Abstract

頸部リンパ節転移制御に関する照射線量因子を明らかにするために，当院で2004年4月から2008年3月にCCRT後にplanned neck dissection （PND）を行ったN2-3中下咽頭扁平上皮癌29例の遡及的解析を行った。治療前の頸部Level別の評価では63領域に転移を認めた。領域別の頸部リンパ節転移に5mmのマージンを加えたものを計画標的体積（PTV）とし，PTV内の最大線量（PTV max），最小線量（PTV min）そして平均線量（PTV mean）を線量体積ヒストグラム（DVH）で評価を行った。病理学的消失（pCR）は全症例の59%であった。原発部位（中咽頭）及び導入化学療法の効果（partial response）がpCRの関連因子であった。領域別ではLevel Vと咽頭後リンパ節に最も病理学的残存を認めた。Level VのPTV maxとPTV meanがLevel II，IIIと比べて有意に低かった。さらに照射線量と頸部リンパ節転移の病理学的残存の有無に相関を認めたことから，頸部リンパ節領域への十分な線量投与により，制御率向上の可能性が示唆された。
