Objective: Hearing loss after removal of vestibular schwannomas with preservation of the cochlear nerve can result from labyrinthine injury of the posterior semicircular canal and/or common crus during drilling of the posterior wall of the internal auditory meatus. Indeed, there are no anatomic landmarks that intraoperatively identify the position of the posterior semicircular canal or of the common crus. We investigated the usefulness of image guidance and endoscopy for exposure of the internal auditory canal (IAC) and its fundus without labyrinthine injury during a retrosigmoid approach., Methods: A retrosigmoid approach to the IAC was performed on 10 whole fresh cadaveric heads after acquiring high-resolution computed tomographic scans (120 kV; slice thickness, 1 mm; field of vision, 40 cm; matrix, 512 x 512) with permanent bone-implanted reference markers. Drilling of the posterior wall of the IAC was executed with image guidance. Its most lateral area was visualized using endoscopy., Results: Target registration error for the procedure was 0.28 to 0.82 mm (mean, 0.46 mm; standard deviation, 0.16 mm). The measured length of the IAC along its posterior wall was 9.7 +/- 1.6 mm. The angle of drilling (angle between the direction of drill and the posterior petrous surface) was 43.3 +/- 6.0 degrees, and the length of the posterior wall of the IAC drilled without violating the integrity of the labyrinth was 7.2 +/- 0.9 mm. The surgical maneuvers in the remaining part of the IAC, including the fundus, were performed using an angled endoscope., Conclusion: Frameless navigation using high-resolution computed tomographic scans and bone-implanted reference markers can provide a "roadmap" to maximize safe surgical exposure of the IAC without violating the labyrinth and leaving a small segment of the lateral IAC unexposed. Further exposure and surgical manipulation of this segment, including the fundus without additional cerebellar retraction and labyrinthine injury, can be achieved using an endoscope. Use of image guidance and an endoscope can help in exposing the entire posterior aspect of the IAC including its fundus without violating the labyrinth through a retrosigmoid approach. This technique could improve hearing preservation in vestibular schwannoma surgery.

Objective: To introduce a novel surgical technique for the dissection of the greater superficial petrosal nerve (GSPN) in the middle fossa approach., Methods: Interdural temporal elevation was performed with a front-to-back technique to preserve the GSPN in 12 sides of 6 injected cadaveric heads dissected through a middle fossa approach., Results: The GSPN emerged from the facial hiatus in a shallow bony groove proximally, ran into a deeper sphenopetrosal groove, and eventually reached the mandibular nerve. With front-to-back dissection, this nerve was easily identified at the posterior border of the mandibular nerve. Dissection from front to back minimized the retraction force applied to the proximal part of the GSPN, which was preserved in all specimens., Conclusion: The temporal dura can be elevated safely with a front-to-back technique to preserve the GSPN and to help maintain the physiological integrity of the facial nerve.

Objective: We have developed an interactive virtual model of the temporal bone for the training and teaching of cranial base surgery., Methods: The virtual model was based on the tomographic data of the Visible Human Project. The male Visible Human's computed tomographic data were volumetrically reconstructed as virtual bone tissue, and the individual photographic slices provided the basis for segmentation of the middle and inner ear structures, cranial nerves, vessels, and brainstem. These structures were created by using outlining and tube editing tools, allowing structural modeling either directly on the basis of the photographic data or according to information from textbooks and cadaver dissections. For training and teaching, the virtual model was accessed in the previously described 3-dimensional workspaces of the Dextroscope or Dextrobeam (Volume Interactions Pte, Ltd., Singapore), whose interfaces enable volumetric exploration from any perspective and provide virtual tools for drilling and measuring., Results: We have simulated several cranial base procedures including approaches via the floor of the middle fossa and the lateral petrous bone. The virtual model suitably illustrated the core facts of anatomic spatial relationships while simulating different stages of bone drilling along a variety of surgical corridors. The system was used for teaching during training courses to plan and discuss operative anatomy and strategies., Conclusion: The Virtual Temporal Bone and its surrounding 3-dimensional workspace provide an effective way to study the essential surgical anatomy of this complex region and to teach and train operative strategies, especially when used as an adjunct to cadaver dissections.

Objective: To assess the advantages and disadvantages of the retrosigmoid intradural suprameatal approach by studying the microsurgical anatomy. This study was performed primarily to assess the advantages of the retrosigmoid intradural suprameatal approach by measuring the amount of increased exposure it provides for lesions of the cerebellopontine and petroclival region as well as to identify the disadvantages of the approach., Methods: Twenty sides of 10 cadaver heads (embalmed and injected) were dissected under x3 to x40 magnification. A standard retrosigmoid craniotomy was made. The cerebellopontine cistern was entered to expose the neurovascular structures, and the internal auditory canal was opened by drilling the margin of the internal auditory meatus. After this, the suprameatal tubercle was drilled, followed by additional drilling to resect the petrous apex. The trigeminal root was mobilized completely after opening Meckel's cave. During drilling, care was taken to preserve the posterior and superior semicircular canals, petrosal sinus, and the internal carotid artery., Results: The approach enhanced the exposure of the cerebellopontine cistern and Meckel's cave. There was an additional exposure of 10.7 +/- 1.16 mm length of trigeminal nerve on the right side and an additional 10.7 +/- 1.25 mm on the left. This helped to mobilize and further retract the trigeminal root. Although it facilitated the view of the neurovascular structures medial to the internal acoustic meatus, the depth of exposure did not vary much from a traditional retrosigmoid approach nor did it increase the angle of exposure or the visualization of the clivus and more medially located structures., Conclusion: This approach is suitable for lesions mainly in the posterior fossa with some extension into the middle fossa in the anterolateral direction. The key benefits of this approach are the length of trigeminal nerve exposure and the subsequent mobilization that improves visualization of the structures medial to the internal auditory canal, to the petrous apex, Meckel's cave, and the posterior end of the cavernous sinus.

Objective: The location of the superior semicircular canal (SSC) is often determined intraoperatively based on its topographic association with the arcuate eminence (AE). This determination is not always possible because of the potential variability in the relationship between these two structures. The goal of this study was to describe the three-dimensional (3-D) relationship between the AE and SSC using 3-D computed tomography (CT) and to evaluate the utility of 3-D CT for preoperative planning for surgical approaches to the middle cranial fossa., Methods: We studied 11 patients (22 sides) radiographically using 0.8- to 1-mm thick reconstructed CT images. A standard set of structural relationships was measured between the AE, SSC, and other regional landmarks., Results: 3-D CT clearly demonstrated the relationships between traditional landmarks along the petrous ridge and middle cranial fossa. The relationship between the arcuate eminence and SSC was found to be highly variable. The average distance between the tips of the two structures was found to be 5.7 mm (range, 2.7-10.4 mm)., Conclusions: There is significant variability in the relationship between the AE and the SSC. The AE is not a consistent or reliable landmark for identifying the precise position of the SSC. Detailed preoperative information regarding the relationship between the AE, SSC, and other bony landmarks can be easily and quickly assessed using 3-D CT.

Objective: Microvascular compression syndromes such as trigeminal neuralgia, hemifacial spasm, and disabling positional vertigo involve an artery or vein compressing a cranial nerve. A cranial nerve is composed of a central nervous system (CNS) segment and a peripheral nervous system (PNS) segment separated by the root entry/exit zone (REZ). Although vascular compression can occur at any point along the cranial nerve, it has been generally assumed that only vascular contact at the REZ of the affected cranial nerve can cause symptoms. On the basis of personal surgical experience, we propose that vascular compression of the CNS segment alone causes symptoms. This has important repercussions for the future diagnosis and treatment of microvascular compression syndromes, especially the cochleovestibular compression syndrome., Methods: For the anatomic study, four autopsy specimens and one surgical biopsy specimen of the vestibulocochlear nerve were microscopically and ultramicroscopically analyzed for structural differences between the CNS and PNS segments. For the clinical study, five patients with the clinical picture of cochleovestibular compression syndrome were treated by microsurgical decompression at the level of the CNS segment and not the REZ. One patient underwent reoperation for recurrent symptoms 4 years later, and a 4-mm vestibular neurectomy was performed at that stage. We performed an epidemiological analysis to demonstrate that the known incidences of trigeminal neuralgia, hemifacial spasm, and glossopharyngeal neuralgia are related to the length of their respective CNS segments., Results: Histological differences between the PNS and CNS segments suggest that the PNS segment is more resistant to compression. This was confirmed by neurophysiological data from intraoperative monitoring in posterior fossa surgery and experimental studies. We found a clear epidemiological correlation between the length of the CNS segment, which differed among cranial nerves, and the incidence of the microvascular compression syndrome. Successful decompression of the CNS segment in patients without compression at the REZ of the vestibulocochlear nerve for disabling positional vertigo provides clinical support for this hypothesis., Conclusion: The evidence we present supports the hypothesis that vascular compression syndromes arise from vascular contact along the CNS segment of the cranial nerves.

Objective: The terms superior vestibular nerve and inferior vestibular nerve have been used in the field of neurosurgery to indicate anatomically the two respective vestibular components of the vestibulocochlear nerve. To reappraise the aptness of this terminology, fascicular patterns and the anatomic relationship of the vestibular and cochlear components were examined., Methods: Twenty vestibulocochlear nerve specimens were obtained from cadavers. The nerves were excised, with care taken to sustain their spatial relationships, then embedded in paraffin blocks and cross sectioned in 10-microm-thick slices. Serial cross sections were stained and examined with a light microscope., Results: The vestibular component was separated into two parts only at the lateral fundus of the internal auditory canal, lateral to the vestibular ganglion. In the internal auditory canal of all specimens, the vestibular component was represented by numerous fascicles. Around the porus acusticus, the fascicular pattern among the specimens was diverse: 13 of the 20 specimens were still polyfascicular, 4 specimens consisted of two large, distinct fascicles, and, in the remaining 3 specimens, a portion of the vestibular fascicles had fused with the cochlear component. Near the root entry zone, all vestibular fascicles fused and merged with the cochlear nerve to form a single trunk., Conclusion: There was no evidence to support the anatomic correctness of specifying the superior and inferior vestibular nerves, except in the lateral fundus of the internal auditory canal.
