OBJECTIVES/HYPOTHESIS Anterior (Wendler) glottoplasty has become a popular surgery for voice feminization. However, there has been some discrepancy between its theoretical pitch-raising potential and what is actually achievable, and downsides to shortening the glottis have not been fully explored. In addition, descriptions of the surgery are inconsistent in their treatment of the vocal ligament. This study aimed to determine 1) how fundamental frequency (fo ) is expected to vary with length of anterior glottic fixation, 2) the impact of glottic shortening on sound pressure level (SPL), and 3) the effect of including the ligament in fixation. STUDY DESIGN Computational simulation. METHODS Voice production was simulated in a fiber-gel finite element computational model using canonical male vocal fold geometry incorporating a three-layer vocal fold composition (superficial lamina propria, vocal ligament, and thyroarytenoid muscle). Progressive anterior glottic fixation (0, 1/8, 2/8, 3/8, etc. up to 7/8 of membranous vocal fold length) was simulated. Outcome measures were fo , SPL, and glottal flow waveforms. RESULTS fo increased from 110 Hz to 164 Hz when the anterior one-half vocal fold was fixed and continued to progressively rise with further fixation. SPL progressively decreased beyond 1/8 to 1/4 fixation. Inclusion of the vocal ligament in fixation did not further increase fo . Any fixation increased aperiodicity in the acoustic signal. CONCLUSIONS The optimal length of fixation is a compromise between pitch elevation and reduction in output acoustic power. The simulation also provided a potential explanation for vocal roughness that is sometimes noted after anterior glottoplasty. LEVEL OF EVIDENCE NA Laryngoscope, 131:1081-1087, 2021.

Objective Most type 1 thyroplasty implants and some common injectable materials are mechanically stiff. Placing them close to the supple vocal fold mucosa can potentially dampen vibration and adversely impact phonation, yet this effect has not been systematically investigated. This study aims to examine the effect of implant depth on vocal fold vibration and vocal output. Study design Computational simulation. Methods Voice production was simulated with a fiber-gel finite element computational model that incorporates a three-layer vocal fold composition (superficial lamina propria, vocal ligament, thyroarytenoid muscle). Implants of various depths were simulated, with a "deeper" or more medial implant positioned closer to the vocal fold mucosa and replacing more muscle elements. Trajectories of surface and within-tissue nodal points during vibration were produced. Outcome measures were the trajectory radii, fundamental frequency (F0 ), sound pressure level (SPL), and smoothed cepstral peak prominence (CPPS) as a function of implant depth. Results Amplitude of vibration at the vocal fold medial surface was reduced by an implant depth of as little as 14% of the total transverse vocal fold depth. Increase in F0 and decrease in CPPS were noted beyond 30% to 40% implant depth, and SPL decreased beyond 40% to 60% implant depth. Conclusions Commonly used implants can dampen vibration "from a distance," ie, even without being immediately adjacent to vocal fold mucosa. Since implants are typically placed at depths examined in this study, stiff implants likely have a negative vocal impact in a subset of patients. Objectives/Hypothesis To test the hypothesis that subligamental cordectomy produces superior acoustic outcome than subepithelial cordectomy for early (T1–2) glottic cancer that requires complete removal of the superficial lamina propria but does not involve the vocal ligament. Study Design Computer simulation. Methods A computational tool for vocal fold surgical planning and simulation (the National Center for Voice and Speech Phonosurgery Optimizer-Simulator) was used to evaluate the acoustic output of alternative vocal fold morphologies. Four morphologies were simulated: normal, subepithelial cordectomy, subligamental cordectomy, and transligamental cordectomy (partial ligament resection). The primary outcome measure was the range of fundamental frequency (F0) and sound pressure level (SPL). A more restricted F0-SPL range was considered less favorable because of reduced acoustic possibilities given the same range of driving subglottic pressure and identical vocal fold posturing. Results Subligamental cordectomy generated solutions covering an F0-SPL range 82% of normal for a rectangular vocal fold. In contrast, transligamental and subepithelial cordectomies produced significantly smaller F0-SPL ranges, 57% and 19% of normal, respectively. Conclusion This study illustrates the use of the Phonosurgery Optimizer-Simulator to test a specific hypothesis regarding the merits of two surgical alternatives. These simulation results provide theoretical support for vocal ligament excision with maximum muscle preservation when superficial lamina propria resection is necessary but the vocal ligament can be spared on oncological grounds. The resection of more tissue may paradoxically allow the eventual recovery of a better speaking voice, assuming glottal width is restored. Objectives/Hypothesis: Viscoelastic properties of numerous vocal fold injectables have been reported but not at speaking frequencies. For materials intended for Reinke's space, ramifications of property values are of great concern because of their impact on ease of voice onset. Our objectives were: 1) to measure viscoelastic properties of a new nonresorbing carbomer and well-known vocal fold injectables at vocalization frequencies using established and new instrumentation, and 2) to predict phonation threshold pressures using a computer model with intended placement in Reinke's space. Study Design: Rheology and phonation threshold pressure calculations. Methods: Injectables were evaluated with a traditional rotational rheometer and a new piezo-rotary vibrator. Using these data at vocalization frequencies, phonation threshold pressures (PTP) were calculated for each biomaterial, assuming a low dimensional model with supraglottic coupling and adjusted vocal fold length and thickness at each frequency. Results were normalized to a nominal PTP value. Results: Viscoelastic data were acquired at vocalization frequencies as high as 363 to 1,400 Hz for six new carbomer hydrogels, Hylan B, and Extracel intended for vocal fold Reinke's space injection and for Cymetra (lateral injection). Reliability was confirmed with good data overlap when measuring with either rheometer. PTP predictions ranged from 0.001 to 16 times the nominal PTP value of 0.283 kPa. Conclusions: Accurate viscoelastic measurements of vocal fold injectables are now possible at physiologic frequencies. Hylan B, Extracel, and the new carbomer hydrogels should generate easy vocal onset and sustainable vocalization based on their rheologic properties if injected into Reinke's space. Vocal fold vibration depends critically on the viscoelasticity of vocal fold tissues. For instance, phonation threshold pressure, a measure of the "ease" of phonation, has been shown to be directly related to the viscosity of the vibrating mucosa. Various implantable biomaterials have been used in vocal fold augmentation surgery, with implantation sites sometimes close to or inside the mucosa. Yet their viscosities or other mechanical properties are seldom known. This study attempts to provide data on viscosities of commonly used phonosurgical biomaterials. Using a parallel-plate rotational rheometer, oscillatory shear experiments were performed on implantable polytetrafluoroethylene (Teflon or Polytef; Mentor Inc., Hingham, MA), collagen (Zyderm; Collagen Corp., Palo Alto, CA), glutaraldehyde crosslinked (GAX) collagen (Phonagel or Zyplast; Collagen Corp.), absorbable gelatin (Gelfoam; Upjohn Co., Kalamazoo, MI), and human abdominal subcutaneous fat. Samples of human vocal fold mucosal tissues were also tested. Under sinusoidal oscillatory shear at 10 Hz and at 37 degrees C, the dynamic viscosity was 116 Pascal-seconds (Pa-s) for polytetrafluoroethylene, 21 Pa-s for gelatin, 8-13 Pa-s for the two types of collagen, 3 Pa-s for fat, and 1 to 3 Pa-s for vocal fold mucosa. Results extrapolated to 100 Hz also show similar differences among the biomaterials, but all values are an order of magnitude lower because of the typical inverse frequency relation (shear thinning effect) for polymeric and biologic materials. The data suggest that the use of fat for vocal fold augmentation may be more conducive to the "ease" of phonation because of its relatively low viscosity, which is closest to physiologic levels. This implication is probably the most relevant in predicting initial outcome of the postoperative voice before there is any significant assimilation (e.g., resorption and fibrosis) of the implanted biomaterial.

Objectives: Previous measurements of viscoelastic properties of Zyderm were to be extended to low audio frequencies, and properties of two other biomaterials not previously measured, thiolated hyaluronic acid (HA-DTPH) and Cymetra, were obtained. Study Design: Rheologic investigation. Methods: Oscillatory shear stress was applied to each sample using a controlled stress rheometer at frequencies between 0.01 and 100 Hz with a parallel plate apparatus. Versuscoelastic moduli were recorded at each frequency. The calculated resonance frequency of the machine and sample were then used to determine the maximum frequency at which reliable data existed. Extrapolation functions were fit to viscoelastic parameters, which predicted the properties up to 1,000 Hz. Results: Frequency trends of Zyderm were similar to those previously reported, whereas magnitudes were different. The elastic moduli logarithmically increased with frequency, whereas dynamic viscosity demonstrated shear thinning, a condition of primary importance for humans to vocalize over a broad frequency range. Previous measurements were extended from 15 Hz up to 74 Hz. Differences in magnitude between a previous study and the present study were attributed to particulate orientation during testing. Cymetra was found to have nearly identical viscoelastic properties to those of bovine collagen, both in magnitude and frequency trend, with reliable measures extending up to 81 Hz. Rheologic properties of the hyaluronic acid gel were the closest match to cadaveric vocal fold mucosa in magnitude and frequency trend. Conclusions: Viscoelastic properties of Cymetra and Zyderm are nearly the same and are significantly greater than those of vocal fold mucosa. HA-DTPH possesses a good viscoelastic match to vocal fold mucosa and may be useful in future lamina propria repair.

Objectives To measure the viscoelastic shear properties of hyaluronic acid, with and without fibronectin, and to compare them with those of the human vocal fold mucosa and other phonosurgical biomaterials. Methods Viscoelastic shear properties of various implantable biomaterials (Teflon, gelatin, collagen, fat, hyaluronic acid, and hyaluronic acid with fibronectin) were measured with a parallel-plate rotational rheometer. Elastic and viscous shear properties were quantified as a function of oscillation frequency (0.01-15 Hz) at 37 degrees C. Results The shear properties of hyaluronic acid were relatively close to those of human vocal fold mucosal tissues reported previously. Hyaluronic acid at specific concentrations (0.5%-1%), with or without fibronectin, was found to exhibit viscous shear properties (viscous shear modulus and dynamic viscosity) similar to those of the average male and female vocal fold mucosa. Conclusions According to a theory that establishes the effects of tissue shear properties on vocal fold oscillation, phonation threshold pressure (a measure of the ease of phonation) is directly related to the viscous shear modulus of the vibrating vocal fold mucosa. Therefore, our findings suggest that hyaluronic acid, either by itself or mixed with fibronectin, may be a potentially optimal bioimplant for the surgical management of vocal fold mucosal defects and lamina propria deficiencies (e.g., scarring) from a biomechanical standpoint.

Objective/hypothesis To examine the interstitial proteins of the vocal fold and their influence on the biomechanical properties of that tissue. Study design Anatomic study of the lamina propria of human cadaveric vocal folds combined with some viscosity testing. Methods Identification of proteoglycans is performed with histochemical staining. Quantitative analysis is performed using an image analysis system. A rheometer is used for viscosity testing. Three-dimensional rendering program is used for the computer images. Results Proteoglycans play an important role in tissue biomechanics. Hyaluronic acid is a key molecule that affects viscosity. Discussion The proteoglycans of the lamina propria have important biological and biomechanical effects. The role of hyaluronic acid in determining tissue viscosity is emphasized. Viscosity, its effect on phonatory threshold pressure and energy expended due to phonation is discussed. Conclusion Proteoglycans, particularly hyaluronic acid, play important roles in determining biomechanical properties of tissue oscillation. Future research will likely make these proteins of important therapeutic interest.

Vocal function was assessed in 15 patients who received thyroplasty type I for the rehabilitation of unilateral vocal fold paralysis. The function was assessed by perceptual evaluation, voice intensity and frequency range profiles, and questionnaire. Goals of the study were to evaluate the efficacy of thyroplasty in improving the voice, to compare postoperative thyroplasty voices to normal voices, and to correlate objective measurements to the results of the questionnaire regarding satisfaction with the voice. Perceptual evaluations were performed by randomizing normal and thyroplasty voices on a recording tape. The voices on the tape were then rated by independent, blinded, trained listeners. The perceptual qualities of pitch, intonation, and loudness were not statistically different than normals; however, voice qualities of strain, breathiness, hoarseness, harshness, and unsteadiness were different than normals. Mean frequency range and mean intensity range were moderately to severely reduced from normals with a wide variation being present in the results. The correlation between the higher threshold phonation pressures and decreased intensity ranges found in some patients is discussed. Questionnaire results indicated that a high degree of satisfaction with the surgery was present (92%). Extreme or general satisfaction with voice was present in 73%. The most difficult voicing was experienced at work, with 25% needing to adjust their employment to accommodate their voice abilities. These results indicate that thyroplasty type I is effective in partially rehabilitating unilateral voice fold paralysis. Voice function is still not normal, probably in part due to the underlying disease.

An excised hemilarynx setup was developed. The phonatory characteristics of nine excised canine larynges were examined. The left vocal fold of each larynx was then removed and substituted with a vertical plexiglass plate. The larynges were phonated again. Recordings were made of phonation threshold pressure, sound pressure level, average glottal flow, fundamental frequency, and amplitude of vocal fold vibration as observed with a video stroboscope. Measurements were made over a range of subglottal pressures. For the hemilarynx, simultaneous recordings of intraglottal pressure and vocal fold contact area were also made. It was found that amplitude and frequency of vocal fold vibration of the hemilarynx, as well as rates of change of amplitude and frequency as a function of subglottal pressure, were similar to those of the full larynx. Also similar were phonation threshold pressures and ranges of subglottal pressure over which the larynges phonated. The average airflow of the hemilarynx was approximately half that of the full larynx, and the sound pressure level, under similar conditions, was one fourth (about 6 dB less) in the hemilarynx.
