31. Effects of Er:YAG and Nd:YAG Lasers on Dentin Permeability in Root Surfaces: A Preliminary in Vitro Study

Abstract

Objective: This in vitro study evaluated the effects of Nd:YAG and Er:YAG lasers on reducing dentin permeability by sealing opened tubules. Background Data: According to hydrodynamic theory, dentine hypersensitivity occurs when dentin is exposed with tubules opened. Consequently, a painful sensation occurs due to an intensification of the dentinal permeability. Treatment, therefore, should be based on a decrease of this permeability, achieved by the obliteration of dentinal tubules. The Nd:YAG laser is known for its capacity to seal dentinal tubules; however, few studies concerning treatment with Er:YAG laser are available. Methods: The Nd:YAG laser was used, based on two parameters: (A) 1.0 W, 10 Hz, and (B) 1.5 W, 15 Hz. The Er:YAG laser was used at 60 mJ, 2 Hz, four applications of 20 sec each, at 6 mm from the surface. After irradiation, all samples were immersed in 1% Rodamine B dye solution, in order to evaluate the penetration of the dye solution and observe the decrease/increase of dentinal permeability after the laser treatment. Results: The laser conditions used in the present study decreased the permeability as follows: (a) when using the Er:YAG laser at 60 mJ, 2 Hz in 26.05%, and (b) when using the Nd:YAG laser at 1.5 W, 15 Hz in 19.03%—with no statistical difference between them. Additionally, the Nd:YAG laser at 1.0 W, 10 Hz, decreased permeability in 4.59%, with a smaller effect. Conclusions: According to the results of this in vitro study, the Er:YAG laser at 60 mJ, 2 Hz, and the Nd:YAG laser at 1.5 W, 15 Hz are useful for decreasing dentin permeability.

32. Intrapulpal Temperature during Preparation with the Er:YAG Laser: An in VitroStudy

Abstract

Objective:This investigation evaluated the variation of the intrapulpal temperature when dentine was irradiated by the Er:YAG laser. Background Data:The effect of preparation with the Er:YAG laser on the intrapulpal temperature is probably the biggest problem in using the laser for preparation of dental hard tissue. Materials and Methods: Seventy-two bovine incisors were studied that had the enamel and dentine of the buccal surface polished to a thickness of 2.0 mm. The teeth were divided into three groups, according to the repetition rate used (Group I = 2 Hz, Group II = 4 Hz, and Group III = 6 Hz), and irradiated, with or without water cooling, using 250, 300, and 350 mJ of energy per pulse. Thermocouples were introduced inside the pulp chamber through the palatine opening of the samples and fixed to the vestibular wall of the pulp chamber using a thermal paste. Results:It was verified that there was a decrease of the intrapulpal temperature for all of the parameters in the Group I irradiated with water cooling and for the parameters of 350 mJ/4 Hz with water cooling. The other irradiations showed an increase of the intrapulpal temperature, varying from 0.03°° to 2.5°°C. Conclusion:We conclude that the use of the Er:YAG laser promoted acceptable temperature increases inside the pulp chamber. However, we do not recommend this procedure without water cooling because macroscopic observations of the dentine irradiated without water cooling showed dark lesions, suggesting carbonization of this tissue.

Abstract

Objective: This in vitro study examines the efficacy of two different dental whitening agents, Opalescence Xtra and Opus White, by analyzing the change in color achieved by the treatment and the temperature increase induced in the pulpal chamber., Background Data: Bleaching techniques achieved significant advances with the use of coherent or incoherent radiation sources to activate the bleaching chemicals., Methods: The bleaching agents, containing 35% of hydrogen peroxide, were stimulated with 0.9 W of xenon arc lamp and 0.9 W or 2 W of a 960-nm diode laser during 60 sec (0.9 W) and 30 sec (2 W) on 33 extracted human teeth. During irradiation, the temperature in the pulpal cavity was monitored. The color change was evaluated using the CIE L*a*b* color space measurement system., Results: The treated groups showed an increase in color saturation (DeltaC*) of 3-32% and a change in whiteness (DeltaL*) of 0-8%. This study found that only some of the irradiated groups show statistically significant difference (p < 0.05) in the effectiveness of their treatment when compared to the control, whereas no significant statistical difference was obtained in between the irradiated groups. Temperature increase was 2-4 degrees C when using the xenon arc lamp, 2-8 degrees C and 4-12 degrees C when using the diode laser at 0.9 W and 2 W, respectively., Conclusions: The results of this study suggest that Opalescence Extra and Opus White are both effective to provide brighter teeth. However, according to the conditions used in this study, only the xenon arc lamp induced a safer temperature increase.
