Background For patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) complicated by respiratory acidosis, noninvasive ventilation therapy is thought to be the first-line treatment. In patients with AECOPD, the effect of high-flow nasal oxygen therapy is not well studied. In this study, the existing data will be synthesized to obtain an effective rate of movement of nasal oxygen therapy in patients with AECOPD. Methods Using PubMed, EMBASE, Cochrane Library, Web of Science, Scopus, a systematic search will be undertaken to identify randomized controlled trails (RCTs) on the clinical therapeutic effects of rate of movement of nasal oxygen therapy in patients with AECOPD without language constraints from their onset to November 2020. To classify potentially qualifying tests, we will also review Google Scholar, ClinicalTrials.gov, and the reference lists of included studies. Two independent reviewers will review inclusion trials and execute data extraction. Research bias and quality will be measured using the Cochrane Collaboration Bias Method 2.0. The findings of the analysis will be pooled using a formula of fixed-effects or random-effects. We will address any dispute by dialogue, and cases of disagreement will be mediated by a third author. Results The current research will examine the clinical therapeutic results of patients with AECOPD with rate of movement of nasal oxygen therapy. Conclusion To assess the efficacy of rate of movement of nasal oxygen therapy in patients with AECOPD, the present analysis would provide consistent facts. Background: The optimal interventions for unprotected left main coronary artery (ULMCA) disease have long been debated, and long-term clinical studies comparing single stenting to double stenting strategies for ULMCA are currently lacking. Methods: We plan to perform a systematic review and meta-analysis of clinical trials comparing single stenting with double stents strategy for ULMCA disease. We will search PubMed, EMBASE, Web of science and Cochrane Library using a comprehensive strategy. The related conference proceedings and reference lists of the included studies will also be checked to identify additional studies. Two reviewers will screen retrieved records, extract information and assess the risk of bias independently. STATA software will be used to conduct data synthesis. There is no requirement of ethical approval and informed consent. Results: This study will be submitted to a peer-reviewed journal for publication. Conclusion: We hope it will provide a relatively comprehensive reference for clinical practice and future relevant clinical trials. Introduction Entropion and secondary trichiasis can lead to irritative symptoms and essential damage of ocular surface. There is no literature reporting the lower eyelid entropion related to thyroid-associated ophthalmopathy (TAO), let alone the treatment. Treatment based on etiology may yield effective and sustained results. We report 3 case reports of lower eyelid entropion associated with TAO, and provide an effective and persistent alternative to cure this entropion via the administration of shallow periorbital injections of triamcinolone acetonide (TA). Patient concerns Three patients presented irritative symptoms of ocular surface and diplopia. Diagnosis According to thyroid dysfunction, physical examination, and imaging findings of extraocular muscle involvement, TAO and unilateral or bilateral lower eyelid entropion were diagnosed. Interventions We administered shallow periorbital injections of TA to the affected eye at 3- to 4-week intervals depending on clinical response. Outcomes All patients underwent complete correction of the lower eyelid entropion and no recurrence was found. Conclusion The cause of lower eyelid entropion related to TAO might be the immunoinflammatory reaction of the lower eyelid retractors, enhancing the traction of pulling the lower eyelid inferoposteriorly. This condition can be treated with shallow periorbital injections of TA. Histopathological evidence and randomized controlled trials are expected to confirm our hypothesis.

Supplemental Digital Content is available in the text, To assess treatment response and overall survival (OS) in refractory or relapsed acute myeloid leukemia (R/R AML) patients treated by different common salvage chemotherapy regimens. Medical records data from 142 R/R AML patients were reviewed in this retrospective study. Patients were treated with regimens based on the following drugs: cytarabine, granulocyte colony-stimulating factor (G-CSF), and fludarabine (FLAG) (n = 46); cytarabine and G-CSF in addition to aclarubicin or daunorubicin (CAG/DAG) (n = 30); cytarabine, G-CSF, and cladribine (CLAG) (n = 27); cytarabine, etoposide, and mitoxantrone (MEA) (n = 17); cytarabine plus idarubicin, daunorubicin, or mitoxantrone (IA/DA/MA) (n = 12); and homoharringtonine, cytarabine, and aclarubicin or daunorubicin (HAA/HAD) (n = 10). A total of 43 (35.2%) patients achieved complete remission (CR), 60 (49.2%) patients achieved overall remission rate (ORR), and 18 (14.8%) patients received allogeneic hematopoietic stem cell transplantation (allo-HSCT) after CR. Median OS was 8.0 (95% CI 6.6–9.4) months with a 1-year OS rate of (29.9 ± 3.9)% and 3-year OS rate of (11.1 ± 3.6)%. No difference of CR (P = .621), ORR (P = .385), and allo-HSCT (P = .537) achievement was observed among different chemotherapy regimens. Interestingly, we observed that the CLAG-based regimen did not affect CR (P = .165), while it achieved a numerically higher ORR (P = .093) and was an independent factor for prolonged OS (P = .016). No other regimens were determined to be correlated with CR, ORR, or OS. FLAG-, CAG/DAG-, CLAG-, MEA-, IA/DA/MA- and HAA/HAD-based regimens were found to achieve similar CR rates, while the CLAG-based regimen achieved numerically higher ORR rates and significant favorable OS. Therefore, CLAG-based regimens should be a prioritized treatment option for R/R AML patients.

Bariatric surgery is effective in treating different components of metabolic syndrome including obesity, type 2 diabetes mellitus (T2DM), and hyperlipidemia. But there is no consensus on the ideal biliopancreatic and Roux limb length. This study aimed to explore the effect of biliopancreatic limb and Roux limb lengths during laparoscopic Roux-en-Y gastric bypass (LRYGB) procedures on weight loss and T2DM control. We studied the clinical records of 58 patients with metabolic syndrome, T2DM, and body mass index (BMI) 32 to 50 kg/m2 who underwent LRYGB in our hospital. The short limb group (Group A) underwent LRYGB with a limb length of 160 to 200 cm (n = 31) and the long limb group (Group B) underwent LRYGB with a limb length of 210 to 240 cm (n = 27) were compared. The occurrence of acute or chronic internal hernia in Group B was higher than that in Group A (P = .026). Twelve months after surgery, patients from the 2 groups were also observed with reduction in BMI, percent excess weight loss (EWL), preoperative FPG, and HbA1c as compared with these indicators before surgery. However, the differences of these indicators between 2 groups were not significant at the time point of before and 3, 6, 12 months after surgery. LRYGB had significant effects on weight loss and diabetes control in obese T2DM patients. However, there was no significant difference in the short term on weight loss and diabetes control in the patients receiving different limb lengths.

Rationale: Although rare, cases of renal failure secondary to a uretero-inguinal hernia have been reported. Patient concerns: Here, we report a case of left renal failure caused by chronic obstructive uropathy due to a uretero-inguinal hernia combined with contralateral renal pelvic carcinoma. Diagnoses: The present case highly indicated that a comprehensive examination is very important when diagnosing an inguinal hernia. In particular, it is necessary to check whether the ureter is involved or not. Interventions: A computed tomography scan should be performed to a uretero-inguinal hernia patient. Outcomes: Unfortunately, this patient was diagnosed too late to attempt surgical management to restore the left renal functions. Lessons: In our opinion, a computed tomography scan is highly recommended for an accurate diagnosis.

Rationale: Simultaneous bilateral mid-clavicle fracture with unilateral dislocation of the acromioclavicular (AC) joint is an extremely rare injury combination. Patient concerns: Herein, we report a case of polytrauma in a 42 years old female following a road traffic accident. Diagnoses: The radiographs showed the fractures in bilateral middle third of the clavicle and AC joint dislocation of left shoulder. Interventions: The patient was treated with a 5-hole locking hook plate to stabilize the left AC joint and a shortly reconstruction plate to fix the left clavicle. Moreover, another 6-hole truncated anatomic reconstruction plate was used to fix the fracture of the right clavicle. Outcomes: At the 1-year follow-up, the patient had excellent range of motion and functional outcomes. Lessons: It is important to achieve stability and aim for excellence in terms of full shoulder function in this rare combination injury.

Currently, the association between sarcopenia and long-term prognosis after gastric cancer surgery has not been investigated. Moreover, the association between sarcopenia and postoperative complications remains controversial. This large-scale retrospective study aims to ascertain the prevalence of sarcopenia and assess its impact on postoperative complications and long-term survival in patients undergoing radical gastrectomy for gastric cancer. From December 2008 to April 2013, the clinical data of all patients who underwent elective radical gastrectomy for gastric cancer were collected prospectively. Only patients with available preoperative abdominal CT scan within 30 days of surgery were considered for analysis. Skeletal muscle mass was determined by abdominal (computed tomography) CT scan, and sarcopenia was diagnosed by the cut-off values obtained by means of optimum stratification. Univariate and multivariate analyses evaluating risk factors of postoperative complications and long-term survival were performed. A total of 937 patients were included in this study, and 389 (41.5%) patients were sarcopenic based on the diagnostic cut-off values (34.9 cm2/m2 for women and 40.8 cm2/m2 for men). This prospective study aims to identify the risk factors for prolonged postoperative ileus (PPOI) and to use these risk factors to generate a risk stratification scoring system for the occurrence of PPOI. Patients who underwent radical gastrectomy for gastric cancer were included in this study. A multivariate logistic analysis was applied to identify independent risk factors for PPOI and to generate the scoring system. A receiver operating characteristic curve was generated and the area under the curve was calculated to demonstrate the predictive power of the scoring system. Finally, 296 patients were included and analyzed, of whom 96 (32.4%) developed PPOI. The multivariate analysis showed that age ≥65 years, operative duration ≥4 hours, tumor–node–metastasis (TNM) stage = III, open/converted operative technique, and total postoperative opiates dose (TOD) ≥0.3 mg/kg were independent risk factors for PPOI. Based on these factors, a risk stratification scoring system was generated, classified by low-risk (score 0–2), moderate-risk (score 3–4), and high-risk (score 5–6) groups. The incidence of PPOI increased by 7.5-fold from low-risk to high-risk group. The area under the curve of the scoring system was 0.841 (95% CI, 0.793–0.890), indicating a good predictive capability for the occurrence of PPOI. We have identified independent risk factors for the occurrence of PPOI and used these factors to construct a risk stratification scoring system.
