Antecedentes: Aunque la reconstrucción mamaria (RM) tras la mastectomía es una opción estética con buenos resultados, se realiza poco en México. Para estudiar sus razones se creó el Cuestionario de Motivos a favor y en contra de la Reconstrucción Mamaria (CMRM). La de validez de contenido del CMRM ha sido comprobada, pero no otras propiedades psicométricas. Objetivos: determinar su estructura factorial y comprobar su fiabilidad y validez de constructo concurrente. Método: Este estudio instrumental con diseño no experimental de corte transversal usó un muestreo intencional. Se aplicó el CMRM integrado por dos escalas, un cuestionario de imagen corporal y preguntas sobre datos sociodemográficos, clínicos y conducta sexual a 110 mujeres mexicanas mastectomizadas. Resultados: La escala de motivos a favor de la RM del CMRM presentó una estructura unidimensional, consistencia interna excelente y validez convergente. La escala de motivos en contra de la RM presentó una estructura de tres factores: miedos/preocupaciones/costos, aceptación/satisfacción con el estado actual y falta de información. Sus tres factores presentaron consistencia interna y validez convergente y discriminante. La correlación entre ambas escalas fue pequeña, pero la consistencia interna del CMRM fue excelente. La fuerza de asociación del CMRM fue muy fuerte con deseo de RM, media con edad e imagen corporal y pequeña con deseo de conservar la mama, frecuencias de relaciones sexuales antes del diagnóstico, búsqueda de información sobre RM y percepción del estado económico. Fue independiente de escolaridad, etapa del cáncer, tipo de mastectomía y estado civil. Conclusión: el cuestionario es fiable y válido.

Introducción: Las pacientes con cáncer de mama presentan dentro de sus principales efectos en su calidad de vida durante o posterior al tratamiento deterioro cognitivo. Su detección y atención oportuna puede mejorar la funcionalidad de este grupo de pacientes. Objetivo: Determinar las propiedades psicométricas del Instrumento de Evaluación de Funcionalidad en Tratamiento Para el Cáncer – Función Cognitiva (FACTCOG) en una muestra de pacientes con cáncer de mama mexicanas. Método: Participaron 200 pacientes del Instituto Nacional de Cancerología, la edad promedio fue de 45.59 años. Se utilizó un diseño transversal no experimental. Instrumentos: FACT-COG, Escala de Ansiedad y Depresión Hospitalaria (HADS) y el Mini Examen del Estado Mental (MMSE). Análisis estadístico: Se realizó un modelo factorial exploratorio y confirmatorio, así como validez concurrente. Resultados: Se identificaron cuatro factores con 36 reactivos. Las consistencias internas de cada subescala tuvieron un valor de entre 0,98 a 0,95, de la escala global 0,98, que explicaron el 77,6% de la varianza. El análisis factorial confirmatorio presenta indicadores adecuados y del ajuste de la estructura del modelo (CFI, RMR y RMSEA) que indican un modelo equilibrado y parsimonioso. La validez, por medio de correlación con las medidas concurrentes, mostró resultados significativos (r de Pearson de ,80 a -,55). Discusión y conclusión El FACT-COG en pacientes con cáncer de mama en población mexicana presentó adecuadas y robustas características psicométricas exploratorias y confirmatorias. La relevancia de los resultados obtenidos radica en que se trata de una población que puede llegar a requerir detección oportuna en afectaciones cognitivas durante su tratamiento.

Objetivo: Elaborar un instrumento que nos permita evaluar algunas estrategias cognitivas que formarían parte de las personas optimistas ante una dificultad de salud como el cáncer de mama. Método: 96 pacientes, de ellas 30 se encontraban en una fase de intervalo libre del cáncer de mama, 35 recibiendo tratamiento activo para el cáncer de mama y 31 tenían un diagnóstico de trastorno adaptativo sin historial médico oncológico. Se han comparado los mecanismos cognitivos del instrumento para valorar el optimismo estratégico entre los grupos de salud; se ha efectuado el análisis factorial del instrumento y se ha observado la relación de éste con variables como la calidad de vida y el optimismo disposicional en cada grupo. Resultados: el análisis factorial muestra cuatro componentes del total de ítems del optimismo estratégico: 1) Expectativas de resultado positivo y percepción de control ante la dificultad de salud; 2) Valoración positiva del pasado en el momento actual de salud; 3) Benefit finding a raíz de la enfermedad; y 4 ) Responsabilidad/culpabilidad por la situación de salud. Introducción: El cáncer de mama es considerado una enfermedad crónica que impacta de modo importante en la calidad de vida (QoL) de las pacientes. En la actualidad no se dispone de un sistema de evaluación totalmente satisfactorio que refleje la complejidad del cáncer de mama. Además, la mayoría de instrumentos están más orientados a la enfermedad en estadios iniciales que a la enfermedad avanzada. Objetivo: Describir la metodología utilizada para evaluar la calidad de vida y las estrategias de afrontamiento en un grupo representativo de cáncer de mama que incluye tanto pacientes metastásicas como no metastásicas. Método: Estudio prospectivo en pacientes con cáncer de mama del Servicio de Oncología Médica del Institut Català d’Oncologia (ICO) en diferentes estadios de la enfermedad, previo consentimiento informado. Se les aplica un protocolo de aproximadamente una hora de entrevista presencial donde se recoge su información sociodemográfica, y se contestan varios cuestionarios de calidad de vida como el cuestionario QLCA-AFex Font, QLQ-C30, QLQ-BR23 HADS, DME, BRCS, MINI-MAC, LOT-R y OE, que se completan con una entrevista semiestructurada. Resultados: De junio de 2017 a marzo de 2020, 257 pacientes han sido incluidas en el estudio. La media de edad es de 57,9 años (SD 10,1), la mayoría son mujeres (98,8%), con hijos (87,9%) y casadas (65,4%). Respecto al estadiaje clínico 75,5% son no-metastásicas y 24,5% metastásicas. El cumplimiento del protocolo se consiguió en más del 90% de los cuestionarios sin diferencias entre pacientes metastásicas y no metastásicas. Conclusión: Este protocolo multidimensional permite hacer una evaluación integral de la calidad de vida, así como reflejar las necesidades no atendidas y las preocupaciones que muestran las pacientes tanto en estadio precoz como avanzado, complementando los sistemas de valoración actualmente disponibles. Después del análisis de los resultados de este estudio sería interesante poder obtener un cuestionario único con las preguntas más relevantes que pudiera ser aplicado a la práctica clínica.

Objetivo: Obtener información sobre las estrategias de afrontamiento que utilizan las pacientes de cáncer de mama y su relación con la calidad de vida, diferenciando dos situaciones de salud: pacientes con y sin metástasis, atendidas en un mismo hospital oncológico especializado de nuestro entorno. Método: 60 mujeres con cáncer de mama fueron entrevistadas individualmente y completaron dos instrumentos que medían diversos componentes de afectación de la calidad de vida (QLTF) y cinco maneras diferentes de enfrentarse a la enfermedad (test mini-MAC). Resultados: Las estrategias de afrontamiento se relacionan con la calidad de vida de las pacientes con cáncer de mama. En nuestro estudio observamos que responder a la situación oncológica mediante “Preocupación Ansiosa” o Desesperanza” correlaciona con una mayor afectación de la calidad de vida general. Sin embargo, no se observan diferencias significativas entre pacientes con metástasis y sin metástasis en cuanto a las estrategias utilizadas. Las pacientes con metástasis presentan más dificultades en la realización de sus actividades cotidianas, pero el malestar emocional es similar en ambos grupos de pacientes. Conclusión: En congruencia con estudios previos, las estrategias y respuestas de afrontamiento activo se relacionan con mejor calidad de vida. Aunque las pacientes con metástasis presentan más dificultades y afectación de la calidad de vida, parece ser que se enfrentan a la enfermedad de manera similar a las pacientes sin metástasis.

Introducción: A pesar de que los tratamientos psico-oncológicos han demostrado su efectividad en disminuir el malestar emocional y mejorar la calidad de vida de las personas con cáncer, aún existen numerosas barreras que limitan su acceso. La transformación a online de esta atención se plantea como una solución para aumentar la cobertura del servicio y mejorar su coste-utilidad. Objetivo: Crear un ecosistema digital de salud para reducir el impacto del cáncer, aumentando el bienestar y la calidad de vida del ciudadano con cáncer. Método: Programa dirigido a pacientes diagnosticadas de cáncer de mama en fase de supervivencia aguda. Es un programa de atención escalonada dividido en 4 niveles de intervención jerarquizados por complejidad: Nivel 1, cribado y monitorización psicosocial; Nivel 2, Campus: psicoeducación y educación sanitaria; Nivel 3, soporte psicosocial comunitario; y Nivel 4, tratamiento psicoterapéutico grupal. Resultados: En 2019, 259 mujeres fueron incluidas en el programa (39,91% de los nuevos casos de cáncer de mama en los centros participantes). Solo el 3,47% (n = 9) requirió atención clínica especializada (Nivel 4). Conclusión: El programa Iconnecta’t adopta un modelo integrado de atención psicosocial en cáncer que se adecúa a las necesidades específicas de los supervivientes. Da solución a algunas de las barreras de la atención sanitaria tradicional, democratizando el acceso a los servicios mediante el uso de tecnologías de uso común en la mayoría de ciudadanos. En un futuro próximo se prevé la implementación progresiva a otras neoplasias, junto con un ensayo clínico controlado y aleatorizado que evaluará su eficacia.

20. La influencia del estrés y el optimismo sobre la calidad de sueño de mujeres con cáncer de mama

Objetivo: El objetivo de este estudio fue determinar la influencia del estrés y el optimismo sobre la calidad de sueño de mujeres con cáncer de mama. Método: Diseño trasversal correlacional. Los instrumentos utilizados fueron el Índice de Calidad del Sueño de Pittsburgh, la Escala de Estrés Percibido y la Escala de Orientación de Vida. Participaron 60 pacientes con cáncer de mama en estadios tempranos, sin metástasis, con una media de edad de 51,4 (±9,3) años. Resultados: Las pacientes obtuvieron una media de 7,5 (±4,4) del Índice de calidad de sueño global, en donde a mayor puntaje indica una mala calidad de sueño, una media de 24,5 (±9,5) para estrés y una media 37,6 (±5,5) para optimismo. Se obtuvo una correlación positiva y significativa entre el estrés y calidad de sueño global (r =0,349, p=0.006), y una correlación negativa y significativaentre optimismo y calidad de sueño (r =-0,524, p=0,001). El optimismo predice la calidad de sueño global en un 25,8%. Conclusión: En mujeres con cáncer de mama en estadios tempranos, la relación entre variables indica que a mayor estrés menor calidad de sueño, menor duración del dormir y más alteraciones de sueño. En cuanto a la variable de optimismo, indica que, a mayor optimismo, mayor calidad subjetiva de sueño, mayor duración, menor latencia de sueño y menos alteraciones del dormir. Objective: Posttraumatic Growth (PTG) is a perceived positive change after a stressful situation. Studies describe different predictors of PTG. The purpose of this study was to (1) review the evidence that rumination and social support are predictors of PTG; (2) analyze the results of the screened studies. Method: A systematic review was conducted by searching for articles with quantitative or mixed methods that evaluated PTG using the Posttraumatic Growth Inventory, rumination and/or social support in women with breast cancer. Results: Were identified twelve articles that corresponded to the inclusion criteria. All of them reported some degree of PTG in their samples. Rumination was evaluated in three studies, social support was evaluated in ten, and both were considered to have a positive correlation with PTG. Conclusions: This review concludes that rumination and social support are predictors of PTG in women with breast cancer. These results contribute to the development of new interventions in mental health.

Objective: Although the reasons for breast reconstruction are considered in clinics and research on breast cancer, there is not any validated scale for its evaluation. The objective of the study was to create a scale to measure the reasons for and against breast reconstruction. Method: Through the technique of natural semantic networks, the items of the scale were obtained, and through an expert judgment these were modified. The technique of semantic networks was applied to 65 Mexican women with breast cancer. The group of experts was formed by 12 specialists in breast cancer. Results: The central core of the reasons in favor of breast reconstruction was composed of 16 reasons,which can be grouped into esthetic/body image, functional, emotional/psychological and relationshipwith children and the couple. The core of the motives against breast reconstruction was composedof 11 motives, which can be grouped into fears, acceptance/comfort with the present situation, costsand health. Based on these contents, 16 items were written about reasons for reconstruction and 11against reconstruction. This 27-item scale was evaluated in adequacy and comprehensibility by thegroup of 12 experts. The 27 items were conserved, but seven items were modified. Finally, two itemson the lack of information were added, which was marked in Mexican studies. Conclusions: It issuggested to study the reliability, validity and distribution of the 29 item scale.

Método: se estudiaron transversalmente a 150 mujeres jóvenes con CaMa en tratamiento y sobrevivientes, con una encuesta de NAT y una de resiliencia mexicanas. Objetive. To assess the effectiveness of psychological interventions in mindfulness with patients surviving breast cancer. Method. We searched the databases: Pubmed, CINAHL and Psycinfo, investigations with interventions in mindfulness to adult patients surviving breast cancer, which included experimental groups and control and monitoring after completing the interventions during the period from January 2011 to October 2017. Results. We obtained 10 investigations with randomized controlled designs and follow-ups that apply the psychological intervention in mindfulness with adult breast cancer survivors. Conclusions. The psychological interventions in mindfulness positively affect the reduction of psychological and physical symptoms of the surviving patients with breast cancer improving their quality of life.

Objective: The aim of this systematic review was to identify the evidence available in the Latin America and Spain scientific literature about the effective psychological intervention for breast cancer, between 2006 and 2016. Method: The databases included were BVS and PsycNetAPA, using the following descriptors: breast cancer, psychological interventions, psychooncology, and psychotherapy, in English and Spanish. The search was also done in Google Academics with the descriptors: “Cáncer de mama and intervenciones psicológicas”, in Spanish. Results: Only 17 articles were found that met our inclusion criteria and showed the evidence of psychological intervention for breast cancer patient in Latin América and Spain between 2006 and 2016. The theoretical model most frequently applied was CBT, followed by other types of psychosocial intervention. Nevertheless, other novel models to these population were also found. Limits in the methodology of these investigation were identified, which hinders the generalization of results. Conclusions: More research with according methodology design to prove the efficacy of a psychological intervention for breast cancer patient, is very much needed in Latin America and Spain.

Introduction: Breast cancer is the malignant tumour which affects more women. A diagnosis that includes the word ‘cancer’ has a negative connotation. This disease impacts on the family dynamics, and communication is one of the dimensions which are frequently studied in the intervention with families. Objective: The aim of this research is to analyse the communication between a mother and her children, husband or partner, developed after a breast cancer diagnosis. Method: A mixed method research design was used. The sample was composed of twelve women diagnosed with breast cancer, aged between 43 and 55. The instruments used in this study were the Family Communication Scale (FCS) and a semi-structured interview. Results: The outcomes show that intra-family communication varies according to the age of the children, being lower when they are under 15 years old. Conclusions: Effective communication in the family is not the cure for any disease, but it improves the way the family as a whole deals with it and it contributes to eliminate stress situations and discomfort in the woman.

Objetivo: Analizar la validez y la fiabilidad de la adaptación de la escala Perceived Personal Control (PPC) en el contexto español para pacientes portadoras de mutación en los genes BRCA1/2 responsables del cáncer de mama y ovario hereditario (CMOH). Método: Adaptación transcultural y validación de la escala Perceived Personal Control (PPC) desarrollada por Shiloh y colaboraradores mediante traducción, retrotraducción y validación a través de un análisis factorial exploratorio con rotación Oblimin en una muestra de 176 mujeres portadoras de genes BRCA 1/2 para CMOH. Resultados: La versión española de la PPC reduce a seis los nueve ítems de la escala original, dado que esta estructura es la que ofrece una solución factorial más satisfactoria. El análisis factorial mostró un solo factor que explica el 51,07% de la varianza, en el que todos los ítems tenían cargas factoriales por encima de 0,4. El coeficiente α de Cronbach fue de 0,84 para el conjunto de la escala, la cual permite obtener valores que oscilan entre 0 (bajo grado de percepción de control) y 2 (alto grado de percepción de control). Conclusiones: La adaptación española de la Escala de Percepción de Control (PPC6) posee propiedades psicométricas satisfactorias en la versión de 6 ítems con un solo factor, por lo que su utilización en contexto español Consejo Genético para cáncer hereditario parece adecuada.

Introducción: la calidad de vida es un concepto fuertemente condicionado por la integridad física e imagen corporal de un individuo. La reconstrucción mamaria se ha convertido en una opción idónea para las mujeres diagnosticadas de cáncer de mama. Es un paso esencial en la recuperación de las secuelas físicas y psíquicas que produce el tratamiento del cáncer de mama en la mujer, y debe ser analizada de forma individual. Procedimientos reconstructivos: existen diversas técnicas reconstructivas, que deben satisfacer el objetivo estético de recuperar el contorno de la mama, así como una mejoría funcional para la mujer con el fin de mejorar su calidad de vida. La reconstrucción puede realizarse mediante implantes, mediante tejido del propio cuerpo o reconstrucción mediante técnicas mixtas. Por sus características, el colgajo DIEP se ha convertido en la técnica quirúrgica más fisiológica y de elección para la reconstrucción mamaria. Discusión: las pacientes tienen derecho a una valoración exhaustiva y rigurosa, por parte de su cirujano plástico, de la técnica más indicada para su intervención de reconstrucción mamaria. El hecho de que sólo el hecho de que sólo el 30% de las mujeres que se someten a una mastectomía lleven a cabo la reconstrucción, se debe principalmente a una falta de información, afectando claramente a la calidad de vida de las pacientes. Conclusión: la reconstrucción mamaria no debería considerarse un paso posterior, si no una parte imprescindible para un tratamiento integral.

Objective: To know the current state of the effectiveness of artistic and creative therapies (ACTs) over psychological distress of women with breast cancer and to delimit it according to the type of CT and the therapeutic moment of the patient. Method: The search was performed using Pubmed, Web of Science, Psychinfo and Google Academic databases for the period 2010-2016. Results: nine controlled RTC and quasi-experimental studies (2 of Dance movement therapy, 1 of Art Therapy, 1 of Music therapy and 5 of Musical Listening) were obtained, including 680 participants. Methodological quality, risk of bias and measures of therapeutic efficacy are discussed. Conclusions: ACT scans seem to be short-term complementary aid during the treatment phase in breast cancer. However, more trials are needed to clarify the mechanisms underlying the improvement as well as to delimit the effectiveness of each modality.

Objective: Most studies on psychological intervention benefits have focused on the effects after several therapeutic sessions. The objective of this research was to describe the patients’ emotional state, and establish the possible immediate emotional effects of an individual psycho-oncological session.Method: The emotional state of 30 women with breast cancer was assessed with an ad- hoc tool that included items from the Distress Thermometer for Patients at the beginning and at the end of a psychological support session. During the session, issues that worried patients were addressed through an approach based on counselling and positive psychology.Results: The mean of emotional distress experienced during the previous week was 6.4 on the emotional thermometer, and more than half of the women had experienced multiple emotional problems during this period. After the session, the intensity of certain emotions decreased significantly in relation to the beginning of it, and the emotional distress compared to the previous week also decreased. In addition, more than half the sample experienced positive emotional states. Improvements were related to neither medical condition nor the number of previous psycho-oncological sessions.Conclusions: In light of the immediate distress relief, it is concluded that it is appropriate to refer all patients to the psych-oncology service, at least for a psychological assessment visit.

Objective: To evaluate the relationship between Perceived Psychological well-being, Optimism and Resilience in women survivors of breast cancer.Method: The sample was composed of 30 women diagnosed with breast cancer who were undergoing adjuvant treatment in Oncology Units General Hospital of Jerez de la Frontera and the University Hospital Puerta del Mar (Cadiz). The average age was 47.47 years (SD = 6,356) and the average of months from diagnosis of the disease was 9.93 (SD = 8,541). Scale of Psychological Well-being administered Perceived Ryff (1989), the Life Orientation Test Revised (LOT-R) Scheier, Carver and Bridges (1994) and the Scale of Resilience Wagnild and Young (1993) to assess psychological well-being the dispositional optimism and resilience respectivelyResults: Descriptive analyzes show that women diagnosed with breast cancer have levels of psychological well-being, dispositional optimism and higher than the average values provided by Resilience scales. On the other hand, regression analyses revealed that only some of the dimensions of resilience allowed to explain and predict some dimensions of psychological wellbeing, not dispositional optimism.Conclusions: The results seem to confirm the idea that if the capacity of women with breast moderate negative affect generated by the diagnosis itself and the consequences of their cancer treatment works and to promote their adaptation to the new situation we can contribute to increasing psychological well-being.

Objetivo: Este estudio se dirige a identificar, describir y analizar los posibles cambios en la experiencia vivida del cuerpo propio y los cambios en las relaciones interpersonales de las mujeres con cáncer de mama derivados de su participación en encuentros grupales de danzaterapia.Método: Ésta es una investigación cualitativa de orientación fenomenológica con utilización del método de danzaterapia de María Fux para las vivencias grupales. Se describen aquí las experiencias de ocho encuentros y se aporta un análisis de las descripciones basándonos principalmente en Merleau-Ponty y María Fux.Resultados: Las participantes han podido expresar dolores y tristeza por las situaciones que les han ocasionado el cáncer de mama y sus entornos relacionales; ellas han podido experimentar momentos de creación y entrega a las sensaciones rítmicas del cuerpo y del entorno vivencial con distintas emociones vividas por separado y conjuntamente; ellas han experimentado recuerdos y sensaciones de su niñez y adolescencia; finalmente, ellas han redescubierto su cuerpo sensible, mediante resignificaciones del cuerpo marcado por la ausencia de la mama y mediante sentimientos de mayor aceptación e integración de lo vivido en nuevas gestalts.Conclusiones: Este proyecto todavía está en marcha, pero ya es posible concluir resultados: las vivencias aportadas en la danzaterapia han permitido a estas mujeres tener experiencias de integración y bienestar, al igual que cambios positivos de percepción de su corporeidad y en cuanto a la forma de ser y estar en el mundo y con los otros sujetos, experimentando, así, el cuerpo de una manera nueva y distinta.

Objective: To know the experiences, expectations and perceived satisfaction in a group of breast cancer women who had undergone immediate breast reconstruction after mastectomy. Material and Methods: 20 women took part in a qualitative study. 4 women were assessed through an in-depth interview and the other 16 women were divided into two focus groups (8 each). The recorded interview and focus group discussions were transcribed and analyzed using “framework analysis”. Results: Five areas were explored: 1) Impact of cancer diagnosis, 2) Information received about the surgery, 3) Post-surgery experiences, 4) Fulfilment of expectations and 5) Satisfaction with the decision. Cancer diagnosis was overwhelming for most of the women. For women, information received was considered good and sufficient, but the study shows that after the post-surgical period, more complaints related to pain and other complications appear. The expectations about immediate breast reconstruction were linked with the final aesthetic results. Some women would choose to do the same procedure again despite the complications but some other women would not choose the same option because of the complications and they regret their decision. Conclusion: There is no clear and unanimous position among women about whether they would go through immediate breast reconstruction again and whether the surgery has fulfilled their expectations. The patients need complete, unbiased and appropriate information in order to be able to make an informed and shared decision. It is for that reason that we think it is necessary to devise patient decision aids which could facilitate the information delivering process to patients.

Objective: The purpose of this study is to describe the relationship between posttraumatic growth (CPT) and Quality of Life (QoL) in a sample of women survivors of Breast Cancer, over three years after completion of treatment. Method: A sample of outpatients (N=156) completed the Argentinean adaptation of Posttraumatic Growth Inventory (PTGI) and Scale for the evaluation of chronic disease (FACIT-B). A longitudinal descriptive correlational study with a non-experimental design was used. The psychometric properties of the questionnaires used in the sample were evaluated. Pearson correlation was used to relate the two constructs and the Friedman test for assessing the existence of significant differences in the values of CPT and CV over time. Results: The median age was 60,7 (DT=9,0). The 54,5%, 50% and 53.9% of participants had moderate to high CPT in the first, second and third visits respectively. There were no significant differences among the three views (PTGI Total p=0,153). The highest levels of CPT are associated with better CV. This relationship is also maintained in the first visit (r =0,227; p=0,022), second (r=0,317; p=0,001) and third (r=0,304; p=0,002). CV was deficient and decreased over time. Conclusions: The CPT remained stable. It was found positive relations between the CPT and the CV, which were maintained over time.

espanol Objetivos: Estudiamos el valor predictivo que tiene sobre la calidad de vida relacionada con la salud (CVRS) de las mujeres con cancer de mama en tratamiento, las variables sociodemograficas, clinicas (tipo de tratamiento) y psicologicas tales como los esquemas desadaptativos tempranos (EDT), las creencias y percepciones sobre la enfermedad y las estrategias de afrontamiento utilizadas. Metodos: Realizamos un estudio descriptivo, correlacional, de subtipo predictivo. Incorporamos una muestra de 228 pacientes adultas con cancer de mama en tratamiento ambulatorio del sistema publico asistencial. Se realizaron entrevistas en las que se aplico un formulario de datos socio demograficos y clinicos, el Cuestionario FACT-B, el Test de Esquemas Tempranos (YSQ-CEY-VR), el Cuestionario de percepcion de enfermedad (IPQ-R) y la escala Brief COPE. Se realizaron analisis descriptivos, de estadistica inferencial, regresion lineal multiple y regresion jerarquica. Resultados: Los diferentes tipos de tratamientos onco-especificos no registraron diferencias significativas en la CVRS, sin embargo las mujeres con mastectomia sin reconstruccion valoraron sensiblemente descendida la CVRS con respecto a los otros tipos de cirugia. Las variables que mostraron relacion estadistica en explicar el FACT-B global fueron los esquemas de Deprivacion emocional, Abandono, Imperfeccion/verguenza, Vulnerabilidad, Apego, Dominio 3 y Dominio 4. Asi tambien todos los valores del IPQ-R guardaron relacion significativa con la CVRS global. Las creencias y percepcion global de la enfermedad (IPQ-R) y los dominios de los EDT, tienen valor predictivo sobre CVRS. Conclusiones: las mujeres con una percepcion amenazante de la enfermedad tienden a activar pautas de funcionamiento caracterizadas por la desconexion emocional, el aislamiento, la verguenza; u otras caracterizadas por la dependencia, la perdida de autonomia y confianza en si mismas. Las creencias y percepciones sobre la enfermedad activan necesidades y pautas de respuestas cognitivas, emocionales y relacionales (esquemas desadaptativos) siendo ambas variables predictivas del mayor o menor ajuste y percepcion global de bienestar. EnglishObjective: We studied the predictive value of sociodemographic, clinical (type of treatment) and psychological variables such as early maladaptive schemas, beliefs and perceptions about the disease and the coping strategies used by patients on health-related quality of life (HRQOL) of women with breast cancer under treatment. Method: We conducted a descriptive and correlational predictive study. Two hundred twenty-eight adult breast cancer patients under outpatient treatment from the public healthcare system were included. Interviews were conducted and sociodemographic and clinical data were collected. Several questionnaires were applied: FACT-B questionnaire, early maladaptive schemas form, Brief Illness Perception Questionnaire (IPQ-R) and COPE dispositional questionnaires. Results: Descriptive and inferential analysis, multiple linear regression, and hierarchical regression were performed. Oncology treatments subtypes did not influence HRQoL. However, women who had undergone a mastectomy without reconstruction showed significantly decreased HRQoL values. Variables that showed a statistical relationship in explaining the global FACT-B were: schemes of emotional deprivation, abandonment, defectiveness/ shame, vulnerability, attachment, domain 3 and domain 4. Also, all the IPQ-R values were significantly related to the Global HRQoL. The beliefs and global perception of the disease (IPQ-R) and EDT domains, have shown predictive value on HRQL. In conclusion, women with a threatening perception of the disease tended to activate functional patterns characterized by emotional disconnection, isolation, and shame; or others characterized by dependency, loss of autonomy and self-confidence. Beliefs and perceptions about the disease activate needs and patterns of cognitive, emotional and relational responses (maladaptive schemas), being both predictive variables of greater or lesser adjustment and global perception of self well-being.

Metodo: El FSFI se aplico a 272 mujeres mexicanas sexualmente activas con diagnostico reciente de cancer de mama que no habian iniciado tratamiento oncologico sistemico. Resultados: El modelo de seis factores de la FSFI se valido por analisis factorial confirmatorio y el inventario y sus factores tienen una fiabilidad de consistencia interna adecuada. Conclusiones: Este estudio proporciona suficiente evidencia sobre la confiabilidad y la estructura factorial del cuestionario FSFI en el contexto de la practica clinica del cancer de mama en Mexico. English Background: Female Sexual Dysfunction (FSD) occurs frequently in women with breast cancer due to oncologic treatments. It is essential to have a validated instrument to diagnose and quantify FSD in this population accurately. Objective: To validate the Female Sexual Function Index (FSFI). Method: The FSFI was applied to 272 sexually active Mexican women with recent diagnosis of breast cancer who had not initiated systemic cancer treatment. Results: The FSFI six-factor model is valid by confirmatory factor analysis, and the inventory and its factors have adequate internal consistency reliability. Conclusions: This study provides enough evidence about the reliability and factor structure of the FSFI questionnaire in the context of breast cancer clinical practice in Mexico.

Introduction: Even when the oncologic treatments are completed, breast cancer patients still may experience physical and psychological distress.Aim: The purpose of this study is to evaluate the effectiveness of a music therapy treatment on mood and quality of life of breast cancer survivors.Method: Eleven free-disease women participated in a music therapy program. Treatment consisted of 8 weekly group music therapy sessions, 90 minutes/session. Participants completed the following instruments before and after the intervention (pre/post): HADS (Hospital Anxiety and Depression Scale), POMS-A (Profile of Mood States), Quality of Life EORTC-QLQ-30 (European Organization for Research and Treatment of Cancer-Quality of Life Core Questionnaire-30). A 3 item Numerical Rating Scale (NRS) for Anxiety, Sadness and Physical Distress was administered at the beginning and ending of each session (pre/post session). An Evaluation Questionnaire was administered at the end of the program.Results: The POMS-A results showed a statistical significant improvement in the Total Mood Disturbance Scores and in the Tension Subscale. No significant changes were found in the Anxiety and Depression HADS subscales. Quality of Life only significantly improved in the Emotional Functioning EORTC Subscale. A significant improvement has been found in the 3 NRS items. The Evaluation Questionnaire shows that participants have perceived this type of intervention as very useful and at a very satisfactory level.

Objetivo: Evaluar la respuesta de ansiedad en tres sistemas (cognitiva, fisiológica y motora) en tres grupos de supervivientes al cáncer de mama y un grupo control para encontrar diferencias entre los grupos en los sistemas de ansiedad.Método: 25 supervivientes al cáncer de mama fueron distribuidas en tres grupos según el tiempo desde que finalizaron el tratamiento (< de 5 años; > de 5 y < 10 años; > de 10 años) y se incluyó un grupo control de 10 participantes de la población general sin historia de cáncer. Todas las participantes en el estudio completaron el Inventario de Situaciones y Respuestas de Ansiedad (ISRA).Resultados: Los tres grupos de supervivientes obtuvieron valores más elevados que el grupo control en los tres sistemas de ansiedad sin llegar a ser significativos. En conjunto los resultados muestran valores elevados en ansiedad fisiológica y motora en los dos grupos más cercanos al final del tratamiento (< de 5 años; > de 5 y < 10 años). Además se observó que las puntuaciones en ansiedad cognitiva y motora aumentan en el segundo grupo comparado con el primero, disminuyendo posteriormente hasta obtener valores similares a los obtenidos por la población general.Conclusiones: En general las supervivientes al cáncer de mama muestran una buena adaptación psicológica en términos de ansiedad, aunque los datos indican que es necesario tener en cuenta el tipo de ansiedad que puede estar más afectado en cada momento temporal.

Objective: The aim of this study was to show the effectiveness of cognitive behavioral therapy in a sample of patients with breast cancer, in order to reduce levels of anxiety and depression, and develop positive coping skills to improve the perception of quality of life.Method: The study subject were a sample of fifteen patients that had accepted to collaborate in the study, they signed an informed consent. They were receiving treatment in the area of oncology at Hospital Central “Dr. Ignacio Morones Prieto” of the state of San Luis Potosi, Mexico. The study began with one group of individual intervention, in a convenience sampling; with a measure of pre and posttest. We used three standardized test to measure the progress of the intervention. We used, the HADS (Hospital Anxiety and Depression Scale), Whoqol Bref test (quality of life) and CAEPO (stress coping questionnaire for cancer patients). We used individual cognitive behavioral therapy an average of ten sessions for person, determined on the basis of a cognitive behavioral treatment manual, previously developed.Results: There have been certain statistically and clinically significant differences, we found among patients who participated in the study. In quality of life we observed differences on physical health domain, t=-2.75, p=0.016 and social relationships t=-2.17, p=0.048, We also found differences, in the anxiety subscale t=2.35, p=0.027 and overall HAD scale, t=2.81, p=0.014. With coping we observed differences, on denial sub-scale, W=74, p=0.048. It is concluded that cognitive behavioral therapy proved efficacy in the study sample.

Objetive: The aims of this research are to study the coping strategies in women with doubtful results after mammography screening for breast cancer who require repeat testing at 6 months, and analyse the predictive value of coping strategies for anxiety and depression at both times of measurement. Methodology: One hundred and five women (mean age=59.9 años, Sx=6.02) with BI-RADS 3 findings in a mammography screening were evaluated in coping (COPE), anxiety and depression (HADS), and 72 of these women were assessed at 6 months after a follow-up mammogram. Conclusions: Coping strategies most used were the active and problem-focused, and the avoidant strategies were the least used. In the second measurement time decreased the use of active coping (p=0.000), planning (p=0.002), positive reinterpretation and growth (p=0.03) and refrain coping (p=0.029). Anxiety and depression were normal in both assessments. Coping strategies problem-focused are more beneficial than emotion-focused, although the correlations between coping and anxiety/depression are modest. Screening mammography does not cause distress and requires few coping resources for participants. Campaigns for breast cancer screening should emphasize these results.

49. Use of internet to assess psychological distress in breast cancer patients

Objective: The object of this study is to determine the prevalence of distress symptoms and coping strategy among a large sample of Spanish-speaking breast cancer patients recruited on-line. Methods: We designed a cross-sectional study with breast cancer survivors (n=434) that were accrued via Internet (http://www.cancermama.org). Participants completed a demographic and medical questionnaire, and they were also evaluated using self-reporting measures of psychological distress and adjustment to cancer; the 14-item Hospital Anxiety and Depression Scale (HADS) and the 29-item version of the Mental Adjustment to Cancer Scale (Mini-MAC), respectively. Results: Global HAD as a measure of distress on-line is reliable in our study. The overall prevalence rate of distress for our sample was 50%, with no differences for socio-demographics or medical status variables. Concerning to anxiety punctuation 66.2% were clinical cases and 34.5% reached the clinical level for depression. These results were associated with Mini-MAC scales as expected. Conclusions: These results offer support for the use of Internet as valid tool for the psychological distress assessment in patients with breast cancer Spanish-speaking patients. Internet brings us the possibility to identify high-risk patients through psychological screening in order to provide early interventions, and Global HAD punctuation may be an accurate tool. However more studies are needed to analyze internet illness consulting situation as an anxious provoking space.
