A 31-year-old man with advanced-stage Duchenne muscular dystrophy (DMD) experienced a lack of efficacy during treatment with scopolamine for aspiration. The man had a history of recurrent aspiration pneumonia and chronic hypercapnia, leading to a tracheostomy and percutaneous endoscopic gastrostomy (PEG) tube placement. Despite receiving scopolamine, he continued to suffer from recurrent aspiration pneumonia. Eventually, he underwent a total laryngectomy, which resulted in a quick recovery, improved quality of life, and no recurrence of aspiration pneumonia. [Extracted from the article]
