Background: Epilepsy is associated with high rates of premature mortality, but the contribution of psychiatric comorbidity is uncertain. We assessed the prevalence and risks of premature mortality from external causes such as suicide, accidents, and assaults in people with epilepsy with and without psychiatric comorbidity., Methods: We studied all individuals born in Sweden between 1954 and 2009 with inpatient and outpatient diagnoses of epilepsy (n=69,995) for risks and causes of premature mortality. Patients were compared with age-matched and sex-matched general population controls (n=660,869) and unaffected siblings (n=81,396). Sensitivity analyses were done to investigate whether these odds differed by sex, age, seizure types, comorbid psychiatric diagnosis, and different time periods after epilepsy diagnosis., Results: 6155 (8.8%) people with epilepsy died during follow-up, at a median age of 34·5 (IQR 21·0-44·0) years with substantially elevated odds of premature mortality (adjusted odds ratio [aOR] of 11·1 [95% CI 10·6-11·6] compared with general population controls, and 11·4 [10·4-12·5] compared with unaffected siblings). Of those deaths, 15·8% (n=972) were from external causes, with high odds for non-vehicle accidents (aOR 5·5, 95 % CI 4·7-6·5) and suicide (3·7, 3·3-4·2). Of those who died from external causes, 75·2% had comorbid psychiatric disorders, with strong associations in individuals with co-occurring depression (13·0, 10·3-16·6) and substance misuse (22·4, 18·3-27·3), compared with patients with no epilepsy and no psychiatric comorbidity., Interpretation: Reducing premature mortality from external causes of death should be a priority in epilepsy management. Psychiatric comorbidity plays an important part in the premature mortality seen in epilepsy. Background: Families of children with newly diagnosed epilepsy worry about death during a seizure. We aimed to assess the frequency and causes of death of children with epilepsy., Method: We did a population-based cohort study. The Nova Scotia epilepsy cohort includes all children who developed epilepsy during 1977-85. In 1999, we matched names and birth dates with provincial health-care, death, and marriage registries. We examined death certificates, necropsy reports, and physician records of children who had died and contacted families if sudden unexpected death in epilepsy could have occurred. We measured the effect of sex, age, epilepsy type, and disorder sufficient to cause functional neurological deficit on death rate. We compared cohort mortality with rates in a reference population matched for age and sex., Findings: 26 (3.8%) of 692 children with epilepsy died. Frequency of death was 5.3 times higher (95% CI 2.29-8.32) than in the reference population in the 1980s and 8.8 times higher (4.16-13.43) in the 1990s. Kaplan-Meier curves showed 6.1% mortality 20 years after onset compared with 0.88% in the reference population. Deaths occurred in one (1%) of 97 children with absence epilepsy, 12 (2%) of 510 with partial and primary generalised epilepsy, and 13 (15%) of 85 with secondary generalised epilepsy. 22 deaths were caused by disorders sufficient to cause functional neurological deficit, one by probable sudden unexpected death in epilepsy, two by suicide, and one by homicide. Functional neurological deficit was the only independent determinant of mortality., Interpretation: Death from epilepsy is uncommon in children without a severe neurological disorder sufficient to cause functional neurological deficit and sudden unexpected death in epilepsy is rare.

Patients with epilepsy may be subject to an increased risk of premature death from the underlying cause, or from the epilepsy itself. The extent and nature of this risk has been insufficiently investigated. Standard mortality ratios (SMRs) of patients with newly diagnosed epilepsy were determined in a prospective national population-based study. 1091 patients with newly diagnosed or suspected epilepsy were ascertained who were attending one of 275 UK general practices from 1984-1987. 1091 patients were classified after 6 months as definite epilepsy (564), possible epilepsy (228), febrile seizures (220), or not epilepsy (79). Over a median follow up of 6.9 years the SMR for patients with definite or possible epilepsy was 2.5 (95% CI 2.1-2.9), and 3.0 (2.5-3.7) for definite epilepsy. The SMR was highest during the first year after diagnosis 5.1 (3.8-6.5), declined to 2.5 (1.5-3.9) at 3 years, and 1.3 (0.7-2.0) at 5 years. The commonest causes of death were pneumonia (SMR 7.2), cancer (3.5), and stroke (3.7). The SMR for patients with idiopathic epilepsy was 1.6 (1.0-2.4), remote symptomatic epilepsy 4.3 (3.3-5.5), and acute symptomatic epilepsy 2.9 (1.7-4.5). Mortality in patients with newly-diagnosed epilepsy was high, mainly due to the underlying cause. The SMR for idiopathic epilepsy was also raised, suggesting that epilepsy per se may carry a small risk of death.

Over 2000 epileptic patients admitted to the Chalfont Centre for Epilepsy between 1931 and 1971 and taking anticonvulsants were followed up to the end of 1977. Mortality between 1951 and 1977 was greatly in excess of that in the general population of England and Wales in that period allowing for age and sex. Some of the excess was directly attributable to epilepsy, but there were also more deaths from suicide and circulatory, respiratory, and malignant disease than would be expected. Apart from the brain and central nervous system, no particular site had a significant excess of tumours. In particular, there were no liver tumours (and only one gallbladder carcinoma). This makes it unlikely that the liver tumours produced on feeding phenobarbitone to mice are indicators of major human risk.

Over 200 children from a group of 628 were followed up after 25 years. The children were identified as having had at least one seizure. Particular attention was given to the long-term prognosis in relation to medical, social, and educational problems. Although nearly two-thirds of the sample suffered minimal ill-effects, the problems of the remainder were considerable. 10-1% had died. Of the survivors 11-2% were confined to institutions, and 6-6% were invalids at home. Just under a quarter had chronic epilepsy. While overall the educational achievement of the sample was good, there was also a considerable number of educational problems. Continuing epilepsy was associated with greatly reduced educational and occupational achievement compared with the group in remission. The study reveals the considerable cost of epilepsy to the community in both human and material terms. It also reveals why epilepsy is regarded as a frightening illness.
