2. The 21st UK Renal Registry Annual Report: A Summary of Analyses of Paediatric Data in 2017

Abstract

Children with end-stage kidney disease (ESKD) requiring renal replacement therapy (RRT) are usually managed in one of the 13 paediatric nephrology centres in the United Kingdom. The UK Renal Registry (UKRR) collects, analyses and reports data on children receiving RRT in these centres.The UKRR annual report presents analyses relating to the attainment of the Renal Association audit measures [1], national averages to enable benchmarking and long-term trends for children on RRT for ESKD. Data are reported by centre to enable between centre comparisons.To improve the timeliness of data reporting, the format of the UKRR 21st Annual Report, which included data to December 31, 2017 [2], differs significantly to that of previous years. For paediatric data, changes include a single chapter reporting demographic and biochemical data, reduced clinical commentary, a higher threshold for data quality control (with data completeness at least 70%) and greater alignment of analyses to the Renal Association guidelines [1].In this article, we summarise the analyses of paediatric data presented in the UKRR 21st Annual Report. Abstract

* A total of 917 children and young people under 18 years with established renal failure (ERF) were receiving treatment at paediatric nephrology centres in 2014.* At the census date (31st December 2014), 79.3% of prevalent paediatric patients aged * In patients aged * The most common primary renal diagnosis was renal dysplasia ± reflux, present in 32.6% of prevalent paediatric patients aged * About a third of patients had one or more reported comorbidity at onset of renal replacement therapy (RRT).* The improvement in rates of pre-emptive transplantation for those referred early has been maintained over the last 10 years at 37.5%, compared to 27.4% in 2000–2004.* At transfer to adult services, 90.3% of patients had a functioning kidney transplant.* Survival during childhood amongst children commencing RRT was the lowest in those aged less than two years compared to those aged 12 to less than 16 years with a hazard ratio of 4.1 (confidence interval 2.2–8.0), and in those receiving dialysis compared to having a functioning transplant with a hazard ratio of 6.3 (confidence interval 3.9–10.2)

Abstract

Introduction: This chapter describes the characteristics of adult patients on renal replacement therapy (RRT) in the UK in 2013. Methods: Data were electronically collected from all 71 renal centres within the UK. A series of crosssectional and longitudinal analyses were performed to describe the demographics of prevalent RRT patients in 2013 at centre and national level. Results: There were 56,940 adult patients receiving RRT in the UK on 31st December 2013. The UK adult prevalence of RRT was 888 pmp which represented an annual increase of 4%, with transplantation the most common treatment modality (52%). There was variation between centres, Clinical Commissioning Groups and Health Boards. The number of patients increased by 1.2% for haemodialysis (HD) and 7.1% for functioning transplant but decreased 3.3% for peritoneal dialysis (PD). The number of patients receiving home HD has increased by 3% since 2012. Median RRT vintage for patients on HD was 3.4 years, PD 1.7 years and for transplant, 10.1 years. The median age of prevalent patients was 58 years (HD 67 years, PD 64 years, transplant 53 years) compared to 55 years in 2000. For all ages the prevalence rate in men exceeded that in women. The most common recorded renal diagnosis was glomerulonephritis (biopsy proven/not biopsy proven) (19%). Conclusions: The HD and transplant population continued to expand; but the prevalent PD population continues to decline. There were national, regional and centre level variations in prevalence rates. This has continued implications for service planning and ensuring equity of care for RRT patients.

Abstract

Acute kidney injury (AKI) is common and associated with extremely poor outcomes. While strategies to tackle deficiencies in basic care delivery are advocated, robust testing of their effectiveness is also needed. The Tackling AKI study was designed to test whether a complex intervention (consisting of an e-alert, care bundle and education programme) can be successfully implemented across a range of UK hospitals, and whether this will deliver improved patient outcomes. This multicentre, pragmatic clinical trial will employ a cluster randomised stepped wedge design to study this in all adult patients who sustain AKI in the 5 participating hospitals over a 2-year period. The intervention will be supported by a comprehensive change management framework. Data collection will include patient outcomes, process measures and a qualitative assessment of barriers and enablers to implementation. Abstract

Aims: To describe the demographics of the paediatric renal replacement therapy (RRT) population under the age of 18 years in the UK and to analyse changes in demography over time. Methods: Data were collected electronically from all 13 paediatric renal centres within the UK. A series of cross-sectional and longitudinal analyses were performed to describe the demographics of paediatric RRT patients. Results: A total of 891 children and young people under 18 with established renal failure (ERF) were receiving treatment at paediatric nephrology centres in 2013. At the census date, 80.2% had a functioning transplant, 11.7% were receiving haemodialysis (HD) and 8.1% were receiving peritoneal dialysis (PD). In patients aged ,16 years the prevalence of ERF was 58.2 per million age related population (pmarp) and the incidence 9.3 pmarp. A third of the prevalent patients had one or more reported comorbidities. At transfer to adult services, 85.2% of patients had a functioning renal transplant. Pre-emptive transplantation was seen to occur in a third of children starting RRT under 16 years, with lower rates seen in girls and ethnic minorities. Living donation as starting modality has continued to improve with an increase from 8.8% in 1999–2003 to 18.4% in 2009–2013. Survival in childhood amongst children starting RRT was the lowest in those aged less than two years. Conclusions: We report continued improvement in data quality and electronic submission of data returns. The data provided in this report show relatively stable trends of incidence and prevalence in children with established renal failure.
