IntroductionTo determine the threshold of umbilical cord blood procalcitonin for early-onset neonatal infection diagnosis.MethodThis prospective study was conducted on 126 neonates in the neonatal care unit of Hue University of Medicine and Pharmacy Hospital, Vietnam, from June 01, 2023 to August 31, 2024. All neonates showed signs at birth or risk factors for early-onset infection (EOI) and were divided into two groups: EOI group and non-EOI group. Umbilical cord blood samples were collected for procalcitonin analysis immediately after birth.ResultsThe median procalcitonin (PCT) levels in umbilical cord blood were significantly higher in the EOI group (0.154 ng/ml [0.092-0.197]) compared to the non-EOI group (0.097 ng/ml [0.082-0.134]; p < 0.001). Receiver operating characteristic (ROC) curve determined the optimal threshold value of PCT of 0.142 ng/ml with an AUC 0.751 (95% CI: 0.661-0.841, pConclusionsUmbilical cord blood PCT concentration were elevated in neonates with EOI. PCT could be a valuable marker for the early diagnosis of EOI.

ObjectiveTo investigate the correlation between oxygen saturation index (OSI) and oxygenation index (OI) for evaluating the blood oxygenation status in neonates with respiratory failure requiring mechanical ventilation support and to assess the predictive capability of OSI in determining clinically relevant OI cutoffs.MethodsA prospective study was conducted on neonates who received invasive mechanical ventilation at the neonatal intensive care unit of tertiary hospital in Vietnam. Bland-Altman analysis was utilized to evaluate the agreement between OSI and OI.ResultsA total of 123 neonates, including both term and preterm infants, were included in the study. A high agreement rate of 94.3% within the 95% limits of agreement (between OI and OSI), with a narrow similarity value of 3.3 (95% CI: -5.1 to 11.8) and high correlation coefficient (r = 0.791, p15 was determined to be 7.45, with a sensitivity of 100% and a specificity of 87.4% (AUC 0.955; 95% CI: 0.922-0.989, p < 0.05). Similarly, an OSI cutoff value of 9.9 corresponded to an OI of 25, displaying a sensitivity of 100% and specificity of 87.4% (AUC 0.92). The receiver operating characteristic (ROC) curves for OSI exhibited statistically significant results (p < 0.05).ConclusionThe findings demonstrate a strong correlation between OSI and OI in neonates with respiratory failure. Furthermore, OSI, as a non-invasive method, can serve as a substitute for OI to evaluate the severity of hypoxic respiratory failure and lung injury in neonates.

Physics informed neural networks have been gaining popularity due to their unique ability to incorporate physics laws into data-driven models, ensuring that the predictions are not only consistent with empirical data but also align with domain-specific knowledge in the form of physics equations. The integration of physics principles enables the method to require less data while maintaining the robustness of deep learning in modelling complex dynamical systems. However, current PINN frameworks are not sufficiently mature for real-world ODE systems, especially those with extreme multi-scale behavior such as mosquito population dynamical modelling. In this research, we propose a PINN framework with several improvements for forward and inverse problems for ODE systems with a case study application in modelling the dynamics of mosquito populations. The framework tackles the gradient imbalance and stiff problems posed by mosquito ordinary differential equations. The method offers a simple but effective way to resolve the time causality issue in PINNs by gradually expanding the training time domain until it covers entire domain of interest. As part of a robust evaluation, we conduct experiments using simulated data to evaluate the effectiveness of the approach. Preliminary results indicate that physics-informed machine learning holds significant potential for advancing the study of ecological systems. [ABSTRACT FROM AUTHOR]

BACKGROUND:Venous thromboembolism (VTE) associated with surgery can cause serious comorbidities or death and imposes a substantial economic burden to society. The study examined VTE cases after surgery to determined how this condition imposed an economic burden on patients based on the national health insurance reimbursement database. Methods: This retrospective analysis adopted the public payer's perspective. The direct medical cost was estimated using data from the national claims database of Vietnam from Jan 1, 2017 to Sep 31, 2018. Adult patients who underwent surgeries were recruited for the study. Patients with a diagnostic code of up to 90 days after surgery were considered VTE cases with the outcome measure being the surgery-related costs within 90 days. RESULTS:The 90-day cost of VTE patients was found to be US$2,939. The rate of readmission increased by 5.4 times, the rate of outpatient visits increased by 1.8 times and total costs over 90 days in patients with VTE undergoing surgery increased by 2.2 times. Estimation using propensity score matching method showed that an increase of US$1,019 in the 90-day cost of VTE patients. CONCLUSION:The VTE-related costs can be used to assess the potential economic benefit and cost-savings from prevention efforts.
