Objective To determine the pathophysiological progresses following bilateral cavernous nerve crushing injury, as an index for a treatment point and establishment of adequate treatment strategies for neurogenic erectile dysfunction. Materials and Methods Thirty-six rats were assigned to 1 of 6 groups, and bilateral cavernous nerve crushing or sham surgery was performed. Functional testing and ultrastructural analyses were performed immediately and at 7, 14, 21, and 28 days after the cavernous nerve (CN) injury (n = 6). Results Intracavernos pressure lowered progressively from 7 days to 14 days post-injury, and histologic staining revealed that the number of neuronal nitric oxide synthase-positive nerve fibers on the dorsal penile nerve decreased significantly and progressively from 7 days until 21 days post-injury. Furthermore, ultrastructural analyses revealed axon loss and demyelination of the CN at 7 and 14 days post-injury. However, it is followed by partial spontaneous recovery of erectile function and regeneration of the CN at 28 days post-injury, suggesting that these time points may be useful for evaluating the effects of drug treatments. Furthermore, we found that CN injury-induced damage to corporal smooth muscle cells was irreversible; therefore, focusing on protecting the corpus cavernosum from apoptosis may be more important than nerve protection when assessing treatment mechanisms in the CN injury model. Conclusion Our study makes a significant contribution to the human diagnostic pathology literature because it describes characteristics of relevant tissue in the rat and provides information regarding time points that may be useful for future studies of pathologic mechanisms or treatment evaluations.

Objective To show the flexibility in magnetic resonance imaging (MRI) of seminal vesicle (SV) and intra-abdominal segment of vas deferens for the patients with congenital absence of the vas deferens (CAVD), including congenital bilateral absence of the vas deferens (CBAVD) and congenital unilateral absence of vas deferens (CUAVD). Methods Fourteen consecutive patients with CAVD had transrectal ultrasonography (TRUS) and further MRI evaluations. TRUS was performed using a 7.5-MHz transducer, and images of the SVs were obtained, calculated in the transaxial plane. MRI studies were performed with a 1.5-7 superconducting system, T1- and T2-weighted axial, coronal, and sagittal imaging of the pelvis was obtained. If the SVs were present, then their size was measured for the morphologic classification and diagnosis. All of the patients also received cystic fibrosis transmembrane conductance regulator ( CFTR ) gene mutation testing. Results In a series of 12 men with CBAVD, only 4 were found to have bilateral SV agenesis using MRI. The remaining 8 men with unilateral hypoplasia still had SV remnants. MRI also detected the intra-abdominal segment of the vas deferens. Through our study of MRI, SV agenesis is not well associated with the presence of CFTR mutation in patients with CAVD. Conclusion MRI provides a precise imaginal diagnosis of SV defect, which is superior to the TRUS examination for the patients with CAVD. Compared with the previous inaccurate examination method of TRUS, this study demonstrates that MRI can provide better images for the patients with CAVD for the clinical diagnosis of existing defects of internal seminal tract and internal organs.

To investigate whether the therapeutic effect of Ginkgo biloba extract (GBE) in a rat model can improve erectile dysfunction after bilateral cavernous nerve injury.Forty-three male Sprague-Dawley rats underwent cavernous nerve crush injury and were randomized into 4 groups, including: vehicle only, high-dose GBE, medium-dose GBE, and low-dose GBE. Eight animals underwent sham operation. Four weeks later, erectile function was assessed by cavernous nerve electrostimulation, and penile tissue was collected for histologic analysis.Significant recovery of erectile function was observed in the high-dose GBE group in a dose-dependent manner as compared with the vehicle-only group (P.001). The high-dose GBE group had a significant increase in neurofilament-1 expression (P.001), preservation of neural nitric oxide synthase nerve fibers of the dorsal penile nerve (P.05), and increased smooth muscle cell content (P.001) compared with the vehicle-only group. In addition, high-dose GBE markedly augments the smooth muscle-to-collagen ratio (P.05) and reduces the apoptotic index.Administration of GBE increases neuron survival and preserves the neural nitric oxide synthase nerve fiber and contents of the corpus cavernosum after bilateral cavernous nerve injury. These implications indicate the beneficial effects of GBE use in the repair of the cavernous nerve and recovery of erectile function after radical prostatectomy.

Objective To investigate the association between serum sex hormone levels and lower urinary tract symptoms in men aged 40-79 years. Methods A cross-sectional study was conducted in 509 men (mean age 58 years). The serum total testosterone (TT), dihydrotestosterone, and estradiol levels were measured. The total prostate volume measured by transrectal ultrasonography and International Prostate Symptom Score (IPSS) questionnaire were obtained. Correlations were determined using univariate and multivariate regression analysis. Results The subjects with moderate to severe lower urinary tract symptoms (total IPSS ≥8) were older, with a greater incidence of hypertension and diabetes, a larger prostate, and had lower serum TT levels. On the univariate analysis, the serum TT levels were negatively associated with the total IPSS, IPSS storage subscore, weak stream, and nocturia. After adjusting for age, hypertension, diabetes, and total prostate volume, only the serum TT level was significantly associated with nocturia (>2 times/night; P = .042), and men with serum TT levels in the greatest quartile had a 44% reduced risk of nocturia than in the lowest quartile ( P = .037). Conclusion In our relative healthy male cohort, most IPSS items showed no significant association with serum sex hormone levels, except for nocturia, which showed a negative correlation with the serum testosterone level.

Objectives To use the International Index of Erectile Function (IIEF) to evaluate the improvement of erectile function and other sexual functions after testosterone monotherapy. Testosterone replacement therapy alone was reported to be effective for the improvement in sexual function in hypogonadal males. However, it is still unclear that which kind of the sexual function is most beneficial and to what extent the sexual function could be improved. Methods A double-blind, randomized, placebo-controlled study was conducted with a treatment group (n = 20) and control group (n = 20). Using a critical review of the different sexual functional domain scores of the IIEF-15 and the scores of the IIEF-5, we evaluated the sexual function of men in hypogonadal status before and after 3 months of testosterone gel treatment. Effect size was used to compare the drug effects for each sexual functional domain, and the results were confirmed by multivariate analysis. Results A total of 30 men remained at the end of the study. After 3 months of testosterone gel therapy for the hypogonadal men, the most beneficial effect on sexual function was erectile function, with sexual desire and orgasmic satisfaction insignificantly affected. Conclusions The results of our study have shown that transdermal testosterone gel treatment for hypogonadal patients can improve their sexual dysfunction mainly through restoring erectile function.

To conduct a cohort study in men older than 40 years in Taiwan to evaluate the prevalence of erectile dysfunction (ED) by self-report and the use of the abridged, five-item version of the International Index of Erectile Function (IIEF-5).From July to August 2003, 990 Taiwanese men older than 40 years of age completed a telephone survey designed to assess ED prevalence and sexual functioning. ED was identified through respondent self-report and investigator assessment using the IIEF-5 instrument. Data were analyzed using the analysis of variance test, t test, and chi-square test, with Yates' correction or Fisher's method.The prevalence of self-reported ED was 13.1%, with 26.2% of this subgroup subsequently deemed to not have ED by IIEF-5 assessment. Among the subgroup of men self-reported to not have ED, 18.8% were found to have ED by IIEF-5 assessment. In total, 26.0% of respondents had ED by IIEF-5 assessment. The ED prevalence increased with age and a history of chronic illness. The frequency of sexual contact and sexual satisfaction were also reduced in the respondents with ED.The prevalence of self-reported ED in Taiwanese men older than 40 years of age was 13.1% compared with 26.0% using the IIEF-5 assessment. Estimates of ED prevalence in the general population should not rely on self-reporting alone because it is likely to underestimate the true prevalence of ED.

Objectives We applied interphase fluorescence in situ hybridization (FISH) to testis sections to examine the evidence of spermatogenesis in patients with nonobstructive azoospermia. This technique was evaluated and compared with conventional testicular histopathologic findings for the possibility of additional clinical applications. Methods Thirty-seven consecutive patients with nonobstructive azoospermia were carefully evaluated clinically. Testes were biopsied for both sperm extraction and histopathologic examination. FISH staining was performed with a CEP 18 SpectrumAqua/CEP X SpectrumGreen/CEP Y SpectrumOrange probe. Results Eight of 11 cases (sensitivity 73%) that were found to have spermatids on the histopathologic slides also were proven to produce haploid cells by FISH staining. On the other hand, 21 of the 26 cases (specificity 81%) for which no spermatids could be found on the histopathologic slides also had only diploid cells by FISH staining. On the basis of the good correlation between the FISH staining and conventional histopathologic findings, we could confirm the diagnosis of spermatogenesis using both methods. Conclusions FISH staining of testicular sections allows more reliable prediction of spermatogenesis and provides benefits for a patient’s decision regarding fertility counseling.

Objectives. To evaluate the utility of no-scalpel vasal sperm aspiration and in vitro fertilization for the treatment of male infertility due to anejaculation. Methods. Vasal aspiration was performed using a no-scalpel technique, and the sperm were used for conventional in vitro fertilization in 1 patient and intracytoplasmic sperm injection in 6 patients. Results. The average total sperm count was 6 × 106, with the motility between 20% and 95%. No complications have been observed to date. A high fertilization rate (74.6%) and pregnancy rate (5 of 7) was achieved with subsequent intracytoplasmic sperm injection or in vitro fertilization. Conclusions. No-scalpel vasal sperm aspiration is a simple, effective alternative to sperm retrieval, especially for anejaculatory males.
