SARS-CoV-2 infection (COVID-19) has had a significant impact on transplant activity in our country. Mortality and the risk of complications associated with COVID-19 in kidney transplant recipients (KT) were expected to be higher due to their immunosuppressed condition and the frequent associated comorbidities. Since the beginning of the pandemic in March 2020 we have rapidly improved our knowledge about the epidemiology, clinical features and management of COVID-19 post-transplant, resulting in a better prognosis for our patients. KT units have been able to adapt their programs to this new reality, normalizing both donation and transplantation activity in our country.This manuscript presents a proposal to update the general recommendations for the prevention and treatment of infection in this highly vulnerable population such as KT. Resumen: La infección por el SARS-CoV-2 (COVID-19) ha supuesto un importante impacto en la actividad trasplantadora en nuestro país. Era esperable que la mortalidad y el riesgo de complicaciones asociadas a la COVID-19 en el receptor de trasplante renal (TR) fueran mayores debido a su condición de inmunosupresión y a las frecuentes comorbilidades asociadas. Desde el inicio de la pandemia en marzo del 2020 hemos mejorado rápidamente nuestro conocimiento acerca de la epidemiología, características clínicas y manejo de la COVID-19 post-trasplante, redundando en un mejor pronóstico para nuestros pacientes. Las unidades de trasplante renal han sabido adaptar sus programas a esta nueva realidad, normalizándose la actividad tanto de donación como de trasplante en nuestro país.Este manuscrito presenta una propuesta de actualización de las recomendaciones generales para la prevención y el tratamiento de la infección en esta población tan vulnerable como son los receptores de un trasplante renal.

Introduction: Death with a functioning graft (DWFG) is the most frequent cause of loss of kidney transplantation (KT). Objective: To analyze the evolution of the causes of DWFG and the frequency of the types of cancer causing DWFG. Methods: Retrospective study of KT in Andalusia from 1984 to 2018. We analyzed the evolution according to eras (1984–1995; 1996–2007; 2008–2018) and according to post-transplant period (early death: first year post-KT; late death: after first year post-KT). Results: A total of 9905 KT were performed, registering 1861 DWFG. The most frequent causes were cardiovascular disease (25.1%), infections (21.5%) and cancer (19.9%).In early death we did not observe changes, and infections were always the main cause. In late death, cardiovascular death decreased (1984−1995: 35.2%, 1996−2007: 22.6%, 2008−2018: 23.9%), but infections (1984−1995: 12.5%, 1996−2007: 18.3%, 2008−2018: 19.9%) and, above all, cancer-related deaths increased (1984−1995: 21.8%, 1996–2007: 29%, 2008−2018: 26.8%) (P

The SARS-CoV-2 (Covid-19) coronavirus pandemic is evolving very quickly and means a special risk for both immunosuppressed and comorbid patients. Knowledge about this growing infection is also increasing although many uncertainties remain, especially in the kidney transplant population. This manuscript presents a proposal for action with general and specific recommendations to protect and prevent infection in this vulnerable population such as kidney transplant recipients. Resumen: La pandemia por coronavirus SARS-CoV-2 (Covid-19) está evolucionando de manera muy rápida y representa un riesgo especial en pacientes inmunodeprimidos y con comorbilidades añadidas. El conocimiento sobre esta infección emergente va también en aumento, si bien, aún sigue habiendo muchas incógnitas, sobre todo en la población con trasplante renal. Este manuscrito presenta una propuesta de actuación con recomendaciones generales y específicas para proteger y prevenir de la infección a esta población tan vulnerable como son los receptores de un trasplante renal.

Background and objective: The Kidney Donor Profile Index (KDPI), together with other donor and recipient variables, can optimise the organ allocation process. This study aims to check the feasibility of the KDPI for a Spanish population and its predictive ability of graft and patient survival. Materials and methods: Data from 2734 kidney transplants carried out in Andalusia between January 2006 and December 2015 were studied. Cases were grouped by recipient age, categorised by KDPI quartile and both graft and patient survival were compared among groups. After contextualizing the generic frameworks of nanotechnology and nanomedicine, the 2 disciplines are discussed in the field of Nephrology. The potential downside to nanonephrology is the renal clearance of nanoparticles, the use of which is ever-increasing both for nanomedicinal purposes and in nanofoods. The positive impact of nanotechnology in Nephrology is centered on the development of renal nanodiagnostics for basic renal function studies, the early diagnosis of acute kidney injury, reliable and simple follow-up of chronic kidney disease and the improvement of magnetic resonance imaging. Renal drug nanotherapies comprise an important and dual-faceted area: the protection of drugs and nephrotoxic agents (e.g. antibiotics, antiretrovirals, contrast media, etc.) on the one hand, and the development of new kidney disease medications on the other. Renal ‘nanotheranostics’ is a promising but little-studied area. The impact of nanostructured supports on renal tissue regeneration is also discussed. The article concludes with a brief analysis of the various nanonephrology perspectives. Resumen: Después de una contextualización en los marcos genéricos de la nanotecnología y la nanomedicina, se exponen las 2 connotaciones nanotecnológicas de la Nefrología. La potencial faceta negativa de la nanonefrología es el aclaramiento renal de las nanopartículas usadas con fines nanomédicos o ingeridas en los nanoalimentos, cada vez más abundantes. El impacto positivo de la nanotecnología en la Nefrología se centra en el desarrollo de nanodiagnósticos renales para estudios básicos de la función renal, diagnóstico precoz del fallo renal agudo, seguimiento fiable y simple de la enfermedad renal crónica o la mejora de las imágenes de resonancia magnética nuclear. Las nanoterapias renales con fármacos es un tema de importancia que tiene 2 connotaciones: la protección de fármacos y agentes nefrotóxicos (ej. antibióticos, retrovirales, medios de contraste, etc.) y el desarrollo de nuevos medicamentos para enfermedades renales. La nanoteragnosis renal es una línea prometedora poco desarrollada. Se explicita también el impacto de los soportes nanoestructurados en la regeneración tisular renal. El artículo finaliza con un breve análisis de las perspectivas de la nanonefrología.
