After making the diagnosis the rheumatic fever as a secondary disease after streptococcal infection is immediately to be treated with penicillin G. Parallel to this or overlapping the rheumatic inflammatory mesenchymal tissue reactions are treated with prednisolone or derivatives, aminophenazone or salicylic acid preparations, in which cases in children prednisolone and aminophenazone are preferred. The combat against the streptococcal infection by means of penicillin transgresses into a recidivation prophylaxis with benzathin-penicillin, which is to be performed up to an age of 5 years. Substitutes for the recidivation treatment with benzathin-penicillin are V-tablopen or erythromycin, perhaps also sulfaclomide. The treatment is to be performed in a hospital up to the time that the patient is free from signs of inflammation and at least 4 weeks without fever, then a gradual rehabilitation begins.

By means of the aminopyrine breath analysis the influence of phenylbutazone, propyphenazone and of the combination preparation Wofapyrin "new" on the aminopyrine-N-demethylation, a hepatic microsomal performance of biotransformation in patients with diseases of the rheumatic circle of forms was examined. The enzyme-inducing effect of phenylbutazone known from literature could be confirmed. On the other hand despite a relatively highly dosed one-week treatment propyphenazone did not lead to changes of the demethylation capacity of the liver in the majority of the patients. In individual cases increases of the capacity are possible. In general, however, the doses of propyphenazone usual in the practice for the treatment of acute feverish infections no disturbing effects of induction or inhibition on the metabolic liver functions represented by the amino-N-demethylation are to be expected. The enzyme-inducing effect of Wofapyrin "new" is to be ascribed to the phenyl-butazone component.

The case of a 48-year-old patient suffering from a malaria tropica with heart involvement is reported. The affection of the heart shows a disturbed conduction. The pathogenesis of the affection of the heart in malaria tropica is discussed.
