Purpose: We aimed to elicit patients' views of research priorities related to pulmonary rehabilitation (PR). Method:We used a modified nominal group technique in which participants with chronic lung conditions who had completed PR attended one of five meetings during which they generated ideas, discussed them with the group, and prioritized their top three topics. Afterward, they completed member checking and satisfaction questionnaires. Results:Fourteen participants (mean age 71 years) prioritized 25 topics, which included music as a motivator, education about the reason for various assessments and exercises, and ongoing patient assessment and follow-up. Most participants "totally agreed" that the summarized topics matched what they thought was important in PR research (75%) and that the meeting was a positive experience (100%). Participants indicated that the findings of this study accurately reflected their priorities. Conclusions:Individuals who have completed PR may have questions about programme components including assessment, treatment, and strategies for promoting motivation. To inform future research projects and identify knowledge translation needs, we will distribute a questionnaire to a larger sample of former PR patients to further prioritize the 25 topics identified in the meetings. Méthodologie : les chercheurs ont utilisé une technique de groupe nominale modifiée selon laquelle les participants atteints d'une pneumopathie chronique qui ont effectué une RP ont assisté à l'une des cinq rencontres pour générer des idées, en parler en groupe et prioriser leurs trois thèmes principaux. Objectif : analyse systématique de l'effet de la mobilité précoce sur la durée d'hospitalisation (DH), la mortalité et les résultats cliniques dans le traitement des adultes hospitalisés à cause d'une pneumonie. Méthodologie : recherche dans quatre bases de données. Les critères d'inclusion étaient 1) une pneumonie aiguë chez l'adulte et 2) une intervention de mobilité précoce. Les chercheurs ont procédé à l'évaluation de la qualité au moyen de l'échelle de la base de données de la physiothérapie fondée sur les preuves et de l'échelle de Newcastle-Ottawa. Résultats : quatre études (trois essais cliniques aléatoires et une étude de cohorte rétrospective) respectaient les critères d'inclusion. La méta-analyse a démontré que la mobilité précoce ne réduisait pas le risque de mortalité par rapport aux soins habituels (risque relatif de 0,9 [IC 95 % : 0,27, 2,97]; p = 0,86), mais réduisait la DH moyenne (−1,1 jour [IC à 95 % : 2,21, −0,04]; p = 0,040). Par ailleurs, la mobilité précoce n'avait pas d'incidence sur le taux de réhospitalisations ou de consultations à l'urgence. Une étude a démontré une amélioration de la capacité fonctionnelle à l'exercice et à la qualité de vie liée à la fonction physique ainsi qu'une exécution plus rapide des mesures d'activités de la vie quotidienne. Purpose: We conducted a systematic review of the effect of early mobility on length of stay (LOS), mortality, and clinical outcomes as a treatment for adults hospitalized with pneumonia. Method: An electronic search of four databases was conducted. Inclusion criteria were (1) acute medical condition of pneumonia in adults and (2) early mobility intervention. Quality appraisal was conducted using the Physiotherapy Evidence Database scale and the Newcastle-Ottawa Scale. Results: Four studies (three randomized controlled trials and one retrospective cohort study) met the inclusion criteria. Meta-analysis demonstrated that early mobility did not reduce the risk of mortality compared with usual care (risk ratio 0.9 [95% CI: 0.27, 2.97]; p = 0.86) but did reduce the mean LOS (–1.1 days [95% CI: 2.21, –0.04]; p = 0.04). Early mobility also did not affect the rate of hospital readmissions or emergency department visits. One study demonstrated an improvement in functional exercise capacity and quality of life related to physical function and faster completion of a measure of activities of daily living. Conclusions: Early mobility reduced LOS in adults hospitalized with community-acquired pneumonia, although there was no effect on mortality or rate of hospital readmissions. Purpose: To investigate the opinions of final-year Canadian physiotherapy students of cardiorespiratory physiotherapy (CRP) and the factors influencing their decision about whether to pursue a career in CRP. Methods: A cross-sectional online survey was completed by final-year Master of Science of Physical Therapy students from three of the largest English-speaking physiotherapy schools in Canada. Results: A total of 120 students responded to the survey (overall response rate was 44%). Fifteen students (12.5%) responded that they were extremely or quite interested in specializing in CRP. The most common factors that positively influenced students’ decision to consider specializing in CRP were job accessibility, potential salary, and experiences in the area, and the most common factors that negatively influenced their decision were the clinical aspects of the area, their experiences in the area, job accessibility, and the influence of others. The most common factors that positively influenced students’ opinion of CRP were their clinical supervisor, educator, or lecturer; their own clinical experience; and evidence in the literature, and the most common factors that negatively influenced their opinion were their own clinical experience and their peers. Conclusion: Strategies focusing on increasing awareness of the role of physiotherapists in the care of patients with cardiorespiratory conditions, exposing students to the positive impact that physiotherapists have in this practice area, and good mentorship experiences may promote the attractiveness of this specialty. Purpose: To determine the prevalence of people with a diagnosis of chronic obstructive pulmonary disease (COPD) among those completing non-pulmonary rehabilitation (NPR) programmes and to describe their characteristics. Methods: Electronic data of participants who completed an in-patient rehabilitation programme between July 1, 2010, and July 1, 2012 were retrospectively reviewed. Data extracted were month and year of birth, sex, height, weight, referral source, admission and discharge dates, programme admitted to, reason for admission, most responsible health condition, number of co-morbidities, referral agency on discharge, and Functional Independence Measure (FIM) scores on admission and discharge. Results: The prevalence of COPD among participants who completed the NPR programmes was 7.5%. The most common reasons for admission were cardiac conditions (n=69, 20%), followed by post-unilateral hip replacement (n=40, 11%) and post-unilateral hip fracture (n=38, 11%). Patients were discharged after an average stay of 20 (SD 13) days. The mean FIM score was 91 (SD 11) at admission and 108 (SD 9) at discharge. Conclusions: The prevalence of a COPD diagnosis among participants in NPR programmes was 7.5%. COPD is a common comorbidity for people in rehabilitation programmes who have had amputations, have a cardiac condition, have undergone organ transplantation, or require complex care. Purpose: To examine the attitudes of a self-selected sample of Canadian physical therapists toward the transition from bachelor's to master's degrees and the implementation of clinical doctorate degrees in physical therapy (PT). Methods: A cross-sectional survey was conducted using a modified Dillman tailored approach. All eligible members of the Canadian Physiotherapy Association (CPA) were invited to participate. Results: Of 1,397 Canadian physical therapists who responded to the survey, 45% favoured the transition from bachelor's to master's degrees, 21% did not, and 34% were neutral; 27% favoured a transition from a master's to a doctoral degree for entry into practice in PT, 53% did not favour this transition, and 20% were neutral. Finally, 56% favoured the implementation of a post-professional clinical doctorate (PPCD) in PT, 23% did not, and 21% were neutral. Conclusions: Overall, a self-selected sample of Canadian physical therapists supported the future implementation of a post-professional clinical doctorate degree in PT but did not support an entry-to-practice doctoral degree. Purpose: To describe and compare the type and quality of evidence published in physical therapy (PT) journals during two time periods (2000-2002 vs. 2010-2012) and to explore scientific editors' opinions on changes in the types and quality of articles. Methods: A quantitative, longitudinal, retrospective journal review was used to categorize and assess the methodological rigour of items published in four PT journals using the Hedges Project. A quantitative, descriptive, cross-sectional survey explored the opinions of scientific editors. Percentages and frequencies of article types (as defined by the Hedges Project criteria), items passing rigour, and editor responses were calculated. Statistical significance of differences in article type and rigour between the two time periods was determined using Fisher's Exact Test. Results: There was a significant increase in original studies and review articles from 2000-2002 to 2010-2012 (p<0.001, p=0.002, respectively). The overall pass rate for rigour was 33.3% in 2000-2002 and 42.5% in 2010-2012, showing a significant increase (p=0.019). The majority of editors reported an increase in systematic reviews, qualitative designs, and randomized controlled trials and believed that quality had improved by 2010-2012. Purpose: To determine the interrater reliability (IRR) of the individual items in the Paediatric Cardiopulmonary Physiotherapy (CPT) Discharge Tool. This tool identifies six critical items that physiotherapists should consider when determining a paediatric patient's readiness for discharge from CPT after upper-abdominal, cardiac, or thoracic surgery: oxygen saturation, mobility, secretion retention, discharge planning, auscultation, and signs of respiratory distress. Methods: A total of 33 paediatric patients (ages 2 to <19 years) who received at least 1 day of CPT following cardiac, thoracic, or upper-abdominal surgery were independently assessed using the Paediatric CPT Discharge Tool by two designated assessors, who assessed each patient within 4 hours of each other. Results: Kappa analysis showed the following levels of interrater agreement for the six items of the Paediatric CPT Discharge Tool: Oxygen Saturation, excellent (κ=0.80); Mobility, substantial (κ=0.62); Secretion Clearance, moderate (κ=0.39); Discharge Planning, fair (κ=0.37); and Auscultation and Respiratory Distress, poor (κ=0.24 and κ=-0.08, respectively). Conclusion: Several of the items in the Paediatric CPT Discharge Tool demonstrate good IRR. The discharge tool is ready for further psychometric testing, specifically validity testing. Purpose: Rehabilitation is increasingly recognized as an important therapeutic intervention for people with cancer. The main objective of this study was to explore the current practice pattern and provision of oncology rehabilitation in Canada. Methods: A descriptive cross-sectional online survey was administered to Canadian facilities offering cancer treatment and/or listed as offering rehabilitation services during or after cancer treatment (cancer centres, rehabilitation hospitals, community centres, and private clinics). Results: Of the 116 sites contacted, 62 completed the questionnaire, 20 of which reported having an oncology rehabilitation programme. The majority of respondents indicated that they are not meeting their clients' rehabilitation needs. Rehabilitation programmes were provided by multidisciplinary health care teams, the majority of which included a physiotherapist. Funding and availability of resources were identified as the main barriers to the development of oncology rehabilitation programmes. Conclusions: Formal oncology rehabilitation programmes appear to be scarce, despite growing evidence that rehabilitation offers benefits across the cancer survivorship continuum. Purpose: To determine (1) the relationship among three common upper-limb tests for patients with chronic obstructive pulmonary disease (COPD): unsupported upper limb exercise test (UULEX), 6-minute pegboard and ring test (6PBRT), and a muscle-strength test using a hand-held dynamometer; and (2) the responsiveness of these three tests to changes after pulmonary rehabilitation that included a resistance arm-training programme. Methods: The study was a secondary analysis of a randomized controlled trial (RCT). The UULEX and the 6PBRT were used to measure peak arm exercise capacity and arm function, respectively. A handheld dynamometer was used to measure elbow and shoulder flexion force. We analyzed baseline data for all participants in the RCT, as well as baseline and post-PR data for those who completed 6-week follow-up testing. Results: 36 patients with COPD (mean forced expiratory volume in 1 second [FEV1]=35% [SD 15%] predicted; age 66 [9] y) participated, of whom 13 completed an arm-training programme. The correlations among the test results ranged from 0.41 to 0.81 (p<0.0001). Standardized response means were 1.0 for muscle force of elbow flexion, 1.2 for shoulder flexion, and 1.8 for the 6PBRT and UULEX. Conclusions: Although the three tests (UULEX, 6PBRT, and muscle-strength test using a hand-held dynamometer) are intended to measure different constructs, they were moderately to highly correlated with one another. Purpose: The purpose of this study was to develop a paediatric cardiopulmonary physiotherapy (CPT) discharge tool. We report on the initial stages of its development and the tool's sensibility (face/content validity, feasibility, and ease of usage). Methods: Using a modified Delphi technique, a panel of paediatric physiotherapy clinicians and academic leaders in the area of CPT (n=25) was recruited. Four rounds of discussion among the members of the Delphi panel focused on (1) generation of discharge items, (2) reduction of items, (3) discussion of contentious items and refinement of criterion definitions, and (4) determination of scoring options for the test instrument. The sensibility of a draft of the tool was assessed using a sample of convenience (n=15). Results: Six items (auscultation, discharge planning, mobility, oxygen saturation, secretion clearance, and signs of respiratory distress) were identified for inclusion in the tool. The global mean of all sensibility domains was 6.4 (median=6.6) of a possible 7.0. Conclusion: Using a modified Delphi process, we developed a six-item paediatric CPT discharge planning tool with good face and content validity. Future work will determine the scoring method for using this tool, interrater reliability, and predictive validity to facilitate optimal timing of hospital discharge for paediatric CPT patients. Purpose: To describe the provision of weekend physiotherapy (PT) services in tertiary-care hospitals in Canada. Methods: A prospective cross-sectional survey was conducted across tertiary-care hospitals, defined as those with university affiliation and at least one intensive care unit (ICU). Data were collected via telephone-administered questionnaires addressing hospital demographics, weekend staffing, workload, and weekend referral criteria. Results: A response rate of 84% (n=36) was obtained. Of facilities providing weekend PT services (97%), 35 (100%) provided care on Saturdays, 32 (91%) on Sundays, and 33 (94%) on statutory holidays. Weekend services were staffed using permanent full-time (n=35; 100%) or part-time (n=28; 80%) in-patient staff or outsourced staff (n=1; 3%). The number of physiotherapists available on the weekend was smaller than the number available during the week (p<0.001). Common weekend referral criteria included cardiorespiratory problems (n=35; 100%), postoperative assessment of patients at risk for deterioration (n=32; 91%), and patients scheduled for discharge pending PT assessment (n=30; 86%). Conclusion: Both the scope and the number of staff available to provide PT services were less on the weekend than during the week. Despite the use of common criteria for weekend referral, variability in this service exists. Knowledge pertaining to current weekend PT services provides opportunities for harmonization of service delivery. [ABSTRACT FROM AUTHOR]

Purpose: The aims of this study were (1) to describe the cardiorespiratory physiotherapy weekend service (PWS) at three tertiary hospitals in the Greater Toronto Area (GTA) and (2) to compare measures of staff burden among the clinical service areas in one of the hospitals that had a programme-based management structure. Method: Two focus-group meetings were held with physiotherapists from hospitals within the GTA. Thereafter, variables characterizing the PWS were collected over 8 months, using a standardized data-collection form. Results: A total of 632 data-collection forms were received. Response rates exceeded 75% at each hospital. Workload variables, including the number of patient visits, new referrals per hour, and the proportion of staff completing unpaid overtime, differed between the hospitals (p<0.002). There was no difference in any variable when data were compared between Saturday, Sunday, and statutory holidays (p>0.13). Workload measures varied between clinical service areas at the hospital that provided PWS using a programme-based approach. Conclusions: These findings highlight the important shortcomings of a programme-based management approach to providing PWS and may constitute a catalyst for change. [ABSTRACT FROM AUTHOR]

Purpose: The purpose of this study was to explore the perspectives of caregivers of persons with stroke with respect to their own physical activity. Methods: A qualitative, descriptive approach was used to study 10 caregivers of persons with stroke, recruited from a stroke exercise class in a large urban rehabilitation facility. Caregivers participated in individual, semi-structured interviews that were audiotaped, transcribed verbatim, and analysed using a constant comparative method. An inductive, iterative approach was applied to determine the codes and themes. Results: Four main themes were identified: change in role, change in activity, barriers to activity and health, and change in meaning of activity. Barriers to activity included guilt, time, and energy. Participants revealed that activity became more therapeutic after stroke and that participants preferred purposeful, functional, and partnered activities. Conclusions: These findings emphasize the importance of the husband–wife dyad and of movement toward a family-centred care approach. Education should be provided to caregivers regarding their role, barriers, and health-promoting activities. Future research should focus on determining appropriate physical-activity programmes for caregivers as well as on evaluating implementation of partnered exercise programmes for caregivers and persons with stroke. The article compares the nature and rehabilitation of individuals with chronic obstructive pulmonary disease (COPD) and HIV/AIDS. COPD is characterized by repeated acute exacerbations due to chronic bronchitis and it mostly affect men. HIV is a retrovirus that targets T cells which help the body in fighting off infection and disease and infections associated with this virus are increasing among women. Pulmonary rehabilitation is effective in improving the symptoms of COPD.

Purpose: This article summarizes the rationale, supporting evidence, and techniques used in training the inspiratory muscles in chronic obstructive pulmonary disease (COPD). Summary of Key Points: COPD is a progressive debilitating lung disease, often caused by smoking, that increases the work of breathing. Inspiratory muscle training (IMT) can alleviate dyspnea and improve exercise tolerance for people with this condition. Before devising a training program, the strength of the respiratory muscles needs to be evaluated by measuring maximal inspiratory pressure (PImax) and expiratory mouth pressure. In addition, inspiratory muscle endurance should be assessed by determining the maximal sustainable inspiratory mouth pressure for a specific endurance time or the maximal inspiratory threshold load that can be achieved during an incremental loading test. Evidence supports the beneficial effects of IMT when training is adequate in duration and frequency (30 minutes daily, 4 to 6 days per week), in mode (threshold or targeted resistive trainer), and in intensity (40 to 70% PImax). Supervision and monitoring are required during training to ensure that target training intensities are achieved, to evaluate patient tolerance, and to prevent CO2 retention, excessive fatigue, or exertion-induced inspiratory muscle injury.
