自然尿中に多数の腫瘍細胞が認められた, 膀胱原発神経内分泌癌の1例を経験したので報告する.症例は64歳男性, 肉眼的血尿を主訴として近医より紹介受診された. 初診時の尿細胞診では, 壊死物質を背景に, 小型で裸核状の腫瘍細胞が, 孤立散在性および結合性の弱い小集塊を形成し多数出現していた. また核のmoldingやindian file状およびrosette様の配列がみられるなど, 肺の小細胞癌にきわめて類似する特徴的な細胞所見であったため, 膀胱原発の神経内分泌癌と推定された. その後, 経尿道的膀胱腫瘍切除術が施行された. その病理組織所見では, 小型の腫瘍細胞が充実性に増殖しており, 免疫組織化学的にNeuron specific enolaseが強陽性を呈し, Grimelius染色で好銀性顆粒が認められた. さらに電子顕微鏡的検索でも神経内分泌顆粒が証明された. 以上の所見より膀胱原発神経内分泌癌と診断された.

心嚢液中に子宮頸部扁平上皮癌細胞が出現することは比較的まれであり, 今回一見すると腺癌細胞と誤りやすい, 球状の集塊を呈して認められた扁平上皮癌細胞の1例を経験したので報告する. 症例は30歳女性. 不正性器出血を主訴として当院を受診し, 子宮頸部擦過細胞診および生検組織診にて子宮頸部扁平上皮癌と診断された. 広汎子宮摘出術後, pT1bN1M0, Stage Ibの病期にて放射線療法および化学療法が施行された. しかし, 術後1年半後に呼吸困難が出現し, 再入院, 心嚢液の貯留が認められ, 血性心嚢液が採取され細胞診検査を実施した. 心嚢液中の細胞所見は, 腺癌との鑑別を要する球状の集塊がみられたが, 球状の集塊を構成する細胞の多くは核中心性の扁平な細胞であり, 同心円状に配列していた. 細胞質縁は厚く, 細胞間橋が明瞭に認められた. PAS染色では, 細胞質が穎粒状に一部びまん性に陽性を呈し, アルシアン青染色は陰性であった. また, 免疫組織学的検索にてケラチン (K-13) 抗体が陽性であった. 以上の所見から, 腫瘍細胞は扁平上皮癌細胞と判定され, 子宮頸癌の心外膜転移と診断された, 再入院から約1ヵ月後に死亡し剖検が施行され扁平上皮癌以外に癌は認められなかった.

われわれは喀痰中に腫瘍細胞が認められた悪性中皮腫の1例を経験した. パパニコロウ染色標本に見出された喀痰中の腫瘍細胞を電顕にて観察し, 悪性中皮腫細胞であることを確診し得た. 症例は, 67歳のアスベスト曝露歴のある男性で, 職場検診にて胸部異常陰影を指摘されて平成6年7月入院し, 胸水貯留が認められ胸水細胞診にて悪性中皮腫が疑われた. 治療により軽快退院したが翌年3月悪化して再入院し, この時喀痰細胞診にて腫瘍細胞が, 平面的で結合性の疎な集団として発見された. 腫瘍細胞はライトグリーン好性の重厚で豊富な細胞質と, 円形で中心性の核を有し, 明瞭な核小体が1ないし2個認められた. 電顕にて腫瘍細胞を観察した結果, 細胞膜表面の長いmicrovilliと核周囲の中間径線維が認められ, 悪性中皮腫細胞と確診された. 死亡時の胸膜穿刺生検から作成された標本では, 線維性結合織中に腫瘍細胞の蜂巣状増殖が認められ, 腫瘍細胞はアルシアン青およびコロイド鉄染色陽性で, ピアルロニダーゼにより消化された. このような所見および免疫組織化学的所見から, 病理組織学的にも悪性中皮腫と診断された.

石灰化上皮腫に穿刺吸引細胞診が施行され, その細胞像を観察する機会をえたので報告する.症例は9歳女児で, 左眉毛の上の結節に気づいていたが増大したように感じられたため, 日之影町国民健康保険病院を受診し, 結節の穿刺吸引細胞診が施行され, 標本が中央微生物検査所に送られてきた.標本を観察した結果, 良性疾患と考えるが組織診の推定は困難であると判定したため, 結節は摘出され, 組織学的に石灰化上皮腫と診断された.細胞診標本上には, 裸核様細胞が結合性の疎な不規則な集団を形成して多数認められ, 核は円ないし類円形で大小不同は軽度であり, クロマチンは細穎粒状で均等に分布し, 明瞭な核小体を有する細胞も存在した.同時に, ライトグリーンで縁どられた, 境界の比較的明瞭な, 無構造で不透明な物質が認められた.組織像と対比した結果, 前者は好塩基性細胞, 後者は陰影細胞であろうと考えられた.

結核予防会大阪府支部では, 平成5年度の老人保健法に基づく肺癌検診で, 6,464件の喀疾細胞診が実施された.その結果, Eと判定された症例8例中, 胸部X線像に異常陰影なしと判定されていた症例は6例あり, そのうち5例は喀疾細胞診で扁平上皮癌と判定されており, 残りの1例が本症例で, 腺癌と判定された.本症例は58歳の女性で, 検診1ヵ月後に精密検査が行われた. 他院で撮影されたCTでは左肺尖部に小結節陰影が認められたが, 肺結核の既往歴があったためそれによる陰影と診断された.同時に行われた喀疾細胞診がやはり陽性であったため, 左上葉の気管支擦過細胞診および気管支肺胞洗浄細胞診が施行されたがいずれも陰性であった. その後, 喀疾細胞診は, ほぼ毎月施行されたが陰性が続いた. 検診から1年7ヵ月後, 喀疾細胞診が再び陽性と判定されたのでCTが施行され, 左肺尖部の病巣は肺癌が疑われた. 気管支擦過細胞診は陽性であり, 手術の結果, 組織学的に高分化乳頭型腺癌と診断された. 組織標本における腫瘍径は15×12mm, 術後病期はpTlN0M0であった.本症例は, 術後16ヵ月の現在も再発の徴候はなく生存中である. 本症例は腺癌の早期発見でも喀疾細胞診が有用であることを示した症例といえる.

乳房内に腫瘤を形成した悪性線維性組織球腫のまれな1例を経験したので, 穿刺吸引細胞所見および組織所見について報告する.症例は54歳, 女性. 1983年に左脛骨に悪性線維性組織球腫が発生し摘出手術を受け, 1987年および1989年, 左右大腿骨および背部皮下に転移巣が発見され, いずれも摘出手術を受けていた. 1991年に左乳房の腫瘤に気づき当院外科を受診した. 触診にて良性と思われたが, マンモグラフィーでは明らかな腫瘤陰影は認められず, エコーグラフィーでは良, 悪性の判定が困難であったため, 腫瘤の穿刺吸引細胞診が施行された. 標本上には, 長楕円形もしくは紡錘形の核を有する線維芽細胞様細胞が多数認められた. 少数であるが, 類円形または分葉状の核を有する組織球様細胞, 連珠状の核を有する多核細胞も認められた. 既往歴を考慮し上記細胞像より悪性線維性組織球腫の乳房内転移と推定され, 確認のために腫瘤の摘出生検が行われた. 組織所見にて悪性線維性組織球腫, 通常型と診断された. 乳房内に転移巣を発見することはまれであり, 原発性乳房内腫瘤との鑑別は治療の選択に重要である. 本症例では, 穿刺吸引細胞診が乳房内の転移巣の診断に有用であった.

自然尿および膀胱洗浄液 (以下洗浄液) 細胞診がともに施行された膀胱の移行上皮癌患者66名のべ81例を対象として, その細胞診成績および細胞所見を比較検討した. 自然尿の陽性例は33例 (40.7%), 洗浄液の陽性例は59例 (72.8%) で, 洗浄液細胞診の方が有意に陽性率が高かった. 洗浄液において, Grade1の膀胱癌8例 (66.7%) およびGrade2の膀胱癌35例 (66.0%), 乳頭状腫瘍の48例 (70.6%) および非乳頭状腫瘍の11例 (84。6%), 浸潤癌の51例 (72.9%) および非浸潤癌の8例 (72.7%), 再発例の29例 (80.6%) がおのおの陽性であった. 他方, 自然尿においてはGrade1の膀胱癌2例 (16.7%) およびGrade2の膀胱癌16例 (30.2%), 乳頭状腫瘍の27例 (39.7%) および非乳頭状腫瘍の6例 (46.1%), 浸潤癌の30例 (42.8%) および非浸潤癌の3例 (27.3%), そして再発例の12例 (33.3%) がおのおの陽性であった. 発育様式をGrade別に分け, 自然尿および洗浄液細胞診の成績を比較すると, 乳頭状浸潤型膀胱癌Grade2において両者の陽性率に有意差が認められた. 細胞所見については, 洗浄液では多数のよく保存された細胞が出現し, また自然尿にみられる細胞より大きく, 核縁は薄く核内構造も明瞭に観察できた.

大阪肺癌集検研究班では, 1981年より胸部X線撮影と喀痰細胞診による肺癌検診を実施しているが, 1990年までの10年間に, 喀痰細胞診でDあるいはEと判定された症例の, 精検および追跡調査の結果について報告する. 喀痰を提出した高危険群所属者は, 男女合計16,992人で, 有効喀痰は16,795人が提出した. 要精検者は402例, 原発性肺癌患者は28例であり, したがって陽性反応適中率は6.97%となった. 要精検者のうち判定D群は53例で, 発見された肺癌患者は14例, 陽性反応適中率は26.42%であった. 一方, 判定E群は14例で, 発見された肺癌患者は9例, 陽性反応適中率は64.29%であった. 臨床病期1期肺癌の比率は, 判定D群14例中12例85.7%と, 判定E群6例中2例33.3%よりかなり高かった. また, 判定D群14例中10例は喀痰細胞診単独発見例であり, すべて臨床病期1期であった. 肺癌または喉頭癌以外の判定D群37例, 判定E群3例を追跡し, 5年以上追跡しえた判定D群の16例, 判定E群の2例からは肺癌を発見しえず, Eと判定した2例はfalse positiveと考えられた

組織学的に甲状腺癌が確認された112例に対する穿刺吸引細胞診の成績は, 陽性94例 (83.9%) であった. これに対し, 触診, 超音波検査, X線検査の診断率は, それぞれ67.9%, 58.6%, 50.0%であり, 穿刺吸引細胞診の成績が最も優れていた. 特に乳頭癌の陽性率は87.1%で, ほかの診断法に比しはるかに優れており, 触診, 超音波検査, X線検査のいずれもが癌の所見を示さず, 穿刺吸引細胞診のみが陽性であった症例が10.9%にみられた.穿刺吸引細胞診が疑陽性あるいは陰性であった症例について標本の再検討を行い, 組織所見と対比した結果, 乳頭癌で細胞判定に問題があったと考えられる症例は4例あり, 1例は核内細胞質封入体と変性空胞との鑑別に困難を感じた例であり, ほかは乳頭癌の特徴的な細胞所見が認められなかった. 組織所見に問題があったと考えられる症例は5例あり, 乳頭癌が濾胞腺腫とともに存在した例, follicular valiantを示した例があり, また腫瘍組織の石灰化の著明な例が3例みられた.濾胞癌の陽性率は25%と低く, 穿刺吸引細胞診による濾胞腺腫との鑑別はきわめて困難であ

われわれの施設では, 1987～1991年の5年間に原発性唾液腺腫瘍に対して穿刺吸引細胞診が行われ, 組織診の判明している症例は19例であった.良性腫瘍の正診率は71.4%, 悪性腫瘍の正診率は60%であったが, 完全に組織診と一致した症例はわずか26.3%であった.今回われわれは, 両者の不一致例14例についてその原因を検討した.多形腺腫では上皮細胞か間質由来の細胞かいずれかの細胞しか認められなかった場合, 単に良性病変とのみ診断される傾向にあった.False positiveのおもな理由は, 核の大小不同や形の不正が認められたことによるが, 背景に粘液基質を認めたことから良性とすべきであった.ワルチン腫瘍を正診できなかったのは, oncocytic cellを腺細胞と誤ったものであり, リンパ球の多い背景, 蜂の巣状の細胞配列, 豊富な細胞質に注意すべきであった.粘表皮癌のfalse negative 2例には, 扁平上皮型, 中間細胞型および粘液産生型のすべての腫瘍細胞が出現していなかった.クロマチンの軽度増量, 濃染した核小体などに注意すべきであったが, 悪性の判定には困難に感じる.本腫瘍19例中7例は, 触診にて頸部リンパ節腫脹と診断されていて判定を迷わせた.

最近, われわれは肺腫瘍としてはまれな, Alpha-fetoprotein産生の低分化腺癌の穿刺吸引細胞診を経験したので報告した.症例は70歳女性, 胸部X線像にて右上肺野, 縦隔寄りに巨大な腫瘤陰影が認められ, 血清AFP値が高値を示した.穿刺吸引細胞診では平面的あるいは不規則重積性に配列した異型細胞が多数出現していた.これらの細胞は細胞質が豊富で顆粒状を呈し, 大小不同がみられた.クロマチンは増量し粗顆粒状不均等分布し, 大型類円形の核小体が1個認められた.PAS染色では一部の異型細胞がびまん性に陽性を呈し, 酵素抗体法により腫瘍細胞内にAFPの局在が証明された.なお, 肝臓や消化器, 卵巣などに腫瘍は認められず, 穿刺吸引細胞診にてAFP産生の肺腺癌が推定された.肺生検および剖検にて肺原発性の低分化腺癌と確診された.

Carcinoid is known to be an endocrine tumor producing the biologically active substances, and is of interest in its histogenesis. Occasionally, bronchial carcinoid shows the malignant potential by the spread resulting in generalized metastasis.Accurate diagnosis of carcinoid is necessary espe cially in relation to its treatment. The cytologic diagnosis of cacinoid is very difficult because of the lack in cytologic malignant characteristics. Therefore, we reviewed the smears from 7 cases of bronchial carcinoid in order to establish the cytologic criteria for bronchial carcinoid.Cytologic findings in the observed cases of bronchial carcinoid were as follows: the arrangement of tumor cells was irregular and had a pavement-like appearance;the cytoplasm was wide, and the N/C ratio was decreased; the cytoplasmic rim was poorly defined; the nuclei were small, oval or round, as compared with lung cancer cells; the nuclear borders were generally thin; anisokaryosis was infrequent;usually, the amount of chromatin was not increased;chromatin was finely granular, and its distribution was irregular; the nucleoli were usually small and round, and almost all of them were located in the center of the nucleus.

A case of mesothelioma of the peritoneum with asbestosis of the lung was reported. Cytological examination of the peritoneal effusion was performed in this case, which was histologically diagnosed to have malignant mesothelioma.Characteristic findings of malignant cells are as follows: Tumor cells appeared singly or as groups consisting of a few-many cells. Some of these cells were overlapping each other. The cytoplasm was abundant and its perinuclear area was staind lightly pale or sometimes appeared to be eosinophilic, with a gradual transition to a deep stain at the periphery. Cytoplasmic rims were distinct and sometimes blister-like protrusions appeared after application of the Giemsa' stain.Nuclei were round or oval and mainly central in location. Their borders were smooth. The chromatin content was increased and chromatin was finely granular with occasional prominent clumps of chromatin.There could be seen 1-4 small nucleoli and, sometimes, 1-2 large irregular-shaped nucleoli.Single tumor cells were large and often multinucleated.Mitoses occured in a small number of tumor cells.Thick strand-like, dark clumping or finely granular PAS-positive substances were irregularly distributed in the cytoplasm, sometimes the whole of the cytoplasm was stained as a ring. Vacuolated cytoplasm gave a negative reaction to staining. Vacuolated cytoplasm and a part of the cytoplasmic border were stained positive by alcian blue staining.

The cytological presentation of nonpapillary in situ carcinoma of the urinary bladder was studied.In all 5 cases, a diagnosis of malignancy has been made by cytological examination of voided urine at a time when the patients had in situ carcinoma. The symptoms of the cases, such as pain on urination and urinary frequency, suggested chronic cystitis. There was no specific picture to be made by cytoscopy. But cancer cells were found in all 5 cases.The characteristic features of malignant cells in urinary sediment from the patients with carcinoma in situ were studied, and a comparison was made with the features of malignant cells from invasive carcino ma. Cytological smears of urine from the 5 patients showed little evidence of inflammation and none of necrosis, a uniform pattern of relatively small malignant cells. Most of these cells were 13-14 microns in size, round or oval in shape. Anisocytosis was rare. Nuclei were slightly enlarged and also round or oval in shape;there were always hyperchromatic nuclei with finely distributed chromatin. A few large cells and malignant cells with degenerated bizarre nuclei were found, mixed with the uniform pattern of malignant cells.These large and malignant cells were numerous in cytological smears of urine from patients with invasive carcinoma.Biopsy specimens appearances are sometimes misle ading and apt to be diagnosed as chronic cystitis, since nonpapillary in situ carcinoma denudes easily. Therefore, routine cytological examination of urinary sediment is most useful in the detection of carcinoma in situ of the urinary bladder.

喀痰中に軟骨肉腫細胞が出現した胸椎原発軟骨肉腫の1例を報告する. 患者は, 54歳, 男性, 右肩甲部, 右肘関節部の軽度の疼痛を主訴として来院. 胸部X線撮影の結果, 縦隔腫瘍が疑われ, 手術が施行された. 臨床的に腫瘍は, 第1, 第2胸椎間原発と考えられた. その後, 局所再発を繰り返し, 縦隔, 肋骨, 右肺へと浸潤部位が拡大し, 7年後に胸髄損傷による呼吸不全にて死亡した. 腫瘍の右肺への著明な浸潤により, 喀痰中に腫瘍細胞を多数認めた. 腫瘍細胞は, ヘマトキシリンに淡く染まる軟骨基質様物質とともに, 1～数個の細胞がシート状に認めれた. 細胞質はライトグリーンに好染し, 円形ないし多辺形, 核は中心性にて, 円ないし類円形であった. 核の大小不同は著明でなかった. まれに2核細胞も認められた. 核小体は円形で大きく, 主として1個認められた.

喀痰細胞診を肺癌集検の場で行うには, 1) 細胞の保存が良好であること, 2) 標本の作製が簡単であること, 3) 標本が美麗で異型細胞のスクリーニングが容易であること, 4) 陽性率が高いこと, 5) 経済的であること, 6) 安全性が高いこと, などが必要な条件となる. 現在までに, 喀疾保存の目的で, ホルマリン固定法, 名市大法, サコマノ法などが行われ, 細胞診陽性率を高める目的で, 蛋白分解酵素あるいは粘液融解剤を用いる集細胞法が行われてきた, 最近では, サコマノ氏固定液に粘液融解剤を加える方法が行われ, 操作が簡単で, 背景がきれいであるために, 異型細胞のスクリーニングが容易であるといわれている. 標本作製法の歴史を知ることは興味のある事実であり, また啓発される点も多い.

男子乳癌に対して細胞診断を行う機会を得たのでその成績を報告する.症例1は68歳男子で, 乳頭分泌を主訴として来診した. 乳頭分泌細胞診は2回行われたが, いずれも細胞成分に乏しく, 2回の全標本中1箇所に, 円形の, 結合性の強い細胞集団を認めたが陰性と判定した. 後日再検討しても1集団のみで癌と診断することは困難であった. 直径0.5cmの腫瘤が乳頭に接して触知されたため, 穿刺細胞診が2回行われた. いずれも細胞成分は豊富で, 細胞問の結合性が弱かったために強く癌を疑ったが, 細胞異型に乏しかったため, 癌と確診するにいたらなかった.症例2は73歳男子で, 左乳腺腫瘤を主訴として来診した. 穿刺細胞診により, 円形ないし乳頭状に配列する, 大小種々の細胞集団が多数標本上に認められた.細胞異型に乏しかったが, 集団を構成する細胞は不規則に重積していたため, 本症例は正しく陽性と診断し得た. 本症例は甲状腺にも腫瘤があり, 穿刺細胞診にて乳頭癌と診断された. 組織学的にも重複癌であった. 乳癌と甲状腺癌の合併は比較的頻度が高いので, 若干の考察を行った.

喉頭癌, 下咽頭癌患者を対象として喀痰細胞診を行い以下の結果を得た.喉頭癌104例の喀痰細胞診陽性率は, 63. 5%で, T因子別にみた陽性率ではT1～T4の各グループの間で有意差が認められ, 癌の進展度と陽性率は平行していた. 下咽頭癌31例の喀痰細胞診陽性率は, 77. 4%で, T因子別の陽性率ではT2～T4の各グループの間に有意差は認められなかった.喀痰中に出現する喉頭, 下咽頭の扁平上皮癌細胞の数は一般に少なく, オレンジG好性の強い角化を呈する癌細胞は出現頻度が高く, N/Cが小さく, クロマチンの増量が軽度であった. ライトグリーン好性の癌細胞は出現頻度があまり高くなく, N/Cはやや大きいがクロマチンの増量は軽度であった. 喀痰中に出現した咽喉頭部の扁平上皮癌細胞は, 異型の乏しいことが特徴といえる.喀痰細胞診は肺癌の診断だけでなく, 咽喉頭癌の診断にも有用であると考える.

Clinically, it is often difficult to distinguish between a mucous carcinoma and a benign lesion of the breast. We have performed cytologic examinations of 6 cases of mucous carcinoma by aspiration cytology and of 2 cases by the use of nipple secretion (Both materials were used in one case)Six out of 7 cases were cytologically positive, that is, cytologic examination was helpful in diagnosing these tumors.All 7 cases were resected and histologically confirmed as mucous carcinoma. Among them, 4 cases were pure mucous carcinoma and those remaining were mixed with other types of carcinoma.As the result of observation of cellular features of cytologic materials of these tumors, tumor cells most often appeared in solid clusters with clear evidence of distinct cell boundaries. The cytoplasm of tumor cells was abundant, anisokaryosis was slight, nuclei were usually small (8-10μ) and nuclear atypia was slight. A lesser degree of cellular atypism was generally found in pure mucous carcinoma compared with the mixed group.The periodic-acid-Schiff reaction was performed by the use of cytologic material of 4 out of 7 cases. The cytologic picture of tumor cells surrounded by amorphous mucin was characteristic in 3 cases. Futhermore, signet-ring type cells with PAS-positive substances were rarely observed.

In this paper is presented the method of sputum cytology which was developed by Saccomanno.This method has been used in the Mayo Lung Project in U.S.A.Comparison between the cellular features of direct smears obtained by the transbronchial brushing or the needle aspiration technique, and of smears that had remained in Saccomanno's solution for 1, 2 and 4weeks showed that epidermoid carcinoma cells did not exhibit much evidence of degeneration, but that adenocarcinoma cells and small cell anaplastic cell carcinoma cells did exhibit some appreciable degree of cellular damage. Cytologic examination, however, was capable of identifying whether they were cancer cells or not.In the authors'laboratory, sputum cytology was performed in 39 cases of lung cancer by Saccomanno's concentration method, and comparison between this method and direct smear was studied.The cytolo gically positive rate was elevated by 18.0%, compared with the direct smear in the same specimens.Morphologic characteristics of cancer cells by Saccomanno's concentration method were observed.Epidermoid carcinoma cells were similar to those found by the direct smear.Adenocarcinoma cells were somewhat different from direct smear cells, that is, irregular overlapping of cells was hardly visible, and scattering of cells was increased;thick cytoplasm was common; chromatin was rather coarsely distributed.In small cell anaplastic carcinoma cells, also chromatin distribution was coarse, and cell clumping was more striking.Since this is a rapid, simple method that permits atypical cells to be readily found, 295 people were screened by chest X-ray and sputum cytology using this method.The sputum cytologic tests utilized 3-day pooled collections of spontaneous sputum.At this point, cancer cells have not yet found, though it may be said that the frequency of discovery of histiocytes in these specimens was higher by 18.5% than in the cases where direct smears were employed.

甲状腺腫瘤穿刺材料に, 上皮細胞とともに, ヘマトキシリンに濃染し, 層状構造を示す砂粒体様石灰を多数認めたため, 細胞診陽性と判定したが, 手術の結果, 腺腫様甲状腺腫であった1例を報告する.摘出された腫瘤は大小種々の濾胞よりなり, 間質内に多くの石灰化巣が認められたが, 砂粒体はみいだし得なかった.油浸にて, 細胞診標本にみられた砂粒体様石灰を再度観察すると, 同心円状かつ層状の構造を示すが, 中心部は淡く, 顆粒状ないし泡沫状で, 癌例にみられる真の砂粒体のようにびまん性に染色されていなかった.PAS染色も陰性であり, 本症例にみられた砂粒体様石灰は, 変性や壊死によって生じた石灰沈着がたまたまこのような形を呈したものと考えられた.

Exfoliated malignant cells of vaginal smears in three cases of botryoid sarcoma of the cervix were observed.Two cases were diagnosed as positive and another was suspicious.Cytology disclosed marked celluar atypism as well as nuclear atypism. Tumor cells showed oval or spindel-shaped cytoplasma with fairly defined celluar border. Nuclear chromatin was slighty increased and irregularly distributed. Prominent, giant nucleoli noted in the majority tumor cells were characteristic.Most of the tumor cells were separated. Small groups of the tumor cells were occasionally observed without cellular overlapping typically found in cases of adenocarcinoma.The cytologic findings were compared with histo logic findings of the resected specimens Myxoma tous changes were common in these botryoid. tumors. Because of this change, it appered difficult to decide whether exfoliated tumor cells were malignant or not.

Breast secretion smears were obtained from 179 patients examined for nipple discharge. Aspiration smears of cyst were obtained from 13 patients.Of the total of 192 patients, 21 were breast cancer. Positive smears were in 13 of 21.Cytologic feature of desquamated cancer cell from papillotubular adenocarcinoma was as follows:1) Several clusters different on number of cells were seen. The cells of the cluster were arranged very irregularly and were overlapped each other.2) The cytoPlasmic boundary was most indistinct. Vacuolation of the cytoplasm was not seen usually.3) The nucleus was large and hyperchromatic and variable in size. The chromatin was most fine, however, difference of chromatin patterns may be seen in each different clusters and even in the cells of a cluster.The characteristics of desquamated cancer cells from comedocarcinoma were as follows:1) The cells usually present as single.2) The some of cells have round distinct cyto plasm, while others have poorly defined one.3) The nucleus varies in size, and abnormality was conspicuous. Necrotic nucleus was always seen.4) Morphologic feature of the cluster of come docarcinoma was similar as of papillotubular adeno carcinoma.

胸部X線像にて腫瘤陰影を認め, 喀疲細胞診断にて燕麦細胞癌と診断し, 治療により腫瘤陰影は一時消失したが, まもなく再発死亡し, 剖検の結果膵島癌の肺転移であった2例を経験したので, 剰離細胞所見および組織学的所見を報告する.

Sputum smears of lung cancer patients were kept in room after the fixation for 30minutes in 95% ethanol-ether solution, in 95% ethanol, in methanol-ether solution, in methanol only, in 90% aceton, in Bouin's solution and in Carnoy's solution respectively. They were also kept in room after the fixation during 24 hours in 10% formalin and after the spraying 2 times with Cytospray.On the first, 2nd, 4th, 7th and 14th days after the fixation, sputum smears were stained with Papanicolaou's method. The slides, stained within 24 hours after the fixation, have good stainability of cells and nuclei, and no degeneration of cell ular structure was observed excepting 10% form alin fixed slides. The slides stained more than 24 hours after the fixation have slightly weak staina bility.Smears fixed by several methods and stained by Papanicolaou's method, of vaginal secretion, sputum, bronchial secretion, gastric juice, urine and serous fluid are reexamined microscopically after 2 years storage. The slides, which are fixed in ethanol-ether solution or by means of Cytospray technique, reveal good preservation of color and good maintenance of cellular structure.Merits of Cytospray technique are summarized as follows: time saving, easy handling, no cellular desquamation from the slde and no cellular contamination.

Ten cases of oat cell carcinoma were studied cytologically, histologically and electron microscopically. Exfoliated tumor cells in sputum stained by Papanicolaou's method were compared with the cell inthe bronchial secretion or material obtained by the TV-brushing method.Two types of tumor cell, i. e., large cell and smallcell, were observed even in the same slide of samematerial from same patient of oat cell carcinoma, and they have also two tinctorially different quality, i. e., one with light stained nucleus (light cell) andthe other with dark stained nucleus (dark cell).Tumor cells of small cell type were found in allcases, while those of large cell type were found in 8 out of 10 cases. These findings were also confirmed by the electron microscopic study.Tumor cells of small cell type have round oroval nuclei, while tumor cells of large cell type haveround or irregularly shaped nuclei. Nuclear borderof these cancer cells is generally thin and nucleolusis not distinct.Pycnotic nucleus which has an appearance justlike india ink spot seems to come from necrotic area.After the Endoxan treatment, the increase in number of the light cells having granular chromatinsubstance and the increase in number of the darkcells having india ink spot were observed, and thisfinding is appeared to be useful for the evaluationof cancer chemotherapy.
