- Abstract
Introduction: Pain management is an integral part of neonatal care. Exposure to pain negatively affects child development with short- and long-term effects, such as persistent and chronic pain, as well as changes in neurobehavioral responses to future painful episodes. The administration of sweet solutions is increasingly used in clinical practice as an established non-pharmacological method. Purpose: To determine the analgesic effect of Sol.Glucose 25% on the reduction of neonatal pain due to heel pricking. Material and methods: Healthy full-term newborns with an average age of 72-84 hours were studied, divided into two groups: non-anesthetised /n= 67/ and anesthetised with Sol. Glucose 25% /n=40/. The child’s reaction to pain is recorded by video surveillance. Its severity was assessed using the scales: the multimodal - Neonatal Infant Pain Scale (NIPS) and the unimodal - Neonatal Facial Coding System (NFCS) 30 sec. before, at the30th-sec. and the 5th min after the painful stimulus caused by the heel prick. Heart rate, transcutaneous saturation, respiratory rate and blood pressure were examined at the same intervals with a Biocare iM/2014 monitor. Physiological markers of pain were monitored in the 12-24 hour interval following heel prick. Descriptive analysis is used to process the results. Results: Before the procedure, the total score according to NIPS and NFCS did not differ significantly between the groups. At the 30th sec. those receiving Sol.Glucose 25% have a higher score than the control group as assessed on both scales.A significant result (p=0.034) was reported only according to NIPS. At the 5th minute, the analgesic effect of Sol.Glucose 25% was confirmed (p=0.016), when applying the multimodal scale - NIPS. When monitoring the physiological indicators, we registered a statistical difference immediately after taking Sol.Glucosae 25% before the procedure - slower breathing (p=0.001), lower transcutaneous oxygen saturation (p=0.012), higher systolic (p=0.005) and diastolic blood pressure (p=0.033). At the 30th sec. a significantly higher heart rate was found in those anesthetised with glucose solution (p=0.023), and on the 5th minute after the procedure, we reported significant differences in two indicators: breathing - slower (p=0.012) and systolic pressure - higher (p=0.020), but they are within physiological norms. In both groups, the oxygen saturation did not reach 95% at the 5th min, but was above 90% and approaching the baseline. Conclusion: Sol.Glucosae 25% has an analgesic effect that allows its application to control procedural pain in fullterm newborns. [ABSTRACT FROM AUTHOR]
