Surgical treatment of pyogenic infections of the sacroiliac joint is indicated in cases of ineffective conservative treatment, abscess formation, septicemia, and neurological deficits. Between 1983 and 1990 in nine patients surgical treatment was performed for pyogenic sacroiliitis under this criteria. The surgical procedure included joint debridement, primary arthrodesis of the sacroiliac joint using a autologous bone graft, antibiotic therapy and postoperative immobilisation. Follow-up examination of 8 patients in average 47 months postoperatively revealed excellent functional and roentgenological results in 6 patients. Two patients suffered only from mild lowback pain, none of the eight patients demonstrated signs of a recurrent infection, one patient died due to complications of a long-lasting preoperative septicemia. Regarding these postoperative follow-up results surgical therapy including primary sacroiliac arthrodesis should be early considered, because this treatment has a low complication rate and the surgical technique is easy to perform and results are excellent or good in most of the patients.

Long-term results after surgical treatment of osteochondritis dissecans of the talar dome and joint knee are dependent on the stage of cartilage damage, the age at operation and on the surgical technique. In cases of osteochondritis dissecans of the talar dome the only loosening of a refixed osteochondral fragment was seen after glueing with acrylate. Using the classification of Arcq in 59.6% an excellent and in 18.3% a good result was observed in cases of osteochondritis dissecans at the femoral condyles. In regard to the development of osteoarthritis in 56% no signs of osteoarthritis were visible. Worst results were obtained in knee joints in which acrylate glue was used for refixation of the osteochondral fragments. In conclusion we recommend the use of fibrin glue for refixation of osteochondral fragments in cases of osteochondritis dissecans even when early mobilisation follows the operation. Because of the long-lasting resorption and barrier effect to ingrowing tissue the use of cyanoacrylate should be avoided.

Hepatic masses are predominantly malignant, and whereas benign tumors are rare. We report the case of a 65-year-old man who presented with an anomalous hepatic mass. Following explorative laparotomy and left lateral segmentectomy (II/III), the patient was diagnosed as having an inflammatory pseudotumor of the liver. So far only 102 cases of “inflammatory pseudotumor of the liver” have been reported in the literature. Asia is a geographical center for this tumor entity, which mostly affects men. The major symptoms are nonspecific: fever, weight loss and general fatigue. The prognosis of “inflammatory pseudotumor of the liver” is very good. In 98% of the cases, a cure has been obtained after surgical therapy, but conservative therapy approaches also yield good results.

The subject of this study consisted of 266 hernia repairs according to Lotheissen-McVay, carried out during the years 1970–1975. The survey evaluated the outcome of operations on direct and indirect inguinal hernias as well as on femoral hernias, but did not include any review of recurrent hernias. The sole aim of the survey was to evaluate the long-term results after 20 years, since the recurrence rate increases with postoperative duration. Through previous studies, partly involving the same patients, partial results on the recurrence incidence were available.

Abstract: Common late complications after esophagectomy and gastric tube reconstruction for esophageal carcinoma are symptomatic, benign fibrotic stenoses of the cervical anastomosis, which require dilatation. Since the prognosis of esophageal carcinoma still remains poor, bad functional results such as dysphagia affect quality of life. In a retrospective analysis, our patients were evaluated with regard to the underlying effects of cervical anastomotic stenosis after esophagectomy and gastric tube reconstruction. From 1 January 1989 to 31 July 1995, 173 patients with carcinoma of the esophagus were operated in our institution. Transhiatal esophageal dissection was performed in 133 patients; 40 patients underwent transthoracic en bloc resection. The 30-day mortality rate was 7.5% (13 patients). Postoperative fibrotic stenosis of the cervical anastomosis requiring dilatation occurred in 36.4% (63 patients) 6 – 12 weeks after operation. Fibrotic stenosis of the cervical anastomosis did not develop in 97 patients. There was a significant difference concerning the incidence of anastomotic leaks within both groups: whereas in 23.8% of the 63 patients who developed a fibrotic stricture of the cervical anastomosis an anastomotic leak preceded this event (P<0.001), no anastomotic leak occurred in the group of 97 patients with normal healing of the cervical anastomosis. In addition, significantly (P<0.01) more patients (37.5%, n = 23) with preexisting diabetes mellitus could be found among the 63 patients who developed a fibrotic stricture of the cervical anastomosis, in contrast to the 97 patients without anastomotic stenosis.

Abstract: The gasless videoendoscopic implantation of GELSOFT aortobifemoral vascular prostheses times 6×6 mm in diameter using an extraperitoneal approach was tested in ten porcine experimental models at the Surgical Department of the University of Cologne, Germany. Gasless videoendoscopic surgery is performed with a laparolift-laparofan system. Aortobifemoral GELSOFT prostheses were successfully implanted in nine of ten animals, whereby one animal died during preparations for surgery of massive coronary infarctions. Average surgical durations using the extraperitoneal approach were 270 min. Dissection of the infrarenal aorta until occlusion took 45 min, average aortic occlusion 75 min, and iliacofemoral occlusion 45 min for the left side and 75 min for the right side. After successful videoendoscopic implantation of aortobifemoral GELSOFT prostheses all nine animals underwent laparotomy and resection of the aortobifemoral prosthetic segment. The quality of the endoscopically sutured aortic end-to-side anastomoses was examined in vitro under artificial circulation of glycerol/Ringer's lactate solution for evaluation of possible leakage and bursting pressures and then compared to conventionally sutured end-to-side anastomoses of 6-h-old porcine abdominal aorta and GELSOFT prostheses 6 mm in diameter. The maximum bursting pressure of all endoscopically sutured anastomoses was 480 mmHg mean pressure; the minimum was 140 mmHg mean pressure. The minimum leakage per minute was less than 10 ml/min for systolic pressure values between 120 and 350 mmHg. All endoscopically sutured aortic end-to-side anastomoses were comparable to conventionally sutured anastomoses concerning in vitro evaluation of bursting pressure and leakage per minute.

Summary The results of the chemiluminescence activity determined in whole blood samples very often were used to compare the unspecific defense mechanism of polymorphonuclear granulocytes (PMN) against causative organisms to preoperative capable risk parameters in surgery. Our investigations try to compare the results of a bacteria-induced chemiluminescence with those we received from a standardized agar killing plate-test. It could be shown that there is no correlation between the used two test systems. Therefore it is not possible to conclude from the chemiluminescence results especially made out of whole blood samples to the potency of the defense mechanism of PMN granulocytes.

Um den Einfluß des chirurgischen Traumas beim Ösophaguskarzinom auf das zelluläre Immunsystem zu erfassen, wurden perioperativ in einer prospektiven Studie die Aktivität der natürlichen Killerzellen sowie die Serumkonzentrationen von Interleukin-2, Interleukin-6 un TNF-α bei transmediastinaler Dissektion (n=12) vs. transthorakaler En-bloc-Resektion (n=10) der Speiserörhre im Vergleich zu einer Kontrollgruppe mit thorakoabdominalen chirurgischen Eingriffen bei nicht maligner Grunderkrankung erfaßt. Die Bestimmung der NK-Zell-Aktivität erfolgte präoperativ sowie am 4. und 10. Tag postoperativ durch einen standardisierten Europiumchlorid-release-Assay unter Verwendung von K-562-Targetzellen, die Lymphokine Interleukin-2, Interleukin-6 und TNF-α wurden zusätzlich am 1. und 7. Tag postoperativ mit stadardisierten ELISA-Assays bestimmt.

Resorbable polyglycohc-acid (PGA)-nets are used successfully for the treatment of traumatic ruptures of parenchymous organs. This experimental study was carried out to investigate whether PGA-nets can also reinforce jeopardized colon anastomoses. In 75 Wistar rats the transverse colon was transsected and reanastomosed. Animals were divided into 2 groups: in one group the anastomosis was applied sufficiently, in the other group the anastomosis was applied potentielly insufficient. Out of each group a PGA-net was placed around the anastomosis in one half of the animals. The results demonstrated that delayed healing of the anastomoses as well as spontaneous suture line disruptions followed by peritonitis only occurred in those animals in which a PGA-net had been used. The evaluation of the bursting strength could confirm this finding of delayed consolidation. The bursting pressure therefore was significantly decreased in all animals in which a PGA-net had been applied, compared to the control group operated without PGA-net application. We therefore conclude that PGA-net application in colon anastomoses inhibits the consolidation of the suture line, probably because of a lack of contact to the peritoneal and omental tissue.

The falciform ligament and the ligamentum teres of the liver are the so-called ventral mesohepaticum. These ligaments are covered on both sides by one layer of mesothelial cells and consist of a dense network of lymphatic capillaries. Preparations of semithin sections were examined light microscopically, ultrathin sections by means of transmission electron microscopy and gold sputtered surface preparations by scanning electron microscopy. It could be shown for the first time that in the mesothelial cell surface there are so-called “stomata”, holes which are considered to be connections between the peritoneal cavity and lymphatic capillaries. This network of lymphatics is able to rapidly absorb fluids and particles like blood cells. The knowledge of localisation, structure and function of the submesothelial lymphatic system is of high importance for surgical processing, because in most infections and tumor diseases it is also affected.

Summary It was intended to establish a model for acute hepatic failure in order to test the function of isolated and auxiliary transplanted hepatocytes. 6 pigs (19.75–27.0 kg) were intoxicated with different dosages of CCl4 and ethanol. According to publications on acute hepatic failure induced in rats by CCl4, adequate dosages were applied in pigs. Three different regimens of CCl4 and C2H5OH applications were used: CCl4 was administered intragastrically alone (group 1) or combined with ethanol (group 2), and CCl4 was given intragastrically and ethanol intravenously (group 3). Animals of group 1 and 2 survived intoxication for the period of observation. Histological examination of the livers revealed central necrosis, ghost cells and swollen hepatocytes. The animals of group 3 died shortly after intoxication. Histological examination did not show any evidence for acute hepatic failure. CCl4 and C2H5OH intoxication in pigs causes effects that are different from those described in humans or laboratory rats. Therefore it should be necessary to use a large number of animals to establish a standardized model for acute hepatic failure in pigs that reflects the observations in humans.

Summary During recent years, noninvasive diagnosis of the pancreatic carcinoma by means of sonography and CT has been improved by the use of new tumor markers and immunoscintigraphy. We present a prospective study demonstrating the value of latter two methods. This study includes patients with pancreatic carcinomas who underwent surgical therapy or who presented with a relapse in our outpatient department for pancreatic diseases. The clinical findings, CT, sonography, tumor markers and immunoscintigraphy with131I-labeled monoclonal antibodies were compared. The results demonstrate that the new tumor markers are essential today for both primary diagnostic and clinical follow-up of patients with pancreatic carcinomas. Generally, in the primary diagnosis immunoscintigraphy is inferior to sonography or CT - however, immunoscintigraphy can be helpful for the identification of infiltrative tumor recurrence. In addition, the first results are presented, demonstrating the potentials of radioimmunotherapy of the pancreatic carcinoma with the antibody BW 494/32.
