Endovascular options for the treatment of basilar apex aneurysms (BAAs) are heterogeneous, and evidence is limited to retrospective cohorts and case series. We seek to evaluate the efficacy and complications associated with various endovascular treatment methods of BAAs. Systematic review of PubMed, Embase, and Web of Science adhering to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Retrospective and prospective studies evaluating endovascular treatment of BAAs between January 2010 and July 2024 were included. Relevant information including occlusion rates, aneurysm recurrence, retreatment rates, and complications were subjected to meta-analysis. Fifteen studies with 1049 BAAs were included. The median aneurysm diameter was 8.5 mm (range, 4.6–19.75), with a median follow-up of 33.7 months (range, 6.0–117.6). Residual aneurysm filling occurred in 24% after primary coiling (95% CI = 0.16–0.32), 25% after single stent-assisted coiling (s-SAC; 95% CI = 0.04–0.46), 25% after Y-stents (95% CI = 0.12–0.37), and 23% after flow diverter stent (FDS; 95% CI = 0.11–0.35). Recurrence rates were high for primary coiling (27%, 95% CI = 0.18–0.36) and s-SAC (19%, 95% CI = 0.13–0.26), but significantly lower for Y-stents (9%, 95% CI = 0.03–0.15) and FDS (4%, 95% CI = −0.04–0.11). Retreatment rates were 19% for primary coiling (95% CI = 0.12–0.26), 17% for s-SAC (95% CI = 0.07–0.27), 5% for Y-stents (95% CI = −0.03–0.12), and 13% for FDS (95% CI = −0.01–0.27). Meta-regression indicated larger aneurysms had higher complication rates (P = 0.02). Thromboembolic events were most frequent with FDS and Y-stents(12%). Occlusion rates were similar across treatments, but recurrence rates were significantly lower after Y-stents and FDS compared to primary coiling, although they carried a higher number of thromboembolic complications. For symptomatic stenosis in the middle cerebral artery (MCA), intracranial angioplasty and stenting are frequently employed. However, limited data exist regarding their long-term impact. Our study demonstrates the long-term advantages in preventing ischemic events through a 5-year follow-up period. A set of 41 individuals with symptomatic stenosis in the MCA who underwent angioplasty or stenting procedures between October 2004 and April 2018 at various hospitals in Southwest China were prospectively enrolled in the study. The rates of successful revascularization, complications, imaging observations, and clinical outcomes were systematically assessed. A total of 41 individuals successfully underwent stenting, respectively. After stenting, the extent of stenosis was decreased from 71.8% (56–87.8%) to 24.9% (0–45%). The mean follow-up period is 36.9 ± 13.68 months (range, 11–67 months). There was no deterioration of neurological function or a new ischemic event. A DSA or CT angiography was conducted after the procedure and demonstrated no in-stent restenosis. No patient experienced restenosis below 50% during the mean follow-up period. The morbidity and mortality rates of the case series were 7.3% and 2.4%, respectively. In the treatment of symptomatic MCA atherosclerotic stenoses, intracranial angioplasty and stenting are demonstrated to be technically feasible and safe. Although numerous case series and meta-analyses have shown the efficacy of venous sinus stenting (VSS) in the treatment of idiopathic intracranial hypertension and idiopathic intracranial hypertension–associated pulsatile tinnitus, there remain numerous challenges to be resolved. There is no widespread agreement on candidacy; pressure gradient and failed medical treatment are common indications, but not all clinicians require medical refractoriness as a criterion. Venous manometry, venography, and cerebral angiography are essential tools for patient assessment, but again disagreements exist regarding the best, or most appropriate, diagnostic imaging choice. Challenges with the VSS technique also exist, such as stent choice and deployment. There are considerations regarding postprocedural balloon angioplasty and pharmacologic treatment, but there is insufficient evidence to formalize postoperative decision making. Although complications of VSS are relatively rare, they include in-stent stenosis, hemorrhage, and subdural hematoma, and the learning curve for VSS presents specific challenges in navigating venous anatomy, emphasizing the need for wider availability of high-quality training. Recurrence of symptoms, particularly stent-adjacent stenosis, poses challenges, and although restenting and cerebrospinal fluid–diverting procedures are options, there is a need for clearer criteria for retreatment strategies. Despite these challenges, when comparing VSS with traditional cerebrospinal fluid–diverting procedures, VSS emerges as a favorable option, with strong clinical outcomes, lower complication rates, and cost-effectiveness. Venous sinus stenosis has garnered increasing academic attention as a potential etiology of idiopathic intracranial hypertension (IIH) and pulsatile tinnitus (PT). The complex anatomy of the cerebral venous sinuses and veins plays a crucial role in the pathophysiology of these conditions. Venous sinus stenosis, often found in the superior sagittal or transverse sinus, can lead to elevated intracranial pressure (ICP) and characteristic IIH symptoms. Stenosis, variations in dural venous anatomy, and flow dominance patterns contribute to aberrant flow and subsequent PT. Accurate imaging plays a vital role in diagnosis, and magnetic resonance (MR) venography is particularly useful for detecting stenosis. Management strategies for IIH and PT focus on treating the underlying disease, weight management, medical interventions, and, in severe cases, surgical or endovascular procedures. Recently, venous sinus stenting has gained interest as a minimally invasive treatment option for IIH and PT. Stenting addresses venous sinus stenosis, breaking the feedback loop between elevated ICP and stenosis, thus reducing ICP and promoting cerebrospinal fluid outflow. The correction and resolution of flow aberrances can also mitigate or resolve PT symptoms. While venous sinus stenting remains an emerging field, initial results are promising. The Wingspan stent system was approved as a rescue device for angioplasty of intracranial atherosclerotic disease (ICAD) in 2012. We conducted the government-recommended Japanese Postmarket Surveillance of Percutaneous Transluminal Angioplasty and Wingspan Stenting for Intracranial Atherosclerotic Disease to monitor Wingspan safety and performance. In this open-label, single-arm study at 76 centers in Japan, ICAD subjects were treated with the Wingspan stent between 2014 and 2016. Efficacy end points were successful stent deployment and technical success, defined as stenosis improvement to <50%. Safety end points were ischemic stroke, modified Rankin Scale and National Institutes of Health Stroke Scale scores, adverse events, and mortality. Subjects were considered treated outside of current recommendations if not already receiving antithrombotics or if percutaneous transluminal angioplasty and stenting occurred within 7 days of onset. The safety cohort included 305 subjects (mean age: 68.7 + 9.6; 83.3% male). Four subjects were removed due to being out of contract. Therefore, 301 subjects were included (mean age: 68.7 ± 9.7; 84.1% male). Successful stent placement was 96.7%. Technical success was observed in 86.8% of lesions. At 1 year, the rate of restenosis was 15.7%, ischemic stroke was 3.9%, and any stroke was 7.9%. Functionally independent outcome (modified Rankin Scale: 0–2) was 88.9% and mortality 3.0%. Significantly more serious adverse events occurred in subjects treated outside of current recommendations (17.9%) versus subjects treated per current recommendations (8.8%) (P = 0.045). Some have suggested that trabeculae within the transverse sinuses (chordae Willisii) might restrict flow and potentially contribute to thrombus organization. In addition, these structures might be encountered with endovascular procedures within the transverse sinus and are now readily seen on imaging. Therefore as anatomic studies of these structures are scant, the current study aimed to better elucidate these structures within the transverse sinus via a morphologic study in cadavers. Thirty fresh-frozen, cadaveric transverse sinuses were dissected, and their detailed morphology was recorded. Classification schemes were applied based on the anatomy and orientation of each chordae. Chordae were found on 70% of sides and were statistically more likely to be found on right sides (86.6%) (P < 0.01). Three types and 3 classes of chordae were identified. There was a statistically significant difference between sides regarding type of chordae (P = 0.02). To date, a comprehensive anatomic evaluation of the intraluminal chordae of the transverse sinuses has been lacking. From their origins as cardiovascular research tools, endovascular techniques have evolved to provide a minimally invasive means of diagnosis and therapy for individuals suffering from occlusive artery disease. The techniques were pioneered by William Harvey, whose work set the stage for all subsequent endovascular experiments. These included the bold self-catheterization procedure performed by Werner Forssmann in 1929, which would lead to his dismissal by his superiors, only to regain respect within the medical community in 1956 on receiving the Nobel Prize. Charles Dotter was the first to understand the true potential of endovascular approaches after a chance recanalization that would catapult arterial catheterization first into the cardiovascular surgical arena, then into neurosurgery for intracranial stenoses. Having been meticulously evaluated and compared with open vascular procedures, endovascular neurosurgery has continued to be refined and optimized. Understanding the history and development of these techniques and their applications in neurosurgery is necessary to appreciate the current clinical utility of these procedures, serving to provide the vascular neurosurgeon a greater array of treatment options for patients. Here we explore the major scientific and technological advancements that facilitated the development of the endovascular approach to cerebral revascularization, as well as current indications and ongoing clinical trials. Up to 20% of patients fail to achieve reperfusion with modified Thrombolysis in Cerebral Infarction (mTICI) scores of 0–1 after mechanical thrombectomy (MT). Furthermore, underlying intracranial atherosclerotic disease, particularly when associated with >70% residual or flow limiting stenosis, is associated with higher rates of failed MT and high failure risk MT. The aim of this study was to systematically review the procedural and clinical outcomes in patients with failed MT and high failure risk MT. We also explored differences between patients receiving acute rescue stenting compared with medical management alone. A systematic literature search was conducted in Ovid MEDLINE, PubMed, Embase, and Cochrane online scientific publication databases for English language publications from their date of inception until October 2018. Studies including adult patients with acute ischemic stroke because of emergent large vessel occlusion with failed (mTICI score 0–1) or high failure risk MT within the anterior circulation who underwent rescue stenting were included. A systematic review and meta-analysis of proportions was performed. Rescue intracranial stenting after failed MT or high failure risk MT results in improved clinical outcomes compared with patients without stenting (48.5% vs. 19.7%, respectively; P < 0.001), without an increase in the rate of symptomatic intracranial hemorrhage, despite additional use of antiplatelet agents (9.7% vs. 14.1%, respectively; P = 0.04). In patients who fail initial attempts at MT or are high risk for acute reocclusion, rescue intracranial stenting could be considered with the aim to improve functional outcomes. The Woven EndoBridge (WEB) device, an intrasaccular flow-diverter that does not require long-term antiplatelet use, has recently emerged as an alternative to embolize complex intracranial aneurysms; however, there are limitations to the WEB device. First, it does not immediately secure the aneurysm in most subarachnoid hemorrhage cases. Second, it may not be suitable for embolization of wide-neck aneurysms with an unfavorable aspect ratio. To overcome these limitations, we have used the WEB device in conjunction with stenting and/or coiling. Here, we present a technical note with an illustrated case series and provide a detailed step-by-step description on how the WEB device can be used in adjunct to coiling and/or stenting to achieve successful angiographic results. Internal jugular vein stenosis (IJVS) results in poor venous outflow and can result in intracranial hypertension. Venous stenting has become a debated topic for correction of this pathology. A 49-year-old male merchant with bilateral IJVS presented with headache, dizziness, and pulsatile tinnitus. He was found to have intracranial hypertension and left IJVS caused by styloid process compression and right IJVS caused by damage of the venous intima. His symptoms improved after undergoing styloidectomy followed by left intra–internal jugular vein (IJV) balloon. However, 1 year later, the prior symptoms reoccurred. At this time, the patient underwent right intra-IJV stenting. After treatment of the contralateral side, the symptoms resolved during the following 3 months. The clinical practice in this case indicated that in patients with bilateral IJVS, a 2-side intervention may be necessary when unilateral correction fails. We advise a staged approach to correction of bilateral IJVS. Styloid compression–induced IJVS should be corrected by styloidectomy in combination with balloon and/or stenting, whereas IJVS induced by venous wall issues needs only stenting. Carotid endarterectomy (CEA) and carotid artery stenting (CAS) are well-established treatments that have been shown to decrease stroke recurrence in patients with underlying carotid artery disease. We assessed clinical outcome, safety, and restenosis rates for patients who underwent standardized CEA or CAS at our tertiary care center using patient selection criteria based on available scientific evidence. Retrospective chart review of patients who underwent CEA or CAS between 2009 and 2016. In total, 314 cases (204 with CEA and 110 with CAS) were analyzed. Patients were predominantly white (84.4%), men (61.1%) with hypertension (86.9%) and hyperlipidemia (81.8%). Most patients (84.5%) had symptomatic carotid disease. No significant differences were observed in median postoperative National Institutes of Health Stroke Scale and modified Rankin scale (mRS) scores based on pretreatment symptomatic status or treatment modality (CEA vs. CAS). Most patients (85.9%) had favorable outcomes (mRS score 0–2) at a median follow-up of 11.7 months (interquartile range, 1.8–28.8). The perioperative complication rate was low (3.2%), and permanent neurologic deficit was seen in only 3 patients (1%). Restenosis was found in 7.3%, without significant difference between CEA and CAS at last follow-up. Restenosis was asymptomatic in most patients. Our findings in a real-world predominantly symptomatic cohort demonstrate that favorable patient outcomes and low restenosis and complication rates can be achieved with both CEA and CAS by the utilization of a consistent institutional patient selection and treatment process. The pathophysiology of de novo aneurysm after stenting is poorly understood. Hemodynamic effects may play an important role. We report and discuss the hemodynamic analysis of de novo aneurysm after intracranial stenting. A 51-year-old female underwent angioplasty and coiling for stenosis associated with an aneurysm of the left intracranial carotid artery. Six years later, the follow-up digital subtraction angiography demonstrated a de novo aneurysm at the proximal tine of the Wingspan stent. Computational fluid dynamics simulations showed that stenting for intracranial stenosis resulted in higher oscillatory shear index and lower time-averaged wall shear stress in the neck region of de novo aneurysm. The hemodynamic change after stenting may lead to formation of a de novo aneurysm. After an initial injury that might result from the tine on the endothelial cells, stenting could produce high oscillatory shear index and low time-averaged wall shear stress near the tine, thus potentially inducing de novo aneurysm formation. A nonbifurcating cervical carotid artery (NBCCA) is a rare anatomic variation of the carotid artery which directly arises from the common carotid artery without forming a bifurcation. A rare case of an NBCCA associated with middle cerebral artery (MCA) stenosis is reported herein. A 70-year-old man was admitted to the hospital for fresh infarctions in the left frontoparietal lobe. The angiographic imaging showed severe atherosclerotic stenosis in the left MCA associated with an ipsilateral NBCCA. The origin of the left internal carotid artery (ICA) was occluded leaving a stump. A tortuous anastomotic vessel arose from the external carotid artery and continued as the cervical ICA. A stent was placed in the left MCA, and the patient recovered uneventfully and without further ischemia during follow-up. Embryogenesis and naming considerations are briefly discussed. This is the first report of an NBCCA accompanied by ipsilateral MCA stenosis. The name hypogenesis/agenesis of the proximal ICA might describe the congenital anatomic variation of an NBCCA more accurately than NBCCA. [ABSTRACT FROM AUTHOR]

Objective High-grade stenoses of the common carotid artery (CCA) are rare and represent a therapeutic dilemma for the treating physician. Endovascular treatment through antegrade access has been proposed as a less invasive treatment option. The aim of this study was to review the existing literature on such a procedure. Methods A meta-analysis of all studies reporting on endovascular treatment for the treatment of CCA was performed. Results Six studies, including 227 patients, were identified. Forty-nine percent of the patients were male, and 55% symptomatic. Lesions were located in CCA in 237 cases, in innominate artery in 4 cases, combined with internal carotid artery in 28 and subclavian artery in 5 cases. The reported technical success of the procedure was 97.9%. In the 234 successful endovascular treatments, a stent was implanted. Thirty-day mortality and stroke rate were 0.4% and 3.1%, respectively. During a mean follow-up of 8.4 to 31.7 months, 4 patients presented symptoms of cerebral ischemia and 18 died (none related to the procedure). Thirty-one patients developed restenosis of the CCA lesion; successful repercutaneous transluminal angioplasty was performed in 9 cases. Conclusions This meta-analysis reported the largest collection of patients having undergone endovascular treatment of CCA stenosis or occlusion. Results from this study showed that 30-day mortality and stroke rate were 0.4% and 3.1%, respectively. The procedure is absolutely effective, and no significant change of stroke was found with or without an embolic protection device. Objective The management of chronic complete internal carotid artery (ICA) occlusion (COICA) has been challenging. Endovascular procedures have been performed with variable success and risks, depending on the type of occlusion and distal revascularization. We present a novel hybrid procedure to recanalize the ICA when previous endovascular interventions have failed or been deemed too risky. Methods Two patients presented with symptomatic COICA after maximal medical management. They were deemed at high risk of endovascular intervention and/or previous endovascular attempts had failed. Thus, they had indications for a hybrid procedure. Results A hybrid technique was used to create a stump by surgical endarterectomy, followed by recanalization using an endovascular approach via femoral access. We have described the technique in detail. Postoperative computed tomography perfusion scanning showed normalization of the mean transient time, cerebral blood volume, and cerebral blood flow compared with the preoperative findings. Cerebral angiography showed successful recanalization of the ICA. Neither patient experienced any complications. Conclusion A hybrid technique is feasible and should be considered for patients with COICA in whom maximal medical management has failed and who have a high-risk profile for endovascular intervention or in whom previous endovascular attempts have failed. Background Thromboembolic complications remain a major reason for morbidity and mortality after flow diversion, in addition to hemorrhagic complications not limited to the brain predicated on the prolonged need for dual antiplatelet therapy. Surface modification to decrease thrombogenicity and accelerate aneurysm occlusion is a promising alteration to mitigate those risks. The Pipeline embolization device with Shield Technology possesses such characteristics; however, it has not yet been approved in the United States. Case Description We present the case of a 38-year-old woman with a Spetzler-Martin grade 4 left parietal arteriovenous malformation with a total of 8 feeding vessel aneurysms and recent rupture of 1 of those aneurysms that was treated with balloon-assisted coil embolization. Four M1-segment middle cerebral artery aneurysms and 1 supraclinoid internal carotid artery aneurysm were treated with 2 Pipeline embolization Shield devices after a compassionate use request was approved. At 3 weeks after the procedure, the patient experienced a minor ischemic stroke after having missed the prescribed dual antiplatelet therapy. At the 3-month follow-up examination, the patient was neurologically intact and all the aneurysms were occluded. Dual antiplatelet therapy was discontinued. Conclusions Flow diverters with surface modification to decrease thrombogenicity and accelerate aneurysm occlusion are promising endovascular tools for patients at high risk of complications with dual antiplatelet therapy. The present case represents one of the first cases performed in the United States using such a device. Despite the favorable outcome in the present case, the question of whether such technology obviates the need for dual antiplatelet therapy remains to be determined. Background/Objective PulseRider is a new endovascular stent dedicated to treat bifurcation intracranial aneurysms with a wide neck. Our purpose was to evaluate 6-month clinical and anatomic results of the device when used to facilitate endovascular coiling of wide-neck bifurcation aneurysms. Methods Unruptured intracranial aneurysms coiled with PulseRider, in 6 European centers and 1 U.S. center, were retrospectively reviewed from June 2014 to October 2015. Immediate and 6-month results were evaluated independently by using the Raymond classification scale. Recanalization was defined as worsening, and progressive thrombosis was defined as improvement on the Raymond scale. Results Nineteen patients (10 women, 9 men; mean age, 63 years) harboring 19 bifurcation aneurysms (mean dome size, 8.8 mm; mean neck size, 5.8 mm) were included. Immediate angiographic outcome showed 11 complete aneurysm occlusions, 6 neck remnants, and 2 residual aneurysms. Follow-up at 6 months, obtained in all patients, included 12 complete aneurysm occlusions (63.1%), 6 neck remnants (31.6%), and 1 residual aneurysm (5.3%). Adequate occlusion (defined as complete occlusion and neck remnant combined) was observed in 94.7%. Progressive thrombosis was observed in 2 cases (10.6%) and recanalization in 1 case (5.3%). There was no in-stent stenosis or jailed branch occlusion. No bleeding was observed during the follow-up period. Permanent morbidity rate was 5.3% (1/19), and the mortality rate was 0% at 6 months. Objective The Pipeline Embolization Device (PED) has been used and shown to be safe under monitored anesthesia care (MAC). We present the results of the first study, to our knowledge, assessing the safety and feasibility of same-day discharge in patients undergoing treatment with the PED, using MAC. Methods A total of 130 patients with 143 cerebral aneurysms (CAs) were identified. Patients were treated under MAC with the PED. All of the patients were counseled preoperatively about the elective nature of the procedure and the same-day discharge. Results The mean age of the participants was 60.7 ± 12.12 years. Men constituted 27.69% (36/130) of the population. The number of procedures was 138. All of our patients elected to return home the same day, whereas only 6.15% (8/130) of them changed their mind in the postoperative setting and elected to stay overnight out of convenience (late discharge, patient preference, or living alone at home). Overall discharge home on the same day occurred after 90.6% of procedures (125/138) and in 91.53% (119/130) of the patients. All same-day discharges took place within 4–6 hours after the procedures. The rate of major complications was 0.75% (1/134). The mortality rate was 0%. Conclusions PED treatment under MAC is feasible and safe. Intracranial stenosis is among the common causes of ischemic strokes, especially in the Asian, African, and Hispanic populations. The Stenting and Aggressive Medical Management for Preventing Recurrent Stroke in Intracranial Stenosis trial showed that, by 30 days, 33 (14.7%) of 224 patients in the stenting group and 13 (5.8%) of 227 patients in the medical group had died or had a stroke. 1 The WEAVE (Wingspan Stent System Post Market Surveillance) trial showed 2%−2.7% periprocedural complications. 2 However, with technical advancements and more experience, angioplasty and stenting in high-risk cases are feasible and successful even in vessels <2 mm. We present the technical nuances of angioplasty and stenting of a sub-2-mm intracranial vessel by submaximal balloon inflation. Both patients had recurrent ischemic stroke symptoms on best medical management. Best medical management includes a dual antiplatelet regimen with a high-dose statin. Objective Cavernous carotid aneurysms are considered benign lesions with indolent natural history. Apart from idiopathic aneurysms, traumatic, iatrogenic, and mycotic aneurysms are common in the cavernous segment of the carotid artery. With rapid advances in endovascular therapy, management of cavernous carotid aneurysms has evolved. Our aim was to review the management options available for cavernous carotid aneurysms. Methods The English literature was searched for various studies describing the management of cavernous carotid aneurysms and the evolution of various treatments was studied. Results Numerous treatment options are available such as conservative management, Hunterian ligation, surgical clipping, and endovascular therapy. The introduction of flow-diverting stents has revolutionized the management of these lesions. The evolution of various treatment strategies are described. Conclusions A thorough knowledge of all the options is paramount to individualize therapy. Subclavian steal syndrome is characterized by the vertebral artery flow inversion as a result of a stenotic lesion in the origin of the subclavian artery. Subclavian origin stenting is an important armamentarium of neurointerventional surgeons. A 79-year-old patient presented with left arm claudication and dizziness while exercising, alongside upper extremity coolness at rest. Examination revealed blood pressure difference of 15 mm Hg in the left arm when compared with the right, with Doppler ultrasonography demonstrating reversal of flow in the left vertebral artery. Aortic arch run with pigtail catheter demonstrated the extent of stenosis and served as a roadmap for stent deployment. Placement of a long sheath across the subclavian stenosis into the aortic arch allowed atraumatic delivery and precise deployment of the covered stent (Video 1). No neurologic deficits were reported postoperatively, with Doppler ultrasonography revealing anterograde flow in the left vertebral artery demonstrating resolution of subclavian steal syndrome. Results from studies comparing carotid artery endarterectomy (CEA) with carotid artery stenting (CAS) in the elderly population are variable in the literature. The objective of this study was to investigate whether CEA or CAS is associated with a better safety profile in older adults (>80 years of age) for treatment of symptomatic and asymptomatic stenosis. A random-effects meta-analysis was performed, and the I2 statistic was used to assess heterogeneity according to Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Subgroup analyses were performed as needed. Nine studies comprising 5955 patients were included in this meta-analysis. No differences were identified in terms of 30-day stroke (CEA: 5.8% [n = 257/4415]; CAS: 10.5% [n = 81/767]; odds ratio [OR], 0.57; 95% confidence interval [CI], 0.30–1.08; I2 = 26.1%), myocardial infarction (MI) (CEA: 1.1% [n = 4/357]; CAS: 0.5% [n = 2/355]; OR, 1.67; 95% CI, 0.37–7.46; I2 = 0%), transient ischemic attack (TIA) (CEA: 0% [n = 0/98]; CAS: 4.2% [n = 7/166]; OR, 0.28; 95% CI, 0.03–2.52; I2 = 0%), death (CEA: 1.5% [n = 8/523]; CAS: 0.9% [n = 4/431]; OR, 1.41; 95% CI, 0.43–4.58; I2 = 0%), and cranial nerve injury (CEA: 5.8% [n = 3/51]; CAS: 0% [n = 0/51]; OR, 4.74; 95% CI, 0.5–44.98; I2 =0%). A subgroup comparing CEA with transfemoral protected CAS showed that patients in the CEA group had a statistically significant lower risk of 30-day stroke (OR, 0.31; 95% CI, 0.17–0.57; I2 = 30.8%). This study shows that CEA is associated with a statistically significant lower risk of 30-day stroke in the elderly population compared with transfemoral CAS with distal or proximal protection. Background Results from studies investigating the effect of contralateral carotid occlusion (CCO) in patients with carotid artery stenosis undergoing carotid artery endarterectomy (CEA) or carotid artery stenting (CAS) are variable in the literature. We sought to determine whether CEA or CAS is the optimal revascularization approach for patients with CCO. Methods This meta-analysis was performed according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. A random effects model meta-analysis was conducted, and the I 2 statistic was used to assess for heterogeneity. Subgroup and sensitivity analyses were performed as needed. Results Five retrospective observational cohort studies comprising 6346 patients were included. Patients in the CEA group had a significantly lower risk of 30-day periprocedural mortality (odds ratio, 0.46; 95% confidence interval, 0.30–0.71; I 2 = 0%). However, no significant differences were identified in terms of stroke, myocardial infarction (MI), and major adverse cardiovascular events (MACEs) between the 2 groups. Subgroup analyses of symptomatic and asymptomatic patients did not yield significant differences for stroke, MI, and death. Conclusions Patients with CCO can safely undergo both CAS and CEA with similar risks of stroke, MI, and MACE. However, patients treated with CEA have a lower risk of 30-day periprocedural mortality. Future studies can help further clarify the ideal approach for these patients. Objective To investigate the endovascular treatment (stenting with or without coiling) of cervical and intracranial lesions associated with internal carotid artery (ICA) redundancy. Methods Nine patients harboring 12 lesions of dissections, pseudoaneurysms, and aneurysms in the presence of ICA redundancy were treated using stent deployment with or without coiling. There were 8 female and 1 male patients with an age range of 31–89 years (mean, 55 years). Results Two patients with ICA dissections were treated with stenting alone while all the other pseudoaneurysms and aneurysms were managed with both stenting and coiling. Fourteen stents were deployed, and the success rate of stent deployment was 100%. All the lesions were successfully occluded with subtotal occlusion in 3 lesions and total occlusions in the remaining lesions. Mean follow-up was 7.3 months (range, 3–25 months), during which time the lesions remained occluded with patent stents. No symptoms existed at follow-up. Conclusions Vascular lesions related to the internal carotid artery redundancy can be successfully managed endovascularly, and the redundancy can no longer represent an obstacle in modern era with advanced techniques and materials. Purpose To investigate the effect and safety of endovascular treatment of intracranial aneurysms concomitant with severe adjacent atherosclerotic stenosis. Materials and Methods Twenty-six patients with aneurysms and adjacent stenosis were prospectively enrolled. The characteristics of the aneurysm, parent artery, atherosclerotic stenosis and endovascular treatment methods were analyzed. Results All aneurysms were successfully embolized (100%), with stent-assisted coiling in 14 (53.8%) cases, coiling alone in 10 (38.5%), double microcatheter coiling in 1 (3.8%), and balloon-assisted coiling in the remaining 1 (3.8%). Immediately after embolization, complete occlusion was achieved in 10 cases (38.5%), nearly complete occlusion in 6 (23.1%) and non-complete occlusion in 10 (38.5%). Ten aneurysms were type I and were managed with coiling alone in 8 cases and stent-assisted coiling in the remaining 2 cases, with complete occlusion achieved in 6 cases (60%), nearly complete in 2 (20%), and noncomplete in the other 2 (20%). Sixteen aneurysms were type II and treated with stent-assisted coiling in 12 cases (75%), single coiling in 2 (12.5%), double microcatheters in 1 (6.3%), and balloon-assisted coiling in the remaining aneurysm (6.3%). Aneurysm occlusion was complete in 4 cases (25%), nearly complete in 4 (25%), and noncomplete in the other 8 (50%). Clinical follow-up of 2 months to 5 years (mean 26 ± 11 months) demonstrated no rebleeding, with the modified Rankin scale score of 0–2 in 20 patients, 3 in 4, and 6 in the remaining 2. Conclusions Intracranial aneurysms concomitant with severe adjacent atherosclerotic stenosis can be successfully treated endovascularly, and careful evaluation of the characteristics of the aneurysm, parent artery, stenosis and collateral circulation can help reducing complications. Background Symptomatic subclavian artery total occlusion is widely treated with an endovascular procedure that often results in distal vertebral artery embolism. Therefore, protection devices are important. Establishing a filter or balloon device in the vertebral artery can protect against this distal embolism. However, the use of embolic protection devices is not easy, and it makes the procedure more complicated. Here, we report a case of symptomatic subclavian artery total occlusion that was treated successfully with a balloon-guiding catheter and the pull-through technique. Case Description A 67-year-old man developed intermittent motor weakness in his left arm. Aortic angiography demonstrated a complete occlusion of the left proximal subclavian artery and a retrograde flow through the left vertebral artery to the distal brachial artery. In this case, we used a balloon-guiding catheter and the pull-through technique to prevent distal embolism. The balloon-guiding system was useful not only for embolic protection but also for scaffold during excavation and for the centering effect against invisible vessels. The pull-through technique enabled our devices to deliver easily and smoothly. The patient was treated successfully without complications. Conclusions The distal and proximal combined endovascular treatment with a transbrachial balloon-guiding catheter is a beneficial treatment option for patients with subclavian artery total occlusion. Objective: This paper discusses indications for and the technique of endoscopic aqueductoplasty with stenting. Methods: We review and summarize the clinical experience with endoscopic aqueductoplasty gained over the last 15 years on pediatric and adult patients. Results: Endoscopic aqueductoplasty with stenting presents a safe procedure. In well selected patients, it provides a long-term, stable clinical course. Aqueductoplasty alone has a high reclosure rate and should be avoided. Conclusions: Aqueductoplasty with stenting is the procedure of choice for the treatment of isolated fourth ventricle. Membranous and tumor-related aqueductal stenosis should be treated by endoscopic third ventriculostomy. [Copyright &y& Elsevier]

Objective: Microsurgical fenestration of the third ventricular floor performed in one session with resection of deep seated tumors has been recently demonstrated as an approach to specifically address the concomitant obstructive hydrocephalus. As with endoscopic third ventriculostomy, occlusion of the stoma may result in progression of the obstructive hydrocephalus. In order to provide reliable communication between the basal cisterns and ventricles, we propose stenting of the stoma in cases of direct surgical approach to deep seated tumors. Methods: After performing tumor resection through the anterior transcallosal approach, premamillar and Liliequist''s membranes were identified and fenestrated. A silicon stent was inserted into the prepontine cistern through the fenestrated floor of the third ventricle; the stent connected the third and lateral ventricles with the basal cisterns. Results: Microsurgical ventriculostomy of the third ventricle and stenting of the stoma was performed in 9 patients simultaneously with tumor resection (5 cases), open biopsy (3 cases), or microsurgical dissection of severe adhesions at the level of Monro foramina (1 case). In 7 cases, the third ventricular floor was infiltrated with the tumor and obstruction of the aqueduct persisted after tumor surgery; in 2 patients, high risk of reocclusion at the level of Monro foramen was expected. Stenting of the ventricular system provided patency of the stoma and Monro foramen. None of the patients required a shunt postoperatively. The follow-up time ranged from 3 to 22 months. Conclusion: Microsurgical fenestration of the third ventricle floor combined with stoma stenting can be a viable option for hydrocephalus control. [Copyright &y& Elsevier]
