3. TIBIOTALOCALCANEAL ARTHRODESIS WITH LATERAL COMPRESSION PLATE: ARTRODESE TIBIO-TALO-CALCANEANA COM PLACA DE COMPRESSÁO LATERAL.

- Abstract
Objective: The objective of this study was to evaluate the short-term clinical and radiographic results from a TTC fusion procedure using a lateral locking plate specifically designed for this purpose., Methods: All the fourteen patients were evaluated using a variety of methods, including a visual analogue scale (VAS), the AOFAS hindfoot score and a subjective satisfaction scale. The average follow-up was 10 months., Results: The average AOFAS score improved from 41 points preoperatively to 63 points postoperatively. The VAS pain score decreased from 7 to 3 points. Four patients reported excellent results, eight good, one fair and one poor., Conclusion: Our study provides support for using a specially designed plate for TTC fusion in hindfoot salvage surgery., Resumo: Objetivo: O propósito deste estudo é avaliar os resultados clínicos e radiológicos a curto prazo do procedimento de fusão tíbio-talo-calcaneana (TTC) com o uso de uma placa lateral bloqueada específicamente concebida para este fim. Métodos: Todos os 14 pacientes foram avaliados usando uma variedade de métodos, incluindo a escala analógico visual da dor (EAV), escore da AOFAS para o retropé e escala subjetiva de satisfação. O acompanhamento médio foi de 10 meses. Resultados: O escore AOFAS médio melhorou de 41 pontos no período pré-operatório para 63 pontos no período pós-operatório. O escore da dor (EAV) caiu de 7 para 3 pontos. Quatro pacientesforam classificados como excelentes resultados, oito bons, um moderado e um ruim. Conclusão: Nosso estudo dá suporte ao uso da placa especialmente desenhada para a fusão tibio-talo-calcaneana (TTC) nas cirurgias de salvamento do retropé.

4. EVALUATION OF METATARSAL RELATIONSHIPS IN THE BIOMECHANICS OF 332 NORMAL FEET USING THE METHOD OF MEASURING RELATIVE LENGTHS.

Unlabelled: To identify the mean normal length of the metatarsals and the most common metatarsal formulas through a simple measurement method, thereby providing surgeons with data for planning treatment on symptomatic individuals with biometric abnormalities of the foot., Methods: We evaluated and measured dorsoplantar weight-bearing radiographs of normal adult feet (83 males and 83 females)., Results: We found relative mean lengths for metatarsus I of 125.4 mm for males and 115.1 mm for females; for metatarsus II, 127.8 mm for males and 117.3 mm for females; for metatarsus III, 123.4 mm for males and 113.5 mm for females; for metatarsus IV, 114.2 mm for males and 105.3 mm for females; for metatarsus V, 99.5 mm for males and 91.7 mm for females. The mean forefoot width was 87.1 mm for males and 80.8 mm for females., Conclusion: Feet with index minus occurred most frequently in both sexes, although all three metatarsal formulas can be considered to be normal patterns. The mean normal pattern for males and females respectively was the following: metatarsus I 2.4 mm and 2.2 mm shorter than metatarsus II; metatarsus III 4.4 mm and 3.8 mm shorter than metatarsus II; metatarsus IV 9.2 mm and 8.2 mm shorter than metatarsus III; metatarsus V 14.7 mm and 13.6 mm shorter than metatarsus IV.

5. TOTAL ANKLE ARTHROPLASTY: BRAZILIAN EXPERIENCE WITH THE HINTEGRA PROSTHESIS.

- Abstract
Ankle arthrosis is becoming more and more common. The search for solutions that preserve joint function has led to a new generation of prosthesis with three components and more degrees of freedom. This paper presents the results achieved for ten patients treated with the HINTEGRA Prosthesis (Integra, New Deal), through collaborative action between the Foot and Ankle Groups of the Orthopedics and Traumatology divisions of Escola Paulista de Medicina, Unifesp, and the School of Medicine of the University of São Paulo (USP). The ten patients (six women and four men, aged between 29 and 66 years), underwent a surgical procedure consisting of Hintermann's technique, between January and June 2005. They were evaluated at prearranged intervals, and the data were subjected to statistical analysis. The surgery led to a significant improvement in ankle mobility. Radiological evaluation showed no signs of loosening or failure in the prosthetic components in any of the patients studied. Although the complication rate in our sample was high, it was equivalent to the rates found by other authors, and directly represents the learning curve associate with this kind of procedure. Four years after the procedure, it was found that the patients pain levels had significantly decreased, and that their functional patterns had significantly improved, with AOFAS and Hintermann scores indicating results that were excellent for 20%, good for 70% and poor for 10%. Treatment of ankle arthritis by means of total arthroplasty using the HINTEGRA prosthesis was capable of providing good results over an average observation period of four years.

6. TIBIOTARSAL COMPRESSION ARTHRODESIS USING A LATERAL LOCKING PLATE.

- Abstract
Objectives: Tibiotalar (TT) arthrodesis is still a very important option in the treatment of primary or post-traumatic arthritis of the ankle but persists the controversy regarding the optimal method for the fixation of the arthrodesis site. No matter the implant used, the goal is to obtain a solid, healthy, pain-free fusion. The purpose of the current study is to present the preliminary results of a novel laterally based tibiotalar compression arthrodesis system using a locked plate., Methods: Thirteen consecutive patients with tibiotalar arthritis were submitted to an arthrodesis using a new lateral plating system. The average age was 59.7 years (range 36~72); nine patients were male and four female. Using a cutting guide, the remaining articular surfaces of the tibia and talus were removed. A compression device was applied to avoid malalignment of the ankle and a precontoured lateral locking plate was used to achieve the joint fusion., Results: Both the AOFAS ankle-hindfoot score and the VAS score improved with the surgery. All patients' ankles fused by 6 months. In all patients we found a very good alignment in the sagittal, coronal and transverse planes., Conclusion: We believe that a combination of a bilateral compression, contoured bony cuts, and lateral locked plating offers a novel, accurate and useful technique for ankle arthrodesis.

7. ANTEROGRADE PERCUTANEOUS TREATMENT OF LESSER METATARSAL FRACTURES: TECHNICAL DESCRIPTION AND CLINICAL RESULTS.

- Abstract
Objective: The aim of this study was to evaluate the results obtained using the anterograde percutaneous fixation technique for treating shaft and neck fractures of the lesser metatarsals., Methods: We prospectively evaluated 14 patients between 2003 and 2008, taking into consideration the topography of the fracture, trauma mechanism, associated comorbidities and AOFAS score for the forefoot., Results: The anatomical region most affected was the metatarsal neck (79%). Involvement of multiple metatarsals (53%) was more common than isolated fractures (47%). Low-energy trauma (79%) was more frequent than high-energy trauma (21%). Female patients with diabetes had the worst postoperative functional results. There were no postoperative complications relating to the type of treatment instituted., Conclusion: The surgical technique presented was efficient for treating fractures of the lesser metatarsals, with a lower complication rate than shown by other established techniques in the literature.

8. How to classify plantar plate injuries: parameters from history and physical examination.

- Abstract
Objective: To find the best clinical parameters for defining and classifying the degree of plantar plate injuries., Method: Sixty-eight patients (100 metatarsophalangeal joints) were classified in accordance with the Arthroscopic Anatomical Classification for plantar plate injuries and were divided into five groups (0 to IV). Their medical files were reviewed and the incidence of each parameter for the respective group was correlated. These parameters were: use of high heels, sports, acute pain, local edema, Mulder's sign, widening of the interdigital space, pain in the head of the corresponding metatarsal, touching the ground, "drawer test", toe grip and toe deformities (in the sagittal, coronal and transversal planes)., Results: There were no statistically significant associations between the degree of injury and use of high-heel shoes, sports trauma, pain at the head of the metatarsal, Mulder's sign, deformity in pronation or displacement in the transversal and sagittal planes (although their combination, i.e. "cross toe", showed a statistically significant correlation). Positive correlations with the severity of the injuries were found in relation to initial acute pain, progressive widening of the interdigital space, loss of "touching the ground", positive results from the "drawer test" on the metatarsophalangeal joint, diminished grip strength and toe deformity in supination., Conclusions: The "drawer test" was seen to be the more reliable and precise tool for classifying the degree of plantar plate injury, followed by "touching the ground" and rotational deformities. It is possible to improve the precision of the diagnosis and the predictions of the anatomical classification for plantar plate injuries through combining the clinical history and data from the physical examination.
