Purpose To determine if a reduced-dose computed tomography (CT) protocol could effectively help to identify patients in the emergency department (ED) with moderate to high likelihood of calculi who would require urologic intervention within 90 days. Materials and Methods The study was approved by the institutional review board and written informed consent with HIPAA authorization was obtained. This was a prospective, single-center study of patients in the ED with moderate to high likelihood of ureteral stone undergoing CT imaging. Objective likelihood of ureteral stone was determined by using the previously derived and validated STONE clinical prediction rule, which includes five elements: sex, timing, origin, nausea, and erythrocytes. All patients with high STONE score (STONE score, 10-13) underwent reduced-dose CT, while those with moderate likelihood of ureteral stone (moderate STONE score, 6-9) underwent reduced-dose CT or standard CT based on clinician discretion. Patients were followed to 90 days after initial imaging for clinical course and for the primary outcome of any intervention. Statistics are primarily descriptive and are reported as percentages, sensitivities, and specificities with 95% confidence intervals. Results There were 264 participants enrolled and 165 reduced-dose CTs performed; of these participants, 108 underwent reduced-dose CT alone with complete follow-up. Overall, 46 of 264 (17.4%) of patients underwent urologic intervention, and 25 of 108 (23.1%) patients who underwent reduced-dose CT underwent a urologic intervention; all were correctly diagnosed on the clinical report of the reduced-dose CT (sensitivity, 100%; 95% confidence interval: 86.7%, 100%). The average dose-length product for all standard-dose CTs was 857 mGy · cm ± 395 compared with 101 mGy · cm ± 39 for all reduced-dose CTs (average dose reduction, 88.2%). There were five interventions for nonurologic causes, three of which were urgent and none of which were missed when reduced-dose CT was performed. Conclusion A CT protocol with over 85% dose reduction can be used in patients with moderate to high likelihood of ureteral stone to safely and effectively identify patients in the ED who will require urologic intervention. Appropriate CT protocol modification in conjunction with adaptive statistical iterative reconstruction provides diagnostic-quality images for evaluating urinary calculi at a substantially reduced radiation dose.

To determine the effect of reduced radiation (tube charge, measured as milliamperes per second) protocols on the sensitivity and specificity of multidetector computed tomography (CT) in the detection of renal calculi.This Health Insurance Portability and Accountability Act-compliant human cadaveric study was approved by the Department of Anatomic Pathology with strict adherence to the university policy for handling donor specimens. Three to five renal stones (range, 2.0-4.0 mm) were randomly placed in 14 human cadaveric kidneys and scanned with a 16-detector CT scanner at 100, 60, and 30 mAs while maintaining other imaging parameters as constant. Following acquisition, images were reviewed independently by two radiologists who were blinded to the location and presence of renal calculi. Interobserver agreement was measured with kappa statistics. The McNemar test was used to compare the sensitivity and specificity between different radiation settings for each reader.Specificity for both readers ranged from 105 (0.95; 95% confidence interval [CI]: 0.90, 0.99) to 109 (0.99; 95% CI: 0.95, 1.0) of 110 without significant differences between 30 and 60 mAs to the standard 100 mAs (P = .500 to.999). Sensitivity ranged from 42 (0.74; 95% CI: 0.60, 0.84) to 48 (0.84; 95% CI: 0.72, 0.93) of 57, also without significant differences (P = .070 to.999). When renal calculi detection rates were analyzed by size, 3.0-4.0-mm stones were detected well at all tube charge settings, ranging from 86%-90% (n = 21 for 3.0-mm stones) to 95%-100% (n = 19 for 4.0-mm stones). However, 2.0-mm stones were poorly detected at all tube charge settings (29%-59%; 5-10 of 17). Overall interobserver agreement for stone detection was excellent, with kappa = 0.862.Decreasing the tube charge from 100 to 30 mAs resulted in similar detection of renal stones while reducing patient radiation exposure by as much as 70%. Multidetector CT scanning parameters should be tailored to minimize radiation exposure to the patients while helping detect clinically significant renal stones.

To estimate organ dose from pediatric chest and abdominopelvic computed tomography (CT) examinations and evaluate the dependency of organ dose coefficients on patient size and CT scanner models.The institutional review board approved this HIPAA-compliant study and did not require informed patient consent. A validated Monte Carlo program was used to perform simulations in 42 pediatric patient models (age range, 0-16 years; weight range, 2-80 kg; 24 boys, 18 girls). Multidetector CT scanners were modeled on those from two commercial manufacturers (LightSpeed VCT, GE Healthcare, Waukesha, Wis; SOMATOM Definition Flash, Siemens Healthcare, Forchheim, Germany). Organ doses were estimated for each patient model for routine chest and abdominopelvic examinations and were normalized by volume CT dose index (CTDI(vol)). The relationships between CTDI(vol)-normalized organ dose coefficients and average patient diameters were evaluated across scanner models.For organs within the image coverage, CTDI(vol)-normalized organ dose coefficients largely showed a strong exponential relationship with the average patient diameter (R(2)0.9). The average percentage differences between the two scanner models were generally within 10%. For distributed organs and organs on the periphery of or outside the image coverage, the differences were generally larger (average, 3%-32%) mainly because of the effect of overranging.It is feasible to estimate patient-specific organ dose for a given examination with the knowledge of patient size and the CTDI(vol). These CTDI(vol)-normalized organ dose coefficients enable one to readily estimate patient-specific organ dose for pediatric patients in clinical settings. This dose information, and, as appropriate, attendant risk estimations, can provide more substantive information for the individual patient for both clinical and research applications and can yield more expansive information on dose profiles across patient populations within a practice.

To prospectively assess parenchymal, vascular, and biliary anatomy of potential living liver donors with an all-inclusive multi-detector row computed tomographic (CT) approach.A total of 250 potential living liver donors (112 women, 138 men; mean age, 37 years) underwent three-phase, dual-enhancement multi-detector row CT to delineate biliary, vascular, and parenchymal morphology according to an institutional review board-approved protocol. Informed consent was obtained from all subjects. For display of the biliary system, the first CT image set was collected after the infusion of a biliary contrast agent. CT angiography was subsequently performed, after automated injection of a conventional iodinated contrast agent, to display the arterial and portal-hepatic venous systems. All data sets were reconstructed in 1-mm sections. Data analysis was based on source images, multiplanar reconstructions, and three-dimensional postprocessing images; was performed in consensus by two radiologists; and was focused on the detection of biliary and vascular variants, exclusion of focal liver lesions, and determination of hepatic volumes. Preoperative findings were correlated with intraoperative findings (available in 62 subjects).Technical failures were experienced in 10 of 250 examinations. Twenty-seven subjects had moderate adverse reactions related to the biliary contrast agent. Benign hepatic lesions were detected in 61 candidates; one candidate had a renal cell carcinoma. Underlying biliary and vascular anatomy was displayed at least to the second intrahepatic branch in all but seven patients. Detected anatomic variants involved the biliary (38.8%), arterial (40.0%), portal venous (21.4%), and hepatic venous (43.5%) systems. Correlation with intraoperative findings was excellent. Some biliary (n = 4), arterial (n = 5), portal venous (n = 1), and hepatic venous (n = 6) variants were missed or misinterpreted at initial reading of preoperative data; however, variants could be retrospectively depicted in all but one biliary case and one hepatic venous case.The outlined three-phase, dual-enhancement multi-detector row CT protocol represents an all-inclusive approach to evaluate potential living liver donors in a single diagnostic step.

To investigate image quality and vascular delineation of multi-detector row computed tomographic (CT) angiography at various kilovoltage settings.Thirty patients were investigated with a standardized CT protocol, with three groups of 10 consecutive patients examined at 80, 120, and 140 kV, respectively. Three blinded readers independently evaluated images and graded image quality parameters, diagnostic confidence, and vascular delineation of intracranial arteries and veins. Vascular CT attenuation values, CT dose indices, and dose length products were assessed quantitatively. For data analysis, a Kruskal-Wallis nonparametric rank F test was used to identify trends and variables that required modeling attention. A proportional odds multinomial regression model was then fit with generalized estimating equations to account for the correlated nature of the data.Image quality was rated higher with higher kilovoltage settings (P.001). The severity of imaging artifacts was higher with lower kilovoltage settings (P.001), while the subjectively rated vessel contrast was lower in the 80-kV group than in the 120-kV group and the 140-kV group (P.05). Diagnostic confidence was higher in the 120-kV group and 140-kV group (P.005). Vascular delineation was higher with higher kilovoltage settings for most arterial and venous structures. Differences were more significant for structures in close topographic proximity to bone and for subsegmental arteries and were less significant and, in parts, not significant for the main arterial branches and the large venous sinus. Attenuation values were higher with lower kilovoltage settings (P.05). The mean dose length product could be reduced from 594 mGy. cm in the 140-kV group to 152 mGy.cm in the 80-kV group.This multireader study of image quality and vessel delineation with cranial multi-detector row CT angiography at various kilovoltage settings demonstrated a superiority of higher voltages with most pronounced effects for vessels adjacent to bone and subsegmental arteries.

Surgical and CT examinations of 104 patients, each of whom presented with a parapharyngeal space mass, has resulted in the development of an updated CT protocol designed to provide a preoperative diagnosis. When dynamic scanning is used, diagnostic angiography or digital venous imaging (DVI) can be avoided in almost all of these cases. A specific preoperative diagnosis can be made in 88% of the patients and a limited differential diagnosis, which includes the final diagnosis, can be made in virtually all cases. The protocol and the problems encountered in differential diagnosis are presented. Evidence to suggest that minor salivary gland benign mixed tumors may arise in salivary rest tissue, rather than pharyngeal mucosal glands, is also presented.
