Chronic inflammation associated with delayed diabetic wound healing is induced by disturbed polarization of macrophages derived mainly from predisposed progenitor cells in bone marrow. Docosahexaenoic acid plays a critical role in regulating the function of macrophage progenitor cells. The authors evaluated whether docosahexaenoic acid accelerates diabetic wound healing in rats.Streptozotocin-induced diabetic rats divided into control and docosahexaenoic acid-treated groups (n = 10) were subjected to paired dorsal skin wounds. Docosahexaenoic acid (100 mg/kg per day) was orally supplemented 2 weeks before wounding until termination. The wound healing process was recorded 0, 7, and 14 days after wounding. At day 7, blood perfusion was measured by laser Doppler perfusion imaging; angiogenesis was compared using immunofluorescent CD31 and α-smooth muscle actin staining; macrophage polarization was detected using immunofluorescence for CD68, CD206, and inducible nitric oxide synthase. Hematoxylin and eosin staining was used to examine wound healing at day 14. Activation status of macrophages derived from bone marrow cells in normal, diabetic, and docosahexaenoic acid-treated diabetic rats was determined in vitro using Western blotting and enzyme-linked immunosorbent assay.Docosahexaenoic acid significantly accelerated wound healing 7 and 14 days (p0.01) after wounding. Increased vessel densities (1.96-fold; p0.001) and blood perfusion (2.56-fold; p0.001) were observed in docosahexaenoic acid-treated wounds. Immunofluorescence revealed more CD206 and fewer inducible nitric oxide synthase-positive macrophages (p0.001) in treated wounds. Furthermore, macrophages derived from diabetic rats expressed higher levels of inducible nitric oxide synthase and tumor necrosis factor-α and lower arginase-1 and interleukin-10 (p0.05).Docosahexaenoic acid accelerates diabetic wound healing at least in part by restoring impaired plasticity of macrophage progenitor cells.

Background The authors aimed to identify clinical characteristics related to the prognosis after one-stage grafting for flexor digitorum profundus reconstruction in zones I and II. Methods A total of 401 patients who underwent one-stage flexor digitorum profundus reconstruction after injuries in zones I and II, between January 1, 2006, and June 1, 2016, were included for analysis. The authors recorded the following clinical characteristics: duration from injury to reconstruction, graft type and length, handling (preservation or removal) of flexor digitorum superficialis, tenorrhaphy technique, and duration of postoperative immobilization. Results There was no significant difference in age, sex, duration between injury and reconstruction, graft type, and handling of flexor digitorum superficialis between subjects who underwent successful reconstruction and those who did not. Univariate and multivariate analyses revealed that end-to-end tenorrhaphy, immediate controlled motion, and immobilization for less than 2 weeks all independently were related to improved final outcomes. Grafting within zones I and II contributed to increased incidence of failure. In a subgroup having an injury-reconstruction duration of less than 2 weeks, the authors observed that grafting from zone III to zones I and II tended to result in better outcomes compared with grafting from zone III to the flexor digitorum profundus insertion. This, however, was not statistically significant (p = 0.11). Conclusions The authors recommend end-to-end tenorrhaphy, reducing immobilization to less than 2 weeks, and avoiding grafting within zones I and II. Endothelial progenitor cells play a critical role in neovascularization. However, the mobilization, recruitment, and functional capacity of endothelial progenitor cells are significantly impaired in diabetes. Statins have been shown to augment the number and improve the function of endothelial progenitor cells. This study investigated the effects of statins on the viability of ischemic skin flaps in diabetic rats.Twenty normal and 40 diabetic Sprague-Dawley rats were included in this study. Atorvastatin (10 mg/kg/day) was administered orally in 20 diabetic rats at 2 weeks before flap surgery for 21 consecutive days. Other rats received equal vehicle. Two weeks after first gavage, a 3 × 10-cm skin flap was established on the backs of rats. The necrotic area of each skin flap was measured at 7 days postoperatively. Capillary density and endothelial progenitor cells recruited to the flaps were analyzed using immunofluorescence staining. Circulating endothelial progenitor cell number was determined by flow cytometry. In vitro migration and tube formation experiments were used to analyze the function of endothelial progenitor cells.Atorvastatin treatment increased flap survival rate and capillary density. In addition, more endothelial progenitor cells were identified in peripheral blood and skin flaps in diabetic rats receiving atorvastatin. Atorvastatin treatment also restored the impaired function of diabetic endothelial progenitor cells in migration and tube formation.Atorvastatin notably promoted neovascularization and enhanced the viability of ischemic skin flaps in diabetic rats, which may be mediated at least partially by augmenting the number and restoring the functional capacity of endothelial progenitor cells.

The authors present a novel nasoalveolar molding protocol by prefabricating sets of nasoalveolar molding appliances using three-dimensional technology.Prospectively, 17 infants with unilateral complete cleft lip and palate underwent the authors' protocol before primary cheiloplasty. An initial nasoalveolar molding appliance was created based on the patient's first and only in-person maxillary cast, produced from a traditional intraoral dental impression. Thereafter, each patient's molding course was simulated using computer software that aimed to narrow the alveolar gap by 1 mm each week by rotating the greater alveolar segment. A maxillary cast of each predicted molding stage was created using three-dimensional printing. Subsequent appliances were constructed in advance, based on the series of computer-generated casts. Each patient had a total three clinic visits spaced 1 month apart. Anthropometric measurements and bony segment volumes were recorded before and after treatment.Alveolar cleft widths narrowed significantly (p0.01), soft-tissue volume of each segment expanded (p0.01), and the arc of the alveolus became more contiguous across the cleft (p0.01). One patient required a new appliance at the second visit because of bleeding and discomfort. Eleven patients had mucosal irritation and two experienced minor mucosal ulceration.Three-dimensional technology can precisely represent anatomic structures in pediatric clefts. Results from the authors' algorithm are equivalent to those of traditional nasoalveolar molding therapies; however, the number of required clinic visits and appliance adjustments decreased. As three-dimensional technology costs decrease, multidisciplinary teams may design customized nasoalveolar molding treatment with improved efficiency and less burden to medical staff, patients, and families.Therapeutic, IV.

Necrotizing fasciitis is perhaps the deadliest form of soft-tissue infection, characterized by a fulminant course and a high mortality rate.1 Meleney in 1924 gave the first modern description of this clinical entity when he introduced the term “acute hemolytic streptococcal gangrene.”2 The term necrotizing fasciitis, coined by Wilson in 1952, is perhaps the most accurate term describing the key features of this infectious process.3 Although our understanding of the pathophysiology of necrotizing fasciitis continues to improve, physicians confronted with this clinical entity continue to thread through “Meleney’s minefield,” fraught with dangers of delayed or even missed diagnosis with catastrophic consequences. In this article, we present a case that we recently managed and highlight the deceptively benign initial presentation of necrotizing fasciitis.

Background: Accurate determination of the depth of burn injury is difficult, even for experienced surgeons. The authors hypothesized that the level of plasma free hemoglobin following burn injury is correlated to the depth of burn injury, and they evaluated this hypothesis in a murine model. Methods: Full-thickness and partial-thickness burn injuries of varying sizes were inflicted on 38 and 36 male Wistar rats, respectively. Blood specimens were taken at 0, 15, 30, 45, and 60 minutes after burn injury, and the levels of plasma free hemoglobin were determined spectrophotometrically. Results: Full-thickness burns cause two times more hemolysis than partial-thickness burns (p < 0.05). A linear correlation was demonstrated between plasma free hemoglobin levels and total body surface area burned in both the full-thickness (r = 0.91, p < 0.001) and partial-thickness burn groups (r = 0.94, p < 0.001). The correlation between the quantity of hemolysis and the total body surface area burned was strongest at 15 minutes after the onset of burn injury. The levels of free hemoglobin peaked rapidly between 15 and 30 minutes after thermal injury and declined thereafter. Conclusions: The authors' data suggest that the level of plasma free hemoglobin after burn injury is related to the size and depth of burn injury. This test can potentially be a valuable diagnostic adjunct in the assessment of burns.

BACKGROUND The fibula flap has been widely used for reconstruction of composite bone and soft-tissue defects. The skin paddle of the fibula flap has played a critical role in providing a cutaneous component and good monitor for the bone component. In this report, the authors designed a perforator-based sural neurocutaneous flap instead of a peroneal perforator flap to enlarge a skin paddle for wound coverage. METHODS A novel chimeric flap was harvested based on the peroneal artery, and consisted of a sural neurocutaneous flap and a fibular graft. The sural neurocutaneous flap was elevated based on a single peroneal perforator and connected with the bone component by means of the peroneal artery. Two patients with complex bone and soft-tissue defects were treated by using this chimeric flap. In case 1, the radius defect was 8 cm in length and the soft-tissue defect measured 23 × 10 cm. In case 2, the distal radius was injured and the defect was 16 cm in length. The area of soft-tissue defect was 23 × 8 cm. In this case, the fibula head was used for wrist reconstruction. The soft-tissue defects in the two cases stretched across the palmar and dorsal surfaces of the forearm. RESULTS The lengths of the bone components were 10 and 16.5 cm, respectively, and the areas of the skin components were 24 × 12 cm and 25 × 10 cm, respectively. Both chimeric flaps survived completely without complications. The follow-up period was 42 months in case 1 and 16 months in case 2. Complete bone union was observed in both cases, and no remarkable degeneration of the fibular head occurred in case 2. The sural neurocutaneous flap showed good texture match and contour. No serious donor-site complications occurred. The two patients were satisfied with their daily lives relative to the severity of their injuries. CONCLUSION The chimeric flap composed of a sural neurocutaneous flap and a fibular graft by means of the peroneal artery is a good candidate for reconstruction of long bone defects associated with extensive soft-tissue defects in the extremities.

Background The sural neurofasciocutaneous flap has been widely used for reconstruction of soft-tissue defects in the lower leg, ankle, and foot. It can provide a large amount of tissue without compromising the function of the leg. The authors report the applications of the free peroneal perforator-based sural neurofasciocutaneous flap. Methods The free sural neurofasciocutaneous flap based on a single peroneal perforator is described. The peroneal perforator, which emerges from the posterior crural septum at the junction of middle and lower thirds of the fibula, was chosen as the pedicle of the flap. Six perforator-based sural neurofasciocutaneous flaps were transferred to resurface large soft-tissue defects in the upper limb. The size of the defects ranged from 15 × 6 cm to 45 × 10 cm. Results All six flaps survived completely without necrosis. The area of the flaps ranged from 16 × 8 cm to 30 × 10 cm. The average diameter of the peroneal perforator ranged from 1.0 to 1.5 mm, and the length of the perforator pedicles ranged from 4 to 6 cm. One patient developed arterial thrombosis after surgery, which was treated by removal of the thrombus and reanastomosis. No severe venous congestion was observed. All the patients were satisfied with the aesthetic outcome postoperatively at 3 to 12 months' follow-up. There were no serious donor-site complications. Conclusion The free perforator-based sural neurofasciocutaneous flap is a good alternative for reconstruction of extensive soft-tissue defects in the upper limb.
