The article describes the case of a 39-year-old man with acquired immunodeficiency syndrome (AIDS) who had successful cardiac transplantation even though his serologic status was known. Despite the progress made in managing human immunodeficiency virus (HIV) infection, there is still a small number of solid-organ transplantations in HIV-positive patients. The chemotherapy of the patient was complicated by the other conditions like leukopenia and anemia. In 1995, his liposomal daunorubicin therapy was replaced with paclitaxel therapy because he presented shortness of breath. In 2001, he was placed on the waiting list for cardiac transplantation because it is believed that his dilated cardiomyopathy resulted from daunorubicin toxicity.
