BACKGROUND: Peripheral nerves play an important role in bone metabolism. In clinical practice, the specific impact of nerve injury on bone transport technology needs further study. OBJECTIVE: To investigate the effect of tibial nerve injury on the treatment of tibial slip by single-plane osteotomy. METHODS: Thirty-two patients with tibial bone defects admitted to Tangshan Second Hospital from May 2011 to June 2022 were selected. According to the presence or absence of tibial nerve injury, patients were divided into the tibial nerve injury group (n=16) and the non-tibial nerve injury group (n=16). Both groups were treated with single-plane osteotomy and bone slip. After treatment, the patients were followed up to collect the mineralization zone healing index, external fixation index, docking point healing and needle infection. After the removal of external fixation, the bone healing and functional evaluation were evaluated by a classification of the Association for the Study and Application of the Method of Ilizarov (ASAMI). RESULTS AND CONCLUSION: (1) All 32 patients were followed up for (25.28±4.79) months. There were no significant differences in bone healing time, external fixation time, healing index and external fixation index between the two groups (P > 0.05). Needle infection occurred in two cases of the tibial nerve injury group and one case of the non-tibial nerve injury group, all of which were PALEY I, and there was no significant difference between the two groups (P > 0.05). The non-union rate of the occlusal end of the tibial nerve injury group was 31%, and that of the non-tibial nerve injury group was 13%; there was no statistical difference between the two groups (P > 0.05). The excellent and good rate of ASAMI bone healing score in the two groups was 100%; the excellent and good rate of limb score was 81% in the tibial nerve injury group and 94% in the non-tibial nerve injury group; there was no statistical difference between the two groups (P > 0.05). (2) Our research shows that tibial nerve injury has no significant effect on the mineralization speed, external fixation time, union of the occlusal end, infection of the needle tract, and the quality of bone formation in the mineralized area of the single-plane osteotomy. Objective To explore the clinical efficacy of oral low-dose tadalafil combined with pelvic floor muscle rehabilitation training in the patients with erectile dysfunction caused by pelvic fracture and urethral injury after bone repositioning and fixation. Methods From September 2018 to September 2022, 110 male patients with erectile dysfunction caused by pelvic fracture and urethral injury admitted to our hospital were selected as the research subjects. All patients received bone repositioning and fixation surgery, and were divided into an observation group and a control group of 55 patients according to the intervention measures they received after surgery. The control group received oral low-dose tadalafil treatment after surgery. On the basis of the control group, the observation group received pelvic floor muscle group rehabilitation training treatment. After continuous intervention for 1 month, the curative effects of the two groups were evaluated, and the IIEF-5 score and NPT were comparatively analyzed between the two groups. Results No significant differences were found in the scores of IIEF-5 between the two groups before intervention (P > 0. 05), whereas IIEF-5 scores of the two groups of patients were all significantly improved after intervention, moreover IIEF-5 scores in the observation group increased more significantly (P <0.05). After intervention, the intensity, duration and frequency of erection in NPT indicators in the two groups were obviously improved, moreover the observation group showed better improvement (P < 0. 05 ). No serious adverse reactions in two groups during the treatment were observed ( P > 0. 05 ). Conclusion For the patients with organic erectile dysfunction caused by pelvic fractures and urethral injury after low-dose tadalafil combined with pelvic floor muscle rehabilitation training can effectively improve the erectile function of patients. BACKGROUND: Bio-oss bone powder and Aobang repair materials are commonly used bone repair materials, but there are few reports on their systematic evaluation of the repair effect of rabbit femoral defect. OBJECTIVE: To systematically evaluate the repair effects of Bio-oss bone powder and Aobang repair materials on femoral defects by using synchrotronradiation-based micro-computed tomography and histopathological staining. METHODS: Thirty-two rabbits were randomly divided into the control group (n=12) and the experimental group (n=20) by the random number table method. Two holes with a size of 4 mm (the diameter of the cavity) × 6 mm (hole depth) of penetrating periosteal bone defect were established on the lateral surface of one side of the femoral body. The control group was filled with normal saline. In the experimental group, Bio-oss bone powder was implanted in one of the bone defects, and Aobang repair material was implanted in the other bone defect. After 8 weeks, the effects of the two kinds of bone repair materials were evaluated by visual observation, imaging analysis and histopathological examination. RESULTS AND CONCLUSION: (1) General view: 8 weeks after the operation, the wounds in the repaired area of bone injury in the experimental group healed well; no infection, suppuration or other symptoms were found, and the animal mobility was significantly better than that of the control group. (2) Imaging examination: synchrotron-radiation-based micro-computed tomography 8 weeks after the operation exhibited that after the implantation of Bio-oss bone powder, there was a large amount of new bone tissue around the bone powder, and the cavity of the bone defect was large, which was not completely filled by the new bone tissue. After the implantation of the Aobang repair material, a large amount of new bone tissue at the bone defect cavity was formed around the repair material. 3D reconstruction showed that both bone filling materials could contact with local bone wound tissue, promote the surrounding osteogenesis to differentiate into new bone tissue and blood vessel formation, and achieve bone repair effect. The new bone around the Aobang repair material was more closely connected with the surrounding bone tissue. 256 iCT scan showed that the CT value of the Aobang repair material side at 8 weeks after operation was higher than that of the Bio-oss bone powder side (P < 0.05). (3) Histopathology: At 8 weeks after operation, hematoxylin-eosin staining showed that new bone tissue could be seen around the side of the Bio-oss bone powder. Bone tissue formed by bone matrix could be seen around the Aobang repair material; the new bone tissue was complete and coherent, and some of the new bone had invaded into the Aobang repair material. (4) These results suggest that Aobang repair material can promote the formation of new bone and damage repair; synchrotron-radiation-based micro-computed tomography echoes the traditional hematoxylin-eosin staining, and it is a good method to evaluate the effect of bone powder repair. Objective: To investigate the effect of iRoot BP Plus on the pulp revascularization of the replanted molars in rats and its influence on VEGF. Methods: Taking 60 male Wistar rats aged 3 weeks as the research object, they were randomly divided into 3 groups with 20 rats in each group, namely the blank group, the control group and the study group. After the reimplantation model was established, pulp revascularization was performed. The control group used MTA to cover the entire clot surface, the study group used iRoot-BP Plus, and the blank group did not cover the surface. The rats were sacrificed 4 weeks after the operation, and imaging, immunohistochemical staining and relative expression of VEGF gene were detected. Results: Compared with the blank group, both the control group and the study group had different degrees of root redevelopment, the root apex enlarged, the apical foramen narrowed, the scope of infection and periapical bone injury decreased, the root canal cavity was narrowed, and the wall thickened. In the control group, the range of apical inflammation was reduced, the destruction of bone tissue did not completely disappear, and there was atypical calcification in the apex of the tooth. In the study group, inflammation and destruction of bone tissue disappeared, and there was no obvious proliferation and abnormal shape of the apical morphology. Compared with the blank group, the MOD value of Nrf2 and bFGF expression in the control group and the study group increased significantly. Compared with the control group, the MOD value of Nrf2 and bFGF expression in the study group increased significantly (P<0.05). Compared with the blank group, the relative expression of TNF-α gene in the control group and the study group were significantly reduced, and the relative expression of VEGF gene was significantly increased (P<0.05); Compared with the control group, the relative expression of TNF-α gene in the study group was significantly reduced, and the relative expression of VEGF gene was significantly increased (P<0.05). BACKGROUND: Cartilage defects due to cartilage lesions such as osteoarthritis are difficult to self-regenerate. How to promote cartilage regeneration and restore smooth wound surface is a hot topic in recent years. OBJECTIVE: To review the general concept, mechanism and effect of Kartogenin as well as the current clinical models, providing new ideas for the regeneration and repair of damaged cartilage. METHODS: WanFang, CNKI, and PubMed were retrieved for studies on Kartogenin combined with tissue engineering in the field of cartilage regeneration and repair published from January 2010 to January 2020. The keywords were “Kartogenin, cartilage regeneration, chondrogenesis, chondroprotection” in English and “Kartogenin, cartilage regeneration and repair, cartilage protection, tissue engineering" in Chinese. After initial screening by reading titles and abstracts, irrelevant literature was excluded and finally 55 articles were included for result analysis according to the inclusion criteria and exclusion criteria. RESULTS AND CONCLUSION: As a newly discovered small molecule, Kartogenin has better stability, lower immunogenicity and easy availability. In addition, Kartogenin plays a synergistic role with growth factors in cartilage regeneration and has great potential in cartilage formation and cartilage protection. Through in vitro experiments or animal experiments, it has been proved that Kartogenin plays a positive role in promoting cartilage generation, osteoarthritis treatment, repair of tendon and bone injury, and wound healing, and helps to protect the degeneration of cartilage and healing of rotator cuff injury. BACKGROUND: In the treatment of bone defect or bone injury by tissue engineering, biomaterials affect the survival rate, proliferation and differentiation of bone marrow mesenchymal stem cells. OBJECTIVE: To review the research progress regarding how biomaterials affect the proliferation and osteogenic differentiation of bone marrow mesenchymal stem cells, and to guide the rational application of biomaterials. METHODS: A computer-based online search of CNKI, PubMed, Web of Science, and Wanfang database with the search terms "Bone marrow mesenchymal stem cells, Osteogenic differentiation, Biological materials, The microstructure". The eligible literatures regarding the effects of different biomaterials on the proliferation and osteogenic differentiation of bone marrow mesenchymal stem cells were included. RESULTS AND CONCLUSION:(1) Metallic materials have the advantages of good biocompatibility, bone conductivity, and mechanical performance. Non-metallic materials exhibit good biocompatibility, bone conductivity, reabsorption, and three-dimensional shaping.(2) There are many factors that affect the surface microstructure of the biomaterial. As for the same biomaterial, greater surface energy/wettability leads to better proliferation and osteogenic differentiation of bone marrow mesenchymal stem cells; greater roughness better promotes the proliferation, adhesion, and differentiation of bone marrow mesenchymal stem cells; larger pore diameter and lower pore diameter rate are more prone to promote the osteogenic differentiation of bone marrow mesenchymal stem cells; greater substrate rigidity and elastic modulus better facilitate the osteogenic differentiation of bone marrow mesenchymal stem cells. The abovementioned factors of the biomaterials affect the proliferation and osteogenic differentiation of bone marrow mesenchymal stem cells. Objective To evaluate the diagnostic value of dual energy CT (DECT) with virtual noncalcium (VNCa) imaging in occult knee fractures. Methods Thirty patients with clinical suspicion of occult knee fracture based on history of trauma and knee movement disorder underwent DECT and MRI of the knee. Each knee was divided into 12 zones of distal femur (6) and proximal tibia (6). Two radiologists independently evaluated the VNCa images for the presence of hone marrow injury. On the coronal image, VNCa CT values of normal and injured bone marrow' were measured, and the difference between the two CT values w'as calculated. MR images w'ere used as the reference standard to evaluate the ability of subjective evaluation and CT difference to detect occult fractures of the knee joint. The sensitivity and specificity of the two methods for bone marrow injury were evaluated by receiver operating characteristic (ROC) curve. Kappa values were used to test the consistency of diagnosis of bone marrow injury by two radiologists. Results There were 480 zones in 40 knee joints. MRI showed bone marrow injury in 74 zones of the distal femur and 166 zones of the proximal tibia. The VNCa diagram showed hone marrow injury in 77 zones of the distal femur and 171 zones of the proximal tibia. The VNCa difference showed bone marrow injury in 76 zones of the distal femur and 174 zones of the proximal tibia. The subjective evaluation (k=0.829) and CT values (k=0.867) in VNCa images were consistent with MRI. The area under the ROC curve (0.876), sensitivity (92.1%), specificity (89.3%) and accuracy (87.6%) of subjective evaluation were comparable to that of CT difference evaluation (0.885, 90.3%, 92.4%, 93.1%). Conclusion DECT and VNCa imaging has excellent diagnostic performance and high consistency with MRI for evaluating occult knee fractures. It can aid clinical diagnosis of occult fractures. Objective: To explore and analyze the current status and causes of medical device-related pressure injuries(MDR PIs)in 7 Class Ⅲ Grade A hospitals in Hainan Province, and put forward corresponding intervention measures. Methods: Using the survey tools in Europe and the United States, a cross-sectional survey of inpatients in 7 Class Ⅲ Grade A hospitals was conducted by trained qualified investigators, and the data was statistically analyzed using SPSS 17.0 software. Results: Among the 7248 hospitalized patients,there were 30 cases of pressure injuries caused by medical devices, and the prevalence rate was 0.41 %, including 18 cases of medical device-related pressure injuries, the prevalence rate was 0.25 %, and 12 cases of mucosal membrane pressure injuries, which prevalence rate was 0.17 %. 83.33 % of the medical device-related pressure injuries were acquired by the hospital, and 82.22 % of the pressure injuries at the bone eminence were present on admission; Patients with intensive care and enteral nutrition are more likely to get pressure injuries from medical devices(P< 0.05). Nurses will be more conscious about using decompression dressings for patients with pressure injuries at the bone eminence(P< 0.05); Nasogastric tube(51.60 %) was the most common cause of medical device-related pressure injuries, followed by oxygen inhalation tube(19.35 %). Mucosal membrane pressure injury could not be staged according to the system,accounting for 38.71 %. Medical device-related pressure injuries mainly concentrated in stage 1 and stage 2, accounting for 51.61 %. The ear(22.58 %) was the most common site of medical device-related pressure injuries, while the nasal mucosa(35.47 %) was the most common site of mucosal membrane pressure injuries. Conclusions: Managers and clinical nurses of Class Ⅲ Grade A hospitals in Hainan Province have already recognized and actively prevented such pressure injuries, but they still need to further strengthen training and regulation, organize regular investigation and continuous monitoring, and build a Continuous care model; for departments, people and specific parts of the relatively high risk of medical device-related pressure injuries, early prevention, identification and protection should be provided. Medical devices should be used correctly, preventive medical device-related pressure injuries should be reduced proactively;At the same time, we actively pay attention to the main causes, sites, stages and progress of medical device-related pressure injuries, and adopt comprehensive intervention measures to establish an organizational structure for pressure injuries early warning management to form a cluster management. Objective To investigate the clinical significance of bone mineral density (BMD), peak bone density (PBD), and the rate of bone loss (RBL) in normal people of Zhuang and Han nationalities of Nanning area. Methods BMD of the lumbar vertebrae L1-L4 and the left femoral neck was measured in 20899 cases of normal people in Nanning area by using dual energy Xray absorptiometry (DAX, Hologic, USA). Objective To observe medium and long-term quality of life of non-surgical treatment and vertebral plasty in patients with osteoporotic vertebral compression fractures. Methods A retrospective study was performed. A total of 108 patients with osteoporotic vertebral compression fractures, who were hospitalized in Guangdong Provincial Hospital of TCM from January 2006 to December 2006, were selected, including 55 patients with non-operative treatment and 53 patients received vertebral plasty. A SF-36 scale and VAS scale were used, in order to compare the quality of life after non-operative treatment or vertebral plasty for more than 3 years after the treatment. The incidence of re fractures was observed. Results The average follow-up was 50. 59 months (36-69 months). According to the SF-36 scale, the physiological function, physical functioning, and role-emotional were better in non-surgical group than those in vertebral plasty group (P < 0. 05). In other dimensions, no significant difference was observed. The VAS score in non-surgical group was also better than that in vertebral plasty group (P < 0. 05 ). Conclusion Considering the medium and long-term quality of life and pain relief, the efficacy in non-surgical treatment group is superior to that in the vertebral plasty group, and the incidence of re fractures is much lower, which indicates that strictly observing the surgical indication is important in clinical work. Objective To analyze the related risk factors of osteoporotic fractures in 98 patients, aiming at taking appropriate preventive measures at the early stage, in order to prevent the complication of the fractures and to improve the quality of life. Methods A total of 98 patients with senile osteoporotic fractures, who were hospitalized in our hospital, aging from 50 to 89 years old, were selected. Their age, status during fractures, and the related risk factors of fractures were all analyzed. Results The occurrence of osteoporotic fractures was more easily observed in the femoral neck, the vertebral body, and the distal radius than in other location. The fractures in women accounted for 63. 27%, and 36. 73% for men. Accidental falling was the main risk factor of the senile osteoporotic fractures. Conclusion Due to the increased bone fragility in patients with osteoporosis, when subjected to minor external impact or vibration, the fractures may occur. Therefore, in view of the risk factors of senile osteoporotic fractures, effective preventive measures should be taken, in order to reduce the incidence of osteoporotic fractures. Purpose: The purpose of this study is to report on the short term follow-up of patients who underwent arthroscopic acetabular labral repair for femoroacetabular impingement and acetabular labral tear.. Materials and Methods: A total of 45 consecutive patients who underwent arthroscopic acetabular labral repair after diagnosis of femoroacetabular impingement and acetabular labral tear from January 2008 to December 2010 were included in this study. Modified Harris hip score (MHHS), VAS, Hip outcomes score (HOS), and patient satisfaction were used for evaluation of the clinical results. Results: Patients included 22 males and 23 females, and the mean age of the patients was 33.0(range, 16-54) years old, and the mean follow up period was 26.7(range, 24-56) months. Of the clinical results, mean VAS score was 6.4 and 2.5 points before and after surgery, respectively, and mean MHHS score improved from 59.5 points before surgery to 85.4 points after surgery. Activities of daily living and sports-related activities of HOS were 58.3% and 51.2%, respectively, before surgery, and 83.0% and 79.8% after surgery. Revision arthroscopic surgery was performed on five cases(12.1%); labro-synovial adhesion, three cases, pull-out of suture anchor, one case, heterotropic ossification, one case. Conclusion: Arthroscopic acetabular labral repair is considered an effective treatment for femoroacetabular impingement and accompanying acetabular labral tear. [ABSTRACT FROM AUTHOR]

PURPOSE: To study the effect of collagen sponge implantation in tooth socket on bone healing. METHODS: Twenty adult New Zealand white rabbits were used in this study. Animal models with tooth extraction were established. In the experimental group, the collagen sponge were embedded in the sockets of left side. In the control group, nothing was embedded in the sockets of right side. The rabbits were sacrificed and the specimens were obtained 1, 2, 4, 8 and 12 weeks after surgery for bone density examination and histological observation, in order to evaluate bone healing of tooth socket. Statistical analysis were conducted using SPSS 12.0 software package. RESULTS: 1 week, 2 weeks, 4 weeks and 8 weeks after surgery, the bone density of the experimental group was significantly higher than that of the control group. 12 weeks after surgery, no significant difference was observed between the two groups. PURPOSE: The study aims to evaluate the value of panoramic films in diagnosis of condylar fractures. METHODS: Panoramic films of 163 cases (220 joints) diagnosed as condylar fractures by CT were examined. Panoramic films of all types of condylar fractures classified by CT were analyzed. According to the position of fracture, the samples were divided into two groups, intracapsular fractures and extracapsular fractures(condylar neck fractures and subcondylar fractures). The detection of fractures in two groups were compared and statistically analyzed with SAS6.12 software package. RESULTS: All 163 condylar fractures (220 joints) could be classified by CT as intracapsular fractures (155 joints), neck fractures (45 joints) and subcondylar fractures (20 joints). Panoramic films showed 38 joints without fractures and 182 joints with fractures. Among them, 33 joints without fractures and 122 joints with fractures were in the group of intracapsular fractures. The detection rate of intracapsular fractures group was significantly lower than that of condylar neck fractures and subcondylar fractures group (P=0.0149). CONCLUSION: Panoramic films can initially diagnose condylar fractures, but the misdiagnosis rate is high for intracapsular fractures, particularly for minor fractures and fractures without fragment displacement. CT should be considered for positioning of fracture lines and confirming the displacement angle of fragment on coronal plane. We examined 200 stress fractures in lower limbs including the pelvis in our hospital. The stress fractures occurred in all bones, but occurred mostly in the tibia. As a whole, diagnosis by X-ray was possible earlier than previously claimed. The rate of X-ray diagnosis by each week from onset was different and the direction of callus formation was characterized by each affected bone. For early diagnosis, it is necessary to take X-ray photographs repeatedly from the appropriate direction. The fracture sites of the tibia in long-distance runners are usually said likely to occur in the proximal and distal third and are rare in the mid third. But actually those were widely distributed proximally to distally including the middle third. Fibular stress fractures occurred in the proximal third or the distal third. Distal type fibular stress fractures occurred a lot in the distal seventh of the fibula more than in the distal third as previously claimed. The concept of the height in the tibia and fibula of the runner's stress fracture was wrong and should be corrected. Fracture sites were revealed by fracture line, edema of bone marrow, periosteal thickening or elevation by MRI. Edema of bone marrow was sometimes detected by STIR image alone. Objective The aim of this study was to investigate the inhibition of different doses of fluvastatin in SPC-A-1 lung adenocarcinoma nude mice bone destruction. Methods 100 nude mice age range of 6 w to 8 w (20-22 g) were treated with zoledronic acid, saline and different doses of fluvastatin, and the inhibition of bone destruction was studied. The data were analyzed using SPSS19.0 statistical software. Results 75 nude mice were valid. In high dose group of fluvastatin, the number of aggravated nude mice bone destruction was the least, followed by zoledronic acid, moderate dose and low dose groups of fluvastatin. Conclusion Fluvastatin has inhibitory effect on bone destruction, and high dose group is better than those of zoledronic acid group, middle dose group, and low-dose fluvastatin group. Deep vein thrombosis (DVT) is a serious complication of fractures. The incidence of ankle fracture which occurs most frequently intra-articular is 200/100 000, about 3.9% of all fractures. Domestic and foreign reports of ankle fractures and DVT were allocated and analyzed to review their correlation. Furthermore, different methods of routined thromboprophylaxis was discussed. DVT incident after ankle fractures and the necessity of routined thromboprophylaxis were reviewed. DVT incidence after ankle fractures was low and routine thromboprophylaxis was not necessary to all ankle fracture patients except the ones who had risk factors of thrombosis which could induce DVT. PURPOSE: To evaluate the efficacy of T-shaped splitting in removal of mandibular third molar with mesioangular and horizontal impaction. METHODS: Sixty-two patients who required removal of mandibular third molar with mesioangular and horizontal impaction from October 2008 to December 2009,in the Department of Oral and Maxillofacial Surgery, Sun Yat-Sen Memorial Hospital, Sun Yat-Sen University were collected. All patients were extracted the impacted mandibular third molar with T-shaped splitting. RESULTS: The operation time last from 8 to 35 minutes. In all patients, 5 developed fractures of the lingual plate, but these plates were reserved after reduction. 6 patients had significant buccal swelling 24 to 48 hours after operation. 5 cases were with minor pain, and 2 cases with limited mouth opening. Only one patient was complicated by dry socket and one with temporally numbness of the lower lip. Pneumoderma and bone fracture did not occur in all patients. CONCLUSION: The method of T-shaped splitting in removal of mandibular third molar with mesioangular and horizontal impaction is safe and with fewer complications.
