This article, composed using literary sources and archival materials, is dedicated to the 155th anniversary of Alexander Grigoryevich Lyutkevich - a prominent Russian ophthalmologist of the late 19th and early 20th centuries (12.09.1867-18.04.1928) and one of the founders of the Voronezh Medical Institute. The paper traces the main stages of his biography, notes his work in rapid-reaction emergency care units, and pays attention to his teaching activities. The importance of Lyutkevich's doctoral dissertation on the treatment of high myopia is emphasized. The article also highlights his important role as editor of the classic textbook "Course of eye diseases" by A.A. Kryukov. In Lyutkevich's public activities, his contribution to the establishment of the Society of Ophthalmologists in Moscow is particularly noteworthy. Lyutkevich's high organizational skills are highlighted, which were clearly manifested in the Yuriev University and in the creation of the medical faculty of the Voronezh University.

The paper presents a detailed analysis of 27700 patients with ocular injury who received a therapeutic-and-diagnostic care at the first specialized stage. The specific features of an oculist's care in an intensive care unit and the required volume of diagnostic studies in different types of posttraumatic pathology of the eye and its appendages are defined. Indications for emergency hospitalization in fresh trauma and sequels are presented. Algorithms of treatment have been developed and are recommended in relation to the pattern and severity of injuries of the organ of vision.

The paper presents the results of studying the prevalence of myopic refraction in adolescents in the Republic of Khakasia in relation to the state of their accommodation-convergence apparatus, constitution, ethnicity, and the type of a settlement. Myopia has been found to occur 3 times more frequently among the urban adolescents. The results have led to the conclusion that urban living conditions may be a determinant of enhanced refraction.

Based of their experience and analysis of the papers available in the medical literature, the authors describe the clinical picture of malignant lymphomas of the organ of vision. They give a concept of the current classification of lymphomas and information on the specific features of their course, by taking into account the site of the process (eyelids, conjunctiva, intraocular and orbital regions) and present recommendations on their treatment.

The paper presents two classifications of the dry eye syndrome: the Madrid and Russian classifications. The creation of its classification is required for the elaboration of treatment policy for this category of patients. The classifications are based on the scientific data on the epidemiology, etiology, and pathogenesis of the dry eye syndrome. Each classification identifies different forms of the dry eye. The classifications virtually published at the same time do not greatly differ from each other and they reflect the current approach to systematizing different forms of the dry eye. The paper analyzes the Madrid versus Russian classifications.

The paper considers the armament development ultimately resulting in the predominance of using minor blasting assemblies and accordingly to a drastic prevalence (as high as 80%) of mine explosion injuries among other battle injuries to the eye. The particular severity of these injuries requires their efficient subdivision into types and severity, which is alleviated by the introduction of a new classification of mechanical injuries into the eyeball. It is obligatory for each ophthalmologist to know this classification. It is the ophthalomologist who may be the first specialist to start rendering an aid to the victim. In terms of the highest level of development of modern ophthalmological diagnosis and intraocular eye surgery, delivery of a patient to a specialized eye traumatological center where he/she will be given a comprehensive aid, including an intraocular one, in 3-7 days rather than the promptest surgical debridement (generally as just wound closure) is a master link in obviating the losses associated with eye injury. There is evidence that the same interregional centers as the army centers that have operated in Kabul and Rostov-on-Don should be set up preferably on the basis of multidisciplinary hospitals in the civil health care system. They could become an original testing area for researches of explosive injuries to the eye and a source of materials for making the centralized national register of eye injury.

The paper covers the issues of blindness and disability associated with pathology of the organ of vision. It lists the principal causes of blindness in the developed and developing countries. Basic tendencies in solving the problems of blindness and weak sight are defined. The issues of disability due to pathology of the organ of vision and the ways of its reduction are discussed.

Outlined in the paper are the basic trends and tasks of clinical-and-social ophthalmology--research and practice related with the medical-and-social consequences of disabling visual impairments as well as of blindness and disability due to vision. They comprise: 1) A study of the epidemiological, social-and-hygienic and clinical-and-ophthalmologic aspects of blindness and disability; 2) Scientific elaboration of methodological, clinical, social and managerial positions and criteria of ophthalmologic and of medical-and-social expertise; 3) A theoretical definition of the methodological principles for medical-and-social rehabilitation of the disabled due to vision. Data are presented of the rate of blindness and disability due to ophthalmic pathologies in the Russian Federation. The modern standpoints of ophthalmologic medical-and-social expertise and rehabilitation of persons with visual lesions are elucidated.

The approach of a country to persons with limited physical abilities is an important component of country's social-and-cultural policy. Blindness is a most severe variety of health disorders leading to social defect and social insufficiency. The role of adaptive physical culture is important within the system of the social-and-medical rehabilitation of the blind because the sedentary life mode has a negative effect on organisms of the disabled due to vision. Sports is not only a method for correcting the general somatic condition, but it is also an important social-and-physiological factor that enables the blind to comprehend their abilities as an example for others. As for our country, the information about the modern sports activities among the disabled due to vision is absolutely insufficient for the public at large. An analysis of results of examination (conducted by using the computer data base of the International Blind Sportsmen Association--IBSA) of 2386 blind and impaired vision sportsmen is presented in the paper. Data about sport disciplines and types of ophthalmic pathologies encountered among the high-class blind and impaired vision athletes from different world countries are described. The above data can be helpful in elaborating the rehabilitation programs for persons with severe disorders of the organ of vision to be used in rehabilitation centers, sport federations and clubs of the disabled as well as in other institutions dealing with rehabilitation of the disabled due to vision in our country.

In this summing-up paper the authors discuss the results of surgical prevention and correction of progressive myopia carried out at Department of Ophthalmic Diseases of Russian University of Peoples' Friendship during two recent decades. Remote results of bandaging scleroplastic and reduced chondroplastic operations recommend scleroplastic interventions for prevention of myopia progress. Combined scleroplasty and cataract extraction stabilized the elongation of the antero-posterior axis of the eye and essentially improved visual acuity of 18-20 diopters. New variants of interlamellar operations are presented: tunnel-circular and fragmented circular, which are protected by patents of the Russian Federation. These operations are recommended for patients with stable myopia of 8-15 diopters after scleroplasty. Their advantage is low traumatism, absence of deep incisions on the cornea, wide optic center, possibility of regulating the refraction effect by replacing the implants or their complete removal without serious consequences for the cornea.
