In Germany, support/counseling with regard to sexual health by social workers is still largely underdeveloped. In the United Kingdom, this is established as Sexual Health Advising (SHA) in the form of an interface management between psychosocial counseling services, the medical system and somatic care services. Based on the conceptual context, the historical development of social work in the German health care system, and the data on sexually transmitted infections (STI), the text examines the extent to which the British model – the development and content of which are presented – could also be useful in a German context and which structures could be integrated. Finally, the WIR – Walk In Ruhr, Center for Sexual Health and Medicine – will be presented and an outlook will be sketched, about the chances an implemented Sexual Health Advising system would open and which challenges would accompany it. Sexual dysfunction remains a common condition and has been on the rise in men since around 2005. Up to four generations present themselves in the practices. The text gives a brief outline of important socio-cultural developments that correlate with the therapeutic developments: 1. ICD-10 still differentiates between organic versus psychological sexual dysfunctions, which is dropped in ICD-11 in favor of an etiological classification. 2. Human rights are given more importance in the newer diagnostic manuals. 3. On the one hand, the biopsychosocial model of sexuality is undisputed. Nevertheless, with medicalization, especially for erectile dysfunction, biological factors are overestimated, which results in unsuccessful treatments with PDE-5 inhibitors. Conclusion: Further research is necessary; available sex-medical findings must be included in the therapeutic training curricula – being able to draw up a sexual anamnesis is still not a matter of course.
