Regulatory guidelines mandate the strong control of the familywise error rate in confirmatory clinical trials with primary and secondary objectives. Bonferroni tests are one of the popular choices for multiple comparison procedures and are building blocks of more advanced procedures. It is usually of interest to find the optimal weighted Bonferroni split for multiple hypotheses. We consider two popular quantities as the optimization objectives, which are the disjunctive power and the conjunctive power. The former is the probability to reject at least one false hypothesis and the latter is the probability to reject all false hypotheses. We investigate the behavior of each of them as a function of different Bonferroni splits, given assumptions about the alternative hypotheses and correlations between test statistics. Under independent tests, unique optimal Bonferroni weights exist; under dependence, optimal Bonferroni weights may not be unique based on a fine grid search. In general, we propose an optimization algorithm based on constrained nonlinear optimization and multiple starting points. The proposed algorithm efficiently identifies optimal Bonferroni weights to maximize the disjunctive or conjunctive power. In addition, we apply the proposed algorithm to graphical approaches, which include many Bonferroni‐based multiple comparison procedures. Utilizing the closed testing principle, we adopt a two‐step approach to find optimal graphs using the disjunctive power. We also identify a class of closed test procedures that optimize the conjunctive power. The accelerated failure time (AFT) model offers an important and useful alternative to the conventional Cox proportional hazards model, particularly when the proportional hazards assumption for a Cox model is violated. Since an AFT model is basically a log‐linear model, meaningful interpretations of covariate effects on failure times can be made directly. However, estimation of a semiparametric AFT model imposes computational challenges even when it only has time‐fixed covariates, and the situation becomes much more complicated when time‐varying covariates are included. In this paper, we propose a penalised likelihood approach to estimate the semiparametric AFT model with right‐censored failure time, where both time‐fixed and time‐varying covariates are permitted. We adopt the Gaussian basis functions to construct a smooth approximation to the nonparametric baseline hazard. This model fitting method requires a constrained optimisation approach. A comprehensive simulation study is conducted to demonstrate the performance of the proposed method. An application of our method to a motor neuron disease data set is provided. Time‐varying covariates can be important predictors when model based predictions are considered. A Cox model that includes time‐varying covariates is usually referred to as an extended Cox model. When only right censoring is presented in the observed survival times, the conventional partial likelihood method is still applicable to estimate the regression coefficients of an extended Cox model. However, if there are interval‐censored survival times, then the partial likelihood method is not directly available unless an imputation, such as the middle point imputation, is used to replaced the left‐ and interval‐censored data. However, such imputation methods are well known for causing biases. This paper considers fitting of the extended Cox models using the maximum penalised likelihood method allowing observed survival times to be partly interval censored, where a penalty function is used to regularise the baseline hazard estimate. Time‐to‐event data in medical studies may involve some patients who are cured and will never experience the event of interest. In practice, those cured patients are right censored. However, when data contain a cured fraction, standard survival methods such as Cox proportional hazards models can produce biased results and therefore misleading interpretations. In addition, for some outcomes, the exact time of an event is not known; instead an interval of time in which the event occurred is recorded. This article proposes a new computational approach that can deal with both the cured fraction issues and the interval censoring challenge. To do so, we extend the traditional mixture cure Cox model to accommodate data with partly interval censoring for the observed event times. The traditional method for estimation of the model parameters is based on the expectation‐maximization (EM) algorithm, where the log‐likelihood is maximized through an indirect complete data log‐likelihood function. We propose in this article an alternative algorithm that directly optimizes the log‐likelihood function. Extensive Monte Carlo simulations are conducted to demonstrate the performance of the new method over the EM algorithm. The main advantage of the new algorithm is the generation of asymptotic variance matrices for all the estimated parameters. The new method is applied to a thin melanoma dataset to predict melanoma recurrence. Various inferences, including survival and hazard function plots with point‐wise confidence intervals, are presented. An R package is now available at Github and will be uploaded to R CRAN. Individuals differ in how they respond to a given treatment. In an effort to predict the treatment response and analyze the heterogeneity of treatment effect, we propose a general modeling framework by identifying treatment‐covariate interactions honoring a hierarchical condition. We construct a single‐step l1 norm penalty procedure that maintains the hierarchical structure of interactions in the sense that a treatment‐covariate interaction term is included in the model only when either the covariate or both the covariate and treatment have nonzero main effects. We developed a constrained Lasso approach with two parameterization schemes that enforce the hierarchical interaction restriction differently. We solved the resulting constrained optimization problem using a spectral projected gradient method. We compared our methods to the unstructured Lasso using simulation studies including a scenario that violates the hierarchical condition (misspecified model). The simulations showed that our methods yielded more parsimonious models and outperformed the unstructured Lasso for correctly identifying nonzero treatment‐covariate interactions. The superior performance of our methods are also corroborated by an application to a large randomized clinical trial data investigating a drug for treating congestive heart failure (N = 2569). Methods for estimating optimal treatment strategies typically assume unlimited access to resources. However, when a health system has resource constraints, such as limited funds, access to medication, or monitoring capabilities, medical decisions must account for competition between individuals in resource usage. The problem of incorporating resource constraints into optimal treatment strategies has been solved for point exposures (1), that is, treatment strategies entailing a decision at just one time point. However, attempts to directly generalize the point exposure solution to dynamic time‐varying treatment strategies run into complications. We sidestep these complications by targeting the optimal strategy within a clinically defined subclass. Our approach is to employ dynamic marginal structural models to estimate (counterfactual) resource usage under the class of candidate treatment strategies and solve a constrained optimization problem to choose the optimal strategy for which expected resource usage is within acceptable limits. We apply this method to determine the optimal dynamic monitoring strategy for people living with HIV when resource limits on monitoring exist using observational data from the HIV‐CAUSAL Collaboration. Multi-arm clinical trials are complex experiments which involve several objectives. The demand for unequal allocations in a multi-treatment context is growing and adaptive designs are being increasingly used in several areas of medical research. For uncensored and censored exponential responses, we propose a constrained optimization approach in order to derive the design maximizing the power of the multivariate test of homogeneity, under a suitable ethical constraint. In the absence of censoring, we obtain a very simple closed-form solution that dominates the balanced design in terms of power and ethics. Our suggestion can also accommodate delayed responses and staggered entries, and can be implemented via response adaptive rules. While other targets proposed in the literature could present an unethical behavior, the suggested optimal allocation is frequently unbalanced by assigning more patients to the best treatment, both in the absence and presence of censoring. We evaluate the operating characteristics of our proposal theoretically and by simulations, also redesigning a real lung cancer trial, showing that the constrained optimal target guarantees very good performances in terms of ethical demands, power and estimation precision. Therefore, it is a valid and useful tool in designing clinical trials, especially oncological trials and clinical experiments for grave and novel infectious diseases, where the ethical concern is of primary importance. This paper considers Cox proportional hazard models estimation under informative right censored data using maximum penalized likelihood, where dependence between censoring and event times are modelled by a copula function and a roughness penalty function is used to restrain the baseline hazard as a smooth function. Since the baseline hazard is nonnegative, we propose a special algorithm where each iteration involves updating regression coefficients by the Newton algorithm and baseline hazard by the multiplicative iterative algorithm. The asymptotic properties for both regression coefficients and baseline hazard estimates are developed. The simulation study investigates the performance of our method and also compares it with an existing maximum likelihood method. We apply the proposed method to a dementia patients dataset.
