Objective: Evaluation of the effectiveness of vestibuloplasty according to microhemodynamics and oxygenation in the mucous membrane of the alveolar ridge., Materials and Methods: To achieve this goal, a clinical and functional study was carried out in the mucous membrane of the alveolar ridge after vestibuloplasty in 80 patients (35 men and 45 women) aged from 18 to 65 years, average age 41.5±1.9 years, without somatic pathology with a diagnosis according to ICD-10 «K08.1» with an insufficient or completely absent zone of attached keratinized gums, 2 groups were separated from them depending on the type of treatment: mail group comprised 40 people and was divided into 2 subgroups. In subgroup A a free gingival graft (FGG) was used as a covering material (subgroup A) with removable device. In subgroup B (20 people), a collagen matrix and a removable device were used. Control group consisted of subgroup 1 (20 people), in which FGG was used as a covering material, and subgroup 2 (20 people), in which collagen matrix was used. Using laser Doppler flowmetry (LDF), we determined the microcirculation index which characterizes the level of tissue blood flow. Oxygenation studies were carried out using optical tissue oximetry (OTO), determining the perfusion oxygen saturation index and the specific oxygen consumption index. Statistical processing of the results was carried out using MS Excel and MS Access., Results: Analysis of the results of LDF in the main group (subgroup A and B) showed that 3 days after surgery, the level of blood flow (M) and its intensity (σ) decreased by 35% and 54%, and 24% and 49%, respectively, indicating the development of ischemia in the microvasculature which resolved after 14 days in subgroup A and 1 month in subgroup B. In the control group (subgroup 1 and 2), the dynamics of tissue blood flow 3 days after surgery were similar, but microcirculation indicators were restored after 1 and 3 months, respectively. According to optical OTO in the main group 3 days after surgery, oxygenation rates decreased by 36% and 50% (subgroup A) and 20% and 41% (subgroup B) indicating the development of hypoxia in the tissues which resolved after 14 days and 1 month after surgery, respectively. In the control group (subgroup 1 and 2), oxygenation indicators were restored after 1 and 3 months, respectively., Conclusions: After vestibuloplasty using FGG or collagen matrix and a removable denture, microhemodynamics and oxygenation in the mucous membrane of the alveolar ridge restored earlier than in controls.

Purpose of the study: to study the features of microhaemodynamics and oxygenation in soft tissues in the area of the plastically reconstructed jaw after the vestibuloplasty., Materials and Methods: The study included 40 patients aged 20 to 65 (21 males and 19 females). The patients were divided into two groups: I group (14 patients) - patients after reconstructive surgery with the use a fibula autograft without the inclusion of a musculocutaneous «islet»; II group (26 patients) - patients after reconstructive surgery with the use a fibula autograft with the inclusion of a musculocutaneous «islet». To correct the prosthetic bed soft tissues, all patients underwent vestibuloplasty with the use of a free dermal autograft. To study microcirculation in tissues, the laser Doppler flowmetry (LDF) method was used. Microcirculation status was assessed by microcirculation index characterizing the level of tissue blood flow; parameter «σ,» which determines the oscillability of the flow of red blood cells and by coefficient of variation, characterizing vasomotor activity of microvessels. According to the Wavelet analysis of LDF-grams the blood flow bypass was determined. An oxygenation study was carried out in the tissues of the plastically restored jaw by optical tissue oximetry, the results of which determined the oxygenation index and the specific oxygen consumption index., Results: According to LDF data after vestibuloplasty, it was found that in I group, the microcirculation in soft tissues of the plastically reconstructed jaw restored in 21 days, and in II group in 2 months, which persisted at 6 months. In I group, the level of oxygenation and specific oxygen consumption normalized in 21 days, and in II group in 2 months, which persisted at 6 months., Conclusion: Based on the results of this functional study, it was found that before vestibuloplasty microcirculation and oxygenation indices in II group patients were lower than those in I group patients. After vestibuloplasty with the use of a free dermal autograft, microcirculation indices in II group patients restored in 2 months, while in I group patients those indices restored in 21 days.

The purpose of the study was to assess the changes in the width and quality of keratinized tissue created in the area of vestibuloplasty on the lower jaw. Preliminary results of the study showed that autotransplantation method allows creating proper buffer zone with the desired mucosa thickness and is particularly justified in cases of "washing board" effect. However, the method is technically more demanding and is more expensive and painfull for patients. The use of alternative materials of artificial origin, such as "Mucograft" and "Tachocomb" simplifies the execution of operation, thereby reducing operational risk, but often does not lead to the formation of a fully keratinized gingiva.

The paper presents short and long-term results of surgical treatment of 80 patients with insufficient attached gingiva. The procedures included vestibuloplasty and gingival plasty with perfluorane and free gingival and subepithelial connective tissue grafts. Proposed surgical treatment method with perfluorane proved to reduce the local inflammatory reaction greatly thus decreasing perioperative morbidity and increasing the long term efficacy of treatment.

Marginal periodontium diseases are quite frequent; such involvement is often local. Pathogenetic therapy of the periodontium is based on surgical excavation of the vestibule of the mouth. The aim of the present paper is clinical evaluation of the 'open' and 'closed' vestibuloplasty methods. A total of 51 patients with hollow vestibule of the mouth were operated on, immediate and late results of multiple-modality treatment were followed up. Immediate results evidence that the maximal values of oral vestibule depth and fixed gingiva width have been achieved in the patients operated on according to Edlan and Meichar. These values were changing as the postoperative cicatrices resolved and in a year after surgery the most stable results were seen in the patients operated on according to the method developed by the authors. The vestibule depth made up 12.2 +/- 0.1 mm and fixed gingiva width 5.86 +/- 0.1 mm at this period.
