Objective To explore the mechanism of Guominjian on atopic dermatitis(AD) based on net- work pharmacology and molecular docking techniques. Methods The active ingredients and corresponding targets of Guominjian were retrieved from TCMSP database, and AD disease related targets were obtained from OMIM, GeneCards and DrugBank database. Cytoscape3. 7. 2 software was used to construct the Chinese medicine component target network and STRING platform for protein interaction network analysis. GO function and KEGG signal pathway database were used for enrichment analysis, active ingredient-target-pathway network was constructed, and molecular docking was performed with AutoDock Vina software. Results There were 123 common targets of Guominjian and AD. GO functional enrichment analysis involved 641 items of biological processes, 68 items of cell components and 129 items of molecular functions. The signaling pathways including MAPK, NF-κB, TNF-α, JAK-STAT and PI3K-Akt, etc. were selected by enrichment analysis of KEGG signaling pathways. The results of molecular docking showed that quercetin and kaempferol had high affinity with FOS, TNF, TGFBI, HIF-1A and IL-1A. Objective To evaluate the therapeutic effect of evodiamine (Evo) on atopic dermatitis (AD) in rat models by regulating the cyclic adenosine monophosphate (cAMP)/protein kinase A (PKA)/cAMP response element binding protein (CREB) signaling pathway. Methods A rat model of AD was established by administration of multiple doses of 2,4-dinitrochlorobenzene (DNCB).The animals were randomly divided into AD group, Evo-low-dose (Evo-L, 5 mg/kg) group, Evo-high-dose (Evo-H, 10 mg/kg) group, Evo-H+H-89 (5 mg/kg) group and dexamethasone (0.1 mg/kg) group. Normal rats were used as the control group and then the degree of skin damage of rats in each group was scored. Abdominal blood was taken to detect the levels of interleukin-4 (IL-4), cAMP, and tumor necrosis factor-α(TNF-α) in serum; Skin lesion tissue was collected to detect pathological change, counting of mast cells, PKA/CREB related protein expression and expression of IL-4 and TNF-α mRNA in the tissue. Results Compared with control group, the level of cAMP in serum, the expression of p-PKA/PKA, and p-CREB/CREB in skin lesions of AD group were reduced, the severity score of skin lesions, level of IL-4 and TNF-α, epidermal thickness, number of mast cells and mRNA expression of IL-4 and TNF-α in skin lesion tissues were all significantly increased (P＜0.05). Compared with AD group, the level of cAMP in serum, the expression of p-PKA/PKA, and p-CREB/CREB in skin lesions in Evo-L group, Evo-H group, and dexamethasone group were increased, the severity score of skin lesions, level of IL-4 and TNF-α, epidermal thickness, number of mast cells, and mRNA expression of IL-4 and TNF-α in skin lesion tissues all reduced (P＜0.05). Compared with the Evo-H group, the level of cAMP in serum, the expression of p-PKA/PKA, and p-CREB/CREB in skin lesions in Evo-H+H-89 group was reduced and the severity score of skin lesion, level of IL-4 and TNF-α, epidermal thickness, number of mast cells, and mRNA expression of IL-4 and TNF-α in skin lesion tissues significantly increased(P＜0.05). Conclusions Evo inhibits inflammatory response and pathological damage through regulation of cAMP/PKA/CREB signaling pathway in AD rat models.

Objective To investigate the differences in gut flora between infants and children aged 0 〜3 years with atopic dermatitis (AD) and healthy infants and children. Methods General information and fecal samples were collected from children with AD aged 0-3 years (AD group) and healthy infants and children (HC group). T Test and Chi 一 Square Tests were used to analyze the general information of infants and children in two groups, and 16S rRNA sequencing technology combined with bioinfbrmatics analysis to compare the diversity and differences of intestinal flora in two groups. Results General information only history of allergy in the immediate family was significantly different between the two groups (丸？= 13. 875, P < 0. 001). The difference in the diversity of Gut microbiota between the two groups was not statistically significant At the gate level, the relative abundance of Verrucomicrobiota was significantly lower in the AD group than in the HC group (P =0. 027), whereas the relative abundance of Fusobacteriota was significantly higher than in the HC group (P = 0. 038). At the genus level, the relative abundance of the genera Cetobacterium (P = 0.010), Dysgonomonas (P = 0.013), and Fusicatenibacter (P = 0. 044) in the AD group was significantly higher than that in the HC group, while the relative abundance of the genera Actinomyces (P = 0. 029), Collinsella (P = 0. 017) and Eggerthella (P = 0. 028) were significantly lower in relative abundance than the HC group. LEfSe analysis showed that the relative abundance of Coriobacteriaceae (P = 0. 017) and Bifidobacterium_longum (P = 0. 002) in the AD group was significantly lower than that in the HC group. Conclusion There are some differences in the composition of the Gut microbiota between the AD and HC groups. Differential microorganisms such as Fusicatenibacter, Eggerthellay Actinomyces and Bifidobacterium ngum may have a correlation with AD development, suggesting that regulating the Gut microbiota early in life may prevent and treat AD. Objective: To analyze the expression levels of serum galectin-10 (Gal-10) and inflammatory factors and their correlation with skin infection in children with atopic dermatitis of different severity. Methods: A total of 110 children with atopic dermatitis admitted to our hospital from January 2022 to January 2023 were included in the observation group. They were divided into mild group, moderate group and severe group according to SCORAD. Another 110 healthy children were included in the control group. The levels of serum Gal-10,inflammatory factors (IL-4, IL-13, IL-31) and percutaneous water loss in different parts of the skin were detected, and the relationship between serum Gal-10, IL-4, IL-13, IL-31, SCORAD score and percutaneous water loss in different parts of the skin was analyzed. The predictive efficacy of serum Gal-10 and inflammatory factors (IL-4, IL-13, IL-31) on skin infection was analyzed using receiver operating characteristic curve (ROC). Results: The levels of serum Gal-10, IL-4, IL-13 and IL-31 in observation group were higher than those in control group (P<0.05). With the aggravation of atopic dermatitis in children, serum levels of Gal-10, IL-4, IL-13 and IL-31 increased, and serum levels of Gal-10, IL-4, IL-13 and IL-31 in mild, moderate and severe groups were significantly different(P<0.05). By Pearson correlation analysis, serum Gal-10, IL-4, IL-13 and IL-31 levels were positively correlated with SCORAD score, and percutaneous water loss in anterior wall extension, anterior wall flexion, cheek and anterior tibia in atopic dermatitis children (P<0.05). Among 110 children with atopic dermatitis, 43 had skin infections. According to ROC curve analysis, the sensitivity of serum Gal-10, IL-4, IL-13 combined with IL-31 to predict skin infection in children with atopic dermatitis was 89.72%, the specificity was 56.97% and the area under ROC curve (AUC) was 0.921. Conclusion: The levels of serum Gal-10 and inflammatory factors (IL-4, IL-13, IL-31) are significantly correlated with the severity of atopic dermatitis in children, and are of certain significance in evaluating skin barrier function and predicting skin infection, which is worthy of clinical attention. Objective To evaluate the effect of intrauterine hyperglycemia exposure on inflammatory cytokine levels in neonatal umbilical cord blood and the incidence of allergic diseases in infants. Methods 112 pregnant women and their infants were enrolled. According to the glucose levels during pregnancy, all women were divided into the intrauterine hyperglycemia exposure (n = 55) and non-intrauterine hyperglycemia exposure groups (n = 57). The levels of inflammatory cytokines in the umbilical cord blood were compared between two groups. During 1-year follow-up, the incidence of allergic diseases such as atopic dermatitis and wheezing in infants was compared between two groups. The impact of intrauterine hyperglycemia on neonatal immunity and infant allergic diseases was evaluated. Results There were no significant differences in the levels of IL-1β, IL-2, IL-6, IL-8, IL-10, IL12 and TNF-α between two groups (all P > 0.05). In the intrauterine hyperglycemia exposure group, the IL-4 level in the umbilical cord blood was (30.180±8.648) pg/mL, significantly higher than (22.500±10.100) in the non-intrauterine hyperglycemia exposure group (P < 0.05). In the intrauterine hyperglycemia exposure group, the incidence of allergic diseases was 34.7%, higher compared with 24.0% in the non-intrauterine hyperglycemia exposure group during 1-year follow-up. However, there was no significant difference between two groups (P > 0.05). Conclusion Intrauterine hyperglycemia exposure exerts certain effect on the IL-4 level in the neonatal cord blood, which may provide evidence for early intervention of allergic diseases. Objective: To investigate the correlation between peripheral blood microRNA (miRNA)-122a and miR-146a levels and T helper cells (Th)1/Th2/Th17 immune balance in children with atopic dermatitis (AD). Methods: 100 children with AD who were admitted to the Dermatology Department of Shijiazhuang Maternal and Child Health Hospital from May 2020 to May 2023 were selected as AD group. According to scoring atopic dermatitis (SCORAD), they were divided into mild group with 31 cases, moderate group with 41 cases and severe group with 28 cases. Another 100 healthy children during the same period were selected as control group. Real-time fluorescence quantitative polymerase chain reaction was used to detect the levels of peripheral blood miR-122a and miR-146a, and flow cytometry was used to detect the proportion of peripheral blood Th1, Th2 and Th17 cells. The levels of peripheral blood Th1, Th2 and Th17-related cytokines [interleukin (IL) -2, interferon-γ (IFN-γ), IL-4, IL-13, IL-17, IL-22] were detected by enzyme-linked immunosorbent assay, and the ratio of Th1/Th2/Th17 was calculated. The correlations of peripheral blood miR-122a, miR-146a and Th1, Th2, Th17 and related cytokines and Th1/Th2/Th17 of children with AD were analyzed by Pearson/Spearman correlation analysis. Results: The levels of peripheral blood miR-122a, miR-146a, Th1, Th1/Th2/Th17, IL-2 and IFN-γ of the AD group were lower than those of the control group, and the levels of Th2, Th17, IL-4, IL-13, IL-17 and IL-22 were higher than those of the control group (all P<0.05). The levels of peripheral blood miR-122a, miR-146a, Th1, Th1/Th2/Th17, IL-2 and IFN-γ of the mild group, moderate group and severe group were decreased successively, while the levels of Th2, Th17, IL-4, IL-13, IL-17 and IL-22 were increased successively (all P<0.05). Pearson/Spearman correlation analysis showed that peripheral blood miR-122a and miR-146a in children with AD were positively correlated with the levels of Th1, Th1/Th2/Th17, IL-2, and IFN-γ (r/rs were 0.679, 0.677, 0.684, 0.706, 0.693, 0.689, 0.671, 0.694, respectively, all P＜0.001), and negatively correlated with Th2, Th17, IL-4, IL-13, IL-17, IL-22 (r/rs were -0.690, -0.680, -0.681, -0.669, -0.675, -0.676, -0.676, -0.686, -0.682, -0.674, -0.680, -0.689, respectively, all P＜0.001). Conclusion: The decreased levels of peripheral blood miR-122a and miR-146a of patients with AD are closely related to the severity of the disease, and which may participate in the occurrence and development of AD by regulating the Th1/Th2/Th17 immune balance. Objective Based on the health big data platform of Yichang city, to analyze the characteristics of allergic diseases of children born in medical institutions from 2017 to 2022 in Yichang city, and to clarify the distribution of allergic diseases among children in Yichang city. Methods Statistical analysis was conducted based on the records of born children with allergic diseases in the outpatient clinic of urban comprehensive medical institutions from 2017 to 2022 in the big data cloud platform of Yichang city. Results The number of visits of male children was 68 707, more than that of female children, and the number of visits had been increasing since 2017. The highest number of hospital visits was 46 190 (37.86%) among children of 0-1 year old, of which the highest number of visits was 3 331 (13.74%) among children of two months. Atopic dermatitis/eczema and allergic rhinitis had a higher number of visits with 46 156 and 14 388 visits, respectively. The age of onset of atopic dermatitis was earlier, with the age of first consultation being 0.42 years, and the age of onset was earlier in males than in females. Atopic dermatitis/eczema and allergic rhinitis were the most common multimorbidities. Conclusion For children with atopic dermatitis/eczema and/or allergic rhinitis, the focus is on the development of concomitant diseases to further interrupt the allergic process and promote early detection, prevention and treatment of the disease. Patients with atopic dermatitis (AD) have skin barrier dysfunction, which is closely related to keratinocyte (KC) abnormalities. Upon stimulation by external factors, KCs release a large number of inflammatory mediators, thereby activating type 2 inflammation, which, in turn, exacerbates skin barrier dysfunction. Thus, the interaction between KCs and type 2 inflammation is a key factor in the pathogenesis of AD, and exacerbation of inflammation and pruritus. At present, the innovative medications, such as biologics and small molecule drugs, have been shown anti-inflammatory effects in AD. Studies also demonstrate their role in skin barrier repair. Therefore, research on the interaction between KCs and type 2 inflammation will help understand the pathogenesis of AD and provide evidence to optimize treatment strategies for AD.

OBJECTIVE: To provide reference for drug research based on registration of clinical trials of atopic dermatitis drugs in children and marketing of atopic dermatitis drugs in China. METHODS: The registration information of clinical trials in children with atopic dermatitis and marketing information of atopic dermatitis drugs were obtained by searching the databases of “platform for registry and publicity of drug clinical trials ” and “drug inquiry” of the National Medical Products Administration (both searched from Jan. 1st, 2017 to Apr. 17th, 2023), and classified statistics were conducted. RESULTS: A total of 40 clinical trials of eczema/ atopic dermatitis drugs for children were registered on the platform, and 10 varieties of atopic dermatitis drugs from 36 domestic and foreign manufacturers had been approved to be listed in China. CONCLUSIONS: Domestic pharmaceutical manufacturers mainly focus on generic drugs in the research and development of atopic dermatitis drugs for children. At present, there are two kinds of biological products for the treatment of class 1 drugs, and it is necessary to further carry out clinical trials of drugs, while giving full play to the advantages of traditional Chinese medicine to explore traditional Chinese medicine varieties to carry out clinical trials for children’s drugs. Objective To investigate the intervention effect of Akkermansia muciniphila (AKK) on atopic dermatitis (AD) induced by 2,4-dicMoronitrobenzene (DNCB) in mice and the effect of intestinal flora. Methods In total 72 BALB/c mice were randomly divided into high and low dose live bacteria group, pasteurization group, model group, and blank group, with 12 mice in each group. The AD mouse model was established by DNCB, and different doses of AKK living bacteria and Pas-teurella multicide were intragastrically administered for 4 weeks. The skin lesions, splenomegaly, and skin histopathology of mice in each group were observed, the level of serum IgE was measured, and the expression levels of IL-4 and IFN-y on mRNA transcription and protein in back skin tissue were detected. The fecal samples of mice were collected and 16SrRNA sequencing was used to understand the changes in the structure and abundance of intestinal microflora. Results After AKK intervention, the skin lesions, splenomegaly, and skin pathological damage of AD mice were not significantly improved, the level of serum IgE increased, the level of IL-4 decreased except the mRNA transcription level of low dose live bacteria group, and the expression of IFN-y increased at mRNA and protein levels in low dose live bacteria group and high dose inactivated group. Conclusion Although AKK did not significantly improve the skin lesions of AD mice, it alleviated the inflammatory reaction in mice to some extent and improved the diversity and structure of intestinal flora. Immunologic skin diseases encompass a spectrum of immune system-mediated autoimmune or inflammatory skin conditions, such as lupus erythematosus, psoriasis, atopic dermatitis, and vitiligo. Immunologic skin diseases are characterized by an unclear pathogenesis, complex disease processes, diverse clinical manifestations, and treatment difficulties, thereby presenting significant diagnostic and therapeutic challenges. Notably, Chinese researchers have achieved numerous innovative research findings on immunologic skin diseases in recent years. Over the past decade, Chinese scholars have contributed 11 919 SCI papers to the field of immune dermatosis, with more than 10 being published in the world's leading medical journals. These publications include one in Science, one in Nature, two in Cell, three in The New England Journal of Medicine, two in The Lancet, and one in Nature Medicine, as well as four in Immunity. Objective To explore the clinical characteristics, the disease disturbance and the treatment needs of patients with atopic dermatitis. Methods We retrospectively analyzed the data of patients with atopic dermatitis who completed the disease management manual for atopic dermatitis at the department of dermatology of the Second Affiliated Hospital of Harbin Medical University from June 2022 to June 2023. The analyses included demographic characteristics, ADCT score, treatment and laboratory test results. Results A total of 119 patients with AD were included in this analysis. The most common involved body site is the upper arms. In the real world, 95.52% of the patients considered that both pruritus and skin lesions affected the quality of their lives and were the main disease trouble. Long-term control of disease and decrease recurrence were expected by 94.67% of the patients. Patients treated with topical glucocorticoids or calcineurin inhibitors accounted for 93.58%, while 85.32% of patients were treated with oral anti-histamines. Disease was poorly controlled in 60.50% of patients in the real world (ADCT ≥7). The patients with long history of disease (>3 years) have higher ADCT scores than the patients with shorter history of disease (≤3 years) (Z=2.41, P=0.016). Conclusions In the real world, patients expect long-term control of the disease. Improper treatment can result in poor disease control in patients with atopic dermatitis.

The pathogenesis of AD has not been fully elucidated, and it is currently recognized that it is related to genetic and environmental factors, skin barrier dysfunction and immune dysregulation dominated by type 2 inflammation. In recent years, with the in-depth study of intestinal microbiota, microbe therapy may make up for the shortage of topical glucocorticoids and other treatments. This article focuses on the relationship of the changes in microecology with the pathogenesis of AD, elaborates the important role of intestinal microbiota, cutaneous microbiota and their metabolites in AD, and reviews the research progress in microbe therapy, so as to provide new enlightenment for the treatment of AD.

Objective To investigate the serum C-type lectin domain family 4 member G（CLEC4G） level and its clinical value in patients with Atopic Dermatitis （AD）. Methods The blood samples of 60 AD patients and 29 control patients were collected, and CLEC4G, Interleukin-33 （IL-33）, total immunoglobulin E （tIgE）, specific IgE （specific IgE）, and eosinophil levels were detected. The correlation between CLEC4G level and clinical data of AD patients and IL-33 was analyzed. The risk of AD was evaluated by Logistic regression analysis of CLEC4G, IL-33 and other indicators. Results Compared with the control group, the serum CLEC4G level in AD patients was significantly decreased （359.4 ± 57.3 vs. 521.8 ± 48.1）pg/mL. There was no significant difference in CLEC4G level between childhood, adolescent and adult, male and female AD patients. Compared with tIgE ≤ 100 kU/L group, CLEC4G level was significantly decreased in 100 ~ 200 kU/L group and tIgE ≥ 200 kU/L group, but there was no significant difference between 100 ~ 200 kU/L group and tIgE ≥ 200 kU/L group. Serum CLEC4G level decreased significantly only in the moderate AD group, but had no significant difference among the other groups. The serum level of IL-33 was increased in AD patients, but there was no significant correlation between CLEC4G and IL-33（ r = 0.090, P = 0.495）. Age less than 14 years old and IL-33 were risk factors for the incidence of AD, with OR values of 2.756 and 1.241,95%CI of 1.076 ~ 7.060 and 1.030 ~ 1.495, respectively. CLEC4G was a protective factor for AD （OR = 0.890,95%CI: 0.809 ~ 0.979）. Conclusion CLEC4G may be a protective factor independent of IL-33 mediated AD pathogenesis. [ABSTRACT FROM AUTHOR]

Objective To explore the distribution characteristics of serum allergens in children with atopic dermatitis (AD), and to provide reference for the prevention and treatment of AD in Chongqing. Methods A total of 482 AD children under 12 years old who were treated in the Department of Dermatology of the hospital from December 2018 to December 2021 were selected, and the detection results of serum total immunoglobulin E(IgE) and specific IgE were analyzed. Results Among the 482 children with AD.394 cases (81.74%) were positive for total IgE antibody and 337 cases (69. 92%) were positive for specific IgE antibody. The positive rates of specific IgE antibody in children with AD under three years old was not signifi- cantly different from that in children with AD aged >3-6 years and >6-12 years old respectively (P> 0.05). Among 337 cases of specific IgE antibody positive children, there were 42 cases (12.46%) of one aller- gen.74 cases(21.96%) of two allergens.85 cases (25.22%) of three allergens. and 136 cases (40.36%) of four or more allergens. The 337 cases of specific IgE antibody positive children were mainly characterized by inhala- tion allergens. The positive rates of dust mite combination and house dust were significantly higher than the average positive rates of inhaled allergens (P<0.05), while the positive rates of soybean and egg white were significantly higher than the average positive rates of ingested allergens (P<0.05). Conclusion Most children with AD have symptoms of elevated serum total IgE and specific IgE levels, and detecting serum specific IgE can help clinical detection of allergens in AD children. Objective: To preliminarily explore therapeutic effects and possible molecular mechanisms of taraxasterol on mice model of atopic dermatitis. Methods: BALB/c male mice were selected to establish mice model of atopic dermatitis using 2, 4-dinitrochlorobenzene, and were intragastrically administrated with low and high concentrations of taraxasterol. Then the skin lesions, scratching times, organ index were observed, and pathological HE staining and CD4+ immunohistochemistry were performed, ELISA was used to detect levels of inflammatory factors TNF-α, IL-4 and IL-6. Results: Taraxasterol could significantly reduce the degree of skin lesions, skin thickness, scratching times and organ index, reduce infiltration of inflammatory cells, keratinization of epidermis, thickening of spinous layer, and reduce levels of TNF-α, IL-4 and IL-6 in serum. Conclusion: Taraxasterol can improve condition of atopic dermatitis in mice, and its mechanism may be related to the inhibition of TNF-α, IL-4 and IL-6. [ABSTRACT FROM AUTHOR]

This article focuses on the relationship of the changes in microecology with the pathogenesis of AD, elaborates the important role of intestinal microbiota, cutaneous microbiota and their metabolites in AD, and reviews the research progress in microbe therapy, so as to provide new enlightenment for the treatment of AD. Objective To explore the clinical characteristics, the disease disturbance and the treatment needs of patients with atopic dermatitis. Methods We retrospectively analyzed the data of patients with atopic dermatitis who completed the disease management manual for atopic dermatitis at the department of dermatology of the Second Affiliated Hospital of Harbin Medical University from June 2022 to June 2023. The analyses included demographic characteristics, ADCT score, treatment and laboratory test results. Results A total of 119 patients with AD were included in this analysis. The most common involved body site is the upper arms. In the real world, 95.52% of the patients considered that both pruritus and skin lesions affected the quality of their lives and were the main disease trouble. Long-term control of disease and decrease recurrence were expected by 94.67% of the patients. Patients treated with topical glucocorticoids or calcineurin inhibitors accounted for 93.58%, while 85.32% of patients were treated with oral anti-histamines. Disease was poorly controlled in 60.50% of patients in the real world (ADCT ≥7）. The patients with long history of disease (＞3 years) have higher ADCT scores than the patients with shorter history of disease (≤3 years) (Z=2.41, P=0.016）. Abrocitinib is a highly selective oral inhibitor of janus kinase 1 (JAK1), which can inhibit JAK-STAT signal transduction and activation pathways, resulting in down-regulation of the expression of IL-4, IL-13 and IL-31, which all are involved in the pathogenesis of AD, and ultimately blocks the occurrence and the development of AD. Abrocitinib has shown a good efficacy, tolerability and safety in the treatment of refractory moderate to severe AD. This article reviews the pharmacological mechanisms, clinical efficacy, safety and clinical applications of abrocitinib.

Here, we review the patient-reported quality-of-life measurement instruments, including generic instruments, dermatology-specific instruments, and disease-specific instruments used in AD. The generic instruments can be used to compare the quality of life of patients with AD and non-skin diseases, while the dermatology-specific instruments can be used to compare the quality of life of patients with different skin diseases. Dermatology-specific instruments and disease-specific instruments used in AD may have greater discriminatory power and higher specificity and sensitivity for subtle changes. How to choose the appropriate scale in clinical practice and research depends on the research question and the applicable population.

Atopic dermatitis (AD) is a common chronic inflammatory skin disease characterized by recurrent eczematous lesions with dry skin and refractory pruritus. The pathogenesis of AD is complex and one of the reasons may be related to the skin microbial environment. Multiple studies have found that bathing may be associated to skin barrier function and different bathing method may affect the severity of AD. This paper focuses on the bathing method of children with AD as well as the cleansers, and reviews relevant guidelines and evidence-based medicine.

Objective To compare the specificity, sensitivity, variability and consistency among three different diagnostic criteria for atopic dermatitis (AD) in subjects with and without AD. Methods We retrospectively analyzed patients with AD initially diagnosed according to the Hanifin Rajka criteria at our clinic between September 2020 and October 2021. Demographic characteristics, clinical manifestations and the results of laboratory tests were used to analyze the sensitivity and specificity of Williams criteria, Zhang's criteria/Chinese criteria and Chinese diagnostic criteria for childhood AD. Patients with other dermatological diseases visiting our clinic during the same period were used as controls. The sensitivity and specificity of the three diagnostic criteria were compared using McNemar's test, while Kappa values were used to determine the consistency of the diagnostic criteria. Results One hundred and ten patients with AD and 91 patients without AD were included in this analysis. The highest sensitivity and lowest specificity were Chinese diagnostic criteria for childhood AD (83.6%, 59.3%). Both Zhang's criteria/Chinese criteria and Williams criterion displayed the highest specificity (90.1% for both). Both positive and negative predictive values (90.2%, 75.2%) were higher for Zhang's criteria/Chinese criteria than the others. The Williams criteria exhibited the highest false negative rate (29.1%), while the highest false positive rate was found for the Chinese diagnostic criteria for AD in children (40.7%). Objective To observe the efficacy of dupilumab in the treatment of refractory atopic dermatitis (AD)，and to provide the reference for the treatment of such patients. Methods The clinical data and follow-up results of two patients with refractory severe AD treated with dupilumab were collected; combined with the literature review, the efficacy and safety of dupilumab in the treatment of refractory AD were analyzed. Results Patient 1 was a 30-year-old female with dense, infiltrated papules on the back of both hands, resembling lichenified skin changes, scattered erythema, and papules with itching on the limbs and trunk for 25 years. Patient 1 also had a history of allergic rhinitis for 5 years. The diagnosis was adults severe AD. After 16 weeks of treatment with dupilumab, the skin lesions were completely regressed, and there were no relapse during the follow-up period. The symptoms of allergic rhinitis was improved significantly, and oral medication was no longer needed. Patient 2 was a 59-year-old female with general dark red erythema, papules, and nodules accompanied by itching for over 10 years. Patient 2 had been hospitalized many times and had previously received treatment with cyclosporine, azathioprine, and steroids with unsatisfactory results. The diagnosis was adults severe AD. Up to the present, the skin lesions had essentially regressed after treatment with dupilumab. Conclusion Dupilumab has good curative effect and safety on refractory AD, and it can improve the life quality, and controll the charicteristics of AD complicated with other allergic diseases driven by type 2 inflammation. Objective To screen out the target genes of miR-155 associated with immune infiltration in atopic der⁃ matitis (AD), and to verify them by cell experiments. Methods The gene expression profiles of AD (GSE121212 and GSE157194) were obtained from the GEO database, and the differentially expressed genes in AD lesions were screened out. GO functional enrichment analysis and KEGG pathway enrichment analysis were performed, and protein-protein inter⁃ action (PPI) network was constructed. Based on CIBERSORT algorithm, the infiltration of immune cells in AD patients was analyzed. The miRNet website was used to predict the target genes of miR-155, and the target genes related to AD im⁃ mune infiltration were screened. Human immortalized epidermal cells (HaCaT cells) were divided into the control group and TI group. The cells in the TI group were treated with TNF-α and IFN-γ to construct the inflammation models, and the cells in the control group were cultured routinely. The qRT-PCR was used to detect miR-155 in the TI group and control group. The HaCaT cells in the TI group were divided into the TI+miR-155 mimics group and TI+mimics NC group. The HacaT cells in the TI+miR-155 mimics group were transfected with miR-155 overexpression plasmid, and cells in the TI+mimics NC group were transfected with the control plasmid mimics NC. The mRNA levels of CXCL1 and CXCL8 in the TI+miR-155 mimics group and TI+mimics NC group were detected by qRT-PCR. Results A total of 519 differentially expressed genes were identified in AD lesions. GO functional enrichment analysis showed that the differentially expressed genes were mainly involved in biological processes such as immune system process, immune response, response to external stimuli, defense response, and regulation of the immune system. KEGG pathway enrichment analysis showed that, path⁃ ways were mainly enriched in cytokine-cytokine receptor interaction, viral protein-cytokine and cytokine receptor interac⁃ tion, chemokine signaling pathway, IL-17 signaling pathway and other inflammation-related signaling pathways. A PPI network graph consisting of 67 nodes and 146 edges was successfully constructed. Dendritic cells, M2 macrophages and mast cells accounted for the highest proportion in the epidermal immune microenvironment, and the immune infiltra⁃ tion was the most obvious. The target genes of miR-155 were CXCL1 and CXCL8, which were highly correlated with AD immune infiltration. The relative expression of miR-155 in the TI group was higher than that in the control group (P< 0. 05) . The relative expression levels of CXCL1 and CXCL8 mRNA in the TI+miR-155 mimics group were higher than those in the TI+mimics NC group (all P<0. 05) . Conclusions CXCL1 and CXCL8 are the target genes of miR-155 asso⁃ ciated with immune infiltration in AD. In recent years, the prevalence of atopic dermatitis in infants and young children has been on the rise. Studies have shown that pregnant women are often affected by environment, diet and other factors during pregnancy and complicated with various diseases. The pregnancy-related risk factors can affect the fetus through the placenta, causing Th1/Th2 imbalance and intestinal flora disorder in newborn, thereby affecting the development of atopic dermatitis in offspring. This article reviews the progress of research on the effects of risk factors, including environmental factors, dietary factors, psychological factors, mode of delivery, and allergies during pregnancy, on atopic dermatitis in the offspring, in order to provide guidance to pregnant women and to reduce the risk of diseases in the offspring.

OBJECTIVE: To systematically evaluate the efficacy and safety of dupilumab in the treatment of adults with moderate to severe atopic dermatitis, so as to provide evidence-based reference for clinical treatment. METHODS: PubMed, Embase, the Cochrane Library and CNKI were retrieved to collect the randomized controlled trial of dupilumab in the treatment of adults with moderate to severe atopic dermatitis (the study group received dupilumab, while the control group was given placebo). The retrieval time was from the establishment of the database to Jun. 2022. RevMan 5. 3 software was used for Meta-analysis. RESULTS: Totally 10 randomized controlled trials were enrolled, including 3 383 patients. Proportion of patients in the study group with eczema area and severity index ≥ 75% of baseline remission (EASI75) were significantly higher than control group (total: RR = 3. 01,95%CI = 2. 62- 3. 46, P<0. 000 01; 300 mg, qw: RR = 2. 91, 95%CI = 2. 38-3. 55, P<0. 000 01; 300 mg, q2w: RR = 3. 16,95%CI = 2. 56-3. 89, P<0. 000 01), proportion of patients in the study group with eczema area and severity index ≥50% of baseline remission (EASI50) were significantly higher than control group (total: RR = 2. 43, 95%CI = 2. 24-2. 64, P< 0. 000 01; 300 mg, qw: RR = 2. 32, 95%CI = 2. 09-2. 59, P<0. 000 01; 300 mg, q2w: RR = 2. 58, 95%CI = 2. 27- 2. 93, P<0. 000 01), with statistically significant differences. Meanwhile, compared with the proportion of patients in the study group with IGA score of 0 or 1 points (clear or almost clear), the proportion of patients in the study group whose score of pruritus index (NRS) decreased by more than 3 points and other six effectiveness indicators, the study group was significantly better than that of control group, with statistically significant differences (P<0. 05). Through the comparative analysis of 300 mg qw and 300 mg q2w, there was no significant difference in each effectiveness index between two administration methods (P>0. 05). In terms of safety, the incidence of serious adverse reactions and skin allergy in the study group was significantly lower than that in the control group, and the incidence of injection site reaction, headache and oral herpes in the study group was significantly higher than that in the control group, with statistically significant differences (P<0. 05). There was no significant difference in the incidence of total adverse drug reactions, upper respiratory tract reaction and other safety indicators between the study group and the control group (P>0. 05). Objective To study the value of neutrophil/lymphocyte ratio (NLR) and platelet/lymphocyte ratio (PLR) in the diagnosis and prediction of disease severity in adolescent atopic dermatitis (AD). Methods A total of 100 AD patients and 50 healthy individuals were selected from Dermatology Department of our hospital.The age, gender, blood routine results of all subjects were collected, and NLR and PLR were calculated.The diagnostic value of NLR and PLR for AD and severe AD was discussed by receiver operating curve (ROC), the relationship between NLR, PLR and the risk of AD was analyzed. Results The AUC of NLR and PLR in the diagnosis of AD was 0.780 and 0.769, with the best cutoff value at 2.95 and 98.50 respectively, and no significant difference in the predictive value between NLR and PLR was noticed (P>0.05). Atopic dermatitis (AD) is a chronic recurrent inflammatory skin disease characterized by intense pruritus. A satisfactory understanding of itch in AD has been proved difficult to reach and the relapsing itch is believed to be primarily a result of dysfunction of the skin barrier, dysregulation of the immune system and multiple environmental factors. Itch-scratch cycle exacerbates skin lesions, promotes inflammation and neurological disorders, and triggers the desire to continue scratching. The current methods of treatment for controlling AD pruritus symptoms include topical and systemic drugs. The exploration on the mechanisms of atopic pruritus has provided novel therapeutic targets, such as IL-4, IL-13, IL-31, JAK, IL-33. The new advances in the mechanism and treatment of itch in AD were systemically reviewed.

Atopic dermatitis (AD) is an inflammatory disease characterized by recurrent itch and skin lesion. The pathological mechanisms of AD include the immunological disorders caused by genetic variants with environment changing and the damage of skin barrier. Recent studies have found that the topical dysbiosis of skin microbiota promotes AD. Compared to the normal skin, the diversity of the microbiome in the lesion skin decreases drastically. Especially, in company with the decreased abundance of protective coagulase-negative Staphylococci (CoNS), the load of Staphylococcus aureus (S. aureus) meets apparent increase. S. aureus is an important clinical pathogen which can cause skin infection, bacteremia, etc. Among AD patients, S. aureus can aggravate the itch and lesion of skin by expressing various toxins, proteases, superantigens, etc. Monitoring the skin flora, especially the characteristics of Staphylococci, provides new ideas for the diagnosis and treatment of AD. Meanwhile, considering the difficulties that the conventional antibiotic therapy failed to reverse and maintain the microbiota of the patient with severe symptom, the application of the commensal microbiome with antimicrobial function provides a new insight into the treatment of AD. Objective·To explore the correlation between serum 25-hydroxyvitamin D [25(OH)D] levels and serum allergens in children with atopic dermatitis (AD), and provide a theoretical basis for the prevention and treatment of AD. Methods·From November 2017 to February 2019, a total of 165 pediatric patients with AD who visited the Dermatology Department of Shanghai Children′s Medical Center were enrolled in this study. AD severity was assessed according to the severity scoring of atopic dermatitis (SCORAD). Serum total IgE and specific IgE (sIgE) levels of 19 common allergens were detected, and serum25(OH)D was determined by liquid chromatography-tandem mass spectrometry. All data were statistically analyzed. Correlation analysis was conducted by spearman's or point-biserial correlation analysis. Chi-square test was used for comparison of count data between groups. Binary linear regression analysis was used to identify the risk factors. Results·SCORAD score was shown to be positively correlated with serum total IgE level, absolute value and percentage of eosinophils in children with AD (r=0.213, P=0.009; r=0.268, P=0.001; and r=0.249, P=0.003, respectively). However, there was no significant correlation between serum 25(OH)D concentrations and SCORAD score in children with AD, while there was a significant negative correlation between the serum 25(OH)D concentrations and the total IgE levels, and the percentage of eosinophils in children with AD (r=-0.212, P=0.011; and r=-0.174, P=0.039, respectively) was shown. In this study, the most common allergens with sIgE grade Ⅲ or above were house dust mite, egg white and cashew nut, followed by crab, cat dander, mixed mold, shrimp, dog dander, house dust and mixed tree pollen from high to low. There was no significant correlation between the number of allergen-positive items and serum 25(OH)D concentrations in children with AD (χ2=6.27, P=0.804). However, AD patients with positive house dust mite and mixed mold were statistically significant in different serum [25(OH) D] groups (P=0.000, and P=0.043, respectively). In addition, univariate logistic regression analysis showed that serum [25(OH)D] levels were risk factor for house dust mite allergy in children with AD. The risk of household dust mite allergy in children with serum 25(OH)D deficiency was 8.196 times than that of children with sufficient serum 25(OH)D (OR=8.196, P=0.000). However, serum [25(OH)D] levels were not the risk factor for house dust mite allergy in children with AD. Conclusion·Serum [25(OH)D] levels might affect the sensitization of children with AD to house dust mite. Allergen immunotherapy （AIT） is the only treatment to change the process of allergic diseases by inducing allergen-specific immunity tolerance. Although a large number of clinical trials and meta-analysis have confirmed the safety and effectiveness of AIT, it has not been widely applied. New allergen vaccines are constantly developed and better AIT treatment strategies are being explored worldwide. However, most of the clinical data are obtained from adults, and the pediatric data are lacking. It is urgent to promote the standardization of diagnosis and treatment and formulate a unified AIT strategy for pediatric allergic diseases. This article reviews the research progress on clinical application of AIT in children, aiming to provide reference for in-depth investigations.

Objective To investigate the relationship among serum 25（OH）D level, IgE-mediated food allergy and the severity of atopic dermatitis （AD） in infants. Methods Seventy-seven children with moderate to severe AD were enrolled in the study group, and 40 healthy children were selected in the control group. The serum levels of 25（OH）D were statistically compared between AD children with and without IgE-mediated food allergy, and the correlation between serum 25（OH）D level and the severity of AD （Severity Scoring of Atopic Dermatitis （SCORAD））was analyzed. Results Among the 77 children with AD, 19 AD cases （24.7%） were complicated with IgE-mediated food allergy. Compared with AD children without food allergy, the serum 25（OH）D level was significantly decreased in those complicated with food allergy （P < 0.001）. There was no significant difference in serum 25（OH）D level between the control group and the moderate AD group （P > 0.05）, whereas the serum 25（OH）D level in the severe AD group was significantly lower compared with that in the moderate AD group （P < 0.05）. There was no significant correlation between serum 25（OH）D level and SCORAD in AD children （P > 0.05）. Conclusion AD infants complicated with IgE-mediated food allergy or those with severe AD are more likely to have low serum levels of 25（OH）D, whereas there is no significant correlation between the severity of AD and serum 25（OH）D level.

Objective Serum levels of vitamin D (VitD), total immunoglobulin E (tIgE) and eosinophil ratio (EOS%) in children with atopic dermatitis (AD) were measured in order to determine the relationship between VitD and AD, and its immunoregulatory role in AD. Methods Serum samples were collected from 120 patients with AD and 60 healthy controls. Enzyme-linked immunosorbent assay (ELISA) was performed to detect the serum levels of 25(OH)D in the AD and controls, and the total serum IgE levels in the AD group. Blood cell analyzer was used to determine the proportion of blood eosinophils in the AD group. Objective: To explore the correlation between severity of atopic dermatitis (AD) and serum 25-hydroxyvitamin D [25(OH)D], total immunoglobulin E (tIgE) and IL-4 levels in children. Methods: A total of 160 children with AD who were treated in our hospital were enrolled. The severity of AD was evaluated and graded with the SCORAD score. Serum 25(OH) D levels were detected by high performance liquid chromatography, total IgE (tIgE) levels were detected by chemiluminescence, and IL-4 levels were detected by enzyme-linked immunosorbent assay. The correlation between serum 25(OH) D, tIgE, IL-4 level and SCORAD score were analyzed. Results: According to the SCORAD score, 55 cases (34.38%) were mild, 84 (52.50%) cases were moderate, and 21 (13.13%) cases were severe. Objective: To establish a mouse model of atopic dermatitis (AD) with overgrowth Malassezia globosa. Methods: Eighteen BALB/c mice were randomly divided into the control group, AD group, NS/M group, AD+NS/M group, Oil/M group and AD+Oil/M group, 3 mice per group. AD model of BALB/c mice were established by topical administration of MC903. Fungi overgrowth experimental models were attempted to establish using suspension of M. globosa with olive oil and normal saline as vehicle, respectively. Clinical manifestations and lesion scoring as well as mycological and histopathological changes of each group were evaluated. Atopic dermatitis is a chronic inflammatory skin disease, which is mainly related to changes in Th2 type signaling pathways. Increased expression of interleukin-4 (IL-4) and IL-13 reduces filaggrin, which leads to skin barrier defects. The Janus kinase-singal transducer and activator of transcriprion (Jak-STAT) signaling pathway is closely related to the pathogenesis of atopic dermatitis. Inhibiting the Jak-STAT signaling pathway is a potential method to treat atopic dermatitis. Janus kinase (JAK) inhibitors can be divided into first-generation and second-generation JAK inhibitors in the selection of JAKs′ inhibition. By summarizing the efficacy and safety of the first-generation and second-generation JAK inhibitors in clinical trials, the clinical research progress of JAK inhibitors in the treatment of atopic dermatitis is clarified. Atopic dermatitis (AD) also known as atopic eczema, Besnier prurigo diathesique or hereditary allergic eczema, is a skin disease characterized by eczema-like skin lesions, severe itching and obvious "atopy" in oneself or his family. In recent years, the theory of AD pathogenesis tends to be abnormal in immune barrier function. At present, it has been found that the method of Jianpi Yangxue Qufeng Decoction has definite curative effect in treating AD with Xuexu Fengzao type, which can effectively control skin lesions, reduce recurrence rate and regulate the immune barrier function of AD.
