Objective To analyze the distribution characteristics of unexpected antibodies in Duffy blood group system and to explore the blood transfusion strategy of patients.Methods The screening and identification data of accidental antibodies in patients from January 2012 to December 2022 in the General Hospital of Western Theater Command PLA were collected.The Duffy phenotype of red blood cells in patients with Duffy blood group was detected.The causes of antibodies were analyzed by disease history, pregnancy history and blood transfusion history, and the blood transfusion strategy was discussed.Results Accidental antibodies were detected in 2 128 (0.6%) of 351 957 patients, and in 34 cases (0.009 7%) of Duffy blood group accidental antibodies, including 26 cases (76.5%) of anti-Fyb, three cases (8.8%) of anti-Fya, three cases (8.8%) of anti-Fyb+anti-E, 1 case (2.9%) of anti-Fyb+anti-Lea, one case (2.9%) of anti-Fyb+anti-E+ anti-c, 33 cases (97.1%) of immunoglobulin G (IgG) antibody, and one case (2.9%) of IgG + IgM antibody. It is mainly produced by immune stimulation of pregnancy or blood transfusion.Among the 34 patients with unexpected antibody of Duffy blood group, two patients were infused with autologous blood (4 U), six patients were infused with allogeneic blood, and the same ABO and Rh blood groups were transfused.The corresponding antigen-negative suspended red blood cells of Duffy antibody were 36.5 U, and there were no adverse reactions of blood transfusion.33 patients were improved and discharged, and one patient gave up treatment due to multiple organ failure.Conclusion Unexpected antibodies of Duffy blood group system are rare, which can lead to hemolytic transfusion reaction and hemolytic disease of the newborn, mainly due to pregnancy or immune stimulation of blood transfusion.The detection rate of anti-Fya is much lower than that of anti-Fyb. Objective To analyze the thromboelastogram(TEG) and coagulation indexs of different stages of cirrhosis, and to understand the correlation between the TEG and coagulation indexs in different stages of cirrhosis. Methods A retrospective analysis was conducted on 299 patients diagnosed with liver cirrhosis from January to December 2020.The patients were divided into two groups according to their different conditions: the compensatory cirrhosis group(82 cases) and the decompensated cirrhosis group(217 cases).And 100 healthy people were selected as the control group. Hemoglobin(Hb), platelet count(PLT),routine coagulation parameters and TEG parameters [coagulation comprehensive index(CI-value), blood clot formation time(Kvalue), blood clot formation rate(Angle), the maximum amplitude of blood clot(MA-value),and blood clot mechanical strength(G-value)] were compared in each group. The correlation between TEG and routine coagulation parameters in patients with liver cirrhosis were analyzed. Results The Hb, PLT, Angle, MA, G and CI values of patients with liver cirrhosis were lower than those of the control group(P <0.05), prothrombin time (PT), activated partial thromboplastin time(APTT), thrombin time(TT), international standardized ratio (INR), fibrinogen(FIB) and K-value of patients with liver cirrhosis were higher than those in the control group, and the differences were statistically significant(P <0.05). Hb, PLT, APTT, INR, and FIB in the de-compensated cirrhosis group were all lower than those in the compensatory cirrhosis group, and the differences were statistically significant(P <0.05).The R, K-values: the decompensated cirrhosis group > the cirrhosis group > the compensatory cirrhosis group > the control group, the differences were statistically significant (P <0.05).The Angle, MA, G, CI values: the decompensated cirrhosis group

OBJECTIVE:To optimize the preparation conditions of Liaoyuanqili controlled release pellets . METHODS: HPLC method was adopted , to set the accumulative release rates within 1.0h. of Loureirin B, Hydroxysaffior yellow A , Notoginsenoside R1 and rerulic acid as review indicators , the preparation technology was optimized by homogeneous design experiments .RESULTS:The optimized preparation conditions were identified as 2%HPMC, extrusion speed 20 Hz, spheronization speed 50 Hz, 2%surelease, controlled-release layer with 5%(weight) coating level.CONCLUSIONS: Studies on release process in vitro reveal that the release of Liaoyuanqili controlled release pellets has been retarded significantly and the release process follows the zero -order kinetics.
