Atherosclerosis is a systemic disease affecting the coronary, carotid, intracerebral, renal and peripherial arteries. The early morphological and functional impairments could be detected in the second or third decades of life and their progression depend on the number and severity of risk factors and individual susceptility. Although the vascular risk factors (smoking, overweight, age, unhealthy diet, lack of physical exercise, hypertension, diabetes mellitus, chronic kidney disease and dyslipidemia) are the same and common in the different vascular diseases, the present clinical routine artificially classifies the diagnosis and therapy of different vascular diseases into different subfields of medicine with the negative impact of possible polypragmasia. Recently, worldwide health surveys (e.g. REACH registry) have proven the usefulness of a holistic approach in the diagnosis and therapy of multiorgan-affected vascular patients. This review summarizes the multidisciplinary advances and future perspective of vascular diseases.

Unlabelled: Medical doctors are not only highly trained professionals but also role models in matters of health and disease. That is what makes the health behaviour of future doctors an important research issue., Aims and Methods: A health behaviour survey was conducted among a representative sample of medical students of a university in Hungary by a standardized self-administered questionnaire. The questionnaire related to demographic data, health status and health behaviour using validated items from previous surveys., Results: Self-perceived (subjective) health was found to be good/very good in more than three-quarters of the students; nearly all of them thought that they can do much/very much for their health. Nearly two-thirds of the students are non-smokers, but 15% smoke daily. More than four-fifth of the students drink alcohol, mainly occasionally. One-quarter had already tried some drugs, mostly marijuana and non-prescription narcotics and sedatives. Comparison of our data to the same age-group of the general population revealed that the proportion of smokers, daily smokers, and weekly alcohol drinkers is lower among medical students., Conclusions: The prevalence of risk behaviour does not seem to have risen in the past years based on comparing our data to those of earlier surveys conducted among medical students in Hungary. However, students had a significantly higher mean score in the general health questionnaire compared to the mean of the same age group of the general population indicating mental health problems that deserve attention.

Study Objective: To develop, introduce and test a methodology, which provides valid data about the prevalence and incidence of chronic, non-communicable diseases of great public health importance., Design: The School of Public Health, University of Debrecen, Hungary and the National Public Health and Medical Officer Service the first time launched a morbidity sentinel stations network of general practitioners in four counties in Hungary in May 1998. Within the framework of this program the participating general practitioners reported the prevalence data of cardiovascular diseases, diabetes mellitus, liver cirrhosis, and major malignant diseases at the beginning, and from than on continuously report the incidence of these diseases. The authors built quality assurance into the program at different levels in order to ensure high quality data., Patients: The study population consists of people belonging to the participating general practitioners' practices at any time, selected in a way to represent the eastern and western part of the country as well as the participating practices in the counties (Gyór-Moson-Sopron, Hajdú-Bihar, Szabolcs-Szatmár-Bereg, Zala) would give a geographically and according to settlement size representative sample of general practitioners in those counties. Eighty general practitioners were approached in the four counties by the county offices and overall 73 of them, 1.4% of the total practices in Hungary (5212), agreed to participate in the study, therefore information can be collected on 1.37% of the Hungarian population's (138,088 people) morbidity status., Main Results: The population of practices participating in the program gives a representative sample of the counties population by age and sex. The prevalence of hypertension, diabetes mellitus and liver cirrhosis is high in all the counties involved. In most of the selected diseases the authors have found differences in the prevalence between the eastern and western part of Hungary with higher values in the western counties. The differences were most apparent in the older age groups in both sexes., Conclusions: The development and sustainability of primary care based, sentinel stations type, euroconform morbidity data collection system is undoubtedly justified in Hungary. The collected and thoroughly analysed data provide not only a valid and comprehensive basis to describe some aspects of the health status of the Hungarian population but also gives an opportunity to continuously monitor the changes in morbidity of the selected diseases. Thus, the program serves an important basis for health care capacity building, priority setting and the evaluation of the effectiveness of public health interventions.

The status and problems of the roma (gipsy) population have been in the forefront in Hungary and have called for numerous benevolent interventions. Successful planning and implementation of programs aimed at the improvement of their health status must be based on solid facts regarding their problems and the causes behind. The authors give a literature review on research papers discussing the health (disease) status of the Hungarian roma population published between 1980 and 2001. They give a summary on the demography of gypsies, an overview of publications on pregnancy, delivery and infant mortality, on adult morbidity and mortality, on genetic investigations among roma people, as well as on their health behaviour and relations with the health care system, and finally, they give a brief overview of their socio-economic status. The authors sum up the major difficulties of research aimed at roma people, express their concern regarding health research papers published on gypsies; and outline their recommendation on the future direction of research on the health of the roma population.

Chronic myelogenous leukaemia is a clonal myeloproliferative stem cell disease. Its cytogenetical hallmark is the Philadelphia chromosome (Ph) or the BCR/ABL fusion gene. Their identification is important both in the diagnosis and the follow-up of the disease. In our department we have investigated the BCR/ABL gene arrangement in 21 patients with fluorescence in situ hybridization. The aim of the analysis in freshly suspected patients without any previous therapy was to confirm diagnosis and mapping the ratio of Philadelphia positive cells. In contrast to the 95-100% Ph-positivity of mononuclear cells by classical cytogenetical examinations we found BCR/ABL gene arrangement only in various but always lower proportions. Therefore the latter examination gives a better representation of residual normal hemopoesis. Out of 9 patients who had received interferon treatment for at least 6 months, 4 gave a major, 4 a minor cytogenetical answer and in 1 case there was no cytogenetical response. Seven patients reached a complete and 2 a partial hematological remission. Among 5 other patients receiving interferon treatment, in 2 cases with double Ph-positivity we found a rapid progression. The data of 3 patients had to be excluded from the evaluation due to the so far short following time.

It is a well known fact that in Hungary--as in the European Countries, in general--the trends of premature mortality are mainly determined by the trends of mortality caused by cardiovascular diseases. The timeliness of our present study on changes in trends of early cardiovascular mortality in the period of 1970-1997 in Hungary in comparison with trends of EU countries is underlined by the upcoming access of Hungary to the European Union. The evaluation is based on WHO data, the relative risk of premature mortality due to different forms of cardiovascular diseases for different sex and age groups of the Hungarian population is expressed as a ratio between standardized mortality rates of Hungarian groups and that of EU-average and of countries specified. Detailed data demonstrate that the risk of early death caused by cardiovascular--especially ischaemic heart and cerebrovascular--diseases, in contrast with EU countries, is significantly increased in Hungary, most significantly in age groups of 35-44 and 45-64 years for both sexes. The authors' results draw the attention to the possible shortcomings of health care systems and the lack of comprehensive health promotion (including prevention) programs in Hungary.
