Urbanization could potentially modify Aedes albopictus' ecology by changing the dynamics of the species, and affecting their breeding sites due to environmental changes, and thus contribute to dengue outbreaks. Thus, this study was conducted to evaluate the biting rhythm, fecundity and longevity of adult female Ae. albopictus in relation to urbanization strata; urban, suburban and rural areas in Penang Island, Malaysia. The experiments were done in comparison to a laboratory strain. Twenty-four hours biting activity of all the mosquito strains showed a clear bimodal biting activity, with morning and evening twilight peaks. The interaction effect between biting time and mosquito strains was not significant. Meanwhile, differences in fecundity among mosquito strains were statistically significant (F(3,442) = 10.559, P < 0.05) with urban areas having higher mean number of eggs (mean = 107.69, standard error = 3.98) than suburban (mean = 94.48, standard error = 5.18), and rural areas (mean = 72.52, standard error = 3.87). Longevity of adult females were significantly higher (F(3,441) = 31.259, P < 0.05) for mosquito strains from urban areas compared to the other strains. These findings would provide crucial information for the planning of control programs in Malaysia, particularly Penang.

In the present study, ion imprinted polymer monoliths (IIPMs) were developed to overcome the limitations of ion imprinted polymer particles (IIPPs) used for the removal of Hg(II) ions from waste water samples. The adsorbents preparation, characterization and Hg(II) removal were very well reported. The IIPMs on porogen optimization was prepared using the molding technique with Hg(II) as a template ion, [2-(methacryloyloxy)ethyl]trimethylammonium cysteine (MAETC) as ligand, methacrylic acid (MAA) as functional monomer, ethylene glycol dimethacrylamide (EGDMA) as cross-linker, benzoyl peroxide as an initiator and methanol and acetonitrile as porogen in the polypropylene tube (drinking straw) as mold. The IIPMs prepared with higher volumes of porogen were indicated to have a good adsorption rate for the Hg(II) removal along with good water permeability and larger porosity as compared to a lower volume of porogen. The IIPMs prepared using the binary porogen were able to improve the porosity and surface area of the monolithic polymers as compared to the single porogen added IIPMs. Finally, we indicate from our analysis that the IIPM having the efficient capacity for the Hg(II) ions is easy to prepare, and has higher water permeability along with high porosity and high adsorption capacity and all these factors making it one of the suitable adsorbent for the successful removal of Hg(II) ions.

The use of rice husks (RH) to reinforce polymers in biocomposites are increasing tremendously. However, the incompatibility between the hydrophilic RH fibers and the hydrophobic thermoplastic matrices leads to unsatisfactory biocomposites. Surface modification of the fiber surface was carried out to improve the adhesion between fiber and matrix. In this study, the effect of surface modification of RH via alkali, acid and ultraviolet-ozonolysis (UV/O3) treatments on the properties of composites recycled high density polyethylene (rHDPE) composites was investigated. The untreated and treated RH were characterized by Fourier Transform Infrared (FTIR) and Scanning Electron Microscope (SEM). The composites containing 30 wt% of RH (treated and untreated) were then prepared via extrusion and followed by compression molding. As compared to untreated RH, all surface treated RH exhibited rougher surface and showed improved adhesion with rHDPE matrix. Tensile strength of UV/O3-treated RH composites showed an optimum result at 18.37 MPa which improved about 5% in comparison to the composites filled with untreated RH. UV/O3 treatment promotes shorter processing time and lesser raw material waste during treatment process where this is beneficial for commercialization in the future developments of wood plastic composites (WPCs). Therefore, UV/O3 treatment can be served as an alternative new method to modify RH surface in order to improve the adhesion between hydrophilic RH fibre and hydrophobic rHDPE polymer matrix.

A wireless sensor network (WSN) comprises small sensor nodes with limited energy capabilities. The power constraints of WSNs necessitate efficient energy utilization to extend the overall network lifetime of these networks. We propose a distance-based and low-energy adaptive clustering (DISCPLN) protocol to streamline the green issue of efficient energy utilization in WSNs. We also enhance our proposed protocol into the multi-hop-DISCPLN protocol to increase the lifetime of the network in terms of high throughput with minimum delay time and packet loss. We also propose the mobile-DISCPLN protocol to maintain the stability of the network. The modelling and comparison of these protocols with their corresponding benchmarks exhibit promising results.

The HPTN 052 trial confirmed that antiretroviral therapy (ART) can nearly eliminate HIV transmission from successfully treated HIV-infected individuals within couples. Here, we present the mathematical modeling used to inform the design and monitoring of a new trial aiming to test whether widespread provision of ART is feasible and can substantially reduce population-level HIV incidence.The HPTN 071 (PopART) trial is a three-arm cluster-randomized trial of 21 large population clusters in Zambia and South Africa, starting in 2013. A combination prevention package including home-based voluntary testing and counseling, and ART for HIV positive individuals, will be delivered in arms A and B, with ART offered universally in arm A and according to national guidelines in arm B. Arm C will be the control arm. The primary endpoint is the cumulative three-year HIV incidence. We developed a mathematical model of heterosexual HIV transmission, informed by recent data on HIV-1 natural history. We focused on realistically modeling the intervention package. Parameters were calibrated to data previously collected in these communities and national surveillance data. We predict that, if targets are reached, HIV incidence over three years will drop by >60% in arm A and >25% in arm B, relative to arm C. The considerable uncertainty in the predicted reduction in incidence justifies the need for a trial. The main drivers of this uncertainty are possible community-level behavioral changes associated with the intervention, uptake of testing and treatment, as well as ART retention and adherence.The HPTN 071 (PopART) trial intervention could reduce HIV population-level incidence by >60% over three years. This intervention could serve as a paradigm for national or supra-national implementation. Our analysis highlights the role mathematical modeling can play in trial development and monitoring, and more widely in evaluating the impact of treatment as prevention.

This cross sectional survey was conducted to determine the support in making Penang UNESCO World Heritage Site (GTWHS) smoke free and to determine the influence of tolerance towards smoking on this support. This is the first phase in making Penang, Malaysia a smoke free state. A multistage sampling process was done to select a sample of respondents to represent the population of GTWHS. Attitude towards smoking was assessed using tolerance as a proxy. A total of 3,268 members of the community participated in the survey. A big majority (n = 2969; 90.9%) of the respondents supported the initiative. Support was lowest among the owners and residents/tenants, higher age groups, the Chinese, men, respondents who had poor knowledge of the places gazetted as smoke free, and respondents with poor knowledge of the health effects on smokers and on passive smokers. The odds (both adjusted and unadjusted) of not supporting the initiative was high among those tolerant to smoking in public areas. Tolerance towards smoking was associated with 80.3% risk of non-support in the respondents who were tolerant to smoking and a 57.2% risk in the population. Health promotion and education concerning the harm of tobacco smoke in Malaysia, which has mainly targeted smokers, must change. Health education concerning the risks of second hand smoke must also be given to non-smokers and efforts should be made to denormalize smoking.

The utility of T-cell based interferon-gamma release assays for the diagnosis of latent tuberculosis infection remains unclear in settings with a high burden of tuberculosis.To determine risk factors associated with positive QuantiFERON-TB Gold In-Tube (QFT-GIT) and tuberculin skin test (TST) results and the level of agreement between the tests; to explore the hypotheses that positivity in QFT-GIT is more related to recent infection and less affected by HIV than the TST.Adult household contacts of tuberculosis patients were invited to participate in a cross-sectional study across 24 communities in Zambia and South Africa. HIV, QFT-GIT and TST tests were done. A questionnaire was used to assess risk factors.A total of 2,220 contacts were seen. 1,803 individuals had interpretable results for both tests, 1,147 (63.6%) were QFT-GIT positive while 725 (40.2%) were TST positive. Agreement between the tests was low (kappa = 0.24). QFT-GIT and TST results were associated with increasing age (adjusted OR [aOR] for each 10 year increase for QFT-GIT 1.15; 95% CI: 1.06-1.25, and for TST aOR: 1.10; 95% CI 1.01-1.20). HIV positivity was less common among those with positive results on QFT-GIT (aOR: 0.51; 95% CI: 0.39-0.67) and TST (aOR: 0.61; 95% CI: 0.46-0.82). Smear positivity of the index case was associated with QFT-GIT (aOR: 1.25; 95% CI: 0.90-1.74) and TST (aOR: 1.39; 95% CI: 0.98-1.98) results. We found little evidence in our data to support our hypotheses.QFT-GIT may not be more sensitive than the TST to detect risk factors associated with tuberculous infection. We found little evidence to support the hypotheses that positivity in QFT-GIT is more related to recent infection and less affected by HIV than the TST.

Although historically tuberculosis (TB) has been associated with poverty, few analytical studies from developing countries have tried to: 1. assess the relative impact of poverty on TB after the emergence of HIV; 2. explore the causal mechanism underlying this association; and 3. estimate how many cases of TB could be prevented by improving household socioeconomic position (SEP).We undertook a case-control study nested within a population-based TB and HIV prevalence survey conducted in 2005-2006 in two Zambian communities. Cases were defined as persons (15+ years of age) culture positive for M. tuberculosis. Controls were randomly drawn from the TB-free participants enrolled in the prevalence survey. We developed a composite index of household SEP combining variables accounting for four different domains of household SEP. The analysis of the mediation pathway between household SEP and TB was driven by a pre-defined conceptual framework. Adjusted Population Attributable Fractions (aPAF) were estimated. Prevalent TB was significantly associated with lower household SEP [aOR = 6.2, 95%CI: 2.0-19.2 and aOR = 3.4, 95%CI: 1.8-7.6 respectively for low and medium household SEP compared to high]. Other risk factors for prevalent TB included having a diet poor in proteins [aOR = 3.1, 95%CI: 1.1-8.7], being HIV positive [aOR = 3.1, 95%CI: 1.7-5.8], not BCG vaccinated [aOR = 7.7, 95%CI: 2.8-20.8], and having a history of migration [aOR = 5.2, 95%CI: 2.7-10.2]. These associations were not confounded by household SEP. The association between household SEP and TB appeared to be mediated by inadequate consumption of protein food. Approximately the same proportion of cases could be attributed to this variable and HIV infection (aPAF = 42% and 36%, respectively).While the fight against HIV remains central for TB control, interventions addressing low household SEP and, especially food availability, may contribute to strengthen our control efforts.

BACKGROUND:The annual risk of tuberculous infection (ARTI) is a key epidemiological indicator of the extent of transmission in a community. Several methods have been suggested to estimate the prevalence of tuberculous infection using tuberculin skin test data. This paper explores the implications of using different methods to estimate prevalence of infection and ARTI. The effect of BCG vaccination on these estimates is also investigated. METHODOLOGY/PRINCIPAL FINDINGS:Tuberculin surveys among school children in 16 communities in Zambia and 8 in South Africa (SA) were performed in 2005, as part of baseline data collection and for randomisation purposes of the ZAMSTAR study. Infection prevalence and ARTI estimates were calculated using five methods: different cut-offs with or without adjustments for sensitivity, the mirror method, and mixture analysis. A total of 49,835 children were registered for the surveys, of which 25,048 (50%) had skin tests done and 22,563 (90%) of those tested were read. Infection prevalence was higher in the combined SA than Zambian communities. The mirror method resulted in the least difference of 7.8%, whereas that estimated by the cut-off methods varied from 12.2% to 17.3%. The ARTI in the Zambian and SA communities was between 0.8% and 2.8% and 2.5% and 4.2% respectively, depending on the method used. In the SA communities, the ARTI was higher among the younger children. BCG vaccination had little effect on these estimates. CONCLUSIONS/SIGNIFICANCE:ARTI estimates are dependent on the calculation method used. All methods agreed that there were substantial differences in infection prevalence across the communities, with higher rates in SA. Although TB notification rates have increased over the past decades, the difference in cumulative exposure between younger and older children is less dramatic and a rise in risk of infection in parallel with the estimated incidence of active tuberculosis cannot be excluded.

The clinical application of cellular immunotherapy in hepatocellular carcinoma (HCC) is impeded by the lack of a cell surface target frequently expressed in HCC cells and with minimal presence in normal tissues to reduce on-target, off-tumor toxicity. To address this, an in silico multomics analysis was conducted to identify an optimal therapeutic target in HCC. A longlist of genes (n = 12,948) expressed in HCCs according to The Human Protein Atlas database were examined. Eight genes were shortlisted to identify one with the highest expression in HCCs, without being shed into circulation, and with restrictive expression profile in other normal human tissues. A total of eight genes were shortlisted and subsequently ranked according to the combination of their transcript and protein expression levels in HCC cases (n = 791) derived from four independent datasets. TM4SF4 was the top-ranked target with the highest expression in HCCs. TM4SF4 showed more favorable expression profile with significantly lower expression in normal human tissues but more highly expressed in HCC compared with seven other common HCC therapeutic targets. Furthermore, scRNA-seq and immunohistochemistry datasets showed that TM4SF4 was absent in immune cell populations but highly expressed in the bile duct canaliculi of hepatocytes, regions inaccessible to immune cells. In scRNA-seq dataset of HCCs, TM4SF4 expression was positively associated with mitochondrial components and oxidative phosphorylation Gene Ontologies in HCC cells (n = 15,787 cells), suggesting its potential roles in mitochondrial-mediated oncogenic effects in HCC. Taken together, TM4SF4 is proposed as a promising cell surface target in HCC due to its high expression in HCC cells with restricted expression profile in non-cancerous tissues, and association with HCC oncogenic pathways. Due to dedollarization and deglobalization trends, countries are pursuing currency diversification to reduce reliance on the U.S. dollar and mitigate currency risks. The research on the drivers of currency internationalization still faces problems such as small sample sizes, fewer methods, and incomplete theoretical frameworks. This study aims to investigate the effects of economic development, money confidence, and the financial market on currency internationalization. It also explores whether purchasing power mediates the relationships between the first two exogenous variables and currency internationalization. The Partial least squares structural equation modeling (PLS-SEM) method is used to analyze secondary data from 9 of the 10 most used currencies (excluding the euro) from 2000 to 2020 to examine these relationships. The findings show that economic development and money confidence have negative and significant relationships with currency internationalization, while financial market and purchasing power have positive and significant relationships with currency internationalization. The relationships between economic development and currency internationalization, as well as between money confidence and currency internationalization, are both mediated by purchasing power. These mediation effects are partially complementary mediation effects. Accordingly, to promote currency internationalization, this study recommends governments should adopt policies to develop the financial market, increase openness, and reduce capital controls. It also highlights the importance of managing inflation, diversifying reserve assets, and maintaining a flexible exchange rate to prevent currency depreciation. The Halophila species exhibit complex characteristics due to their high degree of variation across different bioregions. This study delves into the intricate characteristics of Halophila species in the Tanjung Adang Laut shoal, Johor, Malaysia, and offers valuable insights through morphological and genetic evidence. Employing internal transcribed sequences (ITS), we investigated the phylogeny of Halophila species, revealing distinct clades for H. ovalis, H. major, H. spinulosa, and the newly recorded H. nipponica. Notably, H. nipponica from the Tanjung Adang Laut shoal formed a conspecific relationship with its counterparts from Japan and Korea (98.3–98.5% similarity; 5–11 bp differences). Morphologically, distinguishing features, including the ratio of the half-lamina width (1:4.76–6.13 mm) and cross-vein count (4–7 pairs), supported the identification of H. nipponica. Genetic distance analyses revealed differences between H. nipponica, H. ovalis, and H. major, indicating haplotype diversity. Geographical variations were evident, as H. nipponica presented unique haplotypes (H24) in its clade. The 47 haplotypes network identified significant mutation sites, providing a comprehensive understanding of genetic and morphological distinctions. In conclusion, this study highlights the intricate characteristics and phylogeny of Halophila species in the Tanjung Adang Laut shoal, Johor, and provides valuable insights into their genetic and morphological diversity. PurposeTo compare the diagnostic abilities of vessel density measurements of the optic nerve head (ONH), peripapillary and macular regions on optical coherence tomography (OCT) angiography in eyes with primary open angle glaucoma (POAG) with that of the ONH rim area, peripapillary retinal nerve fiber layer (RNFL) thickness and the macular ganglion cell complex (GCC) thickness measurements.MethodsIn a cross sectional study, 78 eyes of 50 control subjects and 117 eyes of 67 POAG patients underwent vessel density and structural measurements with spectral domain OCT. POAG was diagnosed based on the masked evaluation of optic disc stereo photographs. Area under receiver operating characteristic curves (AUC) and sensitivities at fixed specificities of vessel densities in ONH, peripapillary and macular regions were compared with rim area, RNFL and GCC thickness.ResultsThe AUC (sensitivity at 95% specificity) of average vessel densities within the ONH, peripapillary and macular region were 0.77 (31%), 0.85 (56%) and 0.70 (18%) respectively. Objectives: Accurate subtyping of breast cancer is crucial for its diagnosis, management, and prognostication. This study aimed to determine the association of magnetic resonance imaging (MRI) breast features with the molecular subtype and aggressiveness of breast cancer in a multi-ethnic population. Methods: Treatment-naive patients with invasive breast carcinoma were included in this retrospective study. Breast MRI features were recorded based on the American College of Radiology-Breast Imaging Reporting and Data System (ACR-BIRADS) criteria, with tumour size, and apparent diffusion coefficient value (ADC). The statistical association was tested with Pearson Chi-Square Test of Independence for categorical data or the Kruskal-Wallis/ Mann Whitney U test for numerical data between the MRI features and molecular subtype, receptor status, tumour grade, lymphovascular infiltration (LVI) and axillary lymph node (ALN). Multinomial logistic regression was used to test the predictive likelihood of the significant features. The breast cancer subtypes were determined via immunohistochemistry (IHC) and dual-color dual-hapten in-situ hybridization (D-DISH). The expression statuses of ER, PR, and HER-2, LVI, and ALN were obtained from the histopathology report. The ER / PR / HER-2 was evaluated according to the American Society of Clinical Oncology / College of American Pathologists. Results: The study included 194 patients; 41.8% (n = 81) Chinese, 40.7% (n = 79) Malay, and 17.5% (n = 34) Indian, involving 71.6%(n = 139) luminal-like, 12.9%(n = 25) HER-2 enriched, and 15.5%(n = 30) Triple-negative breast cancer (TNBC). TNBC was associated with rim enhancement (p = 0.002) and peritumoral oedema (p = 0.004). HER-2 enriched tumour was associated with larger tumour size (p = 0.041). Luminal-like cancer was associated with irregular shape (p = 0.005) with circumscribed margin (p = 0.003). Other associations were ER-negative tumour with circumscribed margin (p = 0.002) and PR-negative with round shape (p = 0.001). Tumour sizes were larger in ER-negative (p = 0.044) and PR-negative (p = 0.022). Rim enhancement was significantly associated with higher grade (p = 0.001), and moderate peritumoral oedema with positive axillary lymph node (p = 0.002). Path planning is a crucial element of mobile robotics applications, attracting considerable interest from academics. This paper presents a path-planning approach that utilises the Enhanced Firefly Algorithm (EFA), a new meta-heuristic technique. The Enhanced Firefly Algorithm (FA) differs from the ordinary FA by incorporating a linear reduction in the α parameter. This modification successfully resolves the constraints of the normal FA. The research involves experiments on three separate maps, using the regular FA and the suggested Enhanced FA in 20 different runs for each map. The evaluation criteria encompass the algorithms' ability to move from the initial location to the final position without experiencing any collisions. The assessment of path quality relies on elements such as the distance of the path and the algorithms' ability to converge and discover optimum solutions. The results demonstrate significant improvements made by the Enhanced FA, with a 10.270% increase in the shortest collision-free path for Map 1, a 0.371% increase for Map 2, and a 0.163% increase for Map 3, compared to the regular FA. This work highlights the effectiveness of the Enhanced Firefly Algorithm in optimising path planning for mobile robotics applications, providing potential improvements in navigation efficiency and collision avoidance. To address patterns of genetic connectivity in a mass-aggregating marine fish, we analyzed genetic variation in mitochondrial DNA (mtDNA), microsatellites, and single nucleotide polymorphisms (SNPs) for Nassau grouper (Epinephelus striatus). We expected Nassau grouper to exhibit genetic differentiation among its subpopulations due to its reproductive behavior and retentive oceanographic conditions experienced across the Caribbean basin. All samples were genotyped for two mitochondrial markers and 9 microsatellite loci, and a subset of samples were genotyped for 4,234 SNPs. This study aims to determine, from the perspective of investors, the factors that predict Islamic unit trust (IUT) investment intentions. Additionally, this paper examines the moderating effect of fintech self-efficacy (FSE) on the relationship between attitude and investment intention. A total of 392 data were collected from IUT investors in Malaysia and analyzed using partial least squares structural equation modeling. The findings reveal that subjective norms have the highest impact on investment intention, followed by attitude and FSE, while religiosity is not significantly associated with investment intention in Islamic unit trust funds. Attitude significantly mediates religiosity-intention and Islamic financial literacy-intention relationships. FSE significantly moderates the attitude-intention relationship. The results shed light on the key factors that increase investing behavior and have direct managerial implications with regard to marketing strategies and target markets. These findings suggest that IUT service providers should take the lead in attracting customers through effective and targeted marketing initiatives, particularly by enhancing customers' FSE and capabilities. This study provides empirical evidence on the interrelationships between Islamic financial literacy, religiosity, and FSE in examining investors' behavior using the Theory of Planned Behavior framework. BackgroundThe asymptomatic carriers of the Plasmodium parasite represent a large hidden reservoir of Plasmodium species. These individuals can carry microscopically detectable levels and sub-microscopic levels of Plasmodium parasitemia. As a result, the use of clinical diagnostic methods, such as Rapid Diagnosis Tests (RDTs) and Giemsa microscopy leads to underestimation of the burden of asymptomatic malaria. Thus, the use of highly sensitive molecular methods such as loop-mediated isothermal amplification techniques (LAMP) helps to overcome these limitations and is crucial to reporting the true burden of the disease.ObjectiveThis study aimed to determine the prevalence of Asymptomatic Plasmodium infections (APIs), and evaluate light microscopy for the detection of APIs using the LAMP method as a reference in Gorgora, Western Dembia district, Northwest Ethiopia.MethodA community-based cross-sectional survey was carried out from May 17 to June 30, 2023, among households located in particular kebeles at Gorgora. The study participants were chosen using a multi-stage sampling technique. One Hundred Fifteen households and one household member from each household were selected using systematic random and simple random sampling techniques, respectively. To gather sociodemographic data, semi-structured questionnaires were used. Capillary blood samples were obtained from each study participant and screened for Plasmodium species infections using a LAMP kit and light microscopy. The data were entered into Epi Data version 4.6 and exported to SPSS version 25 for analysis.ResultsThe prevalence of APIs through light microscopy examination and LAMP analysis was 6.1% (95%CI: 2.5-12.1) and 11.3% (95%CI: 6.2-18.6) respectively. Using LAMP as a reference, the sensitivity and specificity of microscopy were 53.8% and 100% respectively. Microscopy examination missed six Plasmodium infections that were positive by LAMP analysis. A measure of agreement between LAMP and Microscopy was found to be 0.67(k = 0.67).ConclusionsA significant proportion of APIs was found which likely act as a reservoir of transmission. This study also showed a significant number of APIs were missed by microscopy techniques. Thus, efforts to control and eliminate malaria should also consider these sub-microscopic infections.

BackgroundParasitic infections in pregnancy are detrimental for both the mother and her fetus. Malaria and intestinal parasite infections among pregnant women at their first antenatal care contact (ANC1) could offer information on their burden in pregnancy, community-level transmission, and intervention coverage, which is vital for targeted interventions. However, data is scarce in Ethiopia. This study investigated Plasmodium and intestinal parasite infections along with their associated risk factors among pregnant women at their ANC1 in northwest Ethiopia.Materials and methodsA cross-sectional study was conducted among 538 pregnant women attending ANC1 at selected health facilities in Jawi District, northwest Ethiopia, between November 2021 and July 2022. Plasmodium infection was diagnosed by light microscopy, rapid diagnostic tests, and multiplex real-time PCR. Whereas, intestinal parasite infections were examined microscopically using stool wet mount and Kato-Katz techniques. Predictors of Plasmodium and intestinal parasite infections were evaluated using logistic regression analysis. A P-value of ResultsOverall, 43.1% (95% CI: 38.9-47.4%) of women were infected with intestinal parasites, 19.1% (95% CI: 15.9-22.7%) had Plasmodium infections, and 11.7% (95% CI: 9.1-14.7%) were co-infected with Plasmodium and intestinal parasites. About 84.9% of malaria cases were asymptomatic, and 39.8% were sub-microscopic infections. Younger maternal age (adjusted odds ratio (AOR) = 2.01, 95% confidence interval (CI): 1.1, 3.65), primigravidity (AOR = 2.37, 95% CI: 1.43, 3.92), lack of insecticide-treated bed net use (AOR = 2.58, 95% CI: 1.26, 5.3), undernutrition (AOR = 1.89, 95% CI: 1.13, 3.15), and intestinal helminth infection (AOR = 2.09, 95% CI: 1.31, 3.36) were significant predictors of Plasmodium infection. Whereas, rural residency (AOR = 1.62, 95% 1.03, 2.57), habit of soil eating (AOR = 2.06, 95% CI: 1.1, 3.9), consuming raw vegetables and fruits (AOR = 1.59, 95% CI: 1.09, 2.3), and lack of latrine use (AOR = 1.66, 95% CI: 1.06, 2.6) were significantly associated with intestinal parasite infections in pregnancy.ConclusionHigh prevalences of Plasmodium and intestinal parasite infections were observed among pregnant women in northwest Ethiopia. These findings highlight the importance of strengthening prevention and control measures for parasitic infections and implementing malaria screening at ANC1, particularly for young and primigravid women.

BackgroundHagenia abyssinica (Bruce) J.F. Gmel (Family: Rosaceae) and Rumex abyssinicus Jacq (Family: Polygonaceae) are valuable medicinal plants traditionally used in Ethiopia to treat various diseases. Recent studies have also demonstrated that solvent extracts of these plants exhibit molluscicidal activities under laboratory conditions, highlighting their potential for snail control. However, limited information is available regarding their safety profiles.ObjectiveThis study aimed to evaluate acute, and sub-acute toxicity of 70% ethanol extracts of H. abyssinica and R. abyssinicus flowers in Swiss albino mice, following the Organization for Economic Co-operation and Development guidelines 423 and 407.MethodsIn the acute toxicity study, both extracts were administered orally to experimental groups at varying concentrations (mg/kg bodyweight): 5, 50, 300, and 2000. For the sub-acute toxicity study, both extracts were given to the experimental groups at doses (mg/kg) of 125, 250, and 500 daily for 28 days. Blood samples were collected from each mouse and analyzed for hematological and biochemical parameters. Additionally, the heart, liver, and kidneys were excised, stained, and examined for potential histopathological effects.ResultsThe acute toxicity study revealed no noticeable changes in behavior at the highest oral dosage of 2000 mg/kg. In the sub-acute toxicity study, no statistically significant changes were observed in hematological and biochemical parameters compared to the control group. Similarly, no abnormal histological findings were noted in the examined organs in comparison to the control group.ConclusionThese findings indicate that flower extracts of both plants did not show significant toxicity to laboratory mammals at an oral dosage of 2000 mg/kg.

Communal respiratory syncytial virus (RSV) causes mild to severe illnesses, predominantly in older adults, or people with certain chronic medical conditions, and in children. Symptoms may include rhinorrhea, cough, fever, and dyspnea. In most cases, the infection is mild and resolves on its own, but in some cases, it can lead to more serious illness such as bronchiolitis or pneumonia. The RSV genome codes for ten proteins, NS1, NS2, N, P, M, SH, G, F, M2 and L. We aimed to identify the RSV geographical transmission pattern based on parsimony and investigate hotspot regions across the complete RSV genomes. We employed Viral Evolutionary Network Analysis System on full-length available RSV genomes and with HyPhy for elucidating type of selection pressure. These results indicated that RSV strains circulating in South and North America are not mixed to the European samples, however, genomes reported from Australia are the direct decedents of European samples. Samples reported from the United Kingdom exhibited significant diversity, spanning almost every cluster. This report provides a complete mutational analysis of all the individual RSV genes, and particularly the 31 hotspot substituting regions circulating across the globe in RSV type A samples. Further, protein G and L displayed higher level of codons experienced positive selection. This analysis of RSV type A highlights mutational frequencies across the whole genome, offering valuable insights for epidemiological control and drug development.

BackgroundThe coexistence of viral hepatitis with diabetes mellitus (DM) significantly escalates the risk of severe outcomes. This study aimed to determine the seroprevalence and associated factors of hepatitis B (HBV) and hepatitis C (HCV) viral infections among DM patients in northern Tanzania.Materials and methodsConducted between February 2023 and May 2023, this hospital-based cross-sectional study enrolled 189 patients with DM from the Diabetic Clinic of Kilimanjaro Christian Medical Centre. A structured questionnaire captured relevant clinical information, and plasma blood sample was assessed for hepatitis B surface antigen and anti-hepatitis C antibody seropositivity. Data analysis employed SPSS v26, and a chi-square test was used to determine the statistical difference of HBV and HCV among patients with DM. Logistic regression was performed to determine factors associated with HBV and HCV.ResultsAmong the 189 patients with DM, the seroprevalence of HBV and HCV infections stood at 2.1% and 0.5%, respectively. Males constituted a significant majority (80%) of those affected by viral hepatitis. Furthermore, 60% of affected patients were in non-union relationships (single, widowed, or divorced), and 40% reported multiple sexual partners. However, the study found no significant association between traditional associated factors and viral infection acquisition.ConclusionThe study's findings reveal a relatively low prevalence of HBV and HCV infections among patients with DM compared to the general population, with no significant association among factors. Nonetheless, the results underscore the importance of early screening and vaccination for HBV and HCV in patients with DM. Such efforts are crucial for curbing infection spread and reducing the risk of hepatocellular carcinoma development in this vulnerable population.

The community-based breeding program was introduced to enhance the productivity of Bonga sheep. Data evaluating animals' potential regarding key traits was gathered from 2009-2021. This study estimated covariance components and genetic parameters for growth (birth, weaning, and six-months weight), reproductive (lambing interval, annual reproductive rate, litter size at birth and weaning, litter weight at birth and weaning), and survival (0-3, 3-6, and 0-6 months) traits using a multivariate animal model. The Average Information Restricted Maximum Likelihood method in WOMBAT was used for growth and reproductive traits, while Derivative-free MUltivariate analysis was employed for survival traits. The results revealed that the additive heritability for growth, reproductive, and survival traits ranged from 0.15-0.26, 0.06-0.08, and 0.10-0.17, respectively. The variance ratio of maternal permanent environment for growth traits ranged from 0.07-0.25, while that of animal environment for reproductive traits ranged from 0.33-0.45. Maternal genetic heritability for birth, weaning, and six-months weights were 0.14, 0.19, and 0.19, respectively, while it ranged from 0.01-0.14 for reproductive traits. BackgroundUnderstanding variations in women's work participation at lower administrative levels, such as districts, is a missing link in identifying trends, patterns and variation that can offer insights into this long-term stagnation. We link data from the 2019-21 Indian National Family Health Survey and the 2011 Indian Population and Housing Census to generate estimates of women's work within 640 districts of India, and to examine the spatial clustering of women's work across these districts. We examine women's work through three outcome variables, namely, district-level estimates of 1) percentage of women who worked in the past 12 months, 2) percentage of women who were self-employed in the past 12 months, and 3) percentage of women who earned cash in the past 12 months.ResultsDiagnostic measures confirm that our model-based estimates are robust enough to provide reliable district-level estimates of women's work in India. Women's work and cash earnings were lowest in the districts of the central, eastern, and northern regions, and highest in the southern region. Self-employment rates for women were generally low in Indian districts, except for districts in Himachal Pradesh and the north-eastern region.ConclusionsConsiderable spatial heterogeneity in women's work has been found across 640 districts of India. Our study demonstrates that estimated percentage of women who worked in the past 12 months, estimated percentage of women who earned cash in the past 12 months and estimated percentage of women who were self-employed in the past 12 months all vary substantially at the district level. Having only state-level estimates may thus be inadequate to inform efforts to remediate low levels of women's work in India. The insights from our current study may help in the formulation and implementation of targeted policies that increase opportunities for women to expand their paid work in India.

IntroductionMaternal tobacco smoking during pregnancy is a significant public health concern with potential long-lasting effects on child development. ADHD, a neurodevelopmental disorder characterized by inattention, hyperactivity, and impulsivity, may be influenced by prenatal nicotine exposure. This systematic review and meta-analysis examine the association between maternal tobacco smoking during pregnancy and the risk of ADHD in offspring.MethodsFollowing PRISMA guidelines, we searched databases including PubMed, Web of Science, Cochrane Central, Embase, Scopus, CINAHL, LILACS, SciELO, Allied and Complementary Medicine Database (AMED), ERIC, CNKI, HTA Database, Dialnet, EBSCO, LENS, and Google Scholar for studies up to November 1, 2024. We included peer-reviewed studies reporting quantitative effect size estimates for the association between maternal tobacco smoking and ADHD. Study quality was assessed using the Newcastle-Ottawa Scale (NOS).ResultsWe identified 2,981 articles and included 55 studies (4,016,522 participants) in the analysis. The meta-analysis showed a significant association between maternal tobacco smoking during pregnancy and increased risk of ADHD in offspring (pooled Odds Ratio (OR) = 1.71, 95% CI: 1.55-1.88; P < 0.001). Egger's test indicated no publication bias (p = 0.204), but Begg's test did (p = 0.042). By employing the trim and fill method, the revised OR was estimated to be 1.54 (95% CI: 1.40-1.70; P < 0.001). The OR were 2.37 (95% CI: 1.72-3.28; P < 0.001) in cross-sectional studies, 1.72 (95% CI: 1.49-2.00; P < 0.001) in case-control studies, and 1.53 (95% CI: 1.34-1.74; P < 0.001) in cohort studies. Meta-regression showed study design and study region significantly influenced heterogeneity (P < 0.10). Sensitivity and subgroup analyses confirmed the robustness of these findings.ConclusionThis systematic review and meta-analysis demonstrate a significant association between maternal tobacco smoking during pregnancy and increased odds of ADHD in offspring. These findings highlight the need for prenatal care guidelines and tobacco smoking cessation programs for pregnant women to reduce ADHD risk and promote optimal neurodevelopmental outcomes. Future research should explore underlying mechanisms and potential confounders further.

BackgroundChronic obstructive pulmonary disease (COPD) and cardiovascular disease (CVD) greatly influence morbidity and mortality, with COPD patients frequently suffering from cardiovascular comorbidities like coronary heart disease and stroke. This study analyzes mortality trends and disparities among individuals in the United States (US) affected by both CVD and COPD.MethodsThis study analyzed death certificates from the CDC WONDER database for individuals aged 25 and older who died between 1999 and 2020 with both CVD (ICD I00-I99) and COPD (ICD J41-J44). Age-adjusted mortality rates (AAMRs) and annual percent change (APC) were calculated by year, sex, age group, race/ethnicity, geographic region, and urbanization status.ResultsBetween 1999 and 2020, there were 3,590,124 reported deaths due to coexisting CVD and COPD, with overall AAMR slightly changing from 82.2 to 81.2 per 100,000 population, and a notable rise from 2018 to 2020 (APC: 5.28; 95% CI: 1.83 to 7.22) coinciding with the onset of COVID-19 pandemic. A similar surge in mortality was observed across multiple demographic subgroups, particularly among older adults. Disparities across age groups, sex, race, and geographic location were also observed in the mortality rates due to CVD and COPD. When analyzed by age group, older adults exhibited the highest AAMR at 824.1. Men had higher AAMRs than women (96.5 vs. 60.7). Ethnoracial analysis showed that non-Hispanic (NH) White individuals had the highest AAMRs (82.0), followed by NH American Indian or Alaska Native (74.5), NH Black (63.6), Hispanic (38.1), and NH Asian or Pacific Islander (25.1) individuals. Additionally, non-metropolitan areas had higher AAMRs compared to metropolitan areas (96.2 vs. 70.9).ConclusionsThe findings suggest that mortality rates for CVD and COPD have increased in recent years, coinciding with the onset of the COVID-19 pandemic, which may have exacerbated outcomes in vulnerable populations. The study highlights the need for targeted interventions to address the overlapping impacts of CVD and COPD, especially in high-risk groups.

BackgroundAntenatal care (ANC) services are essential to reduce maternal and newborn morbidity and mortality rates. However, the trends and socioeconomic inequality of utilizing recommended ANC services has not been well studied in Ethiopia. Therefore, this study aims to investigate the trends and socioeconomic disparities in receiving recommended ANC services among Ethiopian women.MethodsThis study used recent Ethiopian Demographic Health Surveys (EDHS) conducted in 2011, 2016, and 2019. Binary logistic regression model was employed to assess the association between receiving the recommended ANC services and explanatory variables and socioeconomic disparities were estimated through concentration index (CIX) analysis. Moreover, Wagstaff approach was used to decompose the relative CIX to the contribution of explanatory variables for the observed disparities.ResultsThis study found that 37.37% (95%CI: 36.46-38.28%) of mothers utilized the recommended ANC services in Ethiopia. The trend in the coverage of recommended ANC services increased from ~ 30% in 2011 to 44.70% in 2019. Mother's age and education, household wealth status, distance of the nearest health facility, and experiencing domestic abuse (i.e., wife beating) were significantly associated with utilization of recommended ANC services. The relative estimated CIX for wealth index, mothers education, Ethiopian administrative regions, and residence were 0.15 (P ConclusionThe study revealed that Ethiopian women's utilization of recommended ANC services was unequal by their socioeconomic classes, with better off women more likely to utilize the recommended ANC services than worse off women. Bithyniids are freshwater snails that play a crucial role in the transmission of various parasitic trematodes of medical and veterinary importance. In this study, we explored the prevalence of cercarial trematode infections in bithyniid snails from Thailand and examined the species diversity of both the intermediate snail hosts and parasite larvae. A total of 688 bithyniid snails were collected from diverse natural habitats at 24 locations in 16 provinces across 5 regions of Thailand. The presence of larval trematode infections was examined using the cercarial shedding method. Both the collected snails and the emerging cercariae were identified at the species level using a combination of morphological and molecular techniques. The mitochondrial COI and 16S rDNA sequences of bithyniid snails, along with the ITS2 sequences of cercariae, were obtained via PCR amplification and sequencing. Three species of bithyniid snails were identified in this study: Bithynia funiculata, Bithynia siamensis siamensis, and Hydrobioides nassa. Among these species, B. s. siamensis exhibited the highest population density, followed by B. funiculata and H. nassa. The overall rate of cercarial infection in the bithyniid snails was relatively low, at 1.45%. H. nassa snails had the highest infection prevalence, at 11.11%, while B. s. siamensis had a prevalence of 1.39%. Only the morphological type of the xiphidiocercariae was detected. BLASTn searches in GenBank and phylogenetic trees based on xiphidiocercariae were used to classify the samples into four different families spanning two superfamilies of digenean trematodes. The genera Plagiorchis, Prosthogonimus, Paralecithodendrium, and cercaria of Renicolidae are reported for the first time in B. s. siamensis. Plagiorchis and Paralecithodendrium are significant genera of zoonotic trematodes. These findings indicate that B. s. siamensis and H. nassa can act as the first intermediate hosts for various parasitic trematodes in Thailand.

BackgroundParoxysmal tachycardia encompasses various heart rhythm disorders that cause rapid heart rates. Its episodic occurrence makes it difficult to identify and measure its prevalence and trends in the population. Additionally, there is limited data on disparities and trends in mortality due to paroxysmal tachycardia, which is essential for assessing current medical approaches and identifying at-risk populations.MethodsOur study examined death certificates from 1999 to 2020 using the CDC WONDER Database to identify deaths caused by paroxysmal tachycardia in individuals aged 25 and older, using the ICD-10 code I47. Age-adjusted mortality rates (AAMRs) and annual percent changes (APC) were calculated by year, gender, age group, race/ethnicity, geographic location, and urbanization status. Trends in AAMRs were analyzed using the Joinpoint Regression Program to identify significant changes and inflection points in mortality trends throughout the study period.ResultsBetween 1999 and 2020, 155,320 deaths were reported in patients with paroxysmal tachycardia. Overall, AAMR decreased from 4.8 to 3.7 per 100,000 population between 1999 and 2020, despite showing a significant increase from 2014 to 2020 (APC: 4.33; 95% CI: 3.53 to 5.56). Men had consistently higher AAMRs than women (4.7 vs. 2.2). Furthermore, we found that AAMRs were highest among Non-Hispanic (NH) Black or African Americans and lowest in NH Asian or Pacific Islanders (4 vs. 1.9). Nonmetropolitan areas had higher AAMRs than metropolitan areas (3.6 vs. 3.2).ConclusionsOur analysis showed a significant decrease in mortality from paroxysmal tachycardia since 1999, although there has been a slight increase in recent years. However, disparities remain, with higher AAMRs among men, NH Black or African Americans, and residents of non-metropolitan areas. These findings call for immediate public health actions to curb the rising trends and reduce potential disparities.

Infectious diseases and fetal wastage (FW) present major challenges in livestock management, particularly in sub-Saharan Africa, including Nigeria. This study assessed the season variations, incidence rate and economic impact of prevalent diseases and FW at the Kubwa (KB) abattoir in the Federal Capital Territory (FCT) of Abuja, Nigeria. Over a year-long period (January to December 2023), we analyzed 5,779 cattle through daily post-mortem inspections. Disease identification was based on morphological and gross lesions characteristic of the conditions studied. Economic losses from condemned organs and FW were calculated using the formula TEL = N × P × W. The results indicated statistically significant seasonal variations in the incidence of Fasciolosis (FS), Paramphistomosis (PP), and FW, with p-values of 2.52x10^-10, 3.33x10^-9, and 0.003, respectively. In contrast, Tuberculosis (TB), Contagious Bovine Pleuropneumonia (CBPP), Dermatophilosis (DM), Abscess (AB), and Moneziasis (MZ) did not show significant seasonal variation. The total economic impact of condemned organs and FW was 104,348 USD (equivalent to 99,130,600 NGN at the time of the study), representing a considerable threat to food security and substantial economic losses for farmers. Considering the zoonotic potential of some diseases, which can be transmitted to those handling the carcasses, there is a critical need for enhanced inspection protocols, continuous disease surveillance, and timely reporting in slaughterhouses. The notable economic losses from condemned organs also underscore the necessity of implementing pregnancy tests for female animals before slaughter to protect food security and support the nation's economy. These findings highlight the essential role of abattoirs in improving food security, detecting zoonotic diseases, and bolstering public health and economic stability in low-income countries.

Alzheimer's disease is a neurodegenerative disorder that impairs neurocognitive functions. Acetylcholinesterase, Butyrylcholinesterase, Monoamine Oxidase B, Beta-Secretase, and Glycogen Synthase Kinase Beta play central roles in its pathogenesis. Current medications primarily inhibit AChE but fail to halt or reverse disease progression due to the multifactorial nature of Alzheimer's. This underscores the necessity of developing multi-target ligands for effective treatment. This study investigates the potential of phytochemical compounds from Moroccan medicinal plants as multi-target agents against Alzheimer's disease, employing computational approaches. A virtual screening of 386 phytochemical compounds, followed by an assessment of pharmacokinetic properties and ADMET profiles, led to the identification of two promising compounds, naringenin (C23) and hesperetin (C24), derived from Anabasis aretioides. These compounds exhibit favourable pharmacokinetic profiles and strong binding affinities for the five key targets associated with the disease. Density functional theory, molecular dynamics simulations, and MM-GBSA calculations further confirmed their structural stability, with a slight preference for C24, exhibiting superior intermolecular interactions and overall stability. These findings provide a strong basis for further experimental research, including in vitro and in vivo studies, to substantiate their potential efficacy in Alzheimer's disease.

BackgroundAntiretroviral treatment increases the risk of accumulation of resistance mutations that negatively impact the possibilities of future treatment. This study aimed to present the frequency of HIV-1 antiretroviral resistance mutations and the genetic diversity among children with virological failure in five pediatric care facilities in Benin.MethodsA cross-sectional study was carried out from November 20, 2020, to November 30, 2022, in children under 15 years of age who failed ongoing antiretroviral treatment at five facilities care in Benin (VL > 3log10 on two consecutive realizations three months apart). Viral loads were measured using the m2000 RealTime Abbott platform. Genotyping was carried out with the commercial Viroseq kit. Sequences were read on the ABI 3500 sequencer and then edited with ViroSeqHIVv3.0 software. The HIV drug resistance database at Stanford University was used to identify mutations and viral subtypes were assigned by phylogenetic analyses.ResultsThe HIV-1 pol gene was sequenced in 47 participants with virological failure of antiretroviral treatment. The median age was 120 [Interquartile Range 90-144] months. The prevalent treatment was EFV base regimen (22/47; 46.8%). Median viral load was 4.39 log10 [IQR 3.81-4.86 log10] respectively. Resistance testing was successful among (37/47; 78.72%) children, resistance mutations were detected in (32/37; 86.48%) children, and (29/32; 90.62%) had at least one surveillance drug resistance mutation. Respectively (25/32; 78.12%), (28/32; 87.5%), (4/32; 12.90%), (22/32; 68.75%) had at least one resistance mutation associated with NRTIs, NNRTIs, PIs and NNRTIs+NRTIs. (12/32; 37.5%) of children carried mutations related to TAMs. the most frequently NRTIs identified were M184V (21/62; 33.9%) followed by TAMs (20/62; 32.2%) and T69G/D (2/62; 3.2%)s. Among mutations associated with NNRTIs K103N represented (18/64; 28.1%) followed by P225H (7/64; 10.9%). The I54V (3/6; 50%) mutation is the major PI observed. Genetic diversity is characterized by a preponderance of CRF02_AG (72%, 23/32), followed by unique recombinant forms (URFs) (25%, 8/32) and one subtype G.ConclusionA high rate of mutations has been observed in children. These data underline the importance of implementing routine genotypic testing in the biological monitoring of infected children to anticipate the accumulation of resistance mutations and thus compromise the treatment options available in Benin.

Metastatic melanoma causes a high rate of mortality. We conducted an integrated analysis to identify critical regulators associated with the prognosis, pathogenesis, and targeted therapies of metastatic-melanoma. A microarray dataset, GSE15605, including 12 metastatic-melanoma and sixteen normal skin (NS) samples, were obtained from the GEO database. After exploration of DEGs of NS and metastatic-melanoma, identification of relevant transcription factors (TFs) and kinases, the Gene Ontology (GO), and pathways analyses of DEGs were performed. Protein-protein interaction (PPI) networks were evaluated by the STRING and Cytoscape. Subsequently, the hub genes were selected using GEPIA. Survival analysis was performed using the TCGA. To identify microRNA and lncRNA DEGs of the melanoma-associated genes miRwalk and FANTOM6 were employed. In metastatic-melanoma samples 285 and 1173 genes were up and down-regulated, respectively. The upregulated genes were mostly involved in granulocyte chemotaxis, positive regulation of calcium ion transmembrane transport, and melanin biosynthetic process. Five hub genes including CXCL11, ICAM1, LEF1, MITF, and STAT1 were identified, SUZ12, SOX2, TCF3, NANOG, and SMAD4 were determined as the most significant TFs in metastatic-melanoma. Furthermore, CDK2, GSK3B, CSNK2A1, and CDK1 target the highest amounts of genes associated with disease. The DGIdb analysis results show the match drugs for five hub genes. MiRNAs analysis revealed hsa-miR-181c-5p, hsa-miR-30b-3p, hsa-miR-3680-3P, hsa-miR-4659a-3p, hsa-miR-4687-3P, and hsa-miR-6808-3P could regulate the hub genes, whereas RP11-553K8.5 and SRP14-AS1 were identified as the top significant lncRNA. The items recognized in the current study can be used as potential biomarkers for diagnostic, predictive, and might helpful to develop targeted combined therapies.

Cognitive dysfunction in Alzheimer's disease results from a complex interplay of various pathological processes, including the dysregulation of key enzymes such as acetylcholinesterase (AChE), butyrylcholinesterase (BuChE), and monoamine oxidase B (MAO-B). This study proposes and designs a series of novel molecules derived from 8-hydroxyquinoline (Azo-8HQ) as potential multi-target lead candidates for treating AD. An exhaustive in silico analysis was conducted, encompassing docking studies, ADMET analysis, density functional theory (DFT) studies, molecular dynamics simulations, and subsequent MM-GBSA calculations to examine the pharmacological potential of these molecules with the specific targets of interest. Out of the 63 Azo-8HQ derivatives analysed, two molecules, 14c and 17c, demonstrated strong affinities for AChE, BuChE, and MAO-B, along with favourable pharmacokinetic profiles and electronic properties. Molecular dynamics simulations confirmed the stability of these molecules within the active sites of the targets, and MM-GBSA calculations revealed low binding energies, indicating robust interactions. These findings identify molecules 14c and 17c as promising multi-target candidates for the treatment of AD, based on an in-depth computational study aimed at minimizing drug development costs and time. Future work will include the synthesis of these molecules followed by in-depth in vitro and in vivo testing to validate their potential therapeutic efficacy.

IntroductionThe contribution of obesity phenotypes to dyslipidaemia in middle-aged adults from four sub-Saharan African (SSA) countries at different stages of the epidemiological transition has not been reported. We characterized lipid levels and investigated their relation with the growing burden of obesity in SSA countries.MethodsA cross-sectional study was conducted in Burkina Faso, Ghana, Kenya and South Africa. Participants were middle aged adults, 40-60 years old residing in the study sites for the past 10 years. Age-standardized prevalence and adjusted mean cholesterol, LDL-C, HDL-C, triglycerides and non-HDL-C were estimated using Poisson regression analyses and association of body mass index (BMI), waist circumference (WC) and waist-to-hip ratio (WTHR) with abnormal lipid fractions modeled using a random effects meta-analysis. Obesity phenotypes are defined as BMI ≥ 30 kg/m2, increased WC and increased waist-to-hip ratio.ResultsA sample of 10,700 participants, with 54.7% being women was studied. Southern and Eastern African sites recorded higher age-standardized prevalence of five lipid fractions then West African sites. Men had higher LDL-C (19% vs 8%) and lower HDL-C (35% vs 24%) while women had higher total cholesterol (15% vs 19%), triglycerides (9% vs 10%) and non-HDL-cholesterol (20% vs 26%). All lipid fractions were significantly associated with three obesity phenotypes. Approximately 72% of participants in the sample needed screening for dyslipidaemia with more men than women requiring screening.ConclusionObesity in all forms may drive a dyslipidaemia epidemic in SSA with men and transitioned societies at a higher risk. Targeted interventions to control the epidemic should focus on health promoting and improved access to screening services.

IntroductionBreast and cervical cancers are the most frequent and fatal cancers among women. Thus, early detection is necessary to improve the prognosis of affected women. However, in Ethiopia, the rates of screening remain alarmingly low.ObjectiveTo assess the magnitude of screening attendance for breast or cervical cancer, as well as the factors that predict it, among women aged 30-49 years old in Gedeo Zone, South Ethiopia, in 2023.MethodA community-based cross-sectional study was conducted using a multi-stage cluster sampling technique. Data were collected using pretested, structured questionnaires by trained interviewers. Univariate and multivariate logistic regression models were employed to identify factors associated with screening attendance.ResultsA total of 554 women participated in the study. Of them, 132 (23.8%) were screened for breast or cervical cancer. Higher age of 40-44 versus 45-49 years (adjusted odds ratio [AOR] 4.18 [95% CI 1.59, 10.9]), higher education status ([AOR] 5.49 [95% CI 2.01, 13.1]), having family or a friend with history of breast or cervical cancer ([AOR] 5.55 [95% CI 2.47, 12.5]), short anticipated time to seek help ([AOR] 4.66 [1.31, 11.7]), adequate health literacy ([AOR] 6.98 [95% CI 2.82,13.3]) and high self-efficacy ([AOR] 2.32 [95% CI 1.08, 4.96]) were positive factors with higher screening attendance. High response cost ([AOR] 0.19 95% CI [0.08, 0.50]) was a negative factor and associated with lower screening attendance.Conclusion and recommendationThe study found that only one in four women attended breast or cervical cancer screening. Screening uptake was better in women with higher education, health literacy, self-efficacy, and older age-similar to factors associated with other health seeking behavior. Interestingly, history of breast or cervical cancer in a friend or relative was also associated with higher uptake. This indicates that in addition to awareness campaigns, personal testimonials of survivors could encourage women to visit screening facilities.

IntroductionAlthough late initiation of antenatal care has been linked with adverse pregnancy outcomes, a significant number of pregnant women in resource-limited regions are seeking this care late. There has been a lack of information on the extent and factors influencing late initiation of antenatal care in the African context following the implementation of the new antenatal care model in 2016. Thus, we aimed to determine the pooled prevalence of late antenatal care visit and its determinants among women in Sub-Saharan Africa (SSA) using national surveys conducted after the implementation of new guidelines.MethodsWe analyzed data from the recent demographic and health survey (DHS) conducted in 16 SSA between 2018 and 2022 using a weighted sample of 101,983 women who had antenatal care follow-up during their index pregnancy. A multilevel logistic regression models were fitted and likelihood and deviance values were used for model selection. In the regression model, we used adjusted odds ratios along with their corresponding 95% confidence intervals to determine the factors associated with late antenatal care visit.ResultsThe pooled prevalence of late antenatal care visit among pregnant women in SSA was 53.8% [95% CI: 46.2% - 61.3%], ranging from 27.8% in Liberia to 75.8% in Nigeria. Women's age and education, working status, partner's education, health insurance coverage, birth order, household wealth, age at marriage, decision on health care, residence, and community-level women's illiteracy were the factors associated with late antenatal care visit.ConclusionMore than half of pregnant women in SSA initiated attending antenatal care during late pregnancy, with significant differences seen among countries. These results underscore the necessity for focused interventions to tackle these issues and enhance prompt access to antenatal services for better maternal and child health outcomes in the area. Moreover, creating tailored interventions for younger women, those with multiple previous births, employed women, and those who experienced early marriage to address their specific challenges and obstacles in seeking care is crucial.
