Patellar tendinopathy (PT) typically affects jumping‐sport athletes with functional impairments frequently observed. Alterations to the functional organization of corticomotor neurons within the motor cortex that project to working muscles are evident in some musculoskeletal conditions and linked to functional impairments. We aimed to determine if functional organization of corticomotor neuron projections differs between athletes with PT and asymptomatic controls, and if organization is associated with neuromuscular control. We used a cross‐sectional design, and the setting was Monash Biomedical Imaging. Basketball and volleyball athletes with (n = 8) and without PT (n = 8) completed knee extension and ankle dorsiflexion force matching tasks while undergoing fMRI. We determined functional organization via identification of the location of peak corticomotor neuron activation during respective tasks (expressed in X, Y, and Z coordinates) and calculated force matching accuracy for both tasks to quantify neuromuscular control. We observed significant interactions between group and coordinate plane for functional organization of corticomotor projections to knee extensors (p < 0.001) and ankle dorsiflexors (p = 0.016). Compared to controls, PT group peak corticomotor activation during the knee extension task was 9.6 mm medial (p < 0.001) and 5.2 mm posterior (p = 0.036), and during the ankle dorsiflexion task 8.2 mm inferior (p = 0.024). In the PT group, more posterior Y coordinate peak activation location during the knee extension task was associated with greater task accuracy (r = 0.749, p = 0.034). Functional organization of corticomotor neurons differed in jumping athletes with PT compared to controls. Links between functional organization and neuromuscular control in the PT group suggest organizational differences may be relevant to knee extension neuromuscular control preservation. Background: Lower capacity to generate knee extension maximal voluntary force (MVF) has been observed in individuals affected with patellar tendinopathy (PT) compared to asymptomatic controls. This MVF deficit is hypothesized to emanate from alterations in corticospinal excitability (CSE). The modulation of CSE is intricately linked to the excitability levels at multiple sites, encompassing neurones within the corticospinal tract (CST), intracortical neurones within the primary motor cortex (M1), and the alpha motoneurone. The aim of this investigation was to examine the excitability of intracortical neurones, CST neurones, and the alpha motoneurone, and compare these between volleyball and basketball athletes with PT and matched asymptomatic controls. Method: Nineteen athletes with PT and 18 asymptomatic controls participated in this cross‐sectional study. Transcranial magnetic stimulation was utilized to assess CST excitability, corticospinal inhibition (silent period, and short‐interval cortical inhibition). Peripheral nerve stimulation was used to evaluate lumbar spine and alpha motoneurone excitability, including the evocation of lumbar‐evoked potentials and maximal compound muscle action potential (MMAX), and CSE with central activation ratio (CAR). Knee extension MVF was also assessed. Results: Athletes with PT exhibited longer silent period duration and greater electrical stimulator output for MMAX, as well as lower MVF, compared to asymptomatic controls (p < 0.05). Conclusion: These findings indicate volleyball and basketball athletes with PT exhibit reduced excitability of the alpha motoneurone or the neuromuscular junction, which may be linked to lower MVF. To investigate the passive muscle tension of the quadriceps muscle heads in male athletes clinically diagnosed with patellar tendinopathy ( PT) with those of healthy controls and explore the interplay between passive muscle tension and patellar tendon stiffness. Between November 2012 and December 2013, 66 male athletes (mean age of 21.1 ± 4.4 years) were examined using supersonic shear wave imaging technology. The passive tension of the vastus lateralis ( VL) and rectus femoris ( RF) muscles and patellar tendon stiffness were assessed. The shear elastic modulus of the VL muscle was increased by 26.5% ( P < 0.001) in the subjects with PT when compared with the controls. Greater passive tension in the VL was associated with higher patellar tendon stiffness ( r = 0.38; P = 0.001). The vastus lateralis muscle of the quadriceps shows increase in passive muscle tension in jumping athletes with patellar tendinopathy. These findings suggest that increase in muscle tension is not similar in the individual muscles of the quadriceps muscle. Anterior knee pain ( AKP) is a frequent clinical presentation in jumping athletes and may be aggravated by sustained sitting, stair use, and loading of the quadriceps. Corticospinal activation of the quadriceps in athletes with AKP has not yet been investigated, but is important in guiding efficacious treatment. This cross-sectional study assessed corticospinal excitability ( CSE) of the quadriceps in jumping athletes using transcranial magnetic stimulation ( TMS). Groups consisted of Control (no knee pain); patellar tendinopathy (PT) [localized inferior pole pain on single-leg decline squat ( SLDS)]; and other AKP (nonlocalized pain around the patella). SLDS (numerical score of pain 0-10), Victorian Institute of Sport Assessment Patellar tendon (VISA-P), maximal voluntary isometric contraction ( MVIC), active motor threshold ( AMT), CSE, and Mmax were tested. Twenty nine athletes participated; control n = 8, PT n = 11, AKP n = 10. There were no group differences in age ( P = 0.23), body mass index ( P = 0.16), MVIC ( P = 0.38) or weekly activity ( P = 0.22). PT had elevated CSE compared with controls and other AKP ( P < 0.001), but no differences were detected between AKP and controls ( P = 0.47). CSE appears to be greater in PT than controls and other AKP. An improved understanding of the corticospinal responses in different sources of knee pain may direct better treatment approaches. The VISA- P is a questionnaire for assessing the severity of patellar tendinopathy ( PT). Our study aim was to evaluate the equivalence of self-administration of the VISA- P online with the addition of risk factor questions to develop a tool suitable for high-volume remote use. A crossover study design with 107 subjects was used to determine equivalence between online and clinician administration. Three population groups were used to ensure construct validity. Online vs clinician administration revealed an intraclass correlation ( ICC) of 0.79 [confidence interval ( CI): 0.68-0.86] for the VISA- P with a systematic significant difference of 4.99, which is not clinically meaningful. Poor ICCs were seen for questions 7 and 8 of the VISA- P (0.37 and 0.47, respectively) in comparison with earlier questions. There were statistically significant differences between population groups for the VISA- P. The ICC for risk factor questions was excellent at 0.89 ( CI: 0.84-0.93) with no mean difference ( P = 1.00). The online questionnaire enables equivalent collection of VISA- P data and risk factor information and may well improve further with the suggested modifications to the instructions for questions 7 and 8. Tendons adapt in response to sports-specific loading, but sometimes develop tendinopathy. If the presence of ultrasound changes like hypoechoic areas and neovascularization in asymptomatic tendons precede (and predict) future tendon problems is unknown. The aim of this prospective cohort study was to investigate the relationship between the development of ultrasound changes in the patellar and quadriceps tendons and symptoms of jumper's knee, as well to examine the medium-term effects of intensive training on tendon thickness among adolescent athletes. Elite junior volleyball athletes were followed with semi-annual ultrasound and clinical examinations (average follow-up: 1.7 years). Of the 141 asymptomatic athletes included, 22 athletes (35 patellar tendons) developed jumper's knee. In a multivariate logistic regression analysis, a baseline finding of a hypoechoic tendon area (odds ratio 3.3, 95% confidence interval 1.1 to 9.2) increased the risk of developing symptoms of jumper's knee. Patellar tendon thickness among healthy athletes did not change ( Wilk's lambda, P = 0.07) while quadriceps tendon thickness increased ( P = 0.001). In conclusion, ultrasound changes at baseline were risk factors for developing symptoms of jumper's knee. Also, among healthy athletes, we observed a 7-11% increase in quadriceps tendon thickness, while there was no increase in patellar tendon thickness. [ABSTRACT FROM AUTHOR]

Patellar tendinopathy is the most common overuse knee injury in volleyball, with men reporting more than twice the injury prevalence than women. Although high patellar tendon loading is thought to be a causative factor of patellar tendinopathy, it is unknown whether between-sex variations in landing technique account for differences in patellar tendon loading. It was hypothesized that male volleyball players would display differences in landing technique and would generate higher patellar tendon loading than their female counterparts. The landing technique and patellar tendon loading of 20 male and 20 female volleyball players performing a lateral stop-jump block movement were collected. Independent t-tests were used to identify any between-sex differences in landing technique with the data grouped to account for differences in jump height and in anthropometry. Male volleyball players were taller and heavier, landed from a higher height, displayed differences in landing kinematics, generated a significantly greater knee extensor moment, and experienced higher patellar tendon loading than female players when all 40 participants were compared. However, when participants were matched on jump height, they generated similar patellar tendon loading, irrespective of their sex. The purpose of this study was to examine the effects of ankle bracing on knee kinetics and kinematics during volleyball tasks. Fifteen healthy, elite, female volleyball players performed a series of straight-line and lateral volleyball tasks with no brace and when wearing an ankle brace. A 14-camera Vicon motion analysis system and AMTI force plate were used to capture the kinetic and kinematic data. Knee range of motion, peak knee anterior-posterior and medial-lateral shear forces, and peak ground reaction forces that occurred between initial contact with the force plate and toe off were compared using paired sample t-tests between the braced and non-braced conditions ( P < 0.05). The results revealed no significant effect of bracing on knee kinematics or ground reaction forces during any task or on knee kinetics during the straight-line movement volleyball tasks. However, ankle bracing was demonstrated to reduce knee lateral shear forces during all of the lateral movement volleyball tasks. Wearing the Active Ankle T2 brace will not impact knee joint range of motion and may in fact reduce shear loading to the knee joint in volleyball players. [ABSTRACT FROM AUTHOR]

Training volume and body composition have been suggested as risk factors for jumper's knee among athletic youth, but research is lacking. The aim of this 4-year prospective cohort study was to examine the relationship between training and competition load, body composition, and risk for developing jumper's knee. Participants are elite volleyball players, aged 16-18 years. Training and competition load was recorded continuously and body composition semiannually. Jumper's knee was diagnosed on a standardized clinical examination. We recruited 141 healthy students (69 males and 72 females), and 28 developed jumper's knee (22 boys and six girls). In a multivariate analyses, boys had three to four times higher risk compared with girls. Volleyball training had an odds ratio ( OR) 1.72 (1.18-2.53) for every extra hour trained, and match exposure was the strongest sports-related predictor for developing jumper's knee with an OR of 3.88 (1.80-8.40) for every extra set played per week. We did not detect any significant differences between the groups in body composition at the time of inclusion or in the change of body composition during the study period. Appropriate management of patellar tendinopathy requires distinguishing between inflammatory and degenerative conditions, often difficult because tendon thickening can be a normal or pathological adaptation, and micromorphology is not observable on clinical imaging. The purpose of this study was to quantitatively examine patellar tendon micro- and macromorphology in volleyball athletes and relate those findings to reported symptoms. Longitudinal ultrasound images of proximal and distal patellar tendons were acquired from 84 male elite volleyball athletes (44 symptomatic, 40 asymptomatic) and 10 asymptomatic nonathlete controls. Micromorphology was determined using two-dimensional Fast Fourier Transform analysis providing a discriminating peak spatial frequency parameter ( PSF). Macromorphology (patellar tendon thickness) was measured using Image J software. All athletes regardless of symptoms had thicker proximal tendons compared to nonathletes, suggesting a normal adaptation to training loads. However, symptomatic athletes demonstrated lower PSF than asymptomatic athletes and nonathletes at the proximal tendon, suggesting greater collagen disorganization, and tendon degeneration rather than inflammation. Only symptomatic athletes had thicker distal tendons than nonathletes, but there was no difference in PSF distally. Diagnostic ultrasound enhances the understanding of the micromorphology of patellar tendons, supporting the rationale for management that remodels the degenerated tendon instead of treating inflammation. Excessive extensor mechanism loading from repeated landing has been associated with overuse knee injuries, especially patellar tendinopathy. In order to reduce these loads, it is important to establish which landing task places the highest load on the patellar tendon. It was hypothesized that the horizontal landing would create higher patellar tendon force ( FPT) compared with the vertical landing. Sixteen male athletes with healthy patellar tendons performed five successful trials of a stop-jump task, which involved a symmetrical two-foot landing after a horizontal approach (horizontal landing) followed by another symmetrical two-foot landing after a vertical jump (vertical landing). For both lower limbs during each trial, the participants' ground reaction forces were recorded, three-dimensional kinematics measured and FPT calculated by dividing the net knee joint moment by the patellar tendon moment arm. Compared with the vertical landing, significantly higher FPT, posterior ground reaction forces and FPT loading rates were generated during the horizontal landing, despite lower vertical ground reaction forces ( FV), highlighting the notion that FV should not be used to reflect FPT. Understanding that a horizontal landing task places the highest load on the patellar tendon, provides an appropriate framework for future research to investigate lower limb landing strategies in athletes with patellar tendinopathy.
