Cervical cancer continues to be a significant health problem in Latin America. The use of conventional cytology to detect precancerous cervical lesions has had almost no major impact on reducing cervical cancer incidence and mortality rates, which are still high in the region. The availability of new screening tools to detect precancerous lesions provide great opportunities for cervical cancer prevention in the region, as do highly efficacious HPV vaccines able to prevent nearly all lesions associated with HPV-16 and -18 when applied before viral exposure. This paper summarizes the scientific evidence and regional experiences related to: i) the use of HPV testing and visual inspection after the application of acetic acid (VIA) in primary screening and ii) the implementation of adolescent HPV vaccination programs. Finally, we outline a number of recommendations for different resource settings. The feasibility of implementing successful and sustainable national cervical cancer prevention programs in Latin American countries in the region will depend on health priorities and the availability of infrastructure and health personnel--as determined by rigorous local situational analysis.

Exposure to lead is a widespread and serious threat to the health of children in Latin America. Health officials should monitor sources of exposure and health outcomes to design, implement, and evaluate prevention and control activities. To evaluate the magnitude of lead as a public health problem, three key elements must be defined: I) the potential sources of exposure, 2) the indicators to evaluate health effects and environmental exposure, and 3) the sampling methods for the population at risk. Several strategies can be used to select the study population depending on the study objectives, the time limitations, and the available resources. If the objective is to evaluate the magnitude and sources of the problem, the following sampling methods can be used: I) population-based random sampling; 2) facility-based random sampling within hospitals, daycare centers, or schools; 3) target sampling of high risk groups; 4) convenience sampling of volunteers; and 5) case reporting (which can lead to the identification of populations at risk and sources of exposures). For all sampling methods, information gathering should include the use of a questionnaire to collect general information on the participants and on potential local sources of exposure, as well as the collection of biological samples. In interpreting data, one should consider the type of sampling used and the non-response rates, as well as factors that might influence blood lead measurements, such as age and seasonal variability. Objective: This paper characterizes the current stage of traditional medicine in nine countries of Latin America and the Caribbean., Material and Methods: This qualitative study was conducted between March and December 1998. Data were collected on the components of traditional health systems in countries of Latin America and the Caribbean, by means of a network of individuals and institutions from different countries that acted as expert informants from different specialty areas., Results: Findings from the analysis of traditional medicine regulation are presented in three groups: a) Countries with some developments in the area of legislation; b) Countries where legislation is underway; and, c) Countries with no legislation or incipient regulation., Conclusions: Several stages of traditional medical practice legislation are found in the region. This heterogeneity shows the complexity involved in regulating the practice of providers with low levels of formal training, with different therapeutic practices, and with customs that are frequently difficult to include within the standards of the official health system. These findings are important for designing and implementing healthcare policies to adequate traditional medical practices to the needs of populations that commonly use them.

The cost-effectiveness of a health intervention is an estimate of the relation between what it costs to be provided, and the improvement in health which results from such intervention. Health may improve because the incidence of illness or injury is reduced, because death is avoided or delayed, or because the duration or severity of disability is limited. The calculation of this health benefit combines objective factors, such as the age at incidence and whether or not the outcome is death, with subjective factors such as the severity of disability, the judgement as to the value of life lived at different ages, and the rate at which the future is discounted. The construction and interpretation of the estimate are explained. Also, the paper examines whether the concept of cost-effectiveness is consistent with ethical norms such as equity, and concludes that they are not in conflict. Finally, it addresses the question of how to incorporate cost-effectiveness into a health sector reform, and possible ways to implement it.

In this paper the paths taken by developing countries to improve the health and well-being of their populations in a context of diminishing resources is discussed. Emphasis is placed on Latin American countries and the objective is to clarify the changing nature of the relationship between health and social well-being in the 1970s and 1980s. In the second part of the paper three viable and different political scenarios to confront this problem are presented: 1) survival without well-being; 2) survival and eradication of extreme poverty, and 3) survival, eradication of extreme poverty, and selective reconversion of the poor.

In this paper the role of efficiency in health care delivery is discussed. In the first part several definitions of efficiency are presented and the most common efficiency analysis are described. In the second part of the paper some experiences aimed at improving the efficiency of health care delivery in England, Latin America and Mexico are discussed. Finally, the author presents four proposals for the improvement of efficiency of health care delivery in Mexico: monetary reward associated to performance, alternative organization of health services, investment in management training for health manpower, and operational research in the health systems.

In this paper the concepts and criteria related to health coverage are discussed in the context of the organization of national health systems. The main international agreements based on WHO/PAHO proposals are also described. The relationship between primary health care and health coverage is analyzed and the evolution of the programs for the extension of health coverage in Mexico are discussed, with emphasis on the problems of overlap and definition of the universe in the several institutions of the health sector. Finally, the author reviews the problems to measure coverage in order to guarantee social and operative efficiency of the Mexican health system.

This paper discusses the conditions that force the countries in Latin America and the Caribbean to design and produce vaccines for the most common infectious diseases in the Region on a regional basis. The authors discuss as well the goals and strategies of the Regional System for the Production of Vaccines in Latin America, its possible structure and economic feasibility.

This paper discusses, on the basis of the Bruntland Report on development and environment, the relationship between population and development possibilities, and its consequences on natural and man made resources. The author emphasizes the need to design development policies in which the protection of the environment is a priority, specially in Latin American countries.
