Stoffwechselstorungen wirken sich nachteilig auf den postoperativen Heilverlauf aus. In der gastroenterologischen Chirurgie gibt es zahlreiche entzundliche, tumorose und iatrogene Veranderungen, die mit erheblichen Defiziten einhergehen. Das Ausmas der Storung ist abhangig von Organlokalisation, Krankheitsdauer Bowie Schwere und Ausdehnung des Befundes. Entscheidend ist der praoperative Defizitausgleich. Wichtig sind quantitativ und qualitiv ausreichendes Aminosaurenangebot (1,5 g Aminosauren/kg KG in 24 h) und hohe Energiezufuhr (160–200 Joule/kg KG in 24 h).

Studies on gastric emptying after antrectomy and vagotomy with gastroduodenostomy show that the gastric remnant empties in the same way as the intact stomach by hormonal control. If gastrointestinal passage after gastric resection is restored by gastrojejunostomy no endocrine regulation can be found.

Similar to the high pressure zone of the lower esophageal segment (LES) gastric emptying results from alternating intraluminal pressure in the antrum, pylorus and duodenum. Alterations of this antro-duodenal pressure zone depend on hormonal and nervous control mechanisms and are regulated by intraduodenal localized pH-, osmo- and fat-receptors. Experimental investigations in partially gastrectomized dogs emphasize the important role of the duodenum for gastric emptying. After resection of the lower half of the stomach and reconstruction of the orthograde passage between the gastric remnant and the duodenum by gastroduodenostomy (b i-anastomosis), a hormonally controlled gastric emptying comparable to that of the intact organ can be noticed.

The working effect of Gastrin, neostigmine and prostaglandin in regard to the emptying of the stomach was tested on 31 dogs that had undergone a truncular vagotomy. Gastrin and neostigmine were proved to increase the motility at the stomach pylorus but was not able to bring about a coordinated emptying pattern. Prostaglandin shows different effects on the emptying reflex of the stomach. PGF 2 alpha increases the motility on the measuring marks, but at different time intervals. Because of this a coordinated motility is made possible.

The aim of surgical treatment of hiatus hernia is well defined: restoration of the esophago-gastric closing mechanism to prevent further reflux of gastric contents and gastric juice into the esophagus. The main problem to realize this demand lies in the variety of discussed closing mechanisms. Interaction of different mechanisms influencing the "anti-reflux barrier" as well as hormonal and neural regulation of the lower esophagus are not yet examined sufficiently enough. Benefits and hazards of several types of operations for repair of hiatal hernia are discussed before the background of the small but slowly growing understanding of the esophago-gastric closing mechanism.

None of the usual methods of bowel preparation guarantee an aseptic large bowel. The intention is to reduce intestinal flora quantitatively, which can be achieved by mechanical cleansing alone. During the preparation period caloric and amino acid intake is insufficient. Therefore alimentation with elemental diet is necessary. The patient gets an adequate caloric and protein supply. The diet is totally absorbed in the small intestine, the large bowel remains clean, and the bacterial flora is reduced.

Metabolic disturbances are one of the main reasons for complications in gastroenterologic surgery. There is a large number of inflammatory, neoplastic, and iatrogenic illnesses that cause considerable deficiencies. The intensity of disturbances depends on localization, duration, and extension of the pathologic alterations. Important for the postoperative outcome is the preoperative compensation of caloric and protein deficiencies: 1.5 g of amino acids per kg body weight/24 h and energy 160-200 joule per kg body weight/24 h.

Ein ,steriler” Darm ist mit keinem der gebrauchlichen Vorbereitungsregime zu erreichen. Ziel der Vorbereitung ist die quantitative Reduktion der Darmkeime. Qualitative Anderungen sind unerwunscht. Das kann durch alleinige mechanische Darmreinigung erreicht werden. Da bei fehlender Substratzufuhr wahrend der Vorbereitungsphase die grunderkrankungsbedingte Katabolie zunimmt, mus wahrend dieser Zeit ,kiinstlich” ernahrt werden. Bei Anwendung von chemisch-definierter Diat erhalt der Patient ausreichend Energie und Eiweisbausteine. Der Dickdarm bleibt infolge der im Dunndarm komplett absorbierten Nahrung jedoch sauber und keimarm.

Seit 1966 hat die Mortalitdtsrate an Appendicitis kontinuierlich von 3,3 auf 1,7 abgenommen. Eine ernahrungsbedingte hohe Appendicitisfrequenz wird als Ursache der indirekt nachgewiesenen hohen Appendektomiefrequenz eher angenommen, als eine zu grose Op.-Freudigkeit der Chirurgen. Trotzdem ist eine strenge Indikationsstellung zu fordern und moglich. An unserer Klinik sank die Op.-Quote von 87 % auf 33 % bei Appendicitisverdacht, damit stieg die als akut histologisch bewiesene Appendicitis von 46 % auf 72 % an. Die routinemasige histologische Untersuchung stellt eine unverzichtbare Selbstkontrolle fur die Richtigkeit der Op.-Indikation dar.
