage of the receiving coil. Finally, experimental platform of asymmetrical magnetic coupled resonant wireless power transmission system was built and the experimental results verified correctness of theoretical analysis.

In view of the problem that transmission efficiency of small wireless charging devices changes with distance between transmitter and receiver, relevant factors of frequency splitting were analyzed according to mathematic model of wireless energy transmission system. Under the condition that resonant frequency and impedance parameters do not change when frequency splitting occurs, mutual inductance of two resonance coils can be reduced by adjusting radial distance of the two coils, so as to inhibiting frequency splitting phenomenon and improve transmission efficiency. Finally, experiments platform of wireless power transmission system of small size resonator was set up, and the experimental results show that the system can successfully charge the load with good transmission performance, which is suitable for small wireless energy transmission equipment.

Objective This study aims to analyze the current clinic treatment using vitamin D for osteoporosis and its relationship with season via measuring the 25-hydroxyvitamin D3 [25 (OH) D3] level in senile patients with osteoporosis. Methods Fifty cases of senile osteoporosis were included in this study (in accordance with the WHO diagnostic criteria for osteoporosis, 1994), in which 28 were males with mean age of 75.6 ± 1.5 years, and 22 were females with mean age of 69. 3 ± 2.6 years. Radioimmunoassay was applied to determine the serum 25 (OH) D3 level in summer and winter, respectively. The content of calcium, phosphorus, alkaline phosphatase, and parathyroid hormone were also measured. Results Among all of the patients, the serum 25 (OH) D3 levels in summer were significantly higher than those in winter (males: 42.5% higher, females: 54.6% higher, P < 0.05). The vitamin D deficiency situation was 13.64% of women in summer and 36. 36% in winter, 7. 14% of men in summer and 35.71% in winter, respectively. Significantly high vitamin D deficiency happened in winter comparing with that in summer (P < 0. 05). The serum calcium, phosphorus, alkaline phosphatase, and parathyroid hormone levels were within the normal range in two groups. Conclusion There are different degrees of vitamin D deficiency in elderly patients with osteoporosis in different seasons. The serum 25(OH)D3 levels in winter were lower. It is recommended of scientific and rational application of vitamin D among the elderly. The serum 25(OH)D3 levels should be regularly monitored as routine laboratory test for osteoporosis treatment, which may provide theoretical basis for reasonable clinical application of vitamin D in the treatment of senile osteoporosis. [Purpose/Significance] Data empowerment for collaborative transformation of the entire industrial chain has become a national strategy. In the context of multi-source and heterogeneous big data, we face higher demands on the diversity and real-time of recommendation services, as well as the standardization and comprehensiveness of user and item information management, in order to turn data into resources, support the diversification of smart recommendation services, and enhance the user interaction experience. To this end, we should systematically sort out and analyze the research results of user interest modeling and item information management technologies in China's big data empowerment, elaborate the technical system for standardized management of user and item information in intelligent recommendation. [Method/Process] The article summarizes the content of 507 documents using literature review methods, and summarizes and analyzes the domestic big data empowerment resource management technology for smart recommendation from two aspects: user interest modeling and item information management. User interest modeling technology includes model representation, model initialization and model evolution, the item information management technology shows the goals, implementation methods, implementation techniques, and methods of item information management, including data collection, data mining, data storage, data updating, and data interpretation. [Results/Conclusions] First, the user interest modeling technology of big data empowerment mainly studies the following issues. 1) The method of model representation is to accurately formalize the user's needs. 2) The model is initialized to collect user information in an implicit or explicit way and select tools and technologies according to data types to pre-process user data according to a fixed process, and select measurement methods to quantify user interest. 3) Model evolution aims to apply methods and technologies to evolve user interest models offline and online. Second, the item information management technology of big data empowerment mainly studies the following issues. 1) The online technology and offline equipment are used to synchronously collect item data across the whole domain, integrate domain knowledge, and use algorithm libraries and data analysis methods to mine item data. 2) The distributed storage is used such as databases and file systems to store item data. 3) Data collection, mining, and storage technologies are the main techniques, item data are updated through offline and online methods, and visualization technology is used to intuitively interpret ietm data. Finally, existing research needs to strengthen the design and practical research of user-driven and data-driven smart recommendation solutions, as well as strengthen research on data security.

This study explored the active ingredients and mechanism of action of Crataegi Fructus in the treatment of coronary heart disease through network pharmacology and molecular docking technology. With the help of TCMSP, Gene Cards, OMIM and other databases to collect target information, the STRING database was used to construct a PPI network diagram, perform GO and KEGG pathway enrichment analysis on the common target, and finally molecularly dock the active ingredient with the core target to initially verify the network pharmacology results. In this study, a total of 6 Crataegi Fructus active ingredients (sitosterol, kaempferol, stigmasterol, quercetin, ent-Epicatechin, isorhamnetin) and 10 Crataegi Fructus core targets for the treatement of coronary heart disease (JUN, AKT1, TNF, MAPK1, TP53, RELA, IL6, MAPK8, MAPK14, EGFR) were screened; KEGG pathway enrichment results showed that Crataegi Fructus prevention and treatment of coronary heart disease pathway involved pathway in cancer, AGE-RAGE signaling pathway in diabetic complications, hepatitis B, MAPK signaling pathway, etc.; Molecular docking results showed that quercetin, isorhamnetin and kaempferol all had good binding to the core target. It is speculated that these components may be the main active components for the treatment of coronary heart disease. This study revealed that Crataegi Fructus may treat coronary heart disease through multiple components (isorhamnetin, kaempferol, quercetin), acting on key targets such as MAPK8, MAPK1, RELA, and regulating multiple signaling pathways such as MAPK. It preliminarily revealed the potential mechanism of Crataegi Fructus in the treatment of coronary heart disease.

In this paper, we explore the cost evolution and technological diffusion of CCS in the policy context by building an economy-environmental endogenous technological model. We find that CCS is difficult to get its long-term cost advantage, as compared to wind and nuclear, and the increasing of fuel cost should be responsible for this; Resource usage of CO2 could hedge against the use cost of CCS to some extent, while only the fuel cost is relatively stable, would this hedge effect be significant; The effect of CO2 resource usage on the penetration of CCS heavily depends on the CO2 amount used as resource, the introduction of subsidy policy will significantly improve this effect and promote CCS diffuse earlier. The accident probability calculation of the gas fields with high sulfur content, high pressure, and high yield (3-highs) has been the focal and difficult point in risk assessment for gas well drilling accidents in China and abroad. In order to calculate the accident rate reasonably, the paper used the fault tree method and Bayesian updating to research the accident probability of the gas well drilling with 3-highs. Firstly, the paper described the mechanism of the accidents using fault tree analysis (FTA), then made use of Bayesian method to update existing statistic data for reducing uncertainty, and got the probability distributions of the top event and related incidents with Monte Carlo simulation finally. Empirical results show that the method could effectively reduce the uncertainty arisen from limited sample data and judge difference from experts, and could provide reference for quantitative risk assessment of the gas well drilling with 3-highs. Huntington's disease (HD) is a kind of inherited neurodegenerative disorder characterized by movement problems, cognitive decline and psychiatry disturbance. HD is caused by mutation in gene IT ⁃15 involving the expansion of a trinucleotide (CAG) repeat encoding glutamine, which leads to abnormal conformation of huntingtin (Htt) protein and finally emerge cytotoxic functions. Currently, HD remains a fatal untreatable disease. Gene therapy for HD discussed in this review is under preclinical studies. Silencing of mutant IT ⁃15 via RNA interference (RNAi) or antisense oligonucleotide (ASO) has shown some effectiveness in mouse model studies. Increasing the clearance of mutant Htt protein could be achieved by viral⁃mediated delivery of anti⁃Htt intrabodies (iAbs) or induction of autophagy, and beneficial results have been observed. Ectopic expression of neurotrophic factors, such as nerve growth factor (NGF) and brain ⁃ derived neurotrophic factor (BDNF), mediated either by viral vectors or transplantation of genetically modified cells, has also been proved to be effective. Other gene⁃modifying methods aiming at correction of transcriptional dysregulation by histone modification, activation of endogenous neural stem cells, and normalization of calcium signaling and mitochondrial function, are also under intensive research. The nervous, endocrine and immune systems are interconnected through neurotransmitters, endocrine hormones and cytokines, forming a neuro-endocrine-immune (NEI) network that outputs effect information to target organs in a multi-level and multi-system manner. The effects of acupuncture can be amplified through the NEI network, causing local microenvironment responses at acupoints to treat post-stroke depression (PSD). This paper systematically elucidates the mechanisms of acupuncture treatment for PSD from three aspects: nervous, endocrine and immune, and reveals the potential mechanisms and biological pathways of acupuncture treatment for PSD from an overall perspective based on the NEI network, aiming to provide a reliable theoretical basis for fundamental research on acupuncture treatment for PSD.

Professor LIU Cunzhi 's team from Beijing University of Chinese Medicine published Efficacy of intensive acupuncture versus sham acupuncture in knee osteoarthritis: a randomized controlled trial in Arthritis & Rheumatology on November 10th, 2021, which demonstrates that three-session per week acupuncture is safe and effective for knee osteoarthritis patients. Experts from home and abroad discussed in depth the study design, acupuncture protocol, and interpretation of the results of the trial, emphasizing the importance of pretrial implementation, acupuncture dosage, reasonable setting of control group and assessing the efficacy of acupuncture, and pointed out that the mechanism of acupuncture for knee osteoarthritis still needs further study, and how to promote acupuncture for knee osteoarthritis according to the clinical practice abroad while ensuring the efficacy of acupuncture is worthwhile to explore.

The finite element method (FEM) is a mathematical method for obtaining approximate solutions to a wide variety of engineering problems. With the development of computer technology, it is gradually applied to the study of biomechanics of human body. The application of the combination of FEM and biomechanics in exploring the relationship between vascular injury and disease, and pathological mechanisms will be a technological innovation for traditional forensic medicine. This paper reviews the construction and development of human vascular FEM modeling, and its research progress on the vascular biomechanics. This paper also looks to the application prospects of FEM modeling in forensic pathology.

Objective: To explore and compare the safety depths of perpendicular and oblique acupuncture at Yamen (GV 15) in patients with atlantoaxial dislocation (AAD) and healthy volunteers., Methods: One bundred and seventy-seven patients with atlantoaxial dislocation were selected as an AAD group, and 207 patients without atlantoaxial dislocation and with normal anatomical structure were selected as a normal group. All participants were moderately sized. The MRI scanning of the cervical vertebra was performed, and the safety depth of perpendicular and oblique acupuncture at Yamen (GV 15) was calculated on the sagittal image., Results: In the AAD group, the safety depth of men was (45.33±5.17) mm for perpendicular acupuncture and (48.58±4.41) mm for oblique acupuncture; the safety depth of women was (44.17±7.80) mm for perpendicular acupuncture and (47.49±7.32) mm for oblique acupuncture. In the normal group, the safety depth of men was (47.72±5.06) mm for perpendicular acupuncture and (42.69±5.53) mm for oblique acupuncture; the safety depth of women was (44.63±5.85) mm for perpendicular acupuncture and (39.88±6.18) mm for oblique acupuncture. The safety depth of men and women for oblique acupuncture was longer than that for perpendicular acupuncture in the AAD group ( P <0.01); the safety depth of men and women for oblique acupuncture was shorter than that for perpendicular acupuncture in the normal group ( P <0.01). The safety depth of perpendicular and oblique acupuncture between men and women was not significant in the AAD group ( P >0.05); the safety depth of perpendicular and oblique acupuncture for men was longer than that for women in the normal group ( P <0.01). For men, the safety depth of perpendicular acupuncture in the AAD group was shorter than that in the normal group ( P <0.01), while the safety depth of oblique acupuncture in the AAD group was longer than that in the normal group ( P <0.01). For women, the safety depth of perpendicular acupuncture in the AAD group was similar with that in the normal group ( P >0.05), while the safety depth of oblique acupuncture in the AAD group was longer than that in the normal group ( P <0.01)., Conclusion: The safe depth of acupuncture at Yamen (GV 15) has significantly changed under AAD, so during the clinical acupuncture the needle insertion should be less than its safe depth.

Objective: To investigate the clinical features and prognosis of acute renal failure (ARF) caused by rhabdomyolysis (RM) in children., Methods: A retrospective analysis was performed for the clinical data, laboratory examination, and prognosis of 26 RM children with ARF., Results: The causes for all 26 RM children with ARF were non-traumatic diseases, and the three most common causes were infection (69%), diabetes (12%), and metabolic disease (8%). In the RM children with ARF, the five most frequent clinical manifestations were fever (69%), multiple organ dysfunction syndrome (69%), convulsion (46%), oliguria or anuria (35%), and tea-colored urine (27%). All 26 children had a serum creatine kinase (CK) level of >1 000 IU/L, among whom 26 had increased aspartate aminotransferase, 25 had increased alanine aminotransferase, 25 had increased creatine kinase isoenzyme, and 23 had increased lactate dehydrogenase. Serum myoglobin (Mb) was measured in 22 children and was found to increase in all these children. The mean time for CK to decrease to below 1 000 IU/L was 10±5 d. There was no significant difference in the time to CK recovery between the 10 children who were treated with conventional treatment as well as continuous venous-venous hemofiltration and those who were not treated with blood purification (P>0.05). Of all 26 RM children with ARF, 7 were withdrawn from the treatment, and 19 had normal renal function after treatment., Conclusions: ARF and multiple organ dysfunction syndrome are major complications in RM children. The major primary disease for RM children with ARF is infectious disease. CK is the major marker for the diagnosis of RM. Early diagnosis and appropriate treatment may reverse ARF and improve prognosis.

Objective: To screen and identify serum biomarkers for childhood hepatoblastoma (HB)., Methods: The serum samples from 30 children with hepatoblastoma (HB), 20 children with systemic inflammatory response syndrome, and 20 normal children were treated with magnetic bead-based weak cation exchange chromatography. The platform of surface-enhanced laser desorption/ionization-time of flight-mass spectrometry (SELDI-TOF-MS) was used to eliminate the interference of inflammatory factors and to screen out the differentially expressed proteins in serum between tumor group and normal group. After the purification and separation of target proteins were performed using sodium dodecyl sulfate-polyacrylamide gel electrophoresis, matrix-assisted laser desorption/ionization-time of flight-mass spectrometry was used to determine their amino acid sequences. The SwissProt database was searched for matched proteins. Finally, real-time PCR and ELISA were used to verify and measure the expression of target proteins., Results: After SELDI-TOF-MS was used for screening and elimination of the interference of inflammatory factors, a differentially expression protein with a mass-to-charge ratio of 9 348 Da was found in serum between HB group and normal group, and the HB group had significantly lower expression of this protein than the normal group (p<0.05). This protein was identified as apolipoprotein A-1 (Apo A-I). Real-time PCR and ELISA verified the low mRNA and protein expression of Apo A-I in serum in the HB group and high expression in serum in the normal group., Conclusions: Apo A-I can be used as a non-inflammatory protein marker for HB and has a certain value in the early diagnosis of HB.

Comprehensive laboratory courses, which enable students to aptly apply theoretic knowledge and master experiment skills, play an important role in the present educational reform of laboratory courses. We utilized human ABO blood type as the experimental subject, and designed the experiment--"Molecular Genotyping of Human ABO Blood Type and Analysis of Population Genetic Equilibrium". In the experiment, DNA in mucosal cells is extracted from students' saliva, and each student's genotype is identified using a series of molecular genetics technologies, including PCR amplification of target fragments, enzymatic digestion, and electrophoretic separation. Then, taking the whole class as an analogous Mendel population, a survey of genotype frequency of ABO blood type is conducted, followed with analyses of various population genetic parameters using Popgene. Through the open laboratory course, students can not only master molecular genetic experimental skills, but also improve their understanding of theoretic knowledge through independent design and optimization of molecular techniques. After five years of research and practice, a stable experimental system of molecular genetics has been established to identify six genotypes of ABO blood types, namely I(A)I(A), I(A)i, I(B)I(B), I(B)i, I(A)I(B) and ii. Laboratory courses of molecular and population genetics have been integrated by calculating the frequencies of the six genotypes and three multiple alleles and testing population genetic equilibrium. The goal of the open laboratory course with independent design and implementation by the students has been achieved. This laboratory course has proved effective and received good reviews from the students. It could be applied as a genetics laboratory course for the biology majors directly, and its ideas and methods could be promoted and applied to other biological laboratory courses.

Objective: To compare the clinical effects of inguinal varicocelectomy, Palomo varicocelectomy,laparoscopic varicocelectomy, and microscopic varicocelectomy in the treatment of varicocele., Methods: We retrospectively analyzed the pre- and post-operative clinical data about 318 cases of varicocele,108 treated by inguinal varicocelectomy,84 by Palomo varicocelectomy,68 by laparoscopic varicocelectomy, and 58 by microscopic varicocelectomy. We compared the operation time, hospital stay, pre- and post-operative sperm concentration and progressive motility, incidence of complications, rate of recurrence, and rate of sperm quality improvement among the four groups., Results: The operation times for the inguinal, Palomo, laparoscopic, and microscopic varicocelectomy procedures were(50. 3 ± 13. 9),(70. 4 ± 14. 3),(35. 1 ± 11. 1),and(65. 3 ± 13. 2) min, respectively, significantly shorter for the laparoscopic strategy than for the other three( P < 0. 05). The lengths of hospital stay of the four groups of patients were(6. 3 ± 1. 6),(5. 7 ± 1. 5),(4. 3 ± 1. 4),and(3. 4 ± 1. 3) d, respectively, remarkably shorter in the microscopic group than in the other three( P < 0. 05). The microscopic group also showed a significantly lower rate of postoperative complications, incidence of spermatic vein reflux, and recurrence( P < 0. 05) and higher rates of improvement in postoperative sperm concentration and progressive motility than the other three groups( P < 0. 05)., Conclusion: Microscopic varicocelectomy is superior to inguinal,Palomo and laparoscopic varicocelectomy procedures in the treatment of varicocele for its lower incidence of complications, higher rate of sperm quality improvement,and shorter length of hospital stay. It is therefore more suitable to be applied in community hospitals.

Objective: To evaluate the efficacy and safety of cisplatin and capecitabine combination (XP) therapy for patients with metastatic triple negative breast cancer (TNBC) progressing after anthracycline and taxane treatment., Methods: Twenty-nine metastatic TNBC patients were prospectively enrolled to receive capecitabine (1, 000 mg/m(2) twice daily on days 1-14) and cisplatin (75 mg/m(2) on day 1) , repeated every 3 weeks., Results: With a median of 6 cycles of XP, all 29 patients were evaluable for response, including 18 PR (62.1%), 6 SD (20.7%), 5 PD (17.2%) and no CR. The response rate was 62.1%. Patients with earlier stage at diagnosis (stage I to IIIA), longer post-operative disease free survival (>2 years) and less metastatic sites (≤ 3) obtained significantly higher response rate than patients with later stage at diagnosis (stage IIIB to IV), shorter post-operative disease free survival (≤ 2 years) and more metastatic sites (>3). The leading side effects were grade 1/2 gastrointestinal and hematological toxicities. Grade 3/4 toxicities included neutropenia (34.5%), leukocytopenia (31.0%), anemia (6.9%), thrombocytopenia (3.4%), nausea/vomiting (20.7%), stomatitis (3.4%), and hand-foot syndrome (3.4%)., Conclusion: Cisplatin and capecitabine combination therapy is an active and well-tolerated doublet treatment in metastatic TNBC patients progressing after anthracycline and taxane treatments.

Objective: To assess the efficacy of vinorelbine (NVB)-based regimens in patients with metastatic triple negative breast cancer (mTNBC) pretreated with anthracyclines and taxanes., Methods: Clinical data of 48 patients diagnosed and treated for mTNBC between 2004 and 2012 at the Cancer Hospital, Chinese Academy of Medical Sciences (CAMS) were retrospectively analyzed. All patients were pretreated with anthracyclines and at least one taxane in neo-adjuvant, adjuvant or chemotherapy for mTNBC and patients should be having at least one measurable metastatic lesion. Totally, 48 patients were included in this study, of which 21 cases received first-line chemotherapy and 27 cases received second-line chemotherapy. Based on the regimen they received, 22 patients were treated with NVB plus platinum (NP), and 26 patients with NVB plus capecitabine (NX)., Results: After 70 months follow-up, in the total group of patients, the objective response rate was 20.8%, clinical benefit rate was 43.8%, median progression free survival (PFS) was 4.4 months and median overall survival (OS) was 15.5 months. In addition, the ORR was significantly better in the NP arm versus NX arm (33.8% vs.7.7%, P=0.029) as well as PFS was statistically improved in the NP arm than NX arm (5.3 m vs. 3.0 m, P=0.023). Similar trend was observed in the OS, although the difference was not statistically significant (27.7 m vs. 14.8 m, P=0.077). In all, the most frequently reported adverse events were G1/2 gastrointestinal toxicity (68.8%) and neutropenia (62.5%) . No significant difference was observed between the NP arm and NX arm (P>0.05). The percentage of patients who delayed chemotherapy administration in the NP arm and NX arm was 9.1% (n=2), and 3.8% (n=1), respectively., Conclusions: NVB-based combination chemotherapy demonstrates moderate efficacy in mTNBC patients pretreated with anthracyclines and one taxane with manageable toxicity. NP regimen shows potential superiority over NX regimen, and should be further verified in randomized phase III clinical trial in larger cohort.

Objective: To compare the effect of first-line treatment with platinum-based chemotherapy and non-platinum-based chemotherapy in patients with lung metastases from triple negative breast cancer (TNBC)., Methods: Sixty-five eligible patients were divided into platinum-treated group and non-platinum-treated group according to the first-line therapy. Factors predicting the chemotherapeutic efficacy included overall survival (OS), progression-free survival (PFS) and objective response (OR)., Results: In the platinum-treated group of 32 patients, 2 cases (6.3%) achieved CR, 16 cases (50.0%) achieved PR, 11 (34.4%) cases achieved SD, and 3 patients (9.4%) achieved PD. In the non-platinum-treated group of 33 patients, 2 cases (6.1%) achieved CR, 6 cases (18.2%) achieved PR, 16 cases (48.5%) achieved SD, and 9 cases (27.3%) achieved PD. Median PFS was significantly longer in the platinum-treated group than in the non-platinum-treated group (10 months vs. 6.0 months, P = 0.012), and OS was also improved (32 months vs. 22 months, P = 0.006). Multivariate analysis of several factors including local-regional lymph node involvement, lung metastasis-related symptoms, first-line platinum-based chemotherapy, disease-free interval, size and number of lung lesions, showed that first-line platinum-based chemotherapy was an independent prognostic factor for TNBC patients with lung metastases., Conclusions: Compared with non-platinum-based chemotherapy, the first-line platinum-based chemotherapy can improve PFS and OS in TNBC patients with metastases confined to the lungs.
