The main objective of the study was to analyze the spatial distribution of notified and confirmed cases of yellow fever in the state of Piauí, from 2016 to 2019. This was an ecological spatial study of yellow fever, with data from the Notifiable Diseases Information System (SINAN) from 2016 to 2019. The X² association test was applied for bivariate analysis and the Moran Index for spatial analysis. During this period, 21 cases and 1 confirmed case were reported, this case being wild yellow fever and not in humans. The municipality of Teresina (33.3%) and the health region Entre Rios (42.9%) had the highest records. Males (61.9%), the age group between 20 and 34 years (38.1%) and brown ethnicity (61.9%) were the most affected. As for education, 42.9% were ignored. The notifications of yellow fever cases, in the state of Piauí, are concentrated in poles with a large population flow. Although there is no confirmation of yellow fever cases in humans, vector prevention and control strategies must be implemented considering these factors, in addition to professional training for the correct notification of cases.
