Abstract Endothelial dysfunction is acknowledged as a marker for subclinical target organ damage (STOD) in hypertension, though its therapeutic potential has not yet been clarified. This study assessed whether early endothelial function improvement (EEFI) reduced STOD in patients with essential hypertension (EH). We conducted a retrospective cohort analysis of 456 EH patients initially free from STOD. Endothelial function was assessed using brachial artery flow-mediated dilation (FMD), with values ≤ 7.1% indicating dysfunction. Patients were initially categorized by endothelial status (dysfunction: n = 180, normal: n = 276), and further divided into improved or unimproved groups based on changes within three months post-enrollment. During a median follow-up of 25 months, 177 patients developed STOD. The incidence of STOD was significantly higher in patients with initial dysfunction compared to those with normal function. 3. Chinese stroke patients with atrial fibrillation used Robert's age-adjusted warfarin loading protocol obtained good INR results within therapeutic range

Abstract To assess whether Roberts’ age-adjusted warfarin loading protocol is effective in Chinese patients and whether the SAMeTT2R2 score can predict international normalized ratio (INR) control. Roberts’ protocol for warfarin titration was applied to patients with non-valvular atrial fibrillation (NVAF) complicated with ischemic stroke at the Department of Neurology between 2014 and 2019. Clinical and sociodemographic variables were recorded. A minimum of 1-year follow-up was used to calculate the time in therapeutic range (TTR) of the INR. A total of 94 acute ischemic stroke patients with NVAF were included in the study. Seventy-seven (81.9%) of the patients had attained stable INR (2.0–3.0) at the fifth dose, and 90.0% of the patients had achieved stable INR on the ninth day. Seventeen (18.1%) of the patients had an INR > 4 during dose-adjustment period. Patients with INR > 4 had significantly lower body weight (53.8 vs. 63.1 kg, P = 0.014), lower rate of achievement of stable INR (35.3% vs. 92.2%, P = 0.000), and lower rate of TTR ≥ 65% (23.5% vs. 70.1%, P = 0.001), but with no significant increase in bleeding risk. A total of 89 patients underwent long-term INR follow-up, of which 58 (65.2%) patients achieved TTR ≥ 65%. Patients with poor TTR had significantly lower body weight (56.3 vs. 63.7 kg, P = 0.020) and lower rate of stable INR achievement (64.5% vs. 89.7%, P = 0.002). All 94 patients had SAMeTT2R2 score ≥ 2. There was no linear association between SAMeTT2R2 score and the rate of TTR ≥ 65% (P trend = 0.095). Chinese ischemic stroke patients with NVAF on warfarin can safely and quickly achieve therapeutic INR using Roberts’ age-adjusted protocol and can obtain a good TTR. Lower body weight may be a predictor of poor TTR and INR > 4. Patients who have not attained stable INR after adjusting the dose five times are at high risk for poor TTR. SAMeTT2R2 score may not predict TTR in Chinese ischemic stroke patients with NVAF.

4. Broadband All-angle Negative Refraction by Optimized Phononic Crystals

Abstract All-angle negative refraction (AANR) of phononic crystals and its frequency range are dependent on mechanical properties of constituent materials and their spatial distribution. So far, it is impossible to achieve the maximum operation frequency range of AANR theoretically. In this paper, we will present a numerical approach for designing a two-dimensional phononic crystal with broadband AANR without negative index. Through analyzing the mechanism of AANR, a topology optimization problem aiming at broadband AANR is established and solved by bi-directional evolutionary structural optimization method. The optimal steel/air phononic crystal exhibits a record AANR range over 20% and its refractive properties and focusing effects are further investigated. The results demonstrate the multifunctionality of a flat phononic slab including superlensing effect near upper AANR frequencies and self-collimation at lower AANR frequencies.

5. 3D biomimetic artificial bone scaffolds with dual-cytokines spatiotemporal delivery for large weight-bearing bone defect repair

Abstract It is a great challenge to prepare “functional artificial bone” for the repair of large segmental defect, especially in weight-bearing bones. In this study, bioactive HA/PCL composite scaffolds that possess anatomical structure as autogenous bone were fabricated by CT-guided fused deposition modeling technique. The scaffolds can provide mechanical support and possess osteoconduction property. Then the VEGF-165/BMP-2 loaded hydrogel was filled into biomimetic artificial bone spatially to introduce osteoinduction and angioinduction ability via sustained release of these cytokines. It has been revealed that the cytokine-loaded hydrogel possessed good biodegradability and could release the VEGF-165/BMP-2 sustainedly and steadily. The synergistic effect of these two cytokines showed significant stimulation on the osteogenic gene expresssion of osteoblast in vitro and ectopic ossification in vivo. The scaffolds were then implanted into the rabbit tibial defect sites (1.2 cm) for bone regeneration for 12 weeks, indicating the best repair of defect in vivo, which was superior to the pure hydrogel/scaffolds or one-cytokine loaded hydrogel/scaffolds and close to autogenous bone graft. The strategy to construct an “anatomy-structure-function” trinity system as functional artificial bone shows great potential in replacing autogenous bone graft and applying in large bone defect repair clinically in future.

A magnetic graphene/polydopamine (MG/PDA) nanocomposite has been prepared and used as sorbent for magnetic dispersive solid-phase extraction (MDSPE) of four benzoylurea insecticides in environmental water samples. The obtained nanocomposites were characterized by transmission electron microscopy, scanning electron microscopy, vibrating sample magnetometry, powder X-ray diffraction, fourier transform infrared spectroscopy, surface area and porosity analysis and thermogravimetric analysis. To investigate the adsorption performance of MG/PDA for target analytes, various parameters affecting the MG/PDA-based MDSPE procedure were optimized. Under the optimal conditions, the established method exhibits good linearity (R2 ≥ 0.9988) in the concentration range 2.5–500 µg L−1. A low limit of detection (0.75 µg L−1, signal/noise = 3:1), a low limit of quantification (2.50 µg L−1, signal/noise = 10:1), and good precision (intraday relative standard deviation ≤3.6%, interday relative standard deviation ≤4.5%) are also achieved. Finally, the simple, fast, and sensitive sample preparation technique was successfully used to determine benzoylurea insecticides in environmental water samples.

Cornelia de Lange Syndrome (CdLS) is a rare genetic disorder, which causes a range of physical, cognitive, and medical challenges. To retrospectively analyze the clinical characteristics and genetic variations of Chinese patients, and to provide experience for further diagnosis and treatment of CdLS in Chinese children, we identified 15 unrelated Chinese children who presented with unusual facial features, short stature, developmental delay, limb abnormalities, and a wide range of health conditions. In this study, targeted-next generation sequencing was used to screen for causal variants and the clinically relevant variants were subsequently verified using Sanger sequencing. DNA sequencing identified 15 genetic variations, including 11 NIPBL gene variants, two SMC1A gene variants, one RAD21 gene variant, and one HDAC8 variant. The phenotype of these patients was summarized and differences between this cohort and another four groups were compared. The clinical manifestations of the patients in this cohort were mostly consistent with other ethnicities, but several clinical features in our cohort had different frequencies compared with other groups. We identified 15 deleterious variants of which 11 were novel. Variants in the NIPBL gene were the most common cause in our cohort. Our study not only expands upon the spectrum of genetic variations in CdLS, but also broadens our understanding of the clinical features of CdLS.

The fundamental property of photonic crystals is the band gap effect, which arises from the periodic dielectric modulation of electromagnetic waves and plays an indispensable role in manipulating light. Ever since the first photonic-bandgap structure was discovered, the ability to tune its bandgap across a wide wavelength range has been highly desirable. Therefore, obtaining photonic crystals possessing large on-demand bandgaps has been an ever-attractive study but has remained a challenge. Here we present an analytical design method for achieving high-order two-dimensional photonic crystals with tunable photonic band gaps on-demand. Based on the Bloch mode analysis for periodic structures, we are able to determine the geometric structure of the unit cell that will realize a nearly optimal photonic band gap for one polarization between the appointed adjacent bands. More importantly, this method generates a complete bandgap for all polarizations, with frequencies tuned by the number of photonic bands below the gap. The lowest dielectric contrast needed to generate a photonic band gap, as well as conditions for generating complete bandgaps, are investigated. Our work first highlights the systematic approach to complete photonic band gaps design based on Bloch mode analysis. The physical principles behind our work are then generalized to other photonic lattices.

9. Observations of Deep Current at the Western Boundary of the Northern Philippine Basin

One-year time series of current velocities and hydrographic parameters based on four deep moorings deployed east of the Luzon Strait are employed to study the deep current at the western boundary (DCWB) of the northern Philippine Basin. While the mean current is relatively weak, the DCWB is highly variable on an intraseasonal time scale, with dominant periods ranging between 30 and 80 days. During the period of observation (October 2011–October 2012), the DCWB reversed its direction at early April, and pointed southward (−2.4 cm/s) in summer/autumn and northward (1.7 cm/s) in winter/spring. This annual reversal of the DCWB is consistent with the water property distribution in the deep Philippine Basin, with relatively cold and fresh water to the north and relatively warm and salty water to the south. The moored time series also allow for discussion on the stratification of the deep Luzon Strait, which indicates the lower interface of Pacific deep water capable of furnishing the deepwater overflow in the Luzon Strait.

Disorders of sexual development (DSD) are estimated to occur in 1 of 4500 births. Since the genetic etiology of DSD is highly heterogeneous, obtaining a definitive molecular diagnosis by single gene test is challenging. Utilizing a high-throughput sequencing upfront is proposed as an efficient approach to aid in the diagnosis. This study aimed to examine the diagnostic yield of next-generation sequencing in DSD. 32 DSD patients that previously received clinical examinations and single gene tests were selected, with or without a diagnosis. Prior single gene tests were masked, and then samples went through targeted next-generation sequencing of 80 genes from which the diagnostic yield was assessed. A likely diagnosis, with pathogenic or likely pathogenic variants identified, was obtained from nine of the 32 patients (i.e., 28.1%, versus 10% by single gene tests). In another five patients (15.6%), variants of uncertain significance were found. Among 18 variants identified (i.e., 17 single nucleotide variants and one small deletion), eight had not been previously reported. This study supports the notion that next-generation sequencing can be an efficient tool in the clinical diagnosis and variant discovery in DSD.

This study identified the possible threshold to predict dengue fever (DF) outbreaks using Baidu Search Index (BSI). Time-series classification and regression tree models based on BSI were used to develop a predictive model for DF outbreak in Guangzhou and Zhongshan, China. In the regression tree models, the mean autochthonous DF incidence rate increased approximately 30-fold in Guangzhou when the weekly BSI for DF at the lagged moving average of 1–3 weeks was more than 382. When the weekly BSI for DF at the lagged moving average of 1–5 weeks was more than 91.8, there was approximately 9-fold increase of the mean autochthonous DF incidence rate in Zhongshan. In the classification tree models, the results showed that when the weekly BSI for DF at the lagged moving average of 1–3 weeks was more than 99.3, there was 89.28% chance of DF outbreak in Guangzhou, while, in Zhongshan, when the weekly BSI for DF at the lagged moving average of 1–5 weeks was more than 68.1, the chance of DF outbreak rose up to 100%. The study indicated that less cost internet-based surveillance systems can be the valuable complement to traditional DF surveillance in China.

13. A Kirigami Approach to Forming a Synthetic Buckliball

The shape transformation of some biological systems inspires scientists to create sophisticated structures at the nano- and macro- scales. However, to be useful in engineering, the mechanics of governing such a spontaneous, parallel and large deformation must be well understood. In this study, a kirigami approach is used to fold a bilayer planar sheet featuring a specific pattern into a buckliball under a certain thermal stimulus. Importantly, this prescribed spherical object can retract into a much smaller sphere due to constructive buckling caused by radially inward displacement. By minimizing the potential strain energy, we obtain a critical temperature, below which the patterned sheet exhibits identical principal curvatures everywhere in the self-folding procedure and above which buckling occurs. The applicability of the theoretical analysis to the self-folding of sheets with a diversity of patterns is verified by the finite element method.

Dengue fever (DF) epidemics in Australia are caused by infected international travellers and confined to Northern Queensland where competent vectors exist. Recent analyses suggest that global trade and climate change could lead to the re-establishment of Ae. aegypti across the country and promote the spread of dengue nationally. This study aimed to describe the dynamic spatiotemporal trends of imported DF cases and their origins, identify the current and potential future high-risk regions and locate areas that might be at particular risk of dengue transmission should competent mosquito vectors expand their range. Our results showed that the geographical distribution of imported DF cases has significantly expanded in mainland Australia over the past decade. In recent years, the geographical distribution of source countries of DF has expanded from the Pacific region and Asia to include Africa and the Americas. Australia is now exposed to dengue importations from all of the regions involved in the current global pandemic. The public health implications of a range expansion of dengue mosquito vectors are severe. Enhanced mosquito surveillance in those areas that have high imported cases is called for to reduce emerging threats from this globally expanding pathogen.

15. An extreme internal solitary wave event observed in the northern South China Sea

With characteristics of large amplitude and strong current, internal solitary wave (ISW) is a major hazard to marine engineering and submarine navigation; it also has significant impacts on marine ecosystems and fishery activity. Among the world oceans, ISWs are particular active in the northern South China Sea (SCS). In this spirit, the SCS Internal Wave Experiment has been conducted since March 2010 using subsurface mooring array. Here, we report an extreme ISW captured on 4 December 2013 with a maximum amplitude of 240 m and a peak westward current velocity of 2.55 m/s. To the authors’ best knowledge, this is the strongest ISW of the world oceans on record. Full-depth measurements also revealed notable impacts of the extreme ISW on deep-ocean currents and thermal structures. Concurrent mooring measurements near Batan Island showed that the powerful semidiurnal internal tide generation in the Luzon Strait was likely responsible for the occurrence of the extreme ISW event. Based on the HYCOM data-assimilation product, we speculate that the strong stratification around Batan Island related to the strengthening Kuroshio may have contributed to the formation of the extreme ISW.

To date, various waves in homogeneous media have been investigated extensively and have been understood to a great extent. However, the wave behaviors in heterogeneous media have been studied and understood much less. In this work, we investigate waves that are spontaneously generated in one-dimensional heterogeneous oscillatory media governed by complex Ginzburg-Landau equations; the heterogeneity is modeled by multiple interacting homogeneous media with different system control parameters. Rich behaviors can be observed by varying the control parameters of the systems, whereas the behavior is incomparably simple in the homogeneous cases. These diverse behaviors can be fully understood and physically explained well based on three aspects: dispersion relation curves, driving-response relations, and wave competition rules in homogeneous systems. Possible applications of heterogeneity-generated waves are anticipated.

Severe dengue fever is usually associated with secondary infection by a dengue virus (DENV) serotype (1 to 4) that is different to the serotype of the primary infection. Dengue outbreaks only occur following importations of DENV in Cairns, Australia. However, the majority of imported cases do not result in autochthonous transmission in Cairns. Although DENV transmission is strongly associated with the El Niño-Southern Oscillation (ENSO) climate cycle and local weather conditions, the frequency and potential risk factors of infections with the different DENV serotypes, including whether or not they differ, is unknown. This study used a classification tree model to identify the hierarchical interactions between Southern Oscillation Index (SOI), local weather factors, the presence of imported serotypes and the occurrence of the four autochthonous DENV serotypes from January 2000–December 2009 in Cairns. We found that the 12-week moving average of SOI and the 2-week moving average of maximum temperature were the most important factors influencing the variation in the weekly occurrence of the four DENV serotypes, the likelihoods of the occurrence of the four DENV serotypes may be unequal under the same environmental conditions and occurrence may be influenced by changes in global and local environmental conditions in Cairns.

18. Buckling-induced retraction of spherical shells: A study on the shape of aperture

Buckling of soft matter is ubiquitous in nature and has attracted increasing interest recently. This paper studies the retractile behaviors of a spherical shell perforated by sophisticated apertures, attributed to the buckling-induced large deformation. The buckling patterns observed in experiments were reproduced in computational modeling by imposing velocity-controlled loads and eigenmode-affine geometric imperfection. It was found that the buckling behaviors were topologically sensitive with respect to the shape of dimple (aperture). The shell with rounded-square apertures had the maximal volume retraction ratio as well as the lowest energy consumption. An effective experimental procedure was established and the simulation results were validated in this study.
