Defective mucociliary transport contributes to infections and inflammation that destroy cystic fibrosis (CF) lungs. To better understand mechanisms that impair mucociliary transport, we tracked insufflated microdisks with X-ray computed tomography in spontaneously breathing CF pigs. Aerosolized saline increased microdisk movement toward the larynx. Surprisingly, forward progression was repeatedly interrupted as microdisks abruptly recoiled. Thus, although saline increased motion, it had minimal effects on microdisk clearance. To test if abnormally elastic mucus strands were responsible, we aerosolized Tris-carboxyethyl phosphine (TCEP), a reducing agent that breaks disulfide bonds linking mucins. TCEP largely eliminated retrograde movement and increased microdisk clearance. Ex vivo studies confirmed that TCEP broke CF mucus strands, unleashing them from submucosal gland ducts, and cilia carried them rostrally. These findings emphasize the role of abnormally elastic submucosal gland mucus strands in impairing CF mucociliary clearance and suggest that disrupting mucus strands may improve mucociliary clearance in CF. Submucosal glands (SMGs) protect lungs but can also contribute to disease. For example, in cystic fibrosis (CF), SMGs produce abnormal mucus that disrupts mucociliary transport. CF is an ion transport disease, yet knowledge of the ion transporters expressed by SMG acini, which produce mucus, and SMG ducts that carry it to the airway lumen is limited. Therefore, we isolated SMGs from newborn pigs and used single-cell messenger RNA sequencing, immunohistochemistry, and in situ hybridization to identify cell types, gene expression, and spatial distribution. Cell types and transcript levels were the same in non-CF and CF SMGs, suggesting that loss of epithelial anion secretion rather than an intrinsic cell defect causes CF mucus abnormalities. Gene signatures of acinar mucous and acinar serous cells revealed specialized functions in producing mucins and antimicrobials, respectively. However, surprisingly, these two cell types expressed the same ion transporters and neurohumoral receptors, suggesting the importance of balancing mucin and liquid secretion to produce optimal mucus properties. SMG duct cell transcripts suggest that they secrete HCO3 - and Cl-, and thus have some similarity to pancreatic ducts that are also defective in CF. These and additional findings suggest the functions of the SMG acinus and duct and provide a baseline for understanding how environmental and genetic challenges impact their contribution to lung disease. The pulmonary airways are continuously exposed to bacteria. As a first line of defense against infection, the airway surface liquid (ASL) contains a complex mixture of antimicrobial factors that kill inhaled and aspirated bacteria. The composition of ASL is critical for antimicrobial effectiveness. For example, in cystic fibrosis an abnormally acidic ASL inhibits antimicrobial activity. Here, we tested the effect of pH on the activity of an ASL defensin, human p-defensin-3 (hBD-3), and the cathelicidin-related peptide, LL-37. We found that reducing pH from 8.0 to 6.8 reduced the ability of both peptides to kill Staphylococcus aureus. An acidic pH also attenuated LL-37 killing of Pseudomonas aeruginosa. In addition, we discovered synergism between hBD-3 and LL-37 in killing S. aureus. LL-37 and lysozyme were also synergistic. Importantly, an acidic pH reduced the synergistic effects of combinations of ASL antibacterials. These results indicate that an acidic pH reduces the activity of individual ASL antimicrobials, impairs synergism between them, and thus may disrupt an important airway host defense mechanism.
