In this article, the author discusses the study on the benefits of regional nodal irradiation (RNI) in high-risk disease post breast-conserving surgery. He contends that the study extends the indications for RNI to all women with positive lymph nodes receiving full axillary lymph node dissection (ALND). He argues that the study does not directly answer the question of how manage women with positive sentinel lymph nodes with micrometastases.

It focuses on the randomized trial, MAP.3, conducted by the National Cancer Institute of Canada Clinical Trials Group (NCIC CTG) which aims to detect reduction in annual incidence of invasive breast cancer (IBC) using exemestane versus placebo. The study concluded that exemestane significantly reduced invasive breast cancers in postmenopausal women.
