UDP-glucuronosyltransferases(UGTs) are the main phase Ⅱ metabolizing enzymes in the human body, participating in the glucuronidation of various chemicals in the body. Traditional Chinese medicine(TCM) ingredients affect the activities of UGTs involved in drug metabolism, and the fluctuations in the blood concentrations of drugs metabolized by UGTs will lead to the risk of TCM-TCM or TCM-chemical drug interaction, which will cause drug safety problems and affect drug efficacy. Therefore, it is essential to explore the effect of TCM ingredients on the activities of UGTs. The recombinant human UGTs method and liver microsomal incubation method are reliable and effective, and they are thus often used to study the effects of TCM ingredients on UGTs in the liver. Studies have reported that different TCM ingredients can induce or inhibit the activities of UGTs, while the systematical review remains to be carried out. This paper reviews the effects of different ingredients and extracts of TCM and Chinese patent medicines on recombinant human UGTs or liver microsomal UGTs, aiming to provide reference for the rational and safe use of TCM in clinical practice. From the perspective of drug-drug interaction mediated by UGTs, the exacting classical drug pairs and incompatibility principles of traditional Chinese medicine were clarified, which laid the foundation for exploring new drug compatibility.

This paper aimed to investigate the effects of exercise on hepatic platelet-activating factor (PAF) metabolism in rats fed a high-fat diet. Thirty-two male Sprague-Dawley (SD) rats were divided into control group (C), high-fat diet group (H), exercise group (EC), and high-fat diet+exercise group (EH). Serum lipids, glucose, insulin and markers of hepatic injury after a 16-week dietary and/or exercise intervention (60 min/day, 6 times/week) were measured by biochemical analysis; liver lipidomic profiles were analyzed by liquid chromatograph-mass spectrometer (LC-MS). Gene and protein expression of enzymes related to PAF metabolism were determined by qPCR and Western blot respectively. The results showed that high-fat diet feeding significantly increased the levels of low-density lipoprotein-cholesterol (LDL-C) and liver injury markers including purine nucleoside phosphorylase (PNP) and malondialdehyde (MDA) in rats, which were decreased by exercise. Furthermore, high-fat diet feeding significantly increased the hepatic PAF content, which was also attenuated by exercise. In addition, although high-fat diet treatment resulted in an increase in the expression of both PAF synthetase (PAF-CPT and PLA2) and hydrolase (Lp-PLA2 and PAF-AH(II)), induction of PAF synthetase was much greater than that of PAF hydrolase. While exercise increased the expression of Lp-PLA2 and PAF-AH(II) and decreased the expression of PAF-CPT and PLA2, key PAF synthesizing enzymes. In conclusion, high-fat diet-induced increase in hepatic PAF content is mainly due to the increase of its pathological synthesis at the translational level. Exercise reduces hepatic PAF content in high-fat fed rats by increasing PAF hydrolysis and decreasing its synthesis.

Acute-on-chronic liver failure (ACLF) is a group of clinical syndromes related to severe acute liver function damage and multiple organ failure caused by various acute inducing factors on the basis of chronic liver disease. Due to its serious condition, rapid progression and high mortality, it has attracted more and more attention. Recent studies have shown that the pathogenesis of ACLF mainly includes direct injury and immune injury. As the main immune cells in the liver, the immunoregulatory role of liver macrophages in ACLF has been increasingly recognized. Liver macrophages have excellent phenotype conversion function and plasticity characteristics under the influence of epigenetic reprogramming or local microenvironment. This adaptive expression ability can use key mediators to promote the early conversion of anti-inflammatory phenotype to alleviate liver injury. A large number of studies have shown that liver macrophages have a certain potential in reversing the process of ACLF. Therefore, from the perspective of the plasticity characteristics of liver macrophages, this paper expounds the role of liver macrophages in ACLF and the research on the intervention of ACLF disease process, and summarizes its potential significance in the treatment of ACLF. Sjogren's syndrome is a refractory disease for which TCM demonstrates certain advantages in its treatment. "Prevalent dryness affecting the liver" is a classical theory from Huangdi Neijing (Huangdi's Internal Classic), which has important guiding significance for the treatment of Sjogren's syndrome with TCM. In this paper, the connotation and development of the theory of "prevalent dryness affecting the liver" is comprehensively analyzed in combination with clinical practice. Sjogren's syndrome is closely related to the liver itself, its functions, and the coordination between the liver and other viscera. The liver itself pertains to yin, and it nourishes blood and moistens dryness. The liver's functions pertain to yang, and they help to move body fluids without any sign of dryness. It's also of great significance that the liver and other viscera are well coordinated. In ancient books and modern studies, many clinical manifestations of Sjogren's syndrome, including anxiety and depression, are closely related to the liver. Therefore, the treatment methods based on regulating and soothing the liver are summarized, such as soothing the liver qi, clearing the liver heat and nourishing the liver yin, taking into account all the five zang organs, and dredging the channels and collaterals. Taking the theory of "prevalent dryness affecting the liver" as the breakthrough point, this paper systematically sorts out the onset, pathogenesis, pattern differentiation, and the significance of addressing the liver in the treatment of Sjogren's syndrome to provide reference for clinical practice. Multiple biliary hamartomas are a benign incidental finding in the liver. They are not easily detected if one has never seen them, and if appropriate imaging tests are unavailable, and also can be challenging to differentiate from other liver lesions based on imaging alone. Thus, this study aimed to expand the radiologist’s digital image library, enabling a quick and precise differential diagnosis. This paper also highlights the importance of thorough radiological assessment and need for a multidisciplinary approach, involving radiologists, hepatologists, and pathologists, to ensure a precise diagnosis. The patient presented at the hospital for a computed tomography scan and an abdominal magnetic resonance imaging recommended by his general practitioner to assess the biliary tree (magnetic resonance cholangiopancreatography), owing to persistent abdominal pain. The patient had never undergone an abdominal magnetic resonance imaging previously; hence, the discovery of hepatic lesions was incidental and unexpected. Magnetic resonance imaging revealed multiple benign lesions in both the hepatic lobes comparable to the Von Meyenburg complex. These lesions are multiple hamartomas and behave differently in all magnetic resonance imaging sequences. Images acquired with different magnetic resonance imaging sequences were carefully examined. Multiple lesions were found scattered throughout the liver; however, the lesions were benign and consistent with the diagnosis of multiple biliary hamartomas. Medical practitioners should examine the presence of multiple biliary hamartomas and consider them in the differential diagnosis when patients present with hepatic abnormalities. This can prevent unnecessary interventions and guide appropriate patient management.

Depression is a common clinical mental illness affecting patients’ quality of life. There has been a more systematic understanding of the treatment of depression in Traditional Chinese Medicine. This paper intended to explore the etiology, pathogenesis, syndrome differentiation, treatment and Traditional Chinese Medicine characteristic nursing intervention of depression from the point of traditional Chinese medicine, and reviewed relevant literature, in order to provide useful ideas for clinical treatment. Objective To summarize the clinical features of optic neuritis (ON) complicated with chronic hepatitis B virus (HBV) infection, and evaluate the effectiveness of different treatment regimens, recurrence of ON, visual prognosis, HBV reactivation, liver function and liver structure changes. Methods and Results Total 19 patients (23 eyes) with acute ON complicated with chronic HBV infection admitted to Beijing Tongren Hospital, Capital Medical University from January 2015 to June 2021 were included. There were 13 females and 6 males, 15 cases with monocular onset, 17 cases with unidirectional course of disease, and 6 cases with optic disc edema. The time to the worst visual acuity (peak time) was 3 (2, 7) d after onset, and 16/23 eyes had severe visual impairment. Enhanced MRI showed abnormal intensity in orbital, intraductal or intracranial segments of optic nerve, even involving optic nerve sheath and optic chiasm. Anti-nuclear antibody (ANA) was positive in 5 cases and A type Sjögren's syndrome antibody (SSA) was weakly positive in one case. Cerebrospinal fluid (CBF) pressure decreased in 1/7 cases and white blood cell count increased in 2/7 cases. HBV DNA positive group (HBV DNA ≥ 100 IU/ml, 10 cases and 11 eyes) had more severe visual impairment than negative group (HBV DNA < 100 IU/ml, 9 cases and 12 eyes), the difference was statistically significant (Fisher's exact probability: P = 0.005, 0.003). HBV DNA positive group was treated with entecavir, and the recurrence of ON was ignored during follow-up. HBV DNA negative group didn't receive antiviral therapy, one case progressed to cirrhosis, 3 cases had abnormal liver function, and 2 cases had HBV reactivation. Conclusions The visual function of patients with ON complicated with chronic HBV infection was seriously impaired, and the response to glucocorticoid therapy was not good. The visual impairment of patients with positive HBV DNA was more serious than patients with negative HBV DNA. Patients with ON complicated with chronic HBV infection should be treated with antiviral drugs in combination with glucocorticoid therapy. [ABSTRACT FROM AUTHOR]
