Diffuse large B cell non-Hodgkin's lymphomas (DLCL) form a heterogeneous group of tumors with diverse morphology, clinical features, treatment response and prognosis. The biological variables underlying this heterogeneity are unknown. In the present study, we explored the value of the lymphocyte homing receptor CD44, a putative determinant of lymphoma dissemination, in predicting prognosis in DLCL. Expression of the standard form of CD44 (CD44s) and of CD44 isoforms containing exon v6 (CD44v6) on tumor cells was assessed by immunohistochemistry in a cohort of 276 DLCL patients from a population based lymphoma registry. We observed that CD44s as well as CD44v6 expression correlated with tumor dissemination in patients with primary nodal DLCL. Importantly, in patients with localized nodal disease, CD44s was a strong prognosticator predicting tumor related death independent of the other parameters of the International Prognostic Index (IPI). Incorporation of CD44s in the IPI parameter 'stage', increased the prognostic value of this parameter in nodal DLCL. Our data identify CD44 as a biological prognosticator, which can be used to 'fine-tune' the IPI for nodal DLCL.

The dissemination pattern of a human non-Hodgkin's lymphoma (NHL) B cell line (DoHH2) carrying the t(14;18) translocation was analyzed in severe combined immunodeficient (SCID) mice, using different routes of administration. When engrafted intraperitoneally (i.p.) the DoHH2 cells showed a local infiltration into intra- and retroperitoneal mouse tissues, and disseminated to bone marrow and lymph nodes. In contrast, after subcutaneous (s.c.) or intravenous (i.v.) transfer the DoHH2 cells displayed a hematogenous spread, and disseminated predominantly to hematopoietic and lymphoid organs including bone marrow, peripheral blood, spleen, peripheral lymph nodes, and liver. No involvement of the gut and mesenteric lymph nodes was observed, suggesting a specific homing pattern, bypassing the mucosa-associated lymphoid tissue (MALT). This pattern is reminiscent of the human disease. Phenotypic analysis, cytogenetic analysis, and minor histocompatibility antigen (mHA) typing using mHA-specific cytotoxic T-lymphocyte (CTL) clones performed on the original DoHH2 cell line and on DoHH2 cells recovered from mouse tissue, showed that in vivo passage did not alter the characteristics of the DoHH2 cells. After i.v. administration, the survival time of the SCID mice directly correlated with the number of DoHH2 cells inoculated. This model of dissemination of the DoHH2 cell line may be useful for studying the efficacy of (immunotherapeutic) treatment of human lymphoproliferative disorders in vivo.

Malignant cells from 24 cases of hairy cell leukemia were studied by in situ hybridization for evidence of selective aneuploidy using alphoid and satellite probes specific for 16 human chromosomes. Based on these data, hairy cell leukemia appears to be diploid for the chromosomes studied and is a malignancy which displays the phenomenon of pairing of the centromere and p arm of chromosome 15 during interphase.

Patients with hairy cell leukemia (HCL) are prone to opportunistic infections, which suggests an impaired T-cell functioning. To investigate a possible mechanism of such an impairment, we determined the numbers of naive and memory T cells by measuring the expression of CD45R0 on CD4+ and CD8+ T cells in 23 HCL patients. As control, 13 healthy subjects and 13 patients with other chronic B-cell leukemias were studied. In HCL patients with active disease, the percentage of CD4+ CD45R0+ T cells was significantly lower compared to healthy subjects (41% versus 57%, p = 0.01). Also the absolute numbers of CD4+ CD45R0+ T cells were reduced (396 x 10(6)/l versus 615 x 10(6)/l, p = 0.02) compared to healthy subjects, whereas within the CD8+ subset no differences were found. A contrasting elevation of percentages and numbers of CD45R0-expressing T cells (p < 0.0001) was seen in patients with chronic lymphocytic leukemia or leukemic non-Hodgkin's lymphoma. No relationship between CD4+ CD45R0+ and splenectomy, treatment with alpha-interferon or monocyte numbers was found in the HCL population. Despite the fact that the underlying mechanism of the reduced expression of CD45R0 in CD4+ T cells remains unclear, our observations may contribute to the understanding of an impaired T-cell functioning in HCL.

A spontaneously growing EBV-negative B-cell line (DoHH2) was established from the pleural fluid cells of a 60-year-old man with centroblastic/centrocytic non-Hodgkin's lymphoma, that had transformed into an immunoblastic lymphoma. The pleural fluid cells and the DoHH2 cells expressed IgG lambda, were reactive with CD10 and CD19 monoclonal antibodies, and showed by cytogenetic analysis 48,XY, +7, +del(12)(q24), t(14;18)(q32;q21). Southern blot analysis of mini-satellite DNA patterns, and of rearrangements of the immunoglobulin genes and bcl-2, confirmed that the cell line was derived from the patient's clonal lymphoma cells. Direct nucleotide sequence analysis on polymerase chain reaction (PCR) products of the t(14;18) junction revealed an identical sequence for the JH-bcl-2 junction at JH6 and in the major breakpoint region of bcl-2 in both the original tumor cells and the DoHH2 cell line. The cell line was valuable as a standard quantification control for PCR analysis of the t(14;18) breakpoint. Titration experiments demonstrated the detection of up to one tumor cell in 10(5) normal blood lymphocytes.

An overview of the place and use of diagnostic investigations necessary for staging of patients with Hodgkin's disease is given. Staging followed by specific treatment according to stage remains the corner stone in the management. New imaging techniques such as computed tomography and ultrasound cannot completely replace lymphangiography. Staging laparotomy should not be used routinely in clinical stage I and II patients. In the past, staging laparotomy was performed to identify those patients for whom management decisions would depend on the identification of abdominal disease. However, with the identification of prognostic factors and the subsequently selected therapy-regimens including treatment of possible spleen involvement, the role of laparotomy in patients management has largely decreased.

Leukemic B cells with a characteristically sharp nuclear cleft seemingly dividing the nucleus into two or more parts have been entitled "buttock cells" and are subject of this study. These cells were found in leukemic non-Hodgkin's lymphomas (NHL) and usually have been related to follicular center cell lymphomas. However, buttock cells also closely resemble cells present in intermediate lymphocytic lymphoma (ILL) and mantle zone cells of reactive lymphoid tissues. Ultrastructurally, it became apparent that the separate nuclear lobes of buttock cells were connected by chromatin bridges. Immunophenotypically, circulating buttock cells had a variable phenotype, which may indicate either a follicle center or mantle zone origin. The use of CD5 and FMC7 monoclonal antibodies might be of discriminative help. These leukemic NHL have to be differentiated from classical chronic lymphocytic leukemia (CLL) with help of cytomorphology and immunophenotyping, since the former usually have a worse prognosis and generally will require a different treatment.

Malignant B cells, derived from hairy cell leukemia (HCL), non-Hodgkin's lymphoma (NHL) or prolymphocytic leukemia were stimulated with mitogens and interleukin 2 after careful depletion of contaminating T cells resulting in final residual percentages of less than 0.1. No proliferation was found as measured by 3H-thymidine incorporation. Subsequently, to the non-T B cell cultures very small amounts of autologous or allogeneic T cells were added, ending up in final concentrations of 0.1, 0.5, 1, or 2% T cells. It appeared that a marked proliferation occurred which had in various coculture combinations to be ascribed to the T cells alone. Moreover, most HCL and other B cell NHL additionally stimulated the T cells, resulting in a further increase in proliferation. We conclude that 3H-thymidine incorporation by malignant B cells such as HCL and B-NHL stimulated with mitogens and IL-2 will in most cases wrongly be attributed to proliferation by the B cells themselves, and instead has to be ascribed to contaminating T cells.
