A blood oxygenation level-dependent (BOLD)-based apparent relative oxygen extraction fraction (rOEF) as a semi-quantitative marker of vascular deoxygenation has recently been introduced in clinical studies of patients with glioma and stroke, yielding promising results. These rOEF measurements are based on independent quantification of the transverse relaxation times T2 and T2* and relative cerebral blood volume (rCBV). Simulations demonstrate that small errors in any of the underlying measures may result in a large deviation of the calculated rOEF. Therefore, we investigated the validity of such measurements. For this, we evaluated the quantitative measurements of T2 and T2* at 3 T in a gel phantom, in healthy subjects and in healthy tissue of patients with brain tumors. We calculated rOEF maps covering large portions of the brain from T2, T2* and rCBV [routinely measured in patients using dynamic susceptibility contrast (DSC)], and obtained rOEF values of 0.63 ± 0.16 and 0.90 ± 0.21 in healthy-appearing gray matter (GM) and white matter (WM), respectively; values of about 0.4 are usually reported. Quantitative T2 mapping using the fast, clinically feasible, multi-echo gradient spin echo (GRASE) approach yields significantly higher values than much slower multiple single spin echo (SE) experiments. Although T2* mapping is reliable in magnetically homogeneous tissues, uncorrectable macroscopic background gradients and other effects (e.g. iron deposition) shorten T2*. Cerebral blood volume (CBV) measurement using DSC and normalization to WM yields robust estimates of rCBV in healthy-appearing brain tissue; absolute quantification of the venous fraction of CBV, however, is difficult to achieve. Our study demonstrates that quantitative measurements of rOEF are currently biased by inherent difficulties in T2 and CBV quantification, but also by inadequacies of the underlying model. We argue, however, that standardized, reproducible measurements of apparent T2, T2* and rCBV may still allow the estimation of a meaningful apparent rOEF, which requires further validation in clinical studies.

The aim of this study was to quantify the response of the myocardial transverse relaxation times (ΔT2*) to hyperoxic respiratory challenge (HRC) at different field strengths in an intra-individual comparison of healthy volunteers and in a patient with coronary artery disease. Blood oxygenation level-dependent (BOLD) cardiovascular MR (CMR) data were acquired in 10 healthy volunteers (five women, five men; mean age, 29 ± 3 years; range, 22-35 years) at 1.5 and 3.0 T. Medical air (21% O2 ), pure oxygen and carbogen (95% O2 , 5% CO2 ) were administered in a block-design temporal pattern to induce normoxia, hyperoxia and hyperoxic hypercapnia, respectively. Average T2* times were derived from measurements by two independent and blind readers in 16 standard myocardial segments on three short-axis slices per patient. Inter- and intra-reader correlations of T2* measurements were good [intra-class correlation coefficient (ICC) = 0.75 and ICC = 0.79, both p 0.23). The myocardial ∆T2* response to HRC can reliably be imaged and quantified with BOLD CMR at both 1.5 and 3.0 T. During HRC, hyperoxia and hyperoxic hypercapnia induce a significant increase in T2*, with ∆T2* being largest at 3.0 T and during hyperoxic hypercapnia in normal myocardial segments.

The objective of this study is to develop improved methods for renal blood oxygenation level dependent (BOLD) imaging. T2* mapping of the kidneys, or renal BOLD imaging, may depict renal oxygen levels and may be valuable as a noninvasive means of following the progression of renal disease. Current renal BOLD data is limited by imaging in a single breath hold, which results in low resolution and low signal-to-noise ratio (SNR). We compare a new free-breathing renal BOLD method with conventional breath-hold BOLD (BH-BOLD). A multi-echo GRE sequence with continuous prospective respiratory navigation and real-time feedback was developed that allows high resolution and high SNR renal BOLD imaging with constant sequence repetition time (TR) during free-breathing BOLD (FB-BOLD). The sequence was evaluated in 10 normal volunteers and compared with conventional BH-BOLD. Scan time for the FB-BOLD sequence was approximately three minutes, compared with 15 seconds for the BH-BOLD sequence. SNR of source images and residual error of T2* fitting were compared between the two methods. The FB-BOLD sequence produced motion-free T2* maps of the kidneys with SNR 1.9 times higher than BH-BOLD images. Residual error of T2* fitting was consistently lower in the right kidney with FB-BOLD (30% less than BH-BOLD) but higher in the left kidney (80% more than BH-BOLD), likely related to placement of the navigator on the right hemidiaphragm. A free-breathing prospectively navigated renal BOLD sequence allows flexible tradeoff between scan time, resolution, and SNR.

The dynamics of the blood oxygenation level-dependent (BOLD) response are dependent on changes in cerebral blood flow, cerebral blood volume and the cerebral metabolic rate of oxygen consumption. Furthermore, the amplitude of the response is dependent on the baseline physiological state, defined by the haematocrit, oxygen extraction fraction and cerebral blood volume. As a result of this complex dependence, the accurate interpretation of BOLD data and robust intersubject comparisons when the baseline physiology is varied are difficult. The calibrated BOLD technique was developed to address these issues. However, the methodology is complex and its full promise has not yet been realised. In this review, the theoretical underpinnings of calibrated BOLD, and issues regarding this theory that are still to be resolved, are discussed. Important aspects of practical implementation are reviewed and reported applications of this methodology are presented.

The characterisation of the extravascular (EV) contribution to the blood oxygenation level-dependent (BOLD) effect is important for understanding the spatial specificity of BOLD contrast and for modelling approaches that aim to extract quantitative metabolic parameters from the BOLD signal. A new interpretation is proposed for stimulus-induced signal changes in diffusion-weighted functional MRI. T2-weighted spin-echo echo-planar images were acquired at different diffusion-weightings while visual stimulation was presented to human volunteers. The amplitudes of the positive stimulus-correlated response and post-stimulus undershoot (PSU) in the functional time-courses were found to follow different trends as a function of b-value. Data were analysed using a three-compartment signal model, with one compartment being purely vascular and the other two dominated by fast- and slow-diffusing molecules in the brain tissue. The diffusion coefficients of the tissue were assumed to be constant throughout the experiments. It is shown that the stimulus-induced signal changes can be decomposed into independent contributions originating from each of the three compartments. After decomposition, the fast-diffusion phase displays a substantial PSU, while the slow-diffusion phase demonstrates a highly reproducible and stimulus-correlated time-course with minimal undershoot. The decomposed responses are interpreted in terms of the spin-echo blood oxygenation level dependent (SE-BOLD) effect, and it is proposed that the signal produced by fast- and slow-diffusing molecules reflect a sensitivity to susceptibility changes in arteriole/venule- and capillary-sized vessels, respectively. This interpretation suggests that diffusion-weighted SE-BOLD imaging may provide subtle information about the haemodynamic and neuronal responses. Copyright © 2009 John Wiley & Sons, Ltd.

Brain temperature is determined by the interplay between the cerebral metabolic rate of oxygen (CMRO2) and cerebral blood flow (CBF). In this study, single-voxel 1H nuclear MRS, with an accuracy of +/-0.2 degrees C for temperature determination, was used at 3 T to measure human brain temperature during visual stimulation (which increases both CBF and CMRO2) and hypercapnia (which increases CBF only). Visual stimulation had no detectable effect on brain temperature in the parenchyma showing blood oxygenation level dependent activation. Hypercapnia, leading to an increase in the end tidal CO2 by 8 +/- 2 mm Hg above the baseline, caused a short-lasting decrease in brain temperature of 0.30 +/- 0.33 degrees C. These results indicate that increased CBF may be a key factor, bringing about a small decrease in brain temperature during brain activation. However, the increase in CBF is not sufficient to lower brain temperature in the presence of a concomitant increase in endogenous heat production.

This paper describes the use of off-resonance saturation to further manipulate the blood oxygenation level dependent (BOLD) contrast of fMRI. A customized narrow bandwidth radiofrequency pulse, applied with a range of frequency offsets prior to selection of each slice, was designed and incorporated into a gradient echo EPI sequence. This application takes advantage of the resonance frequency and linewidth differences between the oxygenated and deoxygenated state of blood in human brain during task activation and rest, and is capable of creating an enhancement in the contrast of the BOLD effect. Because of a possible contribution to the signal change from cerebro-spinal fluid, which has a much narrower linewidth and smaller frequency shift compared with the brain tissue, data were also collected using a nulling inversion pulse. The inversion pulse was applied before the off-resonance pulse and data acquisition to eliminate the CSF signal. Functional areas are thus more localized to the brain tissue.

International audience; Blood oxygen saturation (SO(2)) is a promising parameter for the assessment of brain tissue viability in numerous pathologies. Quantitative blood oxygenation level-dependent (qBOLD)-like approaches allow the estimation of SO(2) by modelling the contribution of deoxyhaemoglobin to the MR signal decay. These methods require a high signal-to-noise ratio to obtain accurate maps through fitting procedures. In this article, we present a version of the qBOLD method at long TE taking into account separate estimates of T(2), total blood volume fraction (BV(f)) and magnetic field inhomogeneities. Our approach was applied to the brains of 13 healthy rats under normoxia, hyperoxia and hypoxia. MR estimates of local SO(2) (MR_LSO(2)) were compared with measurements obtained from blood gas analysis. A very good correlation (R(2) = 0.89) was found between brain MR_LSO(2) and sagittal sinus SO(2).

Modified gradient-echo MR techniques were applied to study the effects of inflow on functional brain imaging studies using visual and motor cortex stimulation. The results demonstrate that the large signal changes, seen in previously reported gradient-echo studies at 1.5-2.0 T, are dominated by direct inflow effects, in particular when using a large flip angle and a thin slice. The findings suggest that inflow-based functional imaging, along with Blood Oxygenation Level Dependent (BOLD) functional MRI, may play an important role in future research towards the functional organization of the human brain.
