Abstract

We report a case of ocular hypotony secondary to a cyclodialysis cleft that developed following a phacoemulsification procedure in which a scleral tunnel incision was used. A cyclodialysis cleft should be suspected in cases of persistent postoperative hypotony.

Abstract

A bleb infection and subsequent endophthalmitis developed in the left eye of a 68-year-old man who had had a trabeculectomy. Vitreous injections of vancomycin and gentamicin were given, and vitreous cultures grew alpha-Streptococcus. Postoperatively, as the intraocular inflammation resolved, intraocular pressure (IOP) markedly decreased secondary, in our judgment, to ciliary body ischemia. A pars plana vitrectomy relieved the tractional bands and restored the IOP.

Abstract

A 35-year-old woman had a circumscribed pigmented scleral lesion overlying a pigmented mass in the ciliary body and trabecular meshwork. Sclerectomy and iridocyclectomy were performed. Histologically, the lesion proved to be a melanocytoma. This represents to our knowledge the fourth reported case of scleral extension of a ciliary body melanocytoma.

Abstract

Because of decreased scleral rigidity of tissue, penetrating keratoplasty in the pediatric age group is difficult. We have applied an early keratoplasty technique of graft fixation with indirect "bridge" or "overlay" sutures to temporarily stabilize the graft and facilitate wound closure.

5. Exposure keratitis following cataract surgery on the fellow eye.

Abstract

Two patients developed exposure keratitis in the nonoperated eye during intraocular surgery under local anesthesia. These cases demonstrate that potentially serious injuries may occur to the fellow eye even with the use of local anesthesia and that special precautions should be taken to avoid this problem.

7. Effect of ocular compression on intraocular pressure.

Abstract

A prospective double-blind controlled study was undertaken to determine the effects of different compression times applied to soften the eye on normal subjects. Application of the Honan intraocular pressure reducer (HIPR) produced a significantly (p less than 0.02) greater decrease in intraocular pressure when applied for 40 minutes (14.3 mmHg SD = 1.7) when compared to a five-minute application (8.8 mmHg SD = 1.7). No untoward effects were noted in these patients. Physiologic effects of external ocular compression are discussed.
